RESUMEN

Gaucher's disease is a disturbance of lipid storage that results in the accumulation of histiocytes filled with glucosyl ceramide in various organs and bones. Clinical features include history of epistaxis, hemoptysis, and spontaneous gingival hemorrhage. This article reviews Gaucher's disease with reference to its importance to dental practitioners and a case involving mandibular trauma is presented.

RESUMEN

A pathologic condition is described, characterized by rampant necrosis of gingival mucosa, periodontium, and related osseous structures associated with systemic infection with the human immunodeficiency virus (HIV). It is believed that this condition is an extension beyond the normal clinical course of HIV-periodontitis (HIV-P) and manifests itself in three progressive stages: (1) HIV-associated gingivitis, (2) HIV-P, and (3) an extension of HIV-P to osseous necrosis. Two cases of osseous destruction attending HIV-P are reported, one of which led to initial diagnosis of HIV infection. They represent the final stage of disease progression with localized necrosis of gingiva, periodontium, and alveolar bone.

RESUMEN

Hydroxyapatite is a biocompatible material for augmentation of edentulous ridges to improve patient comfort and denture stability. The problems associated with hydroxyapatite are generally related to the amount of material introduced and the site of implantation. This article describes the use of a prefabricated splint that permits localization of the implant material to selected sites of the mandible and restricts the amount of material used. Fabrication of this splint under the guidance of the prosthodontist and oral surgeon permits the most effective use of this technique.

RESUMEN

The supra-eruption of the edentulous posterior maxilla may decrease interarch space, thus compromising the construction of an adequate fixed or removable prosthetic appliance (Henderson et al., 1985). The following case report illustrates a treatment which provides additional interarch space facilitating the restoration of the patient's dentition with a fixed prosthetic appliance. This arch space deficiency was corrected by performance of a unilateral posterior maxillary osteotomy with superior repositioning of this segment. Simultaneously, a hydroxylapatite-coated titanium endosteal dental implant was placed. Pre-operative considerations, the surgical procedure, and post-operative management of this problem are discussed.

RESUMEN

The effects of aspirin and acetaminophen on postextraction hemostasis were evaluated in a double-blind study in a group of 43 patients; 20 received aspirin and 23 received acetaminophen. No significant difference was found in the observed postsurgical alveolus bleeding times or the patients' reported length of postoperative bleeding. However, Ivy bleeding times taken after the ingestion of 80 grains of medication showed an elevation in 75% of the aspirin users and in 60% of the acetaminophen group; the mean change in bleeding times was 2.75 times greater in the aspirin group.

RESUMEN

Two case reports of a modified sagittal split osteotomy of the ramus of the mandible are presented which illustrate a technically simplified modality of treating various dentofacial abnormalities. A brief review of the literature is presented and observations are made on the procedures. The need for future documentation of intraoperative and postoperative results on this type of procedure is recommended.

RESUMEN

A patient with a previously diagnosed case of multiple myeloma was readmitted for a pathologic fracture of the hip. Intraoral examination revealed a soft-tissue swelling over the right posterior mandibular ridge. Radiographs showed multiple radiolucent areas of the maxilla and mandible. A needle-biopsy technique was used to confirm the diagnosis of intraoral manifestations of plasma-cell myeloma.

RESUMEN

A patient with an abnormal maxillary lateral incisor was treated to restore function and to improve appearance. Although specific classification of the problem into accepted terminology was attempted, the term double tooth was used to define the patient's problem. We have shown that the identification of the specific abnormalities, not the etiologic classification, is important to achieve an effective treatment plan.
