RESUMEN

The purpose of study is to establish features of autoimmune reaction of children with Crohn's disease. The sampling included 62 patients aged from 2 to 17 years with diagnosis of Crohn's disease. The evaluation was carried out concerning concentration in blood serum of immunoglobulins IgA, IgM, IgG, IgЕ, antibodies to Saccharomyces cerevisiae (ASCA) classes IgA, IgG и IgЕ, antibodies to Candida albicans classes IgA, IgM, IgG и IgЕ, anti-neutrophilic cytoplasmic antibodies (ANCA) to myeloperoxidase (MPO), to proteinase 3 (PR3), anti-nuclear antibodies (ANA), antibodies to DNAds, DNAss (to double-helical and single-stranded DNA), antibodies to antigens of small and large intestines, pancreas, circulating immune complexes. The hyperimmunoglobulinemia was diagnosed in 47 (75.8%) out of 62 patients with Crohn's disease. The increased level of IgM in blood was detected in 29 patients (46.8%). The hyperimmunoglobulinemia У was established in 19 (30.6%) out of 62 children. The hypoimmunoglobulinemia was detected in 22 (35.5%) of patients and in 17 (77.3%) out of them the disimmunoglobulinemia type IV (isolated decreasing of concentration of IgA). The evaluation of rate of occurrence of specifc antibodies in blood serum demonstrated that in patients most frequently was detected presence of specifc IgE to Saccharomyces cerevisiae (70.9%). The increased level of ASCA (IgA, IgG) was detected in 22 (35.5%) patients. The concentration of antibodies to DNAds, DNAss in blood exceeded standard value in 4.8% and 16.1% patients correspondingly. The increased level of circulating immune complex was established in 20 (32.3%) patients. The concentration of ANA corresponded to standard values in all 62 (100%) patients. The evaluation of results of correlation analysis established a strong positive correlation of concentration in blood of antibodies to antigens of small and large intestines; average positive correlation of level of antibodies to antigens of small intestine and IgM, ANCA PR3, ASCA IgE, antibodies to Candida albicans classes IgM, IgG, IgE, antibodies to antigens of pancreas; average degree of positive correlation between concentration of antibodies to antigens of large intestine and IgA, IgM, circulating immune complex, ANCA PR3, DNAss, ASCA IgE, antibodies to antigens of pancreas; strong positive correlation between concentrations of IgA to Candida albicans and ANA. The detection of auto-to antibodies Saccharomyces cerevisiae, Candida albicans, ANCA, antigens of small and large intestines, pancreas and expressed degree of correlation of many indices of autoimmune reaction indicate to intensity of immune pathological process under Crohn's disease. Under Crohn's disease, the formation of antibodies to ASCA is a prognostically unfavorable sign. The immune diagnostic under Crohn's disease is necessary for evaluating severity of course of disease, differential diagnostic, establishment of prognosis and selection of individual immune correcting therapy.

The study was carried out to compare results of three tests on the basis of antigens CFP-10 and ESAT-6: laboratory tests QuantiFERON (QFT) and Tubinferon (TIF) and intra-cutaneous test with allergen tuberculous recombinant - Diaskintest (DST). Tubinferon is a Russian analog of test QuantiFERON and is based on quantitative analysis of interferon-gamma after incubation of samples of whole blood in the presence of antigens of Mycobacterium tuberculosis (MBT) - tuberculin and recombinant antigens ESAT-6 and САЗ-10. The results QFT, TIF and DST are examined on 63 patients aged from 7 months to 15 years at parallel setup. According complex examination 63 children were divided at three groups: group I - 29 children infected with MBT and without disease; group II - 8 children non-infected with MBT; group III - 26 children with active forms of tuberculosis. It is established that at high rate of coincidence of results of all three tests (68.3%) in particular situations they can react differently, providing thereby additional information. The results of tests QFT and TIF coincide in 74.6% of all cases. The tests in vitro (QFT, TFT) occurred more sensitive under immuno-pathologic conditions in comparison with DST test. The TFT test demonstrated higher sensitivity under infection with MBT (58.6%) as compared with DST (34.5%) and QFT (34.5%) but lesser than in children with tuberculosis: TFT - 61.5%, QFT - 92.3%, DST - 80.8%. The conclusion is made that all three tests provide positive result more often under tuberculosis infection than in case of infected with MBT. The Tubinferon test is recommended to application and especially for children infected with MBT prior development of disease.

The authors studied the properties of myelopeptide (MP) which is a stimulant of antibody production of bone marrow origin. It is shown that MP possesses marked chemotactic activity in relation to polymorphonuclear leukocytes. MP may apparently be a regulator of cell activation by anaphylatoxins. MP intensifies lysosomal secretion by blood leukocytes. On acting on mononuclears, MP induces fibrinolytic activity which is not characteristic of this cell population under physiological conditions. A maximal effect is achieved with MP concentration of 10 micrograms/ml. The possibility of clinical application of MP may therefore be expanded because the agent has an effect not only on processes of antibody production, but possesses a wider action, being, evidently, a mediator providing for immune intensification of cell effector mechanisms of inflammation realization, which is important in the management of inflammation in immunodeficiency.

RESUMEN

The levels of the complement C3 component have been measured in children with various diseases (immunodeficiency states, food allergy, rheumatoid arthritis, severe and medium-severe forms of acute respiratory viral infections), and the complement consumption in the course of alternative activation has been examined. The findings evidence a relationship between the complement C3 component consumption and the blood serum IgA level, as well as between the C3 level and the neutrophilic phagocytic and metabolic activities.

RESUMEN

Clinical analysis of gonorrhea cases in children has shown that girls aged under 10 make up 88% of the patients. In 81% of cases the patients have contracted the infection from their mothers. New cases predominated, only 3% of patients have developed a chronic condition.
