RESUMEN

Subcortical arteriosclerotic encephalopathy was diagnosed in a 56-year-old female normotensive patient with gradually progressing dementia, pseudobulbar palsy and motor deficits. CT scan showed white matter low attenuation in the frontal and parietal lobes. Neuropathological examination revealed degeneration of the white matter. Amyloid was found in walls of small cortical vessels. The walls of small vessels in the white matter showed severe thickening, fibrosis and hyalinization but not amyloid. Cerebral amyloid angiopathy may be responsible for subcortical arteriosclerotic encephalopathy.

RESUMEN

An open pilot study was carried out in 27 patients with severe and frequent migraine attacks who had failed to respond adequately to other drug therapy, to assess the effectiveness and tolerance of prophylactic treatment with suloctidil. Patients received 600 mg suloctidil daily for 6 weeks. Response, assessed on the reduction in frequency of attacks, was considered as good in 7 patients and as significant improvement in 11 patients. Treatment was rated as of doubtful value or as ineffective in the other 9 patients. One patient stopped treatment because of adverse reactions and 10 others reported mild side-effects.

RESUMEN

In a group of 15 patients with Binswanger's subcortical atherosclerotic encephalopathy electroencephalographic investigations were carried out. The obtained EEG records were pathological in most cases, with varying degree of intensity of changes. Slow activity symmetrically distributed prevailed.

RESUMEN

The authors discuss the new form of headache described in 1974 by Sjaastad differing from other headaches known as yet occurring in attacks: chronic paroxymal hemicrania. Attacks resembling cluster headaches have a shorter duration (10-20 minutes) and occur more frequently (up to 30 times daily). An important feature is complete disappearance of headache after indomethacine. Two own cases are reported and the pathomechanism of attacks is discussed.

In 19 patients (14 males, 5 females) the clinical diagnosis of subcortical, arteriosclerotic encephalopathy (s.a.e.) was established on the basis of CT scanning, which showed in subcortical white matter low-attenuation areas and, often, focal ischaemic changes. The age of patients ranged from 46 to 85 years, the duration of illness from 6 months to 12 years. Four patients were normotensive (in 1 of these cases the clinical diagnosis was confirmed neuropathologically). In 2 hypertensive patients the acute development of neurological symptoms was related to considerable fall of systemic RR. On the basis of the CT picture s.a.e. may be diagnosed early, before pronounced neurological and intellectual deficits develop.

RESUMEN

In 11 patients with Horton's headache morphological investigations (differential white blood cell count), cytoenzymatic determinations (alkaline and acid phosphatase, non-specific esterase) and cytoimmunological tests (IgM and IgG binding) were carried out on capillary blood neutrophils obtained from the area of pain, non-painful area of the skin on the head on the contralateral side, and from the finger. The observed changes suggest an active participation of neutrophils in the pathological mechanism of Horton's headache and anaphylactoidal background of the disease.

RESUMEN

The paper has been aimed at searching for the correlation between neck and arm pain and low back pain to test the rate of concomitance of both disorders and the order of their appearance. The studies have been carried out in two big light industry plants in Lódz. The coexistence of the pain syndromes has been found to be frequent, low back pain usually developing first. It seems that the occurrence of one of those disorders disturbs the functioning of the whole vertebral column and thus predisposes to the development of the other one.

RESUMEN

Two cases of chronic paroxysmal hemicrania are presented, both with atypical late onset of illness and lack of pre-CPH stage. In one patient the pain attacks were unusually short, and no associated symptoms during the pain attack were observed. However, the positive reaction to indomethacin substantiates the diagnosis of CPH.

Sixteen patients with a clinical diagnosis of subcortical arteriosclerotic encephalopathy (Binswanger's disease) were studied. Mild or moderate dementia was found in 14 cases, with characteristic disturbances of concentration, and psychic retardation with apathy. Neuropsychological studies revealed that signs of diffuse lesions in the frontal lobes dominated. Impairment of memory was significant in all cases. Although intellectual impairment in subcortical arteriosclerotic encephalopathy resembled subcortical dementia, some significant differences were found.
