RESUMEN

El priapismo es una emergencia urológica que debe ser diagnosticada y tratada apropiadamente. Específicamente en casos conocidos de leucemia el pene turgente doloroso debe hacer surgir la sospecha de priapismo de primera movida y el Doppler peneano debe ser la mera línea de modalidad de imágenes. Esta información debe ser transmitida al paciente y al departamento clínico emergentológico


Priapism is an urological emergency and must be diagnosed and treated appropriately. Specifically in known cases of leukemia painful turgid penis should raise suspicion of priapism in first hand and penile Doppler should be the first line of imaging modality. This information should be forwarded to patient and emergency department clinician

RESUMEN

This is a case report of a 16-year-old boy presenting with a history of oliguria and anasarca. Workup revealed marked proteinuria to the tune of 8 g/day. He was subsequently found to be hepatitis C positive. Genotype was hepatitis C virus type 6. Renal biopsy revealed mesangioproliferative glomerulonephritis with IgA deposits consistent with IgA nephropathy (IgAN). Initiation of oral ribavirin and pegylated interferon caused a marked reduction in proteinuria after 8 weeks of treatment. It was postulated that hepatitis C manifested as IgAN. This case report thus sheds light on the possibility of an association between hepatitis C and IgAN.

RESUMEN

Agricultural spraying involves two major tasks: guiding a sprayer in response to a GPS navigation device, and simultaneous monitoring of rear-attached booms under various illumination and terrain difficulty levels. The aim of the present study was to investigate the effect of illumination, task difficulty, and task level on the mental workload of an individual operating an agricultural sprayer in response to a commercial GPS lightbar, and to explore the sensitivity of the NASA-TLX and SSWAT subjective rating scales in discriminating the subjective experienced workload under various task, illumination, and difficulty levels. Mental workload was measured using performance measures (lateral root mean square error and reaction time), physiological measures (0.1 Hz power of HRV, latency of the P300 component of event-related potential, and eye-glance behavior), and two subjective rating scales (NASA-TLX and SSWAT). Sixteen male university students participated in this experiment, and a fixed-base high-fidelity agricultural tractor simulator was used to create a simulated spraying task. All performance measures, the P300 latency, and subjective rating scales showed a common trend that mental workload increased with the change in illumination from day to night, with task difficulty from low to high, and with task type from single to dual. The 0.1 Hz power of HRV contradicted the performance measures. Eye-glance data showed that under night illumination, participants spent more time looking at the lightbar for guidance information. A similar trend was observed with the change in task type from single to dual. Both subjective rating scales showed a common trend of increasing mental workload with the change in illumination, difficulty, and task levels. However, the SSWAT scale was more sensitive than the NASA-TLX scale. With the change in illumination, difficulty, and task levels, participants spent more mental resources to meet the increased task demand; hence, the illumination, task difficulty, and task level affected the mental workload of an agricultural sprayer operator operating a sprayer in response to a GPS lightbar.

The objective of this study was to perform a complete task analysis to measure the workload associated with operating an agricultural sprayer equipped with a navigation device. The task analysis included a written questionnaire and subsequent observation of sprayer operators. The questionnaire revealed that the lightbar was the most important source of guidance information (as opposed to relying on guidance information from external field cues). Observation consisted of recording eye-glance behaviour and heart rate variability while operators were spraying in a field setting. The eye-glance data suggest that external cues are more important than the lightbar for providing the necessary navigation information. Thus, the questionnaire and observation data contradict each other. Based on heart rate variability, operators who used a lightbar navigation device experienced more mental workload than operators who used an auto-steer navigation device.

RESUMEN

Sixty final year MBBS students were trained in the art of neonatal resuscitation based on the 'NALS' course material. The students scored 11.6 +/- 3.02 in the pretest and 17.85 +/- 4.42 (out of 20) in the post-test. The trainees felt that the programme was extremely useful. Majority of them said that the course content was optimum with appropriate use of teaching-learning media. It is recommended that this training may be imparted to all the undergraduate students at the entry of final year MBBS course before their labor room posting so that they can have effective reinforcement of the training.

RESUMEN

A study was conducted at Jawaharlal Institute of Postgraduate Medical Education and Research, Pondicherry, India, on the pattern of antimicrobial utilization in hospitalized patients. A retrospective study was conducted during a one-month period between January 18 to February 17, 1987. The data were analyzed according to modified Kunin's criteria. The results were compared with a similar study conducted a decade ago. The study revealed a marked increase in the use of antimicrobial agents (from 19.9% to 56.1%). A more appropriate use of antimicrobial agents was observed (from 66.8% to 79.5%). A significant increase in their appropriate use for therapeutic purpose (50% to 88.2%), but not for prophylaxis (79.05% to 71.18%) was seen. The study revealed a marked increase in the use of antimicrobial agents during the last decade resulting in the emergence of nosocomial infections of resistant strains of organisms.

RESUMEN

An extractive spectrophotometric procedure has been developed for the determination of palladium (II) at microgram levels. The palladium(II) chelate of 7-iodo-8-hydroxyquinoline-5-sulphonic acid is extracted into n-butanol. Extraction is maximal (95%) from 0.2M perchloric acid. Beer's law is valid at 430 nm over a wide range of palladium concentration from 2.5 ppm. The molar absorptivity is 958 1.mole(-1).mm(-1). The system can tolerate a large excess of Co(II), Ni(II), Rh(III), Pt(IV), Cr(III), W(VI), chloride, phosphate, citrate and tartrate. Small quantities of Ru(III), IR(III) and EDTA do not interfere, but serious interference is caused by Fe(III), V(V), Mo(VI) and Os(VIII).
