RESUMEN

Objective: This study examined purported change mechanisms in emotion-focused therapy for social anxiety disorder. Methods: The sample included nine clients who had participated in a multiple-baseline case study trial examining the efficacy of emotion-focused therapy for social anxiety disorder (SAD). Multilevel analyses were conducted to examine the trajectories of emotions over the course of treatment, and whether primary adaptive emotions in a given session predicted levels of SAD symptoms, self-criticism, and self-reassurance over the course of the following week. Results: Findings showed a significant decrease in shame, and a marginally significant increase in assertive anger, over the course of treatment. Adaptive sadness/grief in a given session predicted less fear of negative evaluation over the course of the following week. Shame in a given session predicted higher levels of inadequate-self over the course of the following week. Finally, shame, and to a lesser degree assertive anger, in a given session predicted reassurance of self over the course of the following week. Neither assertive anger nor adaptive sadness/grief in a given session predicted levels of self-criticism over the course of the following week. Conclusions: These findings lend partial preliminary support for the therapeutic role of evoking and processing adaptive sadness/grief and assertive anger in the treatment of SAD.

RESUMEN

Objective: This meta-analysis synthesized the literature regarding the effect of therapist experience on internalizing client outcomes to evaluate the utility of lay providers in delivering treatment and to inform therapist training. Method: The analysis included 22 studies, contributing 208 effect sizes. Study and client characteristics were coded to examine moderators. We conducted subgroup meta-analyses examining the relationship of therapist experience across a diverse set of internalizing client outcomes. Results: Results demonstrated a small, but significant relationship between therapist experience and internalizing client outcomes. There was no relationship between therapist experience and outcomes in clients with primary anxiety disorders. In samples of clients with primary depressive disorders and in samples of clients with mixed internalizing disorders, there was a significant relationship between experience and outcomes. The relationship between therapist experience and outcomes was stronger when clients were randomized to therapists, treatment was not manualized, and for measures of client satisfaction and "other" outcomes (e.g., dropout). Conclusions: It appears that therapist experience may matter for internalizing clients under certain circumstances, but this relationship is modest. Continuing methodological concerns in the literature are noted, as well as recommendations to address these concerns.

RESUMEN

OBJECTIVE: A trial of psychotherapy for generalized anxiety disorder (GAD) demonstrated that motivational interviewing (MI) integrated with cognitive-behavioral therapy (CBT) outperformed CBT alone on clients' worry reduction across a 12-month follow-up. In the present study, we hypothesized and tested that less client resistance and greater client-perceived therapist empathy (specific foci of MI) would account for MI's additive effect. Exploratory analyses assessed whether the common processes of homework completion and therapeutic alliance quality mediated the treatment effect. METHOD: Clients with GAD were randomized to 15 sessions of MI-CBT (n = 42) or CBT alone (n = 43). Worry was assessed throughout treatment and follow-up. Observers rated resistance at midtreatment, and clients reported on perceived therapist empathy, alliance, and homework completion throughout treatment. Mediation was tested with bootstrapping methods. RESULTS: Expectedly, MI-CBT clients evidenced less resistance and perceived greater therapist empathy, each of which related to lower 12-month worry. However, when both variables were tested simultaneously, only resistance remained a significant mediator of treatment. No indirect effects through homework completion or alliance emerged. CONCLUSIONS: Reducing client resistance may be a theory-consistent mechanism through which integrative MI-CBT promotes superior long-term improvement than traditional CBT when treating GAD. Clinical or methodological significance of this article: This study further supports the long-term clinical benefit of integrating MI into CBT when treating the highly prevalent and historically difficult-to-treat condition of GAD. In particular, it points to the theory-specific mechanism of MI (helping to reduce/resolve patients' in-treatment resistance) as accounting for the integrative treatment's additive effect on worry reduction across a follow-up period. Therapists using CBT to treat patients with GAD should be trained to incorporate MI principles (e.g., empathy, collaboration, autonomy support) in general and in response to explicit markers of resistance.

RESUMEN

BACKGROUND: Cognitive impairment, anxiety and depression are common in heart failure (HF) patients and its evolution is not fully understood. OBJECTIVES: To assess the cognitive status of HF patients over time, its relation to anxiety and depression, and its prognostic impact. METHODS: Prospective, longitudinal, single center study including patients enrolled in a structured program for follow-up after hospital admission for HF decompensation. Cognitive function, anxiety/depression state, HF-related quality of life (QoL) were assessed before discharge and during follow-up (between 6th and 12th month) using Montreal Cognitive Assessment (MoCA), Hospital Anxiety and Depression Scale (HADS) and Kansas City Cardiomyopathy Questionnaire, respectively. HF related outcomes were all cause readmissions, HF readmissions and the composite endpoint of all-cause readmissions or death. RESULTS: 43 patients included (67±11.3 years, 69% male); followed-up for 8.2±2.1 months. 25.6% had an abnormal MoCA score that remained stable during follow-up (22.6±4.2 vs. 22.2±5.5; p=NS). MoCA score <22 at discharge conferred a sixfold greater risk of HF readmission [HR=6.42 (1.26-32.61); p=0.025], also predicting all-cause readmissions [HR=4.00 (1.15-13.95); p=0.03] and death or all-cause readmissions [HR=4.63 (1.37-15.67); p=0.014]. Patients with higher MoCA score showed a greater ability to deal with their disease (p=0.038). At discharge, 14% and 18.6% had an abnormal HADS score for depression and anxiety, respectively, which remained stable during follow-up and was not related to MoCA. CONCLUSIONS: Cognitive function, anxiety and depressive status remain stable in HF patients despite optimized HF therapy. Cognitive status shall be routinely screened to adopt attitudes that improve management as it has an impact on HF-related QoL and prognosis.

RESUMEN

BACKGROUND: Anxiety is a state of worry caused by the anticipation of external or internal danger. Awareness During Anesthesia (ADA) is an unexpected memory recall during anesthesia. In this study, we aimed to determine the factors that affect preoperative anxiety and observe the incidence of ADA, as well as to determine the anxiety levels of these patients with a history of ADA. METHODS: This study was planned to be prospective, observational, and cross-sectional. Patients in whom septoplasty was planned, who was admitted to the anesthesiology outpatients between March 2018 and September 2018, were ASA I-II, and aged 18-70 years were included in the study. The demographic characteristics of patients were recorded. The State-Trait Anxiety Inventory (STAI) was used to determine anxiety during a preoperative evaluation. The modified Brice awareness score was used simultaneously to determine previous ADA. RESULTS: The anxiety scores of patients who were conscious during anesthesia were higher than other patients. The mean STAI score was 40.85±14.8 in the 799 patients who met the inclusion criteria of this study. When the anxiety scores were compared, the scores were higher in females than in males (p < 0.05). The mean STAI score was found as 40.3±13.8 in patients who dreamed during anesthesia. CONCLUSION: It is important to determine the anxiety levels of patients in the preoperative period to prevent the associated complications. Preoperative anxiety, besides preventing ADA, should be dealt with in a multidisciplinary manner. ADA should be carefully questioned while evaluating previous anesthesia experiences.

RESUMEN

OBJECTIVE: To find evidence of the symptoms of anxiety/depression in children with developmental coordination disorder as compared to their typically developing peers at both the group and individual level, and to identify how many different tools are used to measure anxiety and/or depression. METHODS: Electronic searches in eight databases (PubMed/MEDLINE, Scopus, Web of Science, ERIC, PsycINFO, Embase, SciELO and LILACS), using the following keywords: 'Developmental Coordination Disorder,' 'Behavioral Problems,' 'Child,' 'Anxiety,' 'Depression,' 'Mental Health,' and 'Mental Disorders.' The methodological quality was assessed by Newcastle-Ottawa Scale adapted for cross-sectional studies and the NOS for cohort studies. The studies were classified as low, moderate, or high quality. To provide clinical evidence, the effect size of the symptoms of anxiety and depression was calculated for each study. RESULTS: The initial database searches identified 581 studies, and after the eligibility criteria were applied, six studies were included in the review. All studies were classified as being of moderate to high quality, and the effect sizes for both anxiety and depression outcomes were medium. The evidence indicated that all of the assessed studies presented more symptoms of anxiety and depression in children with developmental coordination disorder than in their typically developing peers. On the individual level, this review found children with clinical symptoms of anxiety in 17-34% (developmental coordination disorder) and 0-23% (typically developing), and of depression in 9-15% (developmental coordination disorder) and 2-5% (typically developing) of the children. CONCLUSIONS: Children with developmental coordination disorder are at higher risk of developing symptoms of anxiety and depression than their typically developing peers.

RESUMEN

BACKGROUND AND OBJECTIVES: It is suggested that 38-45% of patients experience preoperative anxiety. We observe that patients undergoing living donor nephrectomy suffer from anxiety. Preoperative anxiety may complicate a patient's recovery from anesthesia and postoperative pain control. This study investigates the preoperative anxiety rate and its effect on anesthetic recovery and postoperative pain in patients undergoing donor nephrectomy. METHODS: Forty-eight individuals undergoing living-related renal donor nephrectomy were included in this analytic prospective observational cohort study. Their preoperative anxiety was measured with the STAI-I and STAI-II inventories. The relationships between anxiety scores with data regarding demographics, recovery from anesthesia, and postoperative pain scores were investigated. RESULTS: The findings were remarkable in that the anxiety scores of living renal donors were significantly correlated with their recovery variables, which are spontaneous respiration time, sufficient respiration time, extubation time, and PACU discharge time (p<0.01). Anxiety scores were significantly positively correlated with the pain scores of the 30th minute, 1st, 2nd, 4th, 8th, 12th, 24th hours, and the total amounts of analgesic administered in 24hours (p<0.05). A significantly negative correlation was also determined between anxiety scores and patients' satisfaction. CONCLUSION: Our study showed that patients undergoing living-related donor nephrectomy with high anxiety levels had late recovery times and high postoperative pain scores. Thus, determining those patients with high preoperative anxiety level is crucial to providing patients with satisfactory emerging from anesthesia and the control of their postoperative pain during donor nephrectomy.

BACKGROUND AND OBJECTIVES: Preoperative instruction is known to significantly reduce patient anxiety before surgery. The present study aimed to investigate the effects of preoperative education on the level of anxiety of cancer patients undergoing surgery using the self-reported Beck anxiety inventory. METHODS: This study is a short-term observational study, including 72 female patients with a diagnosis of endometrial cancer who were scheduled to undergo surgical treatment under general anesthesia. During the pre-anesthetic consultation 15 days before surgery, one group of patients (Group A, n=36) was given comprehensive information about their scheduled anesthetic and surgical procedures, while the other group of patients (Group B, n=36) did not receive any information pertaining to these variables. The Beck anxiety inventory, blood pressure and heart rate were evaluated before and after the preoperative education in Group A. In Group B, these parameters were evaluated at the beginning and at the end of the consultation. RESULTS: The hemodynamic values were lower in the group that received preoperative education, in comparison with the group that did not receive preoperative education. Educating the patients about the procedure resulted in a reduction in the levels of anxiety from mild to minimum, whereas there was no change in the group that did not receive the preoperative education. This latter group kept the same level of anxiety up to the end of pre-anesthetic consultation. CONCLUSIONS: Patient orientation in the preoperative setting should be the standard of care to minimize patient anxiety prior to surgery, especially for patients with cancer.

RESUMEN

ABSTRACT Objective This study aimed to assessment the risk factors affecting development of hypothyroidism and to examine the selected dietary micronutrient intakes of primary hypothyroidism, subclinical hypothyroidism and euthyroid individuals comparing the healthy control group, and to evaluate the relationship between dietary micronutrient intakes and serum thyroid hormones. Additionally, this study planned to observe anxiety severities in different hypothyroidism groups. Methods This case-control study was carried out with 120 individuals: 60 in the patient group and 60 in the control group. The patient group was further subdivided into primary hypothyroidism, subclinical hypothyroidism, and euthyroid groups. A questionnaire and the Beck Anxiety Inventory were administered to all participants. 24-hour food consumption records, anthropometric measurements, biochemical parameters were taken. Results When serum vitamin D level increased by one unit (ng/mL), risk of disease decreased by 1%; and when age (year) increased by one unit, risk of disease increased by 5.1%. Dietary micronutrient intakes were similar in all groups. There were negative correlations between serum T4 levels and selenium intake in primary hypothyroidism group, and serum TSH levels and copper intake in subclinical hypothyroidism group, and serum TSH levels and iodine intake in control group. Beck Anxiety Inventory scores of patient groups were higher than control group. There was no statistically difference between primary, subclinical and euthyroid hypothyroidism groups in terms of anxiety scores. Conclusion Serum vitamin D level and age affected the risk of hypothyroidism. Micronutrient intake was associated with thyroid parameters. Anxiety was higher in hypothyroid patients, independent of thyroid hormone levels.

OBJECTIVE: To verify the effect of psychological preparation on the relief of preoperative anxiety in children and to correlate parents' and children's levels of anxiety. METHOD: After the approval of the institutional Research Ethics Committee and written consent of the children's parents or guardians, 118 children of both genders were prospectively selected, aged between 2 and 8 years, physical condition classification ASA I, who were treated in the pre-anesthetic evaluation ambulatory of the University Hospital and who underwent ambulatory surgeries at the same hospital. Two controlled groups of 59 children were randomized: control group basic preparation and psychological preparation group. On the day of surgery, all selected children were evaluated regarding their level of anxiety using the modified Yale Preoperative Anxiety Scale and their parents were evaluated regarding their level of anxiety through the Visual Analog Scale. The evaluator was blinded to which study group the child and family member belonged to. RESULTS: Nine children and their family members were excluded per group when the results were analyzed. Children from the prepared group showed significant reductions in their level of anxiety in relation to the control group (p=0.04). There was no correlation between the level of anxiety of children and their parents' levels (p=0.78). CONCLUSION: The psychological preparation was effective in reducing the level of anxiety of children. However, there was no relation between the level of anxiety of children and their parents' level.

Abstract Objective The present study investigated attentional biases in anxiety using facial expressions and eye-tracking measures. Method Seventy-six participants, between 18 and 36 years, took part in the study and were divided based on trait anxiety levels: Low, Moderate, and High. The stimuli were facial expressions, and the measures of interest were Probability of First Fixation and Proportion of Fixation Time. Results The results revealed vigilance biases towards expressions of disgust, regardless of anxiety level, and maintenance biases towards expressions of happiness, regardless of stimulus presentation time, in the Low Trait Anxiety group compared to the High Trait Anxiety group. Conclusion These findings raise questions related to the importance of using positive stimuli in anxiety treatment and the need to specify the types, levels, and characteristics of stimuli used in research.

RESUMEN

ABSTRACT With the COVID-19 pandemic, elective orthopedic surgeries were interrupted in most healthcare services. This leads to impacts on the quality of life, as well as on the emotional, professional, and financial situation of patient athletes who had their surgical treatment postponed. Objective: To evaluate clinical, emotional, and professional impacts on athletes who had their knee surgery postponed. Methods: This study included 21 patients who were diagnosed with knee injuries and were on a surgical waiting list. Participants answered a questionnaire with socioeconomic questions, activity level (amateur/professional), diagnosis, proposed surgery, and questions about anxiety regarding the postponement and uncertainty of performing the surgery, worsening symptoms, and psychological status in general. Results: The most prevalent diagnosis was anterior cruciate ligament injury (81%). Moreover, 42.9% of patients reported being highly anxious about the date of surgery, with 23.8% being highly anxious about the uncertainty of surgery. There was a direct positive correlation (r = 0.418), indicating a higher level of anxiety in patients who faced greater financial impact. Conclusion: The indefinite postponement of surgeries had a great impact on anxiety levels and surgery uncertainty of patients awaiting surgery. Level of Evidence III, Transversal Study.

Abstract Background An implantable cardioverter-defibrillator (ICD) can cause high levels of anxiety and depression, resulting in negative effects on quality of life. Objectives To evaluate the quality of life, anxiety, and acceptance of the ICD using standardized measurement instruments and identify predictors of better responses for each of the outcomes studied. Method This is a prospective cohort study with patients undergoing initial ICD implantation or reoperation to maintain the device. The study outcomes included quality of life, anxiety, and acceptance of the ICD. The change in scores (30 and 180 days) was assessed using the minimal important difference (MID). Univariate analysis and the multivariate logistic regression model were used to identify predictors of better responses, adopting a significance level of 5%. Results A total of 147 patients were included between January/2020 to June/2021, with a mean age of 55.3 ± 13.4 years and a predominance of males (72.1%). The MID for quality of life, anxiety, and ICD acceptance were observed in 33 (22.4%), 36 (24.5%) and 43 (29.3%) patients, respectively. Age equal to or greater than 60 years (OR=2.5; 95%CI=1.14-5.53; p=0.022), absence of atrial fibrillation (OR=3.8; 95%CI=1.26-11.63; p=0.017) and female gender (OR=2.2; 95%CI=1.02-4.97; p=0.045) were independent predictors of better responses to quality of life, anxiety and acceptance of the ICD, respectively. Conclusion The identification of predictors for better quality of life scores, anxiety, and acceptance of the device can support the implementation of specific care for patients with a greater chance of presenting unfavorable results.

RESUMEN

Abstract This study evaluated the relationship between meta-worries and anxiety and depressive symptoms at the beginning of the COVID-19 pandemic in Brazil. In March 2020, 2,042 individuals, aged 18-78 years were recruited. A sociodemographic questionnaire, the 4-item Patient Health Questionnaire, and the Meta-Worry Questionnaire were administered online. Four logistic regression models were used to estimate the effects of the independent variables on anxiety and/or depressive symptoms with explained variances between 28% and 39%. Being younger, not having a steady income, perceiving oneself as sick, and having a high meta-concern level increased the chances of anxiety and/or depressive symptoms. Finally, we hope to contribute to the screening of factors associated with mental disorders in Brazil at the beginning of the pandemic.
