BACKGROUND: Central nervous system fungal infections (CNSFI) are seen in patients with hematologic malignancies and have high morbidity and mortality. Because of their rarity, there is limited data on CNSFI in children with no established treatment protocols or guidelines. MATERIALS AND METHODS: In this multicenter retrospective study, 51 pediatric patients with leukemia, 6 of whom had undergone bone marrow transplantation, with proven or probable CNSFI were evaluated. Fungal infections were defined as proven or probable based on European Organisation for Research and Treatment of Cancer criteria. Proven CNSFI was diagnosed by appropriate central nervous system (CNS) imaging or tissue sample findings in combination with positive microbiological results of cerebrospinal fluid. A positive culture, microscopic evidence of hyphae, a positive result of the galactomannan assays are defined as positive microbiological evidence. Probable CNSFI was defined as appropriate CNS imaging findings together with proven or probable invasive fungal infections at another focus without CNS when there is no other explanatory condition. Data was collected by using the questionnaire form (Supplemental Digital Content 1, http://links.lww.com/JPHO/A541 ). RESULTS: Seventeen patients had proven, 34 patients had probable CNSFI. Headaches and seizures were the most common clinical findings. The median time between the onset of fever and diagnosis was 5 days. The most common fungal agent identified was Aspergillus . Sixteen patients received single-agent, 35 received combination antifungal therapy. Surgery was performed in 23 patients. Twenty-two patients (43%) died, 29 of the CNSFI episodes recovered with a 20% neurological sequelae. CONCLUSION: CNSFIs should be considered in the differential diagnosis in patients with leukemia and refractory/recurrent fever, headache, neurologicalocular symptoms, and a radiologic-serological evaluation should be performed immediately. Early diagnosis and prompt management, both medical and surgical, are essential for improving clinical outcomes.

AIM: This study evaluated the effect of different irrigation protocols on elastic modulus and biomechanics of single-rooted premolar tooth using with nano-indentation and finite element analysis (FEA). MATERIALS AND METHODS: Root canals of single-rooted human teeth were prepared, divided into eight groups, and irrigated with (1) 2.5% NaOCl + 17% EDTA; (2) 2.5% NaOCl + 17% EDTA + 2.5% NaOCl; (3) 2.5% NaOCl + SmearClear; (4) 2.5% NaOCl + 2% chlorhexidine; (5) 1.3% NaOCl + MTAD; (6) 5.25% NaOCl; (7) 17% EDTA; and (8) saline. The roots were vertically sectioned, and elastic modulus of the root dentine was measured using nano-indenter device at coronal, middle, and apical third. Data were recorded as megapascal and statistically analyzed (one-way analysis of variance, Tukey tests). Three-dimensional FEA model of a premolar tooth was created, and the inner root dentine was modified to simulate the effect of irrigation protocols on root dentine. The elastic properties of inner root dentine layer in the FEA models were modified for each group according to the data obtained with nano-indentation. A 300-N load was applied at the buccal cusp and central fossa of the models with a 45° angle. The stresses were calculated using von Mises stress criteria. RESULTS: All irrigation protocols affected the elastic modulus of root dentine. Groups 2 and 3 showed similar elastic modulus values (P > 0.05), whereas the lowest values were obtained in group 7 (P < 0.05). No statistically significant differences were found between groups 4, 5, and 8 (P > 0.05). CONCLUSION: Despite the effect of different clinically used irrigation protocols on elastic modulus of the inner dentine, this does not affect the biomechanics of the roots.

We demonstrate a 4-year-old girl who presented with progressive, asymmetrical, firm abdominal distention and was diagnosed with synchronous Wilms' tumor and left para-aortic ganglioneuroma (GN). Although synchronous tumors in the pediatric population are commonly associated with malignancy-predisposing syndromes, the patient in question was found to be otherwise healthy and had no clinical evidence nor family history of a syndrome. This case is the second one in the literature diagnosed with synchronous presentation of Wilms' tumor and GN in a previously healthy child. In addition, a GN foci presumed to be a previous metastasis of a neurogenic tumor that subsequently matured to GN was depicted within a left para-aortic lymph node. We aimed to emphasize an extremely rare synchronous occurrence of these embryonal tumors, increase the awareness of physicians, and discuss the radiologic differential diagnosis and management.

BACKGROUND: Epidemiologic research has shown that perimenopausal contraception is an important medical issue, because women during the perimenopause still need effective contraception. The objective of the study was to assess the contraceptive choices of perimenopausal Turkish women. METHODS: This is a descriptive cross-sectional study that in a non - random fashion recruited 202 perimenopausal and naturally menopausal women who lived in a suburban area of Istanbul. Women who took part were aged between 45-59 years old. Chief database used to identify the suitable participants in the district. Subjects who voluntarily participated in the study were interviewed in their homes by the researcher. The analysis of the data was evaluated using percentages. RESULTS: The percentage of sexually active women among the participants was 87.6%. A large majority - 80.2% - of the participants did not have any idea of when they should bring contraception to an end. The method most commonly used was withdrawal (Coitus Interruptus), represented by 38.8%. In regard to the participants' choices of medical contraception, those being utilized were the IUD (24.3%), tubal sterilization (8.9%), condom (5.9%) and COC (6.4%). Additionally, 18% of women used other traditional methods including vaginal lavage, vaginal aspirin, and even inserting a small sponge presoaked with fresh lemon juice or cola deep into the vagina. Among the perimenopausal women who participated, the IUD was the most popular and appropriate contraceptive method. CONCLUSION: Most perimenopausal Turkish women are still using traditional methods and women's knowledge about contraception in the menopausal stages is very limited. Health professionals should provide information about perimenopausal contraception.

In this study, an analytical method for indirect measurement of Al bound with tannin in tea infusion was studied. This method utilizes the ability of the tannins to precipitate with protein. Separation conditions were investigated using model solutions. This method is uncomplicated, inexpensive and suitable for real samples. About 34% of the total Al in brew extracted from commercially available teas was bound to condensed and hydrolyzable tannins.
