RESUMEN

OBJECTIVE: We aimed to investigate the potential environmental risk factors, protective factors, and biomarkers of hearing loss (HL), and establish a hierarchy of evidence. DATA SOURCES: Embase, PubMed, Cochrane Library, and Web of Science electronic database from inception to June 1, 2023. REVIEW METHODS: We included meta-analyses of observational studies of associations between HL and environmental risk factors, protective factors, or biomarkers. We calculated summary effect estimates, 95% confidence interval, heterogeneity I2 statistic, 95% prediction interval, small study effects, and excess significance biases. RESULTS: Of the 9211 articles retrieved, 60 eligible articles were included. The 60 eligible articles identified 47 potential environmental risk and protective factors (N = 4,123,803) and 46 potential biomarkers (N = 173,701). Evidence of association was convincing (class I) for rheumatoid arthritis (RA) and every 1 cm increase in height. Evidence of association was highly suggestive (class II) for human immunodeficiency virus (HIV), diabetes, cumulative noise exposure (CNE), smoking, congenital cytomegalovirus (CMV) infection, combined exposure to organic solvents and noise, non-Gaussian noise exposure, each 1 kg increase in birth weight, noise exposure, and alopecia areata (AA). CONCLUSION: In this umbrella review, RA, every 1 cm increase in height, HIV, diabetes, CNE, smoking, congenital CMV infection, combined exposure to organic solvents and noise, non-Gaussian noise exposure, each 1 kg increase in birth weight, noise exposure, and AA were strongly associated with HL.

Many potential environmental risk factors, protective factors, and biomarkers of AR have been published, but so far, the strength and consistency of their evidence are unclear. We conducted a comprehensive review of environmental risk, protective factors, and biomarkers for AR to establish the evidence hierarchy. We systematically searched Embase, PubMed, Cochrane Library, and Web of Science electronic database from inception to December 31, 2022. We calculated summary effect estimate (odds ratio (OR), relative risk (RR), hazard ratio (HR), and standardized mean difference (SMD)), 95% confidence interval, random effects p value, I2 statistic, 95% prediction interval, small study effects, and excess significance biases, and stratification of the level of evidence. Methodological quality was assessed by AMSTAR 2 (A Measurement Tool to Assess Systematic Reviews 2). We retrieved 4478 articles, of which 43 met the inclusion criteria. The 43 eligible articles identified 31 potential environmental risk factors (10,806,206 total population, two study not reported), 11 potential environmental protective factors (823,883 total population), and 34 potential biomarkers (158,716 total population) for meta-analyses. The credibility of evidence was convincing (class I) for tic disorders (OR = 2.89, 95% CI 2.11-3.95); and highly suggestive (class II) for early-life antibiotic use (OR = 3.73, 95% CI 3.06-4.55), exposure to indoor dampness (OR = 1.49, 95% CI 1.27-1.75), acetaminophen exposure (OR = 1.54, 95% CI 1.41-1.69), childhood acid suppressant use (OR = 1.40, 95% CI 1.23-1.59), exposure to indoor mold (OR = 1.66, 95% CI 1.26-2.18), coronavirus disease 2019 (OR = 0.11, 95% CI 0.06-0.22), and prolonged breastfeeding (OR = 0.72, 95% CI 0.65-0.79). OBJECTIVE: Observing the expression changes of serum proteome in model rats after intervention of the Granules of Eliminating Phlegm and Removing Blood Stasis ((see text) also known as GEPRB), screening outand identifying the differentially expressed proteins by mass spectrometry and bioinformatics analysis, discussing the molecular mechanism of control the Diabetes deafness by GEPRB. METHODS: By use of proteomics technology, the serum protein serum proteome of the control group, model control group, Duxil and each observation group were observed for 2-DE gel pattern matching, and the difference in the relative content of 2 times was chosen for the differentially expressed proteins. Identification of differentially expressed proteins by MALDI-TOF MS/MS, the authors further analysis the phosphorylation, subcellular localization, interaction, direct regulation, and transmembrane of the differences proteins by the way of bioinformatics analysis. Sixty SPF level SD rats elected in diabetic rats model group (abbreviated as DM group) were be randomly divided into 5 groups based on random number sheet, namely model control group, positive drug control group (Du-ke-xi group) and Mai-tong-fang high, medium and low dose group respectively. In addition, set of normal control group. 10 rats in each group. RESULTS: By Coomassie brilliant blue staining, identified 51 differential protein spots dug from 2-D gel by mass spectrometry, successfully identified 13 non-redundant proteins. Most of the identified proteins were secreted protein and belong to different protein families. There were about 12 proteins have the transmembrane region from the authors' result, ten of them were plasma membrane proteins. CONCLUSION: It's suggesting that 13 differential proteins is most likely the protein response to GEPRB in vivo, these proteins may play key role for the treatment of GEPRB to Diabetes deafness. The two highly differentially expressed proteins Apolipoprotein E (apoE) and C3 may be a potential drug target of GEPRB.

OBJECTIVE: To study the effectiveness of the granules of eliminating phlegm and removing blood stasis (GEPRB) on glucose metabolism for diabetic rats induced by streptozotocin and that on early diabetic rats cochlear histopathology. METHOD: Intraperitoneally inject streptozotocin disposable of dose of 55 mg x kg(-1), so that a model of diabetic rats is created, and treated by, and simultaneously treated by Duxil as matched groups for positive, then observe the blood glucose of each group, the histopathology of cochlear, and any circumstantial change of their microstructures. RESULT: GEPRB can reduce the cumulating thickness of the basilar membrane and that of the vascular pattern, so as to tighten up the sparsity of the spiral ganglion cells. CONCLUSION: GEPRB is rather effective on hypoglycemic for the diabetic rat model, and therapeutic goal can be achieved by improving the pathological changes of the cochlea damaged. The improvement by GEPRB for microangiopathic change of the cochlea capillary is probably the pathological basis of relieving hearing loss.

OBJECTIVE: To study the effect of Gushen Peiyuan Recipe (GPR) on the expression of Bcl-2 and Bax mRNA in Shen-yang-deficiency (SYD) rats suffering from cochlea apoptosis, thus providing a theoretical basis to the treatment and prevention of sensorineural hearing loss and fill Chinese medine's theory of kidney-ear-correlation with new substance. METHODS: Rats were induced into experimental SYD animal models by injecting cetacort into their buttocks. Rats in the blank and model groups were given 10 mL/kg normal saline by gastrogavage, and 31 g/kg, 15.5 g/kg and 7.5 g/kg GPR were given to the rats in the high, medium and low dose groups by gastrogavage respectively. RT-PCR was adopted to detect the mRNA expressions of Bcl-2 and Bax. RESULTS: Levels of Bcl-2 mRNA expression and Bcl-2/Bax enhanced, and mRNA expression of Bax attenuated in the model rats after GPR treatment, the Bcl-2/Bax ratio increased, showing insignificant difference when compared with the blank control group (P > 0.05). CONCLUSION: GPR plays a significant role in regulating the mRNA expressions of Bcl-2 and Bax, therefore improving the hearing of SYD rats and protecting the structure and function of cochlea.
