BACKGROUND: Little is known about the economic burden of healthcare-associated infections (HAIs) in Brazil. AIM: To analyse the costs of hospitalization by reimbursement from the Brazilian government, via the Brazilian Unified Health System (SUS) affiliation, and direct costs in the adult Intensive Care Unit (ICU). METHODS: The matched-pairs case-control study (83 patients with HAIs and 83 without HAIs) was performed at a referral tertiary-care teaching hospital in Brazil in January 2018. In order to calculate the HAI costs from the perspective of the payer, the total cost for each hospitalization was obtained through the Hospital's Billing Sector. Direct costs were calculated annually for 949 critical patients during 2018. FINDINGS: The reimbursement cost per hospitalization of patients with HAIs was 75% (US$2721) higher than patients without HAIs (US$1553). When a patient has an HAI, in addition to a longer length of stay (15 days), there was an extra increase (US$996) in the reimbursement cost per hospitalization. An HAI in the ICU was associated with a total direct cost eight times higher compared with patients who did not develop infections in this unit, US$11,776 × US$1329, respectively. The direct cost of hospitalization in the ICU without HAI was 56.5% less than the reimbursement (US$1329 × US$3052, respectively), whereas for the patient with an HAI, the direct cost was 111.5% above the reimbursement (US$11,776 × US$5569, respectively). CONCLUSION: HAIs contribute to a longer stay and an eight-fold increase in direct costs. It is necessary to reinforce programmes that prevent HAIs in Brazilian hospitals.

BACKGROUND: Pelvic floor muscles (PFM) and rectus abdominis muscles (RAM) of pregnant diabetic rats exhibit atrophy, co-localization of fast and slow fibers and an increased collagen type I/III ratio. However, the role of similar PFM or RAM hyperglycemic-related myopathy in women with gestational diabetes mellitus (GDM) remains poorly investigated. This study aims to assess the frequency of pelvic floor muscle disorders and pregnancy-specific urinary incontinence (PS-UI) 12 months after the Cesarean (C) section in women with GDM. Specifically, differences in PFM/RAM hyperglycemic myopathy will be evaluated. METHODS: The Diamater is an ongoing cohort study of four groups of 59 pregnant women each from the Perinatal Diabetes Research Centre (PDRC), Botucatu Medical School (FMB)-UNESP (São Paulo State University), Brazil. Diagnosis of GDM and PS-UI will be made at 24-26 weeks, with a follow-up at 34-38 weeks of gestation. Inclusion in the study will occur at the time of C-section, and patients will be followed at 24-48 h, 6 weeks and 6 and 12 months postpartum. Study groups will be classified as (1) GDM plus PS-UI; (2) GDM without PS-UI; (3) Non-GDM plus PS-UI; and (4) Non-GDM without PS-UI. We will analyze relationships between GDM, PS-UI and hyperglycemic myopathy at 12 months after C-section. The mediator variables to be evaluated include digital palpation, vaginal squeeze pressure, 3D pelvic floor ultrasound, and 3D RAM ultrasound. RAM samples obtained during C-section will be analyzed for ex-vivo contractility, morphological, molecular and OMICS profiles to further characterize the hyperglycemic myopathy. Additional variables to be evaluated include maternal age, socioeconomic status, educational level, ethnicity, body mass index, weight gain during pregnancy, quality of glycemic control and insulin therapy. DISCUSSION: To our knowledge, this will be the first study to provide data on the prevalence of PS-UI and RAM and PFM physical and biomolecular muscle profiles after C-section in mothers with GDM. The longitudinal design allows for the assessment of cause-effect relationships between GDM, PS-UI, and PFMs and RAMs myopathy. The findings may reveal previously undetermined consequences of GDM.

The exposure of bees to agrochemicals during foraging and feeding has been associated with their population decline. Sublethal exposure to agrochemicals can affect behavior and the microbiota. Gut microbiota is associated with insect nutritional health, immunocompetence, and is essential for neutralizing the damage caused by pathogens and xenobiotics. Research on the effect of the bioinsecticides and fertilizers on the microbiota of bees remains neglected. In this study, we assessed the sublethal effect of both bioinsecticide spinosad and the fertilizer copper sulfate (CuSO4) on the behavior and gut microbiota in forager adults of the stingless bee Partamona helleri (Friese), which is an important pollinator in the Neotropical region. Behavioral assays and gut microbiota profiles were assessed on bees orally exposed to estimated LC5 values for spinosad and CuSO4. The microbiota were characterized through 16S rRNA gene target sequencing. Acute and oral sublethal exposure to spinosad and CuSO4 did not affect the overall activity, flight take-off, and food consumption. However, CuSO4 decreased bee respiration rate and copper accumulated in exposed bees. Exposure to spinosad increased the proportional abundance of the genus Gilliamella, but CuSO4 did not alter the composition of the gut microbiota. In conclusion, sublethal exposure to CuSO4 induces changes in respiration, and spinosad changes the abundance of gut microorganisms of P. helleri.

Sublethal insecticide exposure may affect foraging of insects, including natural enemies, although the subject is usually neglected. The lady beetle Eriopis connexa (Germar, 1824) (Coleoptera: Coccinelidae) is an important predator of aphids with existing pyrethroid-resistant populations that are undergoing scrutiny for potential use in pest management systems characterized by frequent insecticide use. However, the potential effect of sublethal pyrethroid exposure on this predator's foraging activity has not yet been assessed and may compromise its use in biological control. Therefore, our objective was to assess the effect of sublethal lambda-cyhalothrin exposure on three components of the prey foraging activity (i.e., walking, and prey searching and handling), in both pyrethroid-susceptible and -resistant adults of E. connexa. Both lady beetle populations exhibited similar walking patterns without insecticide exposure in noncontaminated arenas, but in partially contaminated arenas walking differed between strains, such that the resistant insects exhibited greater walking activity. Behavioral avoidance expressed as repellence to lambda-cyhalothrin was not observed for either the susceptible or resistant populations of E. connexa, but the insecticide caused avoidance by means of inducing irritability in 40% of the individuals, irrespective of the strain. Insects remained in the insecticide-contaminated portion of the arena for extended periods resulting in greater exposure. Although lambda-cyhalothrin exposure did not affect prey searching by susceptible lady beetles, prey searching was extended for exposed resistant predators. In contrast, prey handling was not affected by population or by lambda-cyhalothrin exposure. Thus, sublethal exposure to the insecticide in conjunction with the insect resistance profile can affect prey foraging with pyrethroid-exposed resistant predators exhibiting longer prey searching time associated with higher walking activity reducing its predatory performance.

We show how two electrically coupled semiconductor lasers having optical feedback can present simultaneous antiphase correlated fast power fluctuations, and strong in-phase synchronized spikes of chaotic power drops. This quite counterintuitive phenomenon is demonstrated experimentally and confirmed by numerical solutions of a deterministic dynamical system of rate equations. The occurrence of negative and positive cross correlation between parts of a complex system according to time scales, as proved in our simple arrangement, is relevant for the understanding and characterization of collective properties in complex networks.

The leptin gene product is released into the blood stream, passes through the blood-brain barrier, and finds the leptin receptor (LEPR) in the central nervous system. This hormone regulates food intake, hematopoiesis, inflammation, immunity, differentiation, and cell proliferation. The LEPR Gln223Arg polymorphism has been reported to alter receptor function and expression, both of which have been related with prognostics in several tumor types. Furthermore, several studies have shown a relationship between the Gln223Arg polymorphism and tumor development, and its role in oral and oropharyngeal squamous cell carcinoma is now well understood. In this study, 315 DNA samples were used for LEPR Gln223Arg genotyping and 87 primary oral and oropharyngeal squamous cell carcinomas were used for immunohistochemical expression analysis, such that a relationship between these and tumor development and prognosis could be established. Homozygous LEPR Arg223 was found to be associated with a 2-fold reduction in oral and oropharyngeal cancer risk. In contrast, the presence of the Arg223 allele in tumors was associated with worse disease-free and disease-specific survival. Low LEPR expression was found to be an independent risk factor, increasing the risk for lymph node metastasis 4-fold. In conclusion, the Gln223Arg polymorphism and LEPR expression might be valuable markers for oral and oropharyngeal cancer, suggesting that LEPR might serve as a potential target for future therapies.

RESUMO Na Amazônia as plantas medicinais são um dos principais recursos para o tratamento de diversas doenças, dado o contexto cultural, a falta de proximidade com os locais onde poderiam receber atendimento médico, confiabilidade e baixo custo em comparação aos medicamentos industriais. Nesse contexto, encontra-se o Distrito de Marudá, no Município de Marapanim, a 160 Km da capital Belém, onde é comum o uso de plantas medicinais para o tratamento de agravos à saúde. Este trabalho investigou a sabedoria popular no uso de plantas medicinais pelos moradores do bairro do Sossego no Distrito de Marudá - PA, ilustrando a sua importância em termos culturais, econômicos e ambientais. Para isso utilizou-se um levantamento etnofarmacêutico visando oferecer ferramenta de prospecção destas plantas utilizadas como recurso terapêutico pela população local.
ABSTRACT In the Amazon medicinal plants are a relevant resource for the treatment of several diseases, given the cultural context, access, reliability and low cost compared to industrial medicine. Inside this context, it is located the Marudá District, in the municipality of Marapanim,160 km far from Belém state of Pará, where it is common to use medicinal plants in order to treat health problems. In this sense, this work investigates the popular phytotherapy practice of herbal medicine practiced by the residents of the“Sossego” neighborhood in the District of Marudá-PA, illustrating the importance of medicinal plants for this human group in cultural, economic and environment context. For such purpose, we used a ethnopharmaceutical survey aiming to offer a prospecting tool of these plants employed as a therapeutic resource for the local population.

We study a theoretical model describing a laser with a modulated parameter, concentrating on the appearance of extreme events, also called optical rogue pulses. It is shown that two conditions are required for the appearance of such events in this type of nonlinear system: the existence of generalized multi-stability and the collisions of chaotic attractors with unstable orbits in external crisis, expanding the attractor to visit new regions in phase space.

One obstacle to placing artificial posterior teeth in manufacturing complete dentures is a reduction of the space between the maxilla and the mandible. Occasionally, second molar placement is not performed, as it does not affect aesthetics, phonetics or comfort. The aim of this study was to compare the masticatory efficiency between patients wearing maxillary and mandibular complete dentures with reduced dental arches (without second molars) (WSM) and with full dental arches (FDA). Twenty subjects were divided into two groups and randomly received new complete dentures. Patients in Group 1 were given dentures WSM, and those in Group 2 were given dentures with FDA. After the post-placement visits, an initial masticatory efficiency test was performed with Optocal, an artificial test food. Fifteen days later, second molars were placed in Group 1 and removed from Group 2, and a new test was performed. Comminuted material was treated and sieved under vibration. The mean and standard deviation of masticatory efficiency with FDA were 10.4 and 8.1, respectively. In the tests WSM, the mean and standard deviation were 8.4 and 3.3, respectively. After removing the second molars in Group 2 and adding them in Group 1, the mean and standard deviation were 15.7 and 14.7 for Group 1 and 12.5 and 10.4 for Group 2, respectively. Within the limitations of this study, placing artificial teeth up to the first molars can be performed when needed without compromising masticatory efficiency.

O objetivo deste trabalho foi realizar a caracterização físico-química do pó e da tintura, e análise por espectrofotometria e cromatografia do extrato seco de Peperomia pellucida L. (H. B. K.). As metodologias seguiram a Farmacopéia Brasileira IV ed., com exceção da prospecção química, da espectrofotometria, da obtenção do perfil cromatográfico do extrato seco, e determinação do resíduo seco. A prospecção química revelou a presença de saponinas espumídicas; açúcares redutores; proteínas e aminoácidos; fenóis; taninos; flavonóides; esteróides e triterpenóides. Na análise por CCD, o melhor perfil da fração flavonoídica foi obtido com MeOH/CHOOH (90:10). Foi confirmada, através de CLAE, a presença de 3',4',7-tri-O-metoxiflavona no extrato seco deste material vegetal. Os resultados obtidos contribuem para a determinação de especificações de uma futura monografia em Farmacopéias da Peperomia pellucida L. (H.B.K.).
The aim of this study was the physical chemical characterization of the powder and the tincture, and the chromatographic and spectrophotometric analysis of the Peperomiapellucida L. (H. B. K.) dry extract. The methodology followed the Farmacopeia Brasileira IV ed., except for the chemical prospection, the chromatographic profile obtained and the spectrophotometry of the dry extract, and determination of dried residues. The chemical prospection revealed the presence of foaming saponins; reducing sugars; proteins and amino acids; phenols; tannins; flavonoids; steroids and triterpenoids; depsideos and depsidones. The best profile from TLC for flavonoidic fraction was obtained with methanol/formic acid (90:10 v/v). HPLC confirmed the presence of 3 ',4',7-tri-methoxyflavone in the dry extract of the plant material. The results obtained in this work should contribute for the determination of specifications for a future monograph on Peperomia pellucida L. (H.B.K.).

Na indústria farmacêutica de fitoterápicos, o extrato seco é considerado tecnologicamente viável para fins de produção em larga escala, devido à estabilidade física, química e microbiológica, além da facilidade de padronização dos princípios ativos. Entre as técnicas de secagem empregadas na preparação de extratos secos, encontra-se a nebulização ou spray-dryer, o leito de jorro, a liofilização e a evaporação rotativa. A escolha do processo de secagem é motivada pela potencialidade dos diferentes equipamentos secadores, na secagem de materiais presentes na forma líquida, no caso, soluções extrativas de plantas medicinais, fornecendo um produto de alta qualidade e com investimento relativamente baixo.
In the pharmaceutical industry of herbal medicines, the dried extract is considered technologically feasible for large-scale production, due to its physical stability, possibility of chemical and microbiological analyses and the ease of standardization of the active ingredients. Among the drying techniques used in the preparation of dry extracts are a mist or spray-dryer, the spouted bed, freeze-drying and rotary evaporation. The choice of the drying process is motivated by the potential of different drying equipments and materials present in liquid form, in this case, solutions of herbal extracts provide a high quality product and a relatively modest investment.

The effects of the egg density of Diaphania hyalinata (L.) on several biological parameters of Trichogramma exiguum Pinto & Platner and T. pretiosum Riley were investigated. For that, 24h-old egg masses were isolated in glass tubes (15 replicates; 1 egg mass = 1 replicate), and offered to parasitization by a newly-emerged female of T. pretiosum or T. exiguum 24h at the proportion of one female to 5, 10, 15, 20, 25, 30, 35 and 40 eggs of D. hyalinata. The following parameters were evaluated: number of parasitized eggs, number of individuals per egg, viability and sex ratio. Trichogramma exiguum parasitized more eggs than T. pretiosum when more than 25 eggs were available per female. The percentage of emergence was satisfactory to T. pretiosum in densities up to 15 eggs/female and up to 20 eggs/female for T. exiguum. The number of individuals per egg was not statistically different in both species except in the density of 25 eggs/female. It can be concluded that T. exiguum performed better than T. pretiosum at larger clutch sizes, as T. exiguum parasitization capacity increased as a result of the size of the host clutch size.

As plantas utilizadas na medicina tradicional estão sendo cada vez mais estudadas por serem possíveis fontes de substâncias com atividades antimicrobianas. Dentre elas, destacando-se a Peperomia pellucida (erva-de-jabuti) e a Portulaca pilosa (amor-crescido), utilizadas comumente na Amazônia. A P. pellucida é utilizada, popularmente, em casos de hemorragia, como curativo para feridas, dores abdominais, abscessos, acne, furúnculos, cólicas, problemas renais, hipertensão e colesterol, enquanto a P. pilosa é utilizada como hepato-protetor, antidiarreico, diurético, para queimaduras, erisipelas e ferimentos. Neste trabalho, foram realizadas a abordagem fitoquímica e a atividade antimicrobiana in vitro desses dois materiais vegetais. A prospecção fitoquímica revelou a presença de açúcares redutores, fenóis e taninos, esteroides e triterpenoides, glicosídios cardíacos e carotenoides no extrato etanólico seco (EES) de P. pillosa, e a presença de proteínas e aminoácidos, fenóis e taninos, flavonoides, esteroides e triterpenoides, azulenos, carotenoides, depsídios e depsidonas no EES de P. pellucida. Para avaliação da atividade antimicrobiana dos extratos etanólicos brutos, foi empregado o método de disco difusão em ágar, nas concentrações de 500; 250; 125 e 62,5 µg/mL. Os extratos testados que apresentaram atividade antimicrobiana na avaliação preliminar foram submetidos à determinação da concentração inibitória mínima (CIM) pela técnica de microdiluição em caldo. O extrato de P. pellucida possui atividade antimicrobiana frente a S. aureus e P. aeruginosa, e o de P. pilosa contra Pseudomonas aeruginosa.
Plants used in traditional medicine are under increasing scrutiny as possible sources of substances with antimicrobial activity. In this article we focus on Peperomia pellucida (erva-de-jabuti) and Portulaca pilosa (amor crescido), both commonly used in the Amazon. P. pellucida is popularly used as a wound dressing, to stop bleeding, and in cases of abdominal pain and cramps, abscesses, acne, boils, kidney problems, hypertension and raised cholesterol, and P. pilosa as a hepato-protective, antidiarrheal and diuretic and for burns, erysipelas and injuries. In this study, we investigated the phytochemistry and antimicrobial activity of extracts of these two plant materials. Phytochemical screening revealed the presence of reducing sugars, phenols and tannins, steroids and terpenoids, cardiac glycosides and carotenoids in the dried ethanol extract (DEE) of P. pilosa and of proteins and amino acids, phenols and tannins, flavonoids, steroids and triterpenoids, azulenes, carotenoids, depsides and depsidones in the P. pellucida DEE. The antimicrobial activity of the ethanol extracts was assayed by the agar disc diffusion method, at concentrations of 500, 250, 125 and 62.5µg/mL. The plant extracts that showed antimicrobial activity in the preliminary assessment were subjected to the broth microdilution test to determine the minimum inhibitory concentration (MIC). The P. pellucida extract had antimicrobial activity against S. aureus and P. aeruginosa and that of P. pilosa against P. aeruginosa.

O parasitoide Lixophaga sp. (Diptera: Tachinidae) ocorreu em lagartas de Diaphania nitidalis (Cramer, 1782) (Lepidoptera: Crambidae) em plantio comercial de abóbora no Estado do Espírito Santo, Brasil. Não há registro de Lixophaga sp. parasitando Diaphania sp. na América do Sul e mesmo os registros para hospedeiros deste gênero são poucos. A caracterização morfológica dos adultos foi feita no Departamento e Zoologia, Instituto de Biociências da Universidade de São Paulo, Brasil. Das lagartas coletadas em campo, 26,2% estavam parasitadas, obtendo-se uma média de 2,2 pupas por lagarta e viabilidade de 14,3%. Esse parasitoide mostrou ter potencial para o controle de D. nitidalis.
The parasitoid Lixophaga sp. (Diptera: Tachinidae) occurred in larvae of Diaphania nitidalis (Cramer, 1782) (Lepidoptera: Crambidae) in a commercial pumpkin plantation in the State of Espírito Santo, Brazil. This is the first report of Lixophaga sp. parasitizing Diaphania sp. in the South America, and there are few reports concerning hosts of this genus. The morphologic characterization of the adults was made at the Department of Zoology, Institute of Bioscience, of the University of São Paulo, Brazil. Of the larvae collected in the field, 26.2% were parasitized, with an average of 2.2 pupae per larva and a viability of 14.3%. This parasitoid presented potential for the control of D. nitidalis.

Espécies do gênero Aspidosperma apresentam alcalóides do tipo indólico. A partir da espécie Aspidosperma quebracho-blanco Schlecht isolou-se a quebracamina1, de A. olivaceum obteve-se a olivacina2, entre outras. O presente trabalho relata a determinação do perfil cromatográfico por Cromatografia em Camada Delgada e Cromatografia Líquida de Alta Eficiência de frações alcaloídicas de Aspidosperma auriculatum, espécie conhecida no Pará como carapanaúba e indicada popularmente para tratar febre e outras afecções, inclusive malária. O material vegetal foi extraído com ácido clorídrico a 5 por cento. Três frações alcaloídicas foram extraídas a pH 7, pH 8 e pH 11, que analisadas em sistemas adequados apresentaram perfis distintos.
Species from genus Aspidosperma show the presence of indol alkaloids. From Aspidosperma quebracho-blanco Schlecht quebracacidine was obtained1, from A. olivaceum. Schmutz and Hunziker2 isolated olivacine, among others substances. This work reports the determination of the chromatographic profile of alkaloid fractions from Aspidosperma auriculatum, by Thin Layer Chromatography and High Performance Liquid Chromatography. At the Brazilian state Pará this plant species has the vernacular name carapanaúba and is traditionally indicated to heal fever and others affections and also malaria. The plant material - stem bark - was extracted with hydrochloridric acid 5 percent. Three principal alkaloid fractions were extracted at pH 7, pH 8 and pH 11, which were analysed in suitable systems showing characteristic profiles.

No início de 1996, pesquisadores do Programa Pobreza e Meio Ambiente na Amazônia visitaram as aldeias Kayapó A'Ukre e Pykanu no sul do estado do Pará - Brasil. Essas expedições também objetivaram resgatar a tradição fitoterapêutica kayapó e identificar espécies vegetais com potencial farmacêutico. Os pajés apresentaram cinqüenta e três vegetais diferentes, dos quais vinte tiveram sua identificação botânica elucidada com base em fotografias.
In the beginning of 1996 researchers from the program Poverty and Environment in Amazon visited the Kayapó villages A'Ukre and Pykanu in the southern of Pará State - Brazil. These expeditions also aimed to rescue the kayapó phytotherapeutical tradition in the villages and to identify plant species with pharmaceutical potential. Six shamans were interviewed with translation three at each village. The shaman presented fiftythree different plants from, which twenty had elucidated their botanical identification on basis of photography.
RESUMEN
PURPOSE: To determine the effect of cusp coverage with amalgam restorations on the fracture strength of weakened human maxillary premolars. MATERIALS AND METHODS: 30 extracted human maxillary premolars were divided into three groups; Group A (control): uncut teeth, Group B: wide MOD cavities prepared and the pulp chamber's roof removed. The teeth were restored with amalgam without cusp coverage, Group C: same as Group B plus cusp reduction and restored with amalgam. The teeth were included in metal rings with self-curing polystyrene resin and stored in water for 24 hrs. The restorations were refined with rubber points, polished and further stored in water for 24 hrs before being subjected to a compressive axial load in a Universal testing machine at 0.5 mm/min. RESULTS: The mean fracture strength obtained was: Group A: 173.3 Kgf, Group B: 47.3 Kgf and Group C: 127.7 Kgf. All values were statistically significantly different (P < 0.05). The amalgam restoration of weakened human premolars with cusp coverage amalgam significantly increased the fracture strength of the teeth (63%) as compared to teeth restored without cusp coverage. The results showed that cusp coverage with amalgam might be an option for restoring weakened endodontically-treated teeth.

1. The net absorption and the biological efficacy of DL-methionine and of DL-methionine hydroxy analogue free acid (MHA-FA) were evaluated in chickens under heat stress. 2. In a growth assay, finishing broilers 21 to 42 d of age were fed on diets containing graded amounts of the two supplements; the basal diet was composed of practical ingredients. 3. From slope-ratio analysis, equimolar efficacy of MHA-FA relative to DL-methionine was determined to be 83% (confidence limits 61 to 115%) from weight gain responses, and 67% (47 to 91%) from food conversion responses. This indicates that the relative efficacy of MHA-FA is close to previous estimates of about 75% obtained under thermoneutral conditions. 4. In a balance study with caecectomised cockerels, net absorption (intake - excretion in faeces and urine) of DL-methionine and of MHA-FA, respectively, were determined to be 97.2 and 90.8%. The net absorption of MHA-FA was significantly lower than that of DL-methionine.

Analisaram-se os registros genealógicos de 1701 novilhas da raça Nelore, nascidas no período de 1967-84, em seis rebanhos localizados nos Estados de Säo Paulo, Mato Grosso e Mato Grosso do Sul. A média ajustada da idade ao primeiro parto foi de 1246,63 mais ou menos 8,53 dias. As análises estatísticas, desenvolvidas pelo método dos quadrados mínimos com números desiguais nas sub-classes, identificaram os efeitos negativos (P<0,01) de reprodutor, populaçäo, ano de nascimento e efeito linear da endogamia. Mês de nascimento näo afetou (P>0,05) a característica em foco. O coeficiente médio de endogamia foi de 4,45 mais ou menos 5,97. Pela regressäo linear, constatou-se que a cada 1 por cento de aumento no coeficiente de endogamia houve correspondente aumento de 3,22 mais ou menos 0,90 dias na idade do primeiro parto
