BACKGROUND: Intradermal (IDT) and prick (PT) tests are used to select allergens for allergen-specific immunotherapy in dogs with atopic dermatitis (cAD). However, the use of antipruritic drugs before performing these tests may influence the results. OBJECTIVE: To evaluate the influence of the drugs oclacitinib and prednisolone on the immediate-phase reactions of IDT and PT. ANIMALS: Thirty client-owned dogs with cAD with positive reactions to at least one allergen extract on IDT or PT. MATERIALS AND METHODS: Dogs were randomly assigned to receive oclacitinib 0.4-0.58 mg/kg per os, every 12 h (n = 14), or prednisolone 0.37-0.65 mg/kg p.o., every 12 h (n = 16) for 14 days. IDT and PT were performed on Day (D)0 before treatment and on D14. RESULTS: At D14 there was no significant reduction in the means of the orthogonal diameters of the positive immediate-phase reactions of the IDT (p = 0.064) in the oclacitinib group; however, in the PT, the diameter of the positive reactions reduced significantly (p = 0.048). In both tests, there was no significant reduction in the total number of positive reactions (IDT, p > 0.999; PT, p = 0.735). In the prednisolone group, the means of the orthogonal diameters of positive immediate-phase reactions were significantly reduced in both tests (IDT, p = 0.001; PT, p ≤ 0.001) and there also was a reduction in the total number of positive reactions (IDT, p = 0.022; PT, p = 0.001). CONCLUSIONS AND CLINICAL RELEVANCE: The use of oclacitinib 0.4-0.58 mg/kg twice daily for 14 days does not interfere with IDT results in dogs with cAD. However, oclacitinib may reduce PT reactivity. The use of prednisolone 0.37-0.65 mg/kg twice daily results in a reduction in both IDT and PT results.

.: Preventive effect of metformin in hepatocellular carcinoma (HCC) is not entirely clear. We aimed to evaluate the use of metformin as a protective factor of HCC in diabetic patients. .: We carried out an electronic search on PUBMED/MEDLINE, Web of Science and LILACS databases, with no limit of date, from April 2017 to January 2019. Eligible studies included cohort and case-control studies. We adressed data about the use of metformin on the risk of HCC development. Two independent reviewers extracted the data. We evaluated the quality of studies by using the Newcastle-Ottawa scale and carried out a meta-analysis using random-effects models. .: The electronic searches identified 747 studies. After reading abstracts and titles, we excluded 327 duplicated papers and 383 irrelevant references. Eight studies were selected; four case-control and four cohort studies. All studies have observed that the therapy with metformin was associated with a lower risk of HCC, compared with non-metformin therapy. Five articles reported that patients treated with insulin, or insulin secretagogues, presented increased risk of HCC compared to those treated with metformin. One study found that not only statin but also aspirin reduced the risk of HCC, if combined with metformin. A meta-analysis, using the case-control studies, found a combined Odds Ratio of 0.468; 95% CI 0.275-0.799 for the association between HCC and the use of metformin. .: The use of metformin was associated with a reduced risk of HCC, and it may be a relevant factor for preventing HCC in diabetic patients.

RESUMO

Microbial resistance is a major concern for public health worldwide, mainly because of the inappropriate use of antimicrobials. In this scenario, antimicrobial peptides (AMPs) have emerged as a potential therapeutic alternative means by which to control infectious diseases, because of their broad spectrum of action. However, some challenges can make their clinical application problematic, including metabolic instability and toxicity. Here, we provide a clear description of AMPs as promising molecules for the development of unusual antimicrobial drugs. We also describe current strategies used to overcome the main difficulties related to AMP clinical application, including different peptide designs and nanoformulation.

RESUMO

Over the past few decades, the number of health and 'omics-related data' generated and stored has grown exponentially. Patient information can be collected in real time and explored using various artificial intelligence (AI) tools in clinical trials; mobile devices can also be used to improve aspects of both the diagnosis and treatment of diseases. In addition, AI can be used in the development of new drugs or for drug repurposing, in faster diagnosis and more efficient treatment for various diseases, as well as to identify data-driven hypotheses for scientists. In this review, we discuss how AI is starting to revolutionize the life sciences sector.

BACKGROUND: During the COVID-19 pandemic, creating tools to assess disease severity is one of the most important aspects of reducing the burden on emergency departments. Lung ultrasound has a high accuracy for the diagnosis of pulmonary diseases; however, there are few prospective studies demonstrating that lung ultrasound can predict outcomes in COVID-19 patients. We hypothesized that lung ultrasound score (LUS) at hospital admission could predict outcomes of COVID-19 patients. This is a prospective cohort study conducted from 14 March through 6 May 2020 in the emergency department (ED) of an urban, academic, level I trauma center. Patients aged 18 years and older and admitted to the ED with confirmed COVID-19 were considered eligible. Emergency physicians performed lung ultrasounds and calculated LUS, which was tested for correlation with outcomes. This protocol was approved by the local Ethics Committee number 3.990.817 (CAAE: 30417520.0.0000.0068). RESULTS: The primary endpoint was death from any cause. The secondary endpoints were ICU admission and endotracheal intubation for respiratory failure. Among 180 patients with confirmed COVID-19 who were enrolled (mean age, 60 years; 105 male), the average LUS was 18.7 ± 6.8. LUS correlated with findings from chest CT and could predict the estimated extent of parenchymal involvement (mean LUS with < 50% involvement on chest CT, 15 ± 6.7 vs. 21 ± 6.0 with > 50% involvement, p < 0.001), death (AUC 0.72, OR 1.13, 95% CI 1.07 to 1.21; p < 0.001), endotracheal intubation (AUC 0.76, OR 1.17, 95% CI 1.09 to 1.26; p < 0.001), and ICU admission (AUC: 0.71, OR 1.14, 95% CI 1.07 to 1.21; p < 0.001). CONCLUSIONS: In COVID-19 patients admitted in ED, LUS was a good predictor of death, ICU admission, and endotracheal intubation.

BACKGROUND: The first cases of coronavirus disease (COVID-19) in Brazil were diagnosed in February 2020. Our Emergency Department (ED) was designated as a COVID-19 exclusive service. We report our first 500 confirmed COVID-19 pneumonia patients. METHODS: From 14 March to 16 May 2020, we enrolled all patients admitted to our ED that had a diagnosis of COVID-19 pneumonia. Infection was confirmed via nasopharyngeal swabs or tracheal aspirate PCR. The outcomes included hospital discharge, invasive mechanical ventilation, and in-hospital death, among others. RESULTS: From 2219 patients received in the ED, we included 506 with confirmed COVID-19 pneumonia. We found that 333 patients were discharged home (65.9%), 153 died (30.2%), and 20 (3.9%) remained in the hospital. A total of 300 patients (59.3%) required ICU admission, and 227 (44.9%) needed invasive ventilation. The multivariate analysis found age, number of comorbidities, extension of ground glass opacities on chest CT and troponin with a direct relationship with all-cause mortality, whereas dysgeusia, use of angiotensin converting enzyme inhibitor or angiotensin-ii receptor blocker and number of lymphocytes with an inverse relationship with all-cause mortality. CONCLUSIONS: This was a sample of severe patients with COVID-19, with 59.2% admitted to the ICU and 41.5% requiring mechanical ventilator support. We were able to ascertain the outcome in majority (96%) of patients. While the overall mortality was 30.2%, mortality for intubated patients was 55.9%. Multivariate analysis agreed with data found in other studies although the use of angiotensin converting enzyme inhibitor or angiotensin-ii receptor blocker as a protective factor could be promising but would need further studies. TRIAL REGISTRATION: The study was registered in the Brazilian registry of clinical trials: RBR-5d4dj5.

INTRODUCTION: The need to develop new drugs for the control of pathogenic microorganisms has redoubled efforts to prospect for antimicrobial peptides (AMPs) from natural sources and to characterize its structure and function. These molecules present a broad spectrum of action against different microorganisms and frequently present promiscuous action, with anticancer and immunomodulatory activities. Furthermore, AMPs can be used as biopharmaceuticals in the treatment of hospital-acquired infections and other serious diseases with relevant social and economic impacts.Areas covered: The low yield and the therefore difficult extraction and purification process in AMPs are problems that limit their industrial application and scientific research. Thus, optimized heterologous expression systems were developed to significantly boost AMP yields, allow high efficiency in purification and structural optimization for the increase of therapeutic activity.Expert opinion: This review provides an update on recent developments in the recombinant production of ribosomal and non-ribosomal synthesis of AMPs and on strategies to increase the expression of genes encoding AMPs at the transcriptional and translational levels and regulation of the post-translational modifications. Moreover, there are detailed reports of AMPs that have already reached marketable status or are in the pipeline under advanced stages of preclinical testing.

Objectives: To evaluate the first-attempt success rates and complications of endotracheal intubation of coronavirus disease 2019 (COVID-19) patients by emergency physicians. Methods: This prospective observational study was conducted from March 24, 2020 through May 28, 2020 at the emergency department (ED) of an urban, academic trauma center. We enrolled patients consecutively admitted to the ED with suspected or confirmed COVID-19 submitted to endotracheal intubation. No patients were excluded. The primary outcome was first-attempt intubation success, defined as successful endotracheal tube placement with the first device passed (endotracheal tube) during the first laryngoscope insertion confirmed with capnography. Secondary outcomes included the following complications: hypotension, hypoxemia, aspiration, and esophageal intubation. Results: A total of 112 patients with confirmed or suspected COVID-19 were enrolled. Median age was 61 years and 61 patients (54%) were men. The primary outcome, first-attempt intubation success, was achieved in 82% of patients. Among the 20 patients who were not intubated on the first attempt, 75% were intubated on the second attempt and 20% on the third attempt; cricothyrotomy was performed in 1 patient. Forty-eight (42%) patients were hypotensive and required norepinephrine immediately post-intubation. Fifty-eight (52%) experienced peri-intubation hypoxemia, and 2 patients (2%) had cardiac arrest. There were no cases of failed intubation resulting in death up to 24 hours after the procedure. Conclusion: Emergency physicians achieve high success rates when intubating COVID19 patients, although complications are frequent. However, these findings should be considered provisional until their generalizability is assessed in their institutions and setting.

CONTEXT: Adverse reactions associated with prick tests are rare but may be present as serious systemic reactions. CASE REPORT: A 38-year-old female nursing technician complained of three episodes of anaphylaxis in one year, all in the workplace. To investigate latex allergy, the patient underwent the prick test with latex, and immediately developed a rash, itchy skin, hoarseness, dyspnea and dry cough. Her condition improved promptly after appropriate measures were established for controlling her anaphylaxis. CONCLUSION: The skin test must be performed under medical supervision, since complications that can lead to life-threatening reactions, if support measures are not readily implemented, have been attributed to this test.

Objetiva-se apresentar o processo de avaliação e confirmação dos indicadores de Altas Habilidades/Superdotação de um estudante, pré e pós-oferta de enriquecimento intracurricular e extracurricular. A pesquisa configura-se como descritiva, do tipo casuístico. Participou do estudo um estudante com indicadores de Altas Habilidades/Superdotação, matriculado nos anos iniciais do Ensino Fundamental de uma escola pública estadual do interior de São Paulo. A coleta de dados foi realizada na escola e no Centro de Psicologia Aplicada de uma Universidade Pública da cidade, por meio de uma avaliação multimodal, envolvendo uma bateria de instrumentos aplicados junto à mãe e ao estudante, incluindo avaliação pedagógica, psicológica e psicomotora. Os resultados indicaram que o estudante apresenta fortes indicadores de Altas Habilidades/Superdotação, demonstrando desempenho acima da média, com destaque em matemática, leitura e escrita, os quais puderam ser confirmados tanto pela avaliação pedagógica quanto pela psicológica, e alto desempenho na área de psicomotricidade. A oferta de enriquecimento intracurricular e extracurricular contribuiu para melhorar o desempenho do estudante.


The objective is to present the process of evaluating and confirming the indicators of High Abilities/Giftedness of a student, pre and post-offer for intracurricular and extracurricular enrichment. The research is descriptive, of the casuistic type. A student with indicators of High Abilities/Giftedness, enrolled in the early years of Elementary School from a state public school in the state of São Paulo, participated in the study. Data collection was carried out at the school and at the Applied Psychology Center of a Public University in the city through a multimodal assessment, involving a battery of instruments applied to the mother and student, including pedagogical, psychological and psychomotor assessment. The results indicated that the student has strong indicators of High Abilities/Giftedness, demonstrating above-average performance, especially in mathematics, reading and writing, which could be confirmed by both the pedagogical and psychological assessment, and high performance in the area of psychomotricity. Offering intra and extra-curricular enrichment has contributed to improving the student's performance.


El objetivo deste estudio es presentar el proceso de evaluación y confirmación de los indicadores de Altas Capacidades/Superdotación, antes y después de la oferta de enriquecimiento intracurricular y extracurricular. La investigación es descriptiva, de tipo casuístico. Participó en el estudio un estudiante con indicadores de Altas Capacidades/ Superdotación matriculado en los primeros años de la Escuela Primaria, de una escuela pública estatal en el interior de São Paulo. La recolección de datos se realizó en la escuela y en el Centro de Psicología Aplicada de una Universidad Pública de la ciudad. Através de una evaluación multimodal, que involucró una batería de instrumentos aplicados a la madre y al alumno, incluyendo evaluación pedagógica, psicológica y psicomotora. Los resultados indicaron que el estudiante tiene fuertes indicadores de Altas Capacidades/Superdotación, demostrando un desempeño superior al promedio, con énfasis en matemáticas, lectura y escritura, lo cual podría ser confirmado tanto por la evaluación pedagógica, la psicológica y de alto desempeño en el área de Psicomotricidad. Ofrecer enriquecimiento intra y extracurricular ha contribuido a mejorar el rendimiento de los estudiantes.

BACKGROUND: Patients with oral sensitivity are common in our practice. Allergic contact dermatitis is one of the most frequent etiologies. OBJECTIVES: Evaluate oral contact dermatitis using the Brazilian standard series and complementary dental series in patients using dental prostheses, with or without oral complaints. Determine specific dental Brazilian series. METHODS: Patients using dental prostheses with or without oral complaints realized patch tests. Brazilian standard series and complementary dental series were used according to ICDRG recommendations. The results were analysed according to age, sex, race, atopic conditions and symptoms associated. RESULTS: From 54 patients tested, 34 (63%) were positive at least to one substance. Nineteen had oral complaints, such as burning mouth, itch or oral erythema. There was no association between atopic condition and tests results. Without the oral series, just 23(42,6%) patients had a positive result. Using the Brazilian standard series with the complementary dental series we improved the positivity of the patch test to 47%. CONCLUSION: In patients using prostheses and with oral complaints, patch tests with Brazilian standard series with complementary dental series improve the tests positivity.

Estudantes com Altas Habilidades/Superdotação (AH/SD), público-alvo da Educação Especial (PAEE), podem apresentar queixas escolares, quando suas necessidades superiores não são atendidas. O estudo teve como objetivo descrever os tipos de queixas escolares que esses estudantes expressam, por meio do relato materno. Participaram 13 mães, de 29 a 48 anos. As mães foram entrevistadas individualmente, utilizando-se da Entrevista de Anamnese, do Roteiro de Entrevista Semiestruturada para a Família e do Questionário de Capacidades e Dificuldades SDQ-Por. Os resultados demonstraram que as mães recebem queixas escolares de seus filhos na escola, dentre as quais estão os problemas de comportamento, desmotivação em sala de aula, indisciplina e dificuldades na interação social. Todas as mães avaliaram seus filhos dentro do padrão para o comportamento pro-social e não-padrão para os problemas de relacionamento com os colegas. É importante que a escola seja uma rede de apoio para auxiliar no desenvolvimento desses estudantes PAEE.


Estudiantes con Altas Habilidades/Superdotación (AH/SD), público-objetivo de la Educación Especial (PAEE), pueden presentar quejas escolares, cuando sus necesidades superiores no son atendidas. El estudio tuvo como objetivo describir los tipos de quejas escolares que esos estudiantes expresan, por intermedio del relato materno. Participaron 13 madres, de 29 a 48 años. Se entrevistaron las madres individualmente, utilizándose de la Entrevista de Anamnese, del programa de Entrevista Semiestructurada para la Familia y del Cuestionario de Capacidades y Dificultades SDQ-Por. Los resultados demostraron que las madres reciben quejas escolares de sus hijos en la escuela, entre las cuales están los problemas de comportamiento, falta de motivación en sala de clase, indisciplina y dificultades en la interacción social. Todas las madres evaluaron sus hijos dentro del patrón para el comportamiento prosocial y no-patrón para los problemas de relacionamiento con los compañeros. Es importante que la escuela sea una red de apoyo para auxiliar en el desarrollo de esos estudiantes PAEE.


Students with High Ability / Giftedness, target audience of Special Education can present school complaints when their higher needs are not met. The study aimed to describe the types of school complaints that these students express, through the mother's report. Thirteen mothers, aged 29 to 48 years, participated. The mothers were interviewed individually, using the Anamnesis Interview, the semi-structured Interview Guide for the Family and the SDQ-Por Abilities and Difficulties Questionnaire. The results showed that mothers receive school complaints from their children at school, which includes behavior problems, demotivation in the classroom, indiscipline and difficulties in social interaction. All mothers rated their children within the standard for pro-social behavior and non-standard for relationship problems with colleagues. It is important that the school is a support network to assist in the development of these students.

The results were analysed according to age, sex, race, atopic conditions and symptoms associated. RESULTS: From 54 patients tested, 34 (63%) were positive at least to one substance. Nineteen had oral complaints, such as burning mouth, itch or oral erythema. There was no association between atopic condition and tests results. Without the oral series, just 23(42,6%) patients had a positive result. Using the Brazilian standard series with the complementary dental series we improved the positivity of the patch test to 47%. CONCLUSION: In patients using prostheses and with oral complaints, patch tests with Brazilian standard series with complementary dental series improve the tests positivity.

House dust mite antigens have been used for decades to diagnose allergic diseases in humans and animals. The objective of this study was to identify allergens in commercial Dermatophagoides farinae and Blomia tropicalis extracts by immunoblotting using sera from allergic dogs and anti-dog IgE conjugate. The analysis of antigens present in the D. farinae extract (FDA Allergenic) using sera from 10 dogs allergic to D. farinae showed that eight sera recognized a band of approximately 102 kDa, eight recognized two bands of 52 to 76 kDa, five recognized one band of approximately 76 kDa, four recognized one band of 31 to 38 kDa, and two recognized one band of 12 to 17 kDa. Immunoblot assays of the B. tropicalis extract (FDA Allergenic) using sera from 10 animals allergic to B. tropicalis showed that five sera recognized two bands of 52 to 76 kDa. These results demonstrate the importance of the two house dust mite species for the pathogenesis of canine atopic dermatitis in Brazil. In addition, the results indicate which allergens should be present in allergenic extracts used for diagnosis and allergen-specific immunotherapy.


Antígenos de ácaros da poeira domiciliar são utilizados por décadas para diagnóstico de doenças alérgicas em seres humanos e animais. O objetivo do presente trabalho foi identificar proteínas alergênicas presentes em extratos de Dermatophagoides farinae e Blomia tropicalis através de "immunoblotting" utilizando-se soros de animais alérgicos e conjugado anti-IgE canina. A análise por "immunoblotting" dos antígenos presentes no extrato de D. farinae (FDA Allergenic), utilizando soros de dez animais alérgicos, mostrou que oito soros reconhecem uma banda com peso molecular de aproximadamente 102 kDa; oito soros duas bandas entre 52 e 76 kDa; cinco soros uma banda com aproximadamente 76 kDa; quatro soros uma banda entre 31 e 38 kDa; e dois soros uma banda entre 12 e 17 kDa. A análise por "immunoblotting" dos antígenos do extrato de B. tropicalis (FDA Allergenic) mostrou que cinco soros reconhecem duas bandas com pesos moleculares entre 52 e 76 kDa. Esses resultados demonstram a importância dessas duas espécies de ácaros da poeira domiciliar na patogênese da dermatite atópica canina no Brasil, assim como indicam alérgenos que devem estar presentes nos extratos alergênicos utilizados para diagnóstico e imunoterapia alérgeno-específica.

The objective of the study was to evaluate whether allergenic extracts of five house dust and storage mite species standardized for humans might be used for the diagnosis of canine atopic dermatitis (CAD). Extracts of Dermatophagoides pteronyssinus (Pyroglyphidae), D. farinae (Pyroglyphidae), Blomia tropicalis (Glycyphagidae), Lepidoglyphus destructor (Glycyphagidae) and Tyrophagus putrescentiae (Acaridae) were evaluated by intradermal testing in 20 healthy dogs (control) and 25 dogs with allergic dermatitis. A significant difference in the response was observed between the two groups (p<0.05). Only one dog (5 percent) in the control group reacted to the intradermal test, whereas 14 dogs (56 percent) in the allergic group were positive for at least one extract (odds ratio = 24.2). Most of the positive reactions observed in the allergic group occurred against the extracts of T. putrescentiae or L. destructor, each inducing reactions in 10 dogs (40 percent). D. farinae, D. pteronyssinus e B. tropicalis extracts induced reactions in 7 (28 percent), 3 (12 percent) and 3 (12 percent) dogs, respectively. The allergenic extracts standardized for humans evaluated in the present study may be used as a tool to complement the diagnosis of the disease, as well as to select potential allergen candidates for allergen-specific immunotherapy.


O presente trabalho teve como objetivo avaliar se extratos alergênicos de cinco espécies de ácaros da poeira domiciliar e produtos armazenados, padronizados para humanos, podem ser utilizados no diagnóstico da dermatite atópica canina. Extratos de Dermatophagoides pteronyssinus (Pyroglyphidae), D. farinae (Pyroglyphidae), Blomia tropicalis (Glycyphagidae), Lepidoglyphus destructor (Glycyphagidae) e Tyrophagus putrescentiae (Acaridae) foram avaliados através de testes intradérmicos em 45 cães, dos quais 20 normais e 25 com dermatite alérgica. Uma diferença significativa foi observada no padrão de respostas obtidas dos dois grupos (p<0.05). Apenas um animal (5 por cento) do grupo controle reagiu ao teste cutâneo, enquanto que no grupo dos alérgicos 14 cães (56 por cento) apresentaram pelo menos uma reação positiva (odds ratio = 24.2). As maiores freqüências de reações positivas observadas no grupo dos alérgicos foram aos extratos de T. putres-centiae ou L. destructor, cada um induzindo reações em 10(40 por cento) cães. Os extratos de D. farinae, D. pteronyssinus e B. tropicalis foram responsáveis por reações positivas em 7(28 por cento), 3(12 por cento) e 3(12 por cento) cães, respectivamente. Os extratos padronizados para humanos avaliados no presente estudo podem ser utilizados como complemento no diagnóstico da doença, assim como na seleção de alérgenos para a imunoterapia alérgeno-específica.
