RESUMEN

To prevent scar tissue formation after lumbar surgery --for cure of disk herniation or laminectomy for narrowing of the spinal lumbar canal--the authors use an aponeurotic graft picked up by cleavage of lumbar aponeurosis and fixed between the laminae and remaining ligamentum flavum, closing posteriorly the lumbar canal and separating hermetically the dura from the anterior face of lumbar muscles. Presentation and discussion of results after four years' utilisation in 680 patients--and only 6 reinterventions which allowed an experimental study of the previously left in place graft.

RESUMEN

Sacro-iliac osteochondritis is difficult to recognize, it is rather frequently a case of diagnosis by elimination, which can be only confirmed by the course. Its prognosis is excellent. The predominance of lesions on the iliac edge reflects the affinity of this epiphysitis for the accessory iliac ossification point.

RESUMEN

Scintigraphic exploration of the sacroiliac (S.I.) joints by 99 m-technetium pyrophosphate is simple and free of all danger. The fixation of the isotope in the right sacroiliac (R.S.I.) and the "normal" limits of the fixation ratios R.S.I./L.R. and the lumbar rachis (L.R.), visible on the same film. A series of 28 controls having made it possible to calculate the "normal" limits of the fixation ratios R.S.I./L.R. and L.S.I./L.R., the isotopic fixation was measured in 25 patients with sacroiliac inflammation, 21 of whom were rheumatic, 3 infectious. It was shown that scintigraphy could yield useful information on the evolution of sacroiliac inflammation, making an early diagnosis possible, and also contributing to differentiation between rheumatic and infectious inflammation.
