RESUMO

BACKGROUND: Serum amyloid A (SAA), interleukin-6 (IL-6) and neutrophil-to-lymphocyte ratio (NLR) play critical roles in inflammation and are used in clinical laboratories as indicators of inflammation-related diseases. We aimed to provide potential laboratory basis for auxiliary distinguishing coronavirus disease (COVID-19) by monitoring above indicators. METHODS: A total of 84 patients with confirmed COVID-19 were enrolled in the study. Baseline characteristics and laboratory results were collected and analyzed. Receiver operating characteristic (ROC) curve analysis was used to combined detection of SAA and IL-6 in patients with COVID-19, and independent risk factors for severity of COVID-19 were assessed by using binary logistic regression. RESULTS: The main clinical symptoms of patients with COVID-19 were fever (98.8%), fatigue (61.9%), and dry cough (58.3%). SAA, IL-6, and NLR were significantly higher in patients with COVID-19 (all P < .001), and compared with nonsevere patients, three indicators of severe patients were significantly elevated. Besides, combined detection of SAA and IL-6 better separates healthy people from patients with COVID-19 than detection of SAA or IL-6 alone. In addition, elevated SAA, IL-6, and NLR can be used as independent variables for predicting the severity of patients with COVID-19. CONCLUSION: Serum amyloid A and IL-6 could be used as addition parameters to helping the distinguish of patients with COVID-19 from healthy people, and can provide potential basis for separating patients with nonsevere and severe clinical signs.

Objective: Yixishu lotion (YXSL) originates from the summary of traditional Chinese medicine clinical experience and constantly improves in practice in clinical validation of the exact efficacy of traditional Chinese medicine prescription. To explore the mechanism of YXSL in treating vaginitis and the potential mechanisms based on network pharmacology and experimental verification. Methods: The active components and drug-related targets of YXSL were retrieved from the TCMSP (Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform) database, and the target was predicted by the UniProt database. Searching for genes related to 'vaginitis' disease in the GeneCards database, a total of 2581 drug targets were obtained. The interaction between proteins (PPI - protein-protein interaction) relationship was obtained by STRING database and visualized by Cytoscape software. Finally, the 'Bioconductor' installation package in R software was used to analyze the GO (Gene Ontology) and KEGG (Kyoto Encyclopedia of Genes and Genomes) pathways of the target. Results: In this study, by the method of network pharmacology, the key active components of YXSL were flavonoids such as quercetin, apigenin, kaempferol, luteolin, ß-sitosterol; the main core proteins included MAPK14, TP53, FGF2, ESR1, MAPK3, MAPK1, VEGFA, JUN, IL-6, and the KEGG pathway was mainly involved in MAPK pathway, Th17 pathway, Malaria, TNF pathway, and other signaling pathways. Animal experiments showed that the clinical symptoms and vaginal tissue lesions of the YXSL group and the fluconazole group were improved, and the levels of TNF-α (tumor necrosis factor alpha), IL-6 (interleukin-6), MDA (malondialdehyde), SOD (superoxide dismutase), IL-4, and IFN-γ (interferon-γ) in vaginal tissue and serum were better than the model group. Conclusion: YXSL may achieve its therapeutic effect on vaginitis by reducing the inflammatory response, improving oxidative stress response, and improving body immunity, and it provides a theoretical basis for further research on its pharmacodynamic material basis and mechanism of action.

Investigating the effect of the aggregate gradation on the material properties of cemented rockfill is significant for the green mining, economic benefit and engineering safety. Consequently, the ultrasonic test, uniaxial compression experiment and acoustic emission (AE) monitor on cemented rockfill were carried out, for which the aggregate satisfied Talbot gradation. The dilatancy behavior and AE characteristic of cemented rockfill under load were investigated. The damage in the internal structure under compression was revealed by the deformation and AE signals of cemented rockfill. The effect of the Talbot index on the ultrasonic pulse velocity (UPV) and the strength parameters such as stress of dilatancy onset and uniaxial compressive strength (UCS) of cemented rockfill was analyzed. The mechanical properties of cemented rockfill materials were evaluated by the establishment of the relation between the UPV and the strength parameter. The results show that The difference between the stress of dilatancy onset and the UCS, the deformation performance and the activity of AE signals during dilatancy are positive correlated with the Talbot index of aggregate in cemented rockfill. The relation between the UPV and the strength parameters (stress of dilatancy onset and UCS) of cemented rockfill can be characterized by the positive linearity, and the UPV is also suitable for characterizing the stress of dilatancy onset of cemented rockfill material. The cubic polynomial is more suitable for describing the relations between the parameters of strength and UPV and the Talbot index of aggregate than the quadratic polynomial, and the Talbot index with optimal aggregate gradation reflected the maximum strength of cemented rockfill material should be around 0.45-0.47.

It is important to study the mechanical properties of cracked rock to understand the engineering behavior of cracked rock mass. Consequently, the influence of the length of pre-existing fissures on the strength, deformation, acoustic emission (AE) and failure characteristics of cracked rock specimen was analyzed, and the optimal selection of strength parameter in engineering design was discussed. The results show that the strength parameters (stress of dilatancy onset and uniaxial compressive strength) and deformation parameters (axial strain and circumferential strain at dilatancy onset and peak point) of cracked rock specimen decrease with the increase of the number of pre-existing fissures, and the relations which can use the negative exponential function to fit. Compared with the intact rock specimens, the different degrees of stress drop phenomena were produced in the process of cracked rock specimens when the stress exceeds the dilatancy onset. At this moment, the cracked rock specimens with the existence of stress drop are not instantaneous failure, but the circumferential strain, volumetric strain and AE signals increase burstingly. And the yield platform was presented in the cracked rock specimen with the length of pre-existing fissure more than 23mm, the yield failure was gradually conducted around the inner tip of pre-existing fissure, the development of original fissures and new cracks was evolved fully in rock. However, the time of dilatancy onset is always ahead of the the time of that point with the existence of stress drop. It indicates that the stress of dilatancy onset can be as the parameter of strength design in rock engineering, which can effectively prevent the large deformation of rock.

Local recurrence remains a major cause of therapeutic failure in patients with nasopharyngeal carcinoma (NPC) and the effective treatment of recurrent NPC (r-NPC) is still a challenge. Intensity-modulated radiotherapy (IMRT) is considered as a favorable technique in the management of r-NPC, especially for extensive lesions. However, local r-NPC is a highly heterogeneous disease and the survival outcome following salvage IMRT varies. Furthermore, due to varied samples and therapeutic protocols, no consensus has been reached in the establishment of prognostic values. Hence, we used Medline and Embase electronic databases to conducted a meta-analysis to generate the best estimation of the prognostic factors in local r-NPC following salvage IMRT. Finally, a total of 783 patients in seven studies were enrolled. Overall, the pooled HR for OS of recurrent T stage and recurrent tumor volume was 1.77 (95% CI = 1.15-2.39) and 2.12 (95% CI = 1.42-2.82), without any heterogeneity. In addition, despite a significant association was observed in the pooled HR of significant compliance for OS, however, significant heterogeneity was also observed (I2 = 76.6%, p = 0.039). Furthermore, no significant association was observed among the pooled HRs for OS in terms of age, gender, recurrent time interval, synchronous nodal recurrence, chemotherapy and total re-irradiation dose. Therefore, the present meta-analysis demonstrated that recurrent T stage and tumor volume may serve as the prognostic factors for OS in patients with r-NPC who received salvage IMRT. The other factors such as age, gender, and optimal re-irradiation dose warranted further investigation.
