Diabetes mellitus (DM) is caused by hyperglycemia, resulting from defective insulin secretion or function. It is widely believed that the antioxidant micronutrients obtained from plants afford significant protection against diseases like diabetes mellitus. Present study was aimed to examine the effects of Persian shallot (Allium hirtifolium Boiss) on FBS, HbA1c, insulin, triiodothyronine (T3) and thyroxine (T4) levels in type 1 diabetic rats. Thirty two male Wistar rats were divided into 4 groups of 8. The diabetic groups received 100 and 200 mg/kg Persian shallot extract, diabetic control and normal control received %0.9 saline for 30 days. At the end of treatments, fasting blood specimens were collected. The levels of FBS, HbA1c, insulin, T3 and T4 were measured. Our findings indicated that hydroalcoholic extract of Persian shallot significantly decreased serum levels of FBS and HbA1c in treated groups (in a dose dependent manner) (p<0.05). The serum levels of insulin and T3 slightly increased by Persian shallot but the T4 serum level was declined. These beneficial effects of Persian shallot extracts in diabetic rats could probably be due to the antioxidant capacity of its phenolic and diallyl disulfide content.

BACKGROUND: The prevalence of diabetes mellitus (DM) is dramatically increasing worldwide. Prospective studies have reported that high levels of hepatic enzymes, including aspartate aminotransferase (AST), alanine aminotransferase (ALT), Alkaline phosphatase (ALP) and lactate dehydrogenase (LDH) are associated with later development of diabetes. The aim of the present study was to examine the effect of hydroalcoholic extract of Allium hirtifolium (Persian shallot) on the level of liver enzymes in streptozotocin (STZ)-induced diabetic rats. METHODS: Thirty-two male rats were divided into four groups of eight. The diabetic groups received 100 and 300 mg/kg Persian shallot extract, the diabetic control and non-diabetic control groups received 0.9% saline for 30 days. At the end of the experimental period, fasting blood samples were collected, and enzymes levels were measured. RESULTS: Our findings showed that hydroalcoholic extract of Persian shallot can significantly decrease serum levels of liver enzymes (AST, ALT, ALP and LDH) in treated groups in a dose-dependent fashion (p<0.05). CONCLUSIONS: Antioxidant micronutrients in the extract of Persian shallot may rehabilitate liver damages caused by free radicals in diabetic rats.

Background: Recurrent pregnancy loss (RPL) is a major concern among women worldwide. However, the exact mechanisms underlying miscarriage are not well understood. Recent evidence suggests that single nucleotide polymorphisms in various genes, especially miRNAs, may be responsible for RPL. Objective: We surveyed the association between polymorphisms in pre-miR-125a, pre-miR-10a, pre-miR-323b, GPX4, and GPX4 in Iranian women with idiopathic RPL. Materials and Methods: DNA was extracted from blood samples of 116 women with idiopathic RPL and 89 healthy women as controls who had previously had at least two successful pregnancies. Polymerase chain reaction was used for the amplification of the genes. Genotype screening along with SNaPshot were performed to detect different polymorphisms. Finally, the polymorphisms and frequency of each genotype were compared between the two groups. Results: The frequencies of polymorphisms in pre-miR-125a (p < 0.001) and pre-miR-10a (p = 0.04) were calculated among the case and control groups, which showed a statistical difference (p < 0.05), indicating an association between these polymorphisms and the symptoms of RPL. The frequencies of polymorphisms of genotypes in GPX4, COMT and pre-miR-323b did not demonstrate any difference between the two groups. Also, the amount of alleles in pre-miR-125a and pre-miR-10a were significantly different (p < 0.001 and p = 0.02, respectively) and the dominant inheritance model was proposed. Conclusion: In conclusion, pre-miR-125a and pre-miR-10a can be associated with RPL in women. The SNaPshot technique is a valuable tool to evaluate possible associations between polymorphisms and health conditions.

OBJECTIVE: To determine 1) the indexes/indicators used for evaluating the "strengthening community actions" mechanism of the Ottawa Charter for Health Promotion and 2) to extract the characteristics and key components of the indexes/indicators using a scoping review.Data Source:In May 2020, the search was conducted across three databases: Medline (via PubMed), Embase, and Scopus.Inclusion and Exclusion Criteria: All primary studies relating to development, identification, and measurement of health promotion indices/indicators associated to the "strengthening community actions" were included. The review articles were excluded. DATA EXTRACTION: The data were extracted to a data-charting form that was developed by the research team. Two authors reviewed the extracted data. DATA SYNTHESIS: To summarize and report the data, a descriptive numerical analysis and a narrative descriptive synthesizing approach were used. RESULTS: In total, 93 study articles were included. A majority of studies (82%) were conducted in developed countries. Different types of recognized indices were categorized into seven groups: social cohesion (n = 3), community capacity (n = 1), community participation (n = 7), social capital (n = 6), social network (n = 3), social support (n = 1), and others (n = 5). CONCLUSIONS: Having a collection of "strengthening community actions" indices/indicators in hand, health policymakers and health promotion specialists might be able to do their best in considering, selecting, and applying the most appropriate indices/indicators while evaluating community health promotion interventions in different settings.

BACKGROUND: Acute kidney injury (AKI) is a severe and preventable problem of crushed earthquake victims. Early hydration therapy started before fully removing earthquake rubbles has been claimed to play a decisive role in AKI prevention, which saves the necessity of later dialysis. However, the extent, quality, and appropriateness of its know-how are controversial. METHODS: Processing clinical and paraclinical data gathered from Bam earthquake victims older than 15 years, we tried to determine correlations between the time of being under the rubbles (TUR), the level of serum creatine phosphokinase (CPK), the delayed onset of fluid therapy (DFT), and finally the volume of intravenous fluid received per day (VFR) with the formation of AKI and the need for dialysis. RESULTS: There is a direct and significant relation between the intensity of the trauma (TUR and CPK) and DFT with the occurrence of AKI and need for dialysis (P < .001). However, as the VFR increases, the occurrence of AKI and the need for dialysis significantly decrease (P = .005). Based on multivariate analysis, the occurrence of AKI and the need for dialysis are primarily affected by CPK, TUR, and VFR; and DFT has been dropped out. The analysis showed the preventive role of VFR more than 6 L in severe rhabdomyolysis patients and of at least 3 L in moderate ones in development of AKI and dialysis. CONCLUSIONS: In the severely rhabdomyolized patients (CPK ≥ 15,000), higher volumes of prophylactic fluid (VFR >6 L) are required, whereas in less-traumatized patients, lower volumes (3-6 L) would be effective.

RESUMEN

A total of 2,305 consecutive newborns at Shariati Hospital, Tehran, Iran, and 1,706 at Lolagar Hospital, Tehran, Iran, were examined for 2 years (2004-2006). Mongolian spots were found in 11.4% and 37.3% of newborns at Shariati and Lolagar hospitals, respectively. The most frequent site of involvement is the sacral, followed by the gluteal area.

Non–small cell lung cancer (NSCLC) is an important cause of cancer-associated deaths worldwide. Therefore, the exact molecular mechanisms of NSCLC are unidentified. The present investigation aims to identify the miRNAs with predictive value in NSCLC. The two datasets were downloaded from the Gene Expression Omnibus (GEO) database. Differentially expressed miRNAs (DEmiRNA) and mRNAs (DEmRNA) were selected from the normalized data. Next, miRNA-mRNA interactions were determined. Then, co-expression network analysis was completed using the WGCNA package in R software. The co-expression network between DEmiRNAs and DEmRNAs was calculated to prioritize the miRNAs. Next, the enrichment analysis was performed for DEmiRNA and DEmRNA. Finally, the drug-gene interaction network was constructed by importing the gene list to dgidb database. A total of 3,033 differentially expressed genes and 58 DE miRNA were recognized from two datasets. The co-expression network analysis was utilized to build a gene co-expression network. Next, four modules were selected based on the Zsummary score. In the next step, a bipartite miRNA-gene network was constructed and hub miRNAs (let-7a-2-3p, let-7d-5p, let-7b-5p, let-7a-5p, and let-7b-3p) were selected. Finally, a drug-gene network was constructed while SUNITINIB, MEDROXYPROGESTERONE ACETATE, DOFETILIDE, HALOPERIDOL, and CALCITRIOL drugs were recognized as a beneficial drug in NSCLC. The hub miRNAs and repurposed drugs may act a vital role in NSCLC progression and treatment, respectively; however, these results must validate in further clinical and experimental assessments.

In this study, NiO/Rosa Canina-L seeds activated carbon nanocomposite (NiO/ACNC) was prepared by adding dropwise NaOH solution (2mol/L) to raise the suspension pH to around 9 at room temperature under ultrasonic irradiation (200W) as an efficient method and characterized by FE-SEM, FTIR and N2 adsorption-desorption isotherm. The effect of different parameters such as contact time (0-120min), initial metal ion concentration (25-200mg/L), temperature (298, 318 and 333K), amount of adsorbent (0.002-0.007g) and the solution's initial pH (1-7) on the adsorption of Pb (II) was investigated in batch-scale experiments. The equilibrium data were well fitted by Langmuir model type 1 (R2>0.99). The maximum monolayer adsorption capacity (qm) of NiO/ACNC was 1428.57mg/L. Thermodynamic parameters (ΔG°, ΔH° and ΔS°) were also calculated. The results showed that the adsorption of Pb (II) onto NiO/ACNC was feasible, spontaneous and exothermic under studied conditions. In addition, a fuzzy-logic-based model including multiple inputs and one output was developed to predict the removal efficiency of Pb (II) from aqueous solution. Four input variables including pH, contact time (min), dosage (g) and initial concentration of Pb (II) were fuzzified using an artificial intelligence-based approach. The fuzzy subsets consisted of triangular membership functions with eight levels and a total of 26 rules in the IF-THEN approach which was implemented on a Mamdani-type of fuzzy inference system. Fuzzy data exhibited small deviation with satisfactory coefficient of determination (R2>0.98) that clearly proved very good performance of fuzzy-logic-based model in prediction of removal efficiency of Pb (II). It was confirmed that NiO/ACNC had a great potential as a novel adsorbent to remove Pb (II) from aqueous solution.

This is a systematic review and meta-analysis of existing evidence regarding the possible effects of epothilones on spinal cord injury (SCI). This study aimed to investigate the possible effects of epothilone administration on locomotion recovery in animal models of SCI. Despite increasing rates of SCI and its burden on populations, no consensus has been reached about the possible treatment modality for SCI. Meanwhile, low-dose epothilones have been reported to have positive effects on SCI outcomes. Electronic databases of Web of Science, Scopus, Embase, and Medline were searched using keywords related to epothilones and SCI until the end of 2020. Two researchers screened the articles, and extracted data were analyzed using STATA ver. 14.0. Final results are reported as a standardized mean difference (SMD) with a 95% confidence interval (CI). After the screening, five studies were included in the analysis. Rats were used in all the studies. Two types of epothilones were used via intraperitoneal injection and were shown to have positive effects on the motor outcomes of samples with SCI (SMD, 0.87; 95% CI, 0.51 to 1.23; p =0.71). Although a slightly better effect was observed when using epothilone B, the difference was not significant (coefficient, −0.50; 95% CI, −1.52 to 0.52; p =0.246). The results of this study suggest that epothilones have positive effects on the improvement of motor function in rats, when administered intraperitoneally until a maximum of 1 day after SCI. However, current evidence regarding the matter is still scarce.

Background@#Current therapies are quite unsuccessful in the management of neuropathic pain. Therefore, considering the inhibitory characteristics of GABA mediators, the present systematic review and meta-analysis aimed to determine the efficacy of GABAergic neural precursor cells on neuropathic pain management. @*Methods@#Search was conducted on Medline, Embase, Scopus, and Web of Science databases. A search strategy was designed based on the keywords related to GABAergic cells combined with neuropathic pain. The outcomes were allodynia and hyperalgesia. The results were reported as a pooled standardized mean difference (SMD) with a 95% confidence interval (95% CI). @*Results@#Data of 13 studies were analyzed in the present meta-analysis. The results showed that administration of GABAergic cells improved allodynia (SMD = 1.79;95% CI: 0.87, 271; P < 0.001) and hyperalgesia (SMD = 1.29; 95% CI: 0.26, 2.32; P = 0.019). Moreover, the analyses demonstrated that the efficacy of GABAergic cells in the management of allodynia and hyperalgesia is only observed in rats. Also, only genetically modified cells are effective in improving both of allodynia, and hyperalgesia. @*Conclusions@#A moderate level of pre-clinical evidence showed that transplantation of genetically-modified GABAergic cells is effective in the management of neuropathic pain. However, it seems that the transplantation efficacy of these cells is only statistically significant in improving pain symptoms in rats. Hence, caution should be exercised regarding the generalizability and the translation of the findings from rats and mice studies to large animal studies and clinical trials.

BACKGROUND: Quality of services, making communications with target groups, and educating them are among the most important success factors in implementation of family planning programs. Provision of public access to contraception and related methods, counseling services, and paying attention to social specification and cultures are important in promotion of service quality. With regard to applicability of health education theories and models, the present study aimed to find the impact of an educational program based on BASNEF model to choose contraceptive methods in women referring to health care centers in Minoodasht in 2012. MATERIALS AND METHODS: This is a quasi-experimental study. Data were collected using BASNEF questionnaire by the researcher from women referring to health care centers in two groups of study and control (n = 100 in each group). Educational intervention (in the form of four educational 1-h sessions once a week during 1 month and two additional review sessions) was conducted in the form of group and face-to-face discussions and educational booklets were distributed. Data were analyzed by Chi-square, analysis of covariance (ANCOVA), paired t-test, and t-test through SPSS version 14. RESULTS: After intervention, mean score of knowledge was significantly higher in the study group compared to control (P < 0.003). Intervention led to a better attitude in the study group (P < 0.0001). Mean score of subjective norms and enabling factors after intervention was significantly higher in the study group compared to control (P < 0.0001). The change in practice had a significant increase in the study group (54%) compared to control (9%). CONCLUSIONS: BASNEF-based educational intervention was effective in increasing women's knowledge, attitude, and practice. As family and educational facilities are among the influencing factors on contraceptive method selection, interventional planning is hoped to be based on educational model.

BACKGROUND: Multi-state models are appropriate for cancer studies such as gastrectomy which have high mortality statistics. These models can be used to better describe the natural disease process. But reaching that goal requires making assumptions like Markov and homogeneity with time. The present study aims to investigate these hypotheses. MATERIALS AND METHODS: Data from 330 patients with gastric cancer undergoing surgery at Iran Cancer Institute from 1995 to 1999 were analyzed. To assess Markov assumption and time homogeneity in modeling transition rates among states of multi-state model, Cox-Snell residuals, Akaikie information criteria and Schoenfeld residuals were used, respectively. RESULTS: The assessment of Markov assumption based on Cox-Snell residuals and Akaikie information criterion showed that Markov assumption was not held just for transition rate of relapse (state 1-state 2) and for other transition rates - death hazard without relapse (state 1-state 3) and death hazard with relapse (state 2-state 3) - this assumption could also be made. Moreover, the assessment of time homogeneity assumption based on Schoenfeld residuals revealed that this assumption - regarding the general test and each of the variables in the model - was held just for relapse (state 1-state 2) and death hazard with a relapse (state 2-state 3). CONCLUSIONS: Most researchers take account of assumptions such as Markov and time homogeneity in modeling transition rates. These assumptions can make the multi-state model simpler but if these assumptions are not made, they will lead to incorrect inferences and improper fitting.

BACKGROUND: Occupational accidents are of most important consequences of globalization in developing countries. Therefore, investigating the causes of occupational accidents for improving the job situation and making operational policy is necessary. So the aim of this study was to investigate factors affecting the fatal occupational accidents and also calculate the years of life lost for dead workers. METHODS: This cross-sectional study was conducted on data related to the 6052 injured workers that was registered in the 2013 registry system of the Ministry of Health and Medical Education of Iran. Variables including sex, education, age, job tenure, injury cause, referred location of injured workers, occupation, shift work, season, accident day, damaged part of the body were chosen as independent variables. The Chi-squared and Fisher exact tests were used for univariate analysis and then exact multiple logistic regression was carried out to identify independent risk factors of fatal occupational accidents. Finally, for dead workers, years of life lost, according to the injury causes was calculated. RESULTS: Among the 6052 accidents reported, 33 deaths were recorded. Chi-square and Fisher exact tests showed that factors including: current job tenure (p = 0.01), damaged parts of the body (p < 0.001) and injury cause (p < 0.001) are associated with the fatal accidents. Also exact multiple logistic regression analysis showed a significant association between electric shocks as a cause of injury (OR = 7.04; 95% CI: 1.01–43.74; p = 0.02) and current job tenure more than 1 year (OR = 0.21; 95% CI: 0.05–0.70; p = 0.005) with the fatal accidents. The total amount of years of life lost based on causes of injuries was estimated 1289.12 years. CONCLUSIONS: In Iran, fatal accident odds in workers with job tenure more than 1 year was less in comparing to the workers with job tenure less and equal to 1 year. Also odd of death for electrical shock was more than other causes of injuries. So it seems that employing of workers who have more than one-year work experience in a specific job and using of appropriate safeguards will be useful for the reducing of fatal occupational accidents.

This study aimed to compare evidence-based medicine (EBM) vs. conventional approaches to journal club sessions in teaching critical appraisal skills in reading papers by emergency medicine residents. This double cut off discontinuation regression quasi-experimental study was conducted among emergency medicine residents. EBM vs. the conventional approach were applied to teach critical appraisal skills for half of the residents as an experimental group and another half as a control group respectively. Both groups participated in one hour monthly journal club sessions for six months. Before and after the study, all participants were examined by two tests: the Fresno Test (FT) [to evaluate their knowledge about EBM] and the Critical Appraisal Skills Test (CAST) [to evaluate their competency with critical appraisal skills]. The allocation of the participants into the experimental or control groups was according to their CAST scores before the study. 50 emergency medicine residents participated. After the study, the scores of both groups in the FT and CAST significantly improved (p<0.01), and the promotion of scores of the FT and CAST in the experimental group were more than that of the conventional group (p<0.0001). The current study indicated that an evidence-based medicine approach in journal club sessions was comparatively more advantageous compared to the conventional approach in teaching critical appraisal skills for reading papers among the residents of emergency medicine.

RESUMEN

The present study aimed to determine the relationship between the level of air pollutants and the rate of ischemic stroke (IS) admissions to hospitals. In this retrospective cross-sectional study, stroke admissions (January-March 2012 and 2013) to an emergency department and air pollution and meteorological data were gathered. The relationship between air pollutant levels and hospital admission rates were evaluated using the generalize additive model. In all 379 patients with IS were referred to the hospital (52.5% male; mean age 68.2+/-13.3 years). Both transient (p<0.001) and long-term (p<0.001) rises in CO level increases the risk of IS. Increased weekly (p<0.001) and monthly (p<0.001) average O3 levels amplifies this risk, while a transient increase in NO2 (p<0.001) and SO2 (p<0.001) levels has the same effect. Long-term changes in PM(10) (p<0.001) and PM(2.5) (p<0.001) also increase the risk of IS. The findings showed that the level of air pollutants directly correlates with the number of stroke admissions to the emergency department.

RESUMEN

Metastatic breast cancer is a disease of early breast cancer that usually occurs several years after the early breast cancer. Breast cancer is the most common cancer among Iranian women. According to the new statistics in Iran 6160 breast cancers are diagnosed in the country each year and 1063 cases lead to death. In this paper, epidemiology, diagnosis and treatment have been investigated. In this study, case-control clinical trials and open studies with adequate data were collected. Due to the higher risk of age group 40-49 years and the advent of advanced breast cancer in Iranian women, the early diagnosis and determination of the exact size of the tumor before surgery is important in choosing a therapy plan. The decision on the therapy of invasive breast cancer depends on several factors such as cancer stage, tumor size and type, pathological and cytological status of the tumor, the patient's opinion, the presence or absence of estrogen and progesterone receptors in the cytoplasm of tumor cells and so on.

Objective: To study agents of animal wound myiasis in various geographical districts of Fars province. Methods: This study has been done in Fars province, located in the southern part of Iran. Sums of 10 358 domestic animals have been visited from April 2011 to March 2012. The infected wounds in any parts of animal body were sampled by means of forceps. Results: About 61% of all animal wound myiasis were caused by larvae of Wohlfahrtia magnifica. The most wound myiasis cases due to this species occurred in central part of Fars province. There wasn't any significant difference between sheep and goat in infestation with myiasis (P>0.05). The infestation rate of myiasis in cattle community was 0.86%. Conclusions: The infestation rate of livestock was lower than other works in Iran and some other countries like Saudi Arabia. Chrysomya bezziana has been mentioned as main myiasis agent in Iran. But in this study it cleared that similarly to some European countries, the common animal myiasis agent in Iran is Wohlfahrtia magnifica. Introducing new species as principal agent for myiasis can help public health and animal husbandry policy makers to prepare sufficient and effective control and/or preventive measures for this disease.
