RESUMEN

BACKGROUND: Understanding the experiences of general practice (GP) residents caring for dependent elderly people during the first lockdown as part of the countries COVID-19 pandemic strategy. The aim was to explore themes that could explain the gap between the missions and values at the heart of GP practice during this period of strict isolation. METHOD: Qualitative study using an iterative approach. Semi-structured interviews were conducted with 13 GP residents using a pre-established interview guide. Audio recordings were transcribed verbatim. Data were analyzed according to a coding grid, developed using Nvivo software (NVivo Qualitative Data Analysis Software; QSR International Pty Ltd. Version Release 1.5.1 (940) 2021), to identify emerging themes. RESULTS: Three themes emerged from this qualitative research: cognitive dissonance, psychosocial risks, and fear. General practice residents have lived in the paradox between care and deprivation of liberty of dependent elderly people. CONCLUSION: The results suggest that the GP residents experienced a form of work-related suffering in this situation of deprivation of liberty of dependent elderly people. The present research serves as a pilot study to explore how GP residents experienced their care of locked-up dependent elderly people.

RESUMEN

The innovative and learning collective actions initiative, steered by the Ministry of Health and coordinated and led by the network of the National Agency for the Improvement of Working Conditions and regional associations for the improvement of working conditions with the support of the regional health agencies, has made it possible to deploy quality of life at work initiatives in 277 institutions for the elderly and disabled, between 2018 and 2021.

RESUMEN

Anticipated directives are of great value among patients living in residential care facilities for the elderly. Their prevalence in cases of cardiac arrest was studied. It was found that this issue is not systematically addressed. This results in the inappropriate use of resuscitation manoeuvres that could be avoided. This is an area that needs to be addressed in depth.

RESUMEN

During the second epidemic wave of COVID-19 in the fall of 2020, residents of accommodation facilities for dependent elderly people were not totally confined. They were able to continue to receive visits from their families under a strict health protocol. But these facilities are very fragile, and many have been affected. The marketing of rapid detection tests for severe acute respiratory syndrome coronavirus 2 using an antigenic method has made it possible to carry out massive screening campaigns for staff in these facilities since the beginning of November.

The objective of this pilot study was to provide data on the oral health status and oral health behaviour of care-dependent elderly people in the canton of Uri. The study aimed at assessing whether age and duration of living in a nursing home is associated with dental attendance, whether there is a relationship between age and the DMF-T index, and how high the risk was for participants to develop oral health problems. The study offered mobile dental services to people residing in nursing homes (NHG) and to community dwellers (CDG). Data was collected on demographics, medical history, presence of pain, ability to eat and oral health behaviour by means of a questionnaire. A clinical examination was performed and data on the oral status was collected. Statistical methods were: median tests, linear regressions and descriptive statistics. Uptake of the programme was low. 56 participants were examined. 47 participants (24 females, 23 males, average age 86.3 ± 7.36 years) resided in nursing home, nine participants (seven females, two males, average age 76 ± 12.8 years) were community dwellers. No significant differences were found between frequency of dental attendance and age (p = 0.35) or duration of stay in a nursing home (p = 0.55). The number of decayed teeth (p=0.005), missing teeth (p=0.01), and the DMF-T index (average in NHG = 26.5, CDG = 20.2, p < 0.001) increased, the number of filled teeth (p=0.02) decreased as age increased. Upon calculation of the 'Teamwerk-index', which takes into account oral health behaviour, dental caries, periodontal status, the majority of participants had a medium risk of developing oral health problems. Our pilot study in rural Switzerland provides first results of the oral health status in dependent elderly people. As people age, oral health deteriorates. More attention should be given in order to achieve better oral hygiene maintenance and more regular dental visits, to ensure a better oral health status in dependent elderly people.

The pandemic has severely affected some accommodation facilities for dependent elderly people, but all of them have had to reorganize. Their operation will never be the same again and transparency towards employees and the families of residents has become an obligation. Cooperation with dispensing pharmacists, who can act as information relays for families, must be stepped up.

OBJECTIVE: This work aimed at describing mobile dental services provided by both the private sector and the universities in Switzerland to dependent elderly people. It also aimed at presenting the views of both sectors on the current status of these services. BACKGROUND: As people age and become more care-dependent, their oral health requirements increase, as access to dental care decreases. METHOD: A survey was sent to practitioners who potentially provided a mobile dental service (n = 253). Five interviews were conducted with six individuals who have a leading role in Geriatric dentistry in the four universities in Switzerland that provide dental training. RESULTS: The private sector provides a mobile dental service to the elderly mainly through the use of portable equipment, while university services are provided by different models. The interviewees discussed the reasons as to why these services were set up. They discussed the challenges associated with treating dependent elderly patients, which often makes work in this field hard and unattractive. Both sectors see the need for better collaboration with other professionals. CONCLUSION: The private sector in Switzerland provides a mobile dental service to elderly people mostly through the use of portable equipment, while university services are provided in a variable configuration of models. Better collaboration is required to improve dental care and service delivery to dependent elders. There is a potential for improvement of these services, in order to meet the demand of an ageing population, catering for both the institutionalised elders and those living at home.

AIM: To report New Zealand rest home (rest home is the New Zealand term for residential aged care facilities where dependent, institutionalised, elderly people reside.) managers' opinions on access and barriers to oral health care for rest home residents. METHODS: An exploratory study using qualitative descriptive methods. Eleven rest home managers in a region of New Zealand were interviewed about admission assessments for new residents, oral health care, availability of dental facilities, staff training in oral hygiene, residents' nutritional needs and oral hygiene aids. RESULTS: Interviews with managers indicated that rest homes have no policies for the provision of oral health care and only arrange dental check-ups or treatment when requested by residents or family. Barriers to accessing care include making appointments, availability of staff and transport particularly for bedridden patients. Staff receive basic oral hygiene training only and face resistance to oral hygiene from some residents. No homes had a dental chair at their premises. All rest homes stocked oral hygiene aids such as toothbrushes and toothpastes. Electric toothbrushes were not generally promoted with cost being a key factor. CONCLUSION: While rest home managers were aware of the importance of good oral health, there are barriers to care. Finance is a challenge as dental care is not publicly funded and many residents rely on a limited income. Recommendations include policy changes to better fund oral health, specific oral health training for rest home staff, provision of mobile dental services to rest homes and the inclusion of gerodontology in the dental school curriculum.

AIM: To clarify the association between nutritional status and the prevalence of geriatric conditions in dependent older adults. METHODS: A cross-sectional observational study of dependent older adults aged 65years or older who were living either in the community (n = 511, mean age 81.2years) or in nursing homes (n = 587, mean age 85.2years) was carried out. Data included the participants' demographic characteristics, basic activities of daily living, Charlson Comorbidity Index and the prevalence of eight geriatric conditions (visual impairment, hearing impairment, falls, bladder control problems, cognitive impairment, impaired mobility, swallowing disturbance and loss of appetite). Nutritional status was assessed by the Mini Nutritional Assessment short form (MNA-SF). RESULTS: Of 1098 participants, 21.4% (n = 235) were categorized as "malnourished", according to the MNA-SF classification. Participants in the "malnourished" group had a greater number of geriatric conditions than those in the other two groups. A higher prevalence of all the geriatric conditions except for falls was detected in the group with poorer nutritional status. Multivariate logistic regression analysis showed that malnutrition was associated with the number of geriatric conditions, but not with that of comorbidities, even after controlling for confounders. CONCLUSIONS: Malnutrition was confirmed to have significant associations with geriatric conditions in dependent older adults.
