RESUMEN

To effectively use (Li) lithium metal anodes, it is becoming increasingly necessary to create membranes with high lithium conductivity, electrochemical and thermal stabilities, as well as adequate mechanical properties. Composite gel polymer electrolytes (CGPE) have emerged as a promising strategy, offering improved ionic conductivity and structural performance compared to polymer electrolytes. In this study, a simple and scalable approach was developed to fabricate a crosslinked polyethylene oxide (PEO)-based membrane, comprising two different glass fiber reinforcements, in terms of morphology and thickness. The incorporation of a solvated ionic liquid into the developed membrane enhances the ionic conductivity and reduces flammability in the resulting CGPE. Galvanostatic cycling experiments demonstrate favorable performance of the composite membrane in symmetric Li cells. Furthermore, the CGPE demonstrated electrochemical stability, enabling the cell to cycle continuously for more than 700 h at a temperature of 40 °C without short circuits. When applied in a half-cell configuration with lithium iron phosphate (LFP) cathodes, the composite membrane enabled cycling at different current densities, achieving a discharge capacity of 144 mAh·g-1. Overall, the findings obtained in this work highlight the potential of crosslinked PEO-based composite membranes for high-performance Li metal anodes, with enhanced near room temperature conductivity, electrochemical stability, and cycling capability.

Sarcopenia screening tools have a low capacity to predict adverse outcomes that are consequences of sarcopenia in the elderly population. This study aimed to evaluate the ability of a new sarcopenia screening tool SARC-GLOBAL to predict negative clinical outcomes in the elderly. A total of 395 individuals were evaluated in a 42-month period. The screening tools SARC-GLOBAL, SARC-F, and SARC-CalF and the diagnosis of sarcopenia according to European Working Group on Sarcopenia in Older Persons (EWGSOP2) were performed at the beginning of the study. Logistic and Poisson regression models were applied to assess the predictive value of the tools for the odds and risks of negative clinical outcomes, respectively. The most common negative clinical outcome in the followed population was falls (12.9%), followed by infections (12.4%), hospitalizations (11.8%), fractures (4.3%), and deaths (2.7%). Both SARC-GLOBAL and SARC-F were similar in predicting the odds of falls and hospitalizations during the follow up period, however SARC-CalF only predicted the odds of hospitalizations at 42 months.

OBJECTIVE: Acute kidney injury (AKI) in the neonatal period is associated with worst outcomes as increased mortality and increased length of hospital stay. Very low birth weight (VLBW) newborns are at higher risk for developing several other conditions that are associated with worst outcomes. Understanding the risk factors for AKI may help to prevent this condition and improve neonatal care for this population. METHODS: This retrospective cohort study included 155 very low birth weight newborns admitted between 2015 and 2017. The authors compared the newborns who developed neonatal AKI with the non-AKI group and analyzed the main risk factors for developing AKI in the population. The authors also performed an analysis of the main outcomes defined as the duration of mechanical ventilation, length of stay, and death. RESULTS: From the cohort, a total of 61 (39.4%) patients had AKI. The main risk factors associated with Neonatal AKI were necrotizing enterocolitis (aOR 7.61 [1.69 - 34.37]; p = 0.008), neonatal sepsis (aOR 2.91 [1.17 - 7.24], p = 0.021), and hemodynamic instability (aOR 2.99 [1.35 - 6.64]; p = 0.007). Neonatal AKI was also associated with an increase in the duration of mechanical ventilation in 9.4 days (p = 0.026) and in an increase in mortality 4 times (p = 0.009), after adjusting for the other variables. CONCLUSION: The present results highlight the importance of minimizing sepsis and necrotizing enterocolitis, as well as the importance of identifying hemodynamic instability, to prevent AKI and diminish the burden of morbimortality in VLBW newborns.

Poor nutrition increases the risk of diseases and adverse health outcomes in older adults. We evaluated the potential inadequacy of nutrient intake among older adults in Brazil and its association with body anthropometry and composition outcomes. Dietary intake was obtained from 295 community-living older adults (>60 years old), of both genders, using a seven-day food record. Nutrient inadequacy was further identified based on the Dietary Reference Intakes and European Guidelines. Skeletal muscle mass (SM), strength and performance, and the diagnosis of sarcopenia were assessed using reference methods. Nutritional inadequacy was high, with energy, dietary fiber, and six micronutrients exhibiting the greatest inadequacy levels (>80%). Energy intake was correlated with SM strength (p = 0.000) and performance (p = 0.001). Inadequate energy, fiber, and protein intakes influenced BMI, while inadequate intake of vitamin B6 directly affected the diagnosis of sarcopenia (p ≤ 0.005). Further research is required to investigate whether these inadequacies can be associated with other clinical health outcomes.

RESUMEN

The repeated administration of non-degradable dendrimers can lead to toxicity due to their bioaccumulation. Furthermore, in drug delivery applications, carrier stability can result in low biological performance due to insufficient intracellular cargo release. A novel family of versatile, biosafe, water-soluble, and fully biodegradable PEG-dendritic nanosystems is proposed, which overcomes the limitations of the most used dendrimers. Their novelty relies on the full and adjustable degradability thanks to the presence of tunable ester bonds in every dendritic arm. These dendritic nanosystems present peripheral azides that allow their easy multivalent functionalization, by "click" chemistry, with a vast range of ligands to act as versatile carriers. Here, their amine-functionalization to serve as nucleic acid vectors for gene therapy is explored. These nanosystems complex and protect efficiently siRNA in very small dendriplexes (<60 nm), being successfully cell-internalized, including in hard-to-transfect neuronal cells even when in full tissue explants (dorsal root ganglia). Importantly, full biodegradability was crucial for an efficient nucleic acid intracellular release and the attainment of excellent transfection efficiencies. The reported fully biodegradable dendritic nanosystems can act as multi-function nanotherapeutics for gene therapy, and also for broader applications in nanomedicine. Therefore, they represent top-notch and clinically translatable health facilitating nanotechnologies for further developments in theranostics.

As part of an arbovirus vector control strategy, chemical control continues to be a mainstay in mitigating the burden of disease. The current arsenal of chemicals used for this purpose, however, are becoming challenged rapidly because of issues of insecticide resistance and environmental pressure. Newer, environmentally friendly actives are of interest to supplement aging chemistries; therefore efforts to screen compounds for insecticidal activity are warranted. This study evaluated the efficacy of the high-throughput screening system (HITSS) for measuring the behavior-modifying actions of Brazilian Cerrado plant extracts, oils, and other compounds against Aedes aegypti. Different concentrations were evaluated, with 8 of 34 samples tested demonstrating either contact irritancy, spatial repellency, or attractiveness. We concluded several natural products screened in this study showed promise for use against mosquito vectors like Ae. aegypti, and that the compact modular HITSS assay constitutes a robust tool for measuring the behavioral responses of mosquitoes in the search for novel insecticides derived from natural products.

RESUMEN

Solid-state electrolytes are a promising family of materials for the next generation of high-energy rechargeable lithium batteries. Polymer electrolytes (PEs) have been widely investigated due to their main advantages, which include easy processability, high safety, good mechanical flexibility, and low weight. This review presents recent scientific advances in the design of versatile polymer-based electrolytes and composite electrolytes, underlining the current limitations and remaining challenges while highlighting their technical accomplishments. The recent advances in PEs as a promising application in structural batteries are also emphasized.

Vismia gracilis extracts were tested against Aedes aegypti to assess mortality and behavioural effects. The leaf hexanic extract (L-Hex) presented increased larvicidal activity with exposure period: LC50 46.48 µg/mL (24 h) and LC50 20.57 µg/mL (48 h). Eight compounds were annotated/isolated from the L-Hex active extract, 4 benzophenones and 4 anthraquinones. Considering chemometric findings, the benzophenone moiety, tested as the commercial benzophenone, promoted larval mortality (LC50 16.35 µg/mL). Both the L-Hex extract and benzophenone induced intestinal damage in larvae. Benzophenone also promoted toxicity and behavioural effects in female adults. These findings highlighted the potential use of this class of compounds for developing vector-control products.

OBJECTIVES: The use of easily accessible methods to estimate skeletal muscle mass (SMM) in patients with cirrhosis is often limited by the presence of edema and ascites, precluding a reliable diagnosis of sarcopenia. The aim of this study was to design predictive models using variables derived from anthropometric and/or biochemical measures to estimate SMM; and to validate their applicability in diagnosing sarcopenia in patients with cirrhosis. METHODS: Anthropometric and biochemical data were obtained from 124 male patients (18-76 y of age) with cirrhosis who also underwent dual-energy x-ray absorptiometry (DXA) and handgrip strength (HGS) assessments to identify low SMM and diagnose sarcopenia using reference cutoff values. Univariate analyses for variable selection were applied to generate predictive decision tree models for low SMM. Model accuracy for the prediction of low SMM and sarcopenia (when associated with HGS) was tested by comparison with reference cutoff values (appendicular SMM index, obtained by DXA) and clinical sarcopenia diagnoses. The prognostic value of the models for the prediction of sarcopenia and mortality at 104 wk of follow up was further tested using Kaplan-Meier graphics and Cox models. RESULTS: The models with anthropometric variables, alone and combined with biochemical variables, showed good accuracy (0.89 [0.83; 0.94] and 0.90 [0.84; 0.95], respectively) and sensitivity (0.72 [0.56; 0.85] and 0.74 [0.59; 0.86], respectively) and excellent specificity (0.96 [0.90; 0.99] and 0.97 [0.92; 0.99], respectively) in predicting SMM. Both models showed excellent accuracy (0.94 [0.89; 0.98], good sensitivity (0.68 [0.45; 0.86]), and excellent specificity (1.00 [0.96; 1.00]) in predicting sarcopenia. The models predicted mortality in patients with sarcopenia, with the likelihood of death sixfold greater relative to patients not predicted to have sarcopenia. CONCLUSIONS: Our simple and inexpensive models provided a practical and safe approach to diagnosing sarcopenia patients with cirrhosis along with an estimate of their mortality risk when other reference methods are unavailable.

BACKGROUND: Plant extracts and isolated compounds are known for their insecticidal activity. The Aedes aegypti mosquito has a significant medical impact as it transmits a number of arboviruses and is able to develop resistance to the commercially available insecticides. This study investigates larvicidal compounds isolated from Machaerium acutifolium, designated by the Brazilian Forest Service as a sustainable species. RESULTS: A M. acutifolium trunk ethyl acetate extract was fractionated using chromatographic methods with full structural elucidation by mass spectrometry (MS), nuclear magnetic resonance and specific rotation analyses revealing: one new 3-arylcoumarin derivative 1; two flavonoids 2 and 3; a trans-stilbene 4, and an unprecedented natural indene 5. The larvicidal activity against Ae. aegypti after 24 h exposure was: crude extract (median lethal dose, LC50 205 mg L-1 ), fraction C (LC50 27 mg L-1 ) and 5 (LC50 24 mg L-1 ). CONCLUSION: A M. acutifolium extract showed larvicidal activity, which increased with prolonged exposure, demonstrating LC50 75 mg L-1 after 72 h. Although the flavonoids 2 and 3 and trans-stilbene 4 were deemed inactive according to the adopted mortality limit, additional tests revealed their ability to cause 65% Ae. aegypti larvae mortality, suggesting they could contribute to the larvicidal activity. Compound 5, identified by liquid chromatography-MS, was over eight-fold more toxic to larvae than the crude extract after 24 h. Therefore, 5 constitutes a structural model for new prototypes to control Ae. aegypti. These data reinforce the potential of natural products as a source of commercial alternatives for vector control strategies, respecting both sustainability and eco-friendly principles. © 2020 Society of Chemical Industry.

BACKGROUND Osteonecrosis of the jaw is a condition manifesting as necrotic bone for 8 weeks or more, with no history of radiotherapy. It is linked to the use of antiresorptive drugs, such as bisphosphonates and denosumab. We discuss in this case report the importance of infection control during clinical procedures in patients taking antiresorptives. CASE REPORT Our case report describes a 52-year-old woman who had received zoledronic acid injections for 7 years, who presented with osteonecrosis of the jaw in the region of teeth 24, 25, and 26, with no local trauma. The report proceeds to describe the extraction of these teeth. After the dental extractions, she did not have any recurrence of the lesion, and she is currently in follow-up care. CONCLUSIONS Osteonecrosis of the jaw can be prevented through infection control and local trauma prevention. Patients who are taking antiresorptive drugs must receive special care from dental surgeons and doctors, and receive the necessary oral treatments before starting drug therapy.

Complex coacervation between whey proteins and carboxylated or highly sulphated polysaccharides has been widely studied. The aim of this work was to characterise a slightly sulphated polysaccharide (agar) and whey protein insoluble complexes in terms of yield, composition and physicochemical properties as well as to study their rheological behaviour for better understanding their structure. Unlike other sulphated polysaccharides, complexation of agar and whey protein at pH 3 in the absence of a buffering agent resulted in a coacervate that was a gel at 20°C with rheological properties and structure similar to those of simple agar gels, reinforced by proteins electrostatically aggregated to the agar network. The behaviour towards heat treatment was similar to that of agar alone, with a high thermal hysteresis and almost full reversibility. In the presence of citrate buffer, the result was a "flocculated solid", with low water content (75-81%), whose properties were governed by protein behaviour.

A partir das experiências de intervenções em serviços públicos de saúde e em outros setores da cidade, este trabalho afirma uma prática oficineira descentralizada capaz de compor na construção de redes sociais, servindo de instrumento de análise e possibilitando a invenção de modos de vida e afirmação de singularidades. As intervenções na cidade abrem caminhos para mudanças nos processos em curso e contribuem para a produção de saúde/cidadania por meio da potência conectiva das lutas. O enlace dessas temáticas visa ampliar o campo problemático da atenção psicossocial e fortalecer a luta antimanicomial no Espírito Santo, apostando em novas formas de atuação.


From the experiences of interventions in health public services and another sectors from the city, is our intention to assert a oficine practice decentralized able to compose in the construction of social networks in configure themselves as artifice for the analysis, opening to possibilities of alternatives ways of living and affirmations of singularities. The city interventions produce changes in the already started process to expand the production of citizenship/health trough the connective power of the collective battle. The link of these thematic may contribute to expand the problematic field of the psychosocial attention and the appliance to the strengthening of the struggle against mental intitutions in Espírito Santo, producing new ways of acting in the already mentioned sectors.

RESUMEN

O estudo da dor e suas particularidades é de grande importância para o tratamento de diversas patologias e para a melhora na qualidade de vida dos pacientes. A maioria das disfunções orgânicas tem a dor como um ponto importante da sua manifestação. Dessa maneira, é justificável a elaboração de conteúdo para auxiliar os profissionais da saúde no entendimento e tratamento das principais causas de dores agudas e crônicas. Este livro foi elaborado com o objetivo de servir como um guia prático para o manejo da dor por profissionais e acadêmicos de Medicina. Engloba temas como conceitos e aspectos biopsicossociais da dor, além de questões mais complexas como a fisiologia da dor e o tratamento medicamentoso com o arsenal terapêutico existente. Finalmente, também trata dos diversos tipos de dor mais prevalentes e o conhecimento básico que envolve seu manejo.

RESUMEN

Powdered tomato was produced by spray drying the tomato pulp. A full 2³ factorial design with the central point was used, varying the feed flow rate (127-276 g/min), air inlet temperature (200-220ºC) and the atomisation speed (25,000-35,000 rpm). The responses analysed were: moisture content, solubility, wettability consistency and colour, but the factors only significantly affected the colour parameter. All the samples became significantly darker and less red with an increase of the variables under study. A low atomisation speed (25,000 rpm) and lower inlet air temperature (220ºC) produced the powders with a higher colour index (a/b) and less darkening.


Tomate em pó foi produzido por secagem por atomização da polpa de tomate. Um planejamento 2³fatorial completo com ponto central foi conduzido variando a taxa de alimentação (127276 g/min), temperatura de entrada do ar (200-220ºC) e a velocidade de atomização (25000-35000 rpm). As respostas analisadas foram: conteúdo de umidade, solubilidade, molhabilidade, consistência e cor, ma os fatores somente afetaram significativamente a cor. Todas as amostras tornaram significantemente mais escuras e menos vermelhas com o aumento das variáveis sob estudo. Baixa velocidade de atomização (25000 rpm) e menor temperatura do ar de secagem (220ºC) produziram pós de maior índice de cor (a/b) e menor escurecimento.

Introdução: Aids e câncer representam, atualmente, um desafio para a saúde pública, com sérias implicações para os aspectos biológicos, sociais e psicológicos dos indivíduos. Essas doenças crônicas interferem no cotidiano familiar, especialmente, quando atingem crianças e adolescentes. Objetivo: Neste artigo, são apresentados os principais resultados de uma pesquisa sobre processos de estigmatização de pacientes pediátricos com câncer ou HIV, com especial atenção à recepção do diagnóstico e de mudanças no cotidiano familiar ocasionadas pela doença. Casuística e Métodos: Foram realizadas 56 entrevistas semiestruturadas com: pacientes de 7 a 15 anos com câncer ou com HIV, atendidos nos ambulatórios da Faculdade de Medicina do ABC (FMABC) (n = 20); acompanhantes desses pacientes (n = 18); população usuária dos ambulatórios da FMABC (n = 18). Resultados: Em geral, os pacientes e acompanhantes não referiram a presença de estigma relacionado à doença; em contrapartida, a população usuária dos ambulatórios aponta a sua existência. Os pacientes com HIV são orientados pelos seus cuidadores (leigos e profissionais) a não revelar o diagnóstico para sua a rede social, evitando assim situações estigmatizantes. Conclusões: Embora poucas situações de estigmatização tenham sido identificadas nos relatos obtidos, estes revelam estratégias de controle da identidade do doente, objetivando evitar preconceitos. O HIV e o câncer geram diferentes expectativas e sofrimentos nos pacientes e familiares, requisitando dos profissionais de saúde conhecimentos e ações sensíveis aos impactos específicos desses problemas de saúde no cotidiano familiar.


Introduction: Aids and cancer represent, nowadays, a challenge for the public health, with important connotation for biological, social e psychological aspects of the patients. These chronic diseases change the familiar daily routine, especially when children and teens are involved. Objective: In this paper, the core results of a research about stigma among children with cancer or HIV are presented, with special attention to the reception of the diagnostic and the presence of changes on the daily routine caused by both diseases. Patients and Methods: Fifty-six pre-structured interviews were done with three distinct groups, with free selection: pediatric patients between 7 and 15 years old (n = 20); companions of the patients (n = 18) and people that use the clinics of Faculdade de Medicina do ABC (n = 18). Results: In the patient’s group the stigma was not present but, on the other hand, in the general population this stigma was more prevalent. HIV patients were oriented by the doctors and the family to not reveal the diagnostic for the social net, avoiding, with it, prejudice situations. Conclusions: Even thus few prejudice situations have been identified; the results show strategies for the control of the patient identity, trying, in this way, to avoid this prejudice. HIV and cancer promote different expectations and kinds of suffering in patients and in their families, demanding to thehealth professionals’ knowledge and sensible actions for the specific impacts for these problems in the familiar quotidian.
