RESUMEN

The incidence of abdominal tuberculosis has increased over the last decade and can mimic any other disease. Pancreatic involvement is an uncommon cause of jaundice in children. A case of obstructive jaundice due to pancreatic tuberculosis in an infant, with hepatic involvement is reported. The sonographic and computed-tomographic features are described.

RESUMEN

We report a case of Lemierre syndrome with extensive thrombophlebitis of the internal jugular and subclavian veins with multiple septic emboli to the lungs and pericardium. This report illustrates a case of this forgotten disease in a child and the unusual presence of pericardial involvement.

RESUMEN

Chondrosarcoma is a rare cartilaginous tumor, representing less than 0,5% of intracranial tumors. This tumor occurs primarily in elderly patients and involves long and pelvic bones. The authors report a case of intracerebral chondrosarcoma in a 12 year old child, probably fronto-ethmoidal in origin. CT and MR imaging showed a lytic partly calcified soft tissue mass.

RESUMEN

Osteochondroma or exostosis is the most common benign tumor of bone, but vertebral involvement is rare. The authors report the case of a 16 years old male with a family history of hereditary multiple exostoses who presented with spinal cord compression. MR examination showed an intraspinal extradural bone lesion at the T1-T2 level, hyperintense on T1 weighted and hypointense on T2 weighted images, causing marked cord deformity. The CT scan showed a tumor of the body and left pedicle of T2 with severe narrowing of the spinal canal.

RESUMEN

Tuberculosis of the spine usually involves the vertebral body and intervertebral disk. Involvement of the posterior arch is rare. We report a case of tuberculosis involving the posterior elements of the T4 and T5 vertebrae in a 38 year old woman. CT is helpful to assess bony structures whereas MRI is ideal to evaluate the neural structures. Clinical, radiographic, and therapeutic considerations regarding tuberculosis of the spine are reviewed.

Natural autoantibodies (NaAb) and IgG antibodies to tetanus toxoid (TT) were analysed in the sera of 38 children with active visceral leishmaniasis (VL) previously vaccinated with TT and in 30 healthy controls matched for sex and age. Patients exhibited high levels of NaAb to a panel of self antigens (tubulin, myosin, myoglobin, actin) contrasting to a low level of IgG to TT. Analysis of the circulating B cells in 26 untreated patients showed a low percentage of CD5+ per total B cells (3-66%, mean 36.6%) compared with 14 normal controls (17.8-66.6%, mean 52.7%) (P < 0.001). Evaluation of these parameters after antimonial therapy showed a significant decrease of the level of the NaAb (P < 0.0005), and a spontaneous increase of the level of the IgG to TT without any vaccine boosting (P < 0.01). In contrast, there was a significant increase in CD5+ B cells (P < 0.0005). This result suggests that CD5+ B cells may be sequestrated in parasitized lymphoid organs and may be released after remission. These findings show that the polyclonal B cell activation that occurs during active VL involves mainly B cells bearing NaAb and are in favour of a functional dichotomy of B cells.
