BACKGROUND: Chagas disease affects an estimated 326 000-347 000 people in the United States and is severely underdiagnosed. Lack of awareness and clarity regarding screening and diagnosis is a key barrier. This article provides straightforward recommendations, with the goal of simplifying identification and testing of people at risk for US healthcare providers. METHODS: A multidisciplinary working group of clinicians and researchers with expertise in Chagas disease agreed on 6 main questions, and developed recommendations based on the Grading of Recommendations Assessment, Development and Evaluation (GRADE) methodology, after reviewing the relevant literature on Chagas disease in the United States. RESULTS: Individuals who were born or resided for prolonged time periods in endemic countries of Mexico and Central and South America should be tested for Trypanosoma cruzi infection, and family members of people who test positive should be screened. Women of childbearing age with risk factors and infants born to seropositive mothers deserve special consideration due to the risk of vertical transmission. Diagnostic testing for chronic T. cruzi infection should be conducted using 2 distinct assays. CONCLUSIONS: Increasing provider-directed screening for T. cruzi infection is key to addressing this neglected public health challenge in the United States.

RESUMO

Chagas is a complex, multidimensional phenomenon in which political, economic, environmental, biomedical, epidemiological, psychological, and sociocultural factors intersect. Nonetheless, the hegemonic conceptualisation has long envisioned Chagas as primarily a biomedical question, while ignoring or downplaying the other dimensions, and this limited view has reinforced the disease's long neglect. Integrating the multiple dimensions of the problem into a coherent approach adapted to field realities and needs represents an immense challenge, but the payoff is more effective and sustainable experiences, with higher social awareness, increased case detection and follow-up, improved adherence to care, and integrated participation of various actors from multiple action levels. Information, Education, and Communication (IEC) initiatives have great potential for impact in the implementation of multidimensional programs of prevention and control successfully customised to the diverse and complex contexts where Chagas disease persists.

RESUMO

Chagas disease (CD) in the United States is severely underdiagnosed, due to an absence of systematic screening as part of routine healthcare. We screened 189 relatives of 86 existing patients and found a CD prevalence of 7.4%. Screening close relatives of previously diagnosed individuals can effectively identify new CD cases.

The vast majority of people with Chagas disease (CD) are undiagnosed and untreated. Improving access to diagnosis and treatment for CD involves confronting a wide range of barriers. This report discusses a collaborative approach to eliminate barriers and increase the availability of CD testing and treatment. Potential areas for intervention are selected based on burden of disease, support of local champions, and commitment from national and local authorities. A 4D approach (diagnose, design, deliver, and demonstrate impact) is then implemented. The diagnose step involves gathering key stakeholders at a seminar to collaboratively identify important barriers and propose solutions. The design step creates a specific plan to act upon the seminar's conclusions with consensus on core indicators. The deliver step entails implementing the plan at pilot locations, while simultaneously strengthening health system capacity for CD testing and treatment. Lastly, the demonstrate impact step compares baseline data with annual post-implementation data to measure progress. In Colombia, this approach has helped simplify testing procedures and increase CD testing and treatment access in pilot communities, though challenges remain. The 4D approach represents one of several pathways toward ensuring that the best therapeutic and diagnostic products reach people affected by neglected tropical diseases.


Una amplia mayoría de las personas con la enfermedad de Chagas no reciben diagnóstico ni tratamiento. Para mejorar el acceso al diagnóstico y al tratamiento de esta enfermedad es necesario superar multiples obstáculos. En el presente informe se considera un método colaborativo diseñado para superar las barreras y aumentar la disponibilidad de las pruebas de detección y el tratamiento de la enfermedad de Chagas. Se seleccionan los posibles ámbitos de intervención en función de la carga de enfermedad, el apoyo por parte de los líderes locales y el compromiso de las autoridades locales y nacionales. Posteriormente, se aplica un método 4D basado en cuatro pasos: diagnóstico, diseño, desarrollo y demonstración de impacto. El paso correspondiente al diagnóstico consiste en reunir en un seminario a los principales socios con el fin de establecer de manera colaborativa las barreras más importantes y ofrecer soluciones. En el paso de diseño, se elabora un plan específico para actuar conforme a las conclusiones del seminario de manera consensuada sobre los indicadores centrales. El paso de ejecución supone la aplicación del plan en las ubicaciones piloto, al tiempo que se fortalece la capacidad de los sistemas de salud en lo relativo a las pruebas de detección y el tratamiento de la enfermedad de Chagas. Finalmente, en el paso de demonstración de impacto se comparan los datos iniciales con los datos anuales posteriores a la ejecución para evaluar el progreso. En Colombia, este enfoque ha contribuido a simplificar los procedimientos de las pruebas de detección y brindar mayor acceso a la detección y el tratamiento de la enfermedad de Chagas en las comunidades piloto, si bien todavía hay retos que superar. El método de cuatro pasos es una de las diferentes vías para garantizar que los mejores productos de tratamiento y diagnóstico estén al alcance de las personas afectadas por las enfermedades tropicales desatendidas.


A ampla maioria das pessoas com doença de Chagas não é diagnosticada nem é tratada. Para melhorar o acesso ao diagnóstico e ao tratamento desta doença, é necessário superar uma série de barreiras. Neste relato, é exposta uma abordagem colaborativa com o objetivo de vencer as barreiras e aumentar a disponibilidade do exame de detecção da doença de Chagas. As possíveis áreas de intervenção são selecionadas de acordo com a carga de doença, o apoio de defensores da causa e o compromisso das autoridades nacionais e locais. Uma abordagem em 4Ds (diagnóstico, desenho, desenvolvimento e demonstração de impacto) é implementada. No passo ''diagnóstico'', um seminário com os principais atores é realizado para identificarem as maiores barreiras e proporem soluções de modo colaborativo. No passo ''desenho'', um plano de ação é elaborado a partir das conclusões tiradas do seminário mediante o consenso sobre os indicadores básicos. No passo ''desenvolvimento'', o plano é implementado em áreas-piloto e, em paralelo, a capacidade do sistema de saúde de realizar os exames de detecção e o tratamento da doença de Chagas é reforçada. Por último, no passo ''demonstração de impacto'', os dados de referência são comparados aos dados anuais pós-implementação com o objetivo de avaliar o progresso. Na Colômbia, apesar de persistirem os desafios, esta abordagem contribuiu para simplificar o procedimento para o exame de detecção da doença de Chagas, aumentar o número de exames realizados e melhorar o acesso ao tratamento nas comunidades-piloto. A abordagem em 4Ds representa um dos vários cursos a serem seguidos para garantir que os melhores recursos diagnósticos e terapêuticos cheguem até as pessoas afetadas por doenças tropicais negligenciadas.

Background: According to an estimate from the Centers for Disease Control and Prevention (CDC), Chagas disease (CD) may affect 1.31% of Latin American immigrants in the United States, with >300 000 cases. However, there is a lack of real-world data to support this estimate. Little is known about the actual prevalence of this neglected tropical disease in the United States, and the bulk of those infected are undiagnosed. Methods: From April 2008 to May 2014, we screened 4,755 Latin American-born residents of Los Angeles County. Blood samples were tested for serologic evidence of CD. We collected demographic data and assessed the impact of established risk factors on CD diagnosis, including sex, country of origin, housing materials, family history of CD, and awareness of CD. Results: There were 59 cases of CD, for an overall prevalence of 1.24%. Prevalence was highest among Salvadorans (3.45%). Of the 3,182 Mexican respondents, those from Oaxaca (4.65%) and Zacatecas (2.2%) had the highest CD prevalence. Salvadoran origin (aOR = 6.2; 95% CI = 2.8-13.5; P < .001), prior knowledge of CD (aOR = 2.4; 95% CI = 1.0-5.8; P = .047), and exposure to all 3 at-risk housing types (adobe, mud, and thatched roof) (aOR = 2.5; 95% CI = 1.0-6.4; P = .048) were associated with positive diagnosis. Conclusions: In the largest screening of CD in the United States to date outside of blood banks, we found a CD prevalence of 1.24%. This implies >30 000 people infected in Los Angeles County alone, making CD an important public health concern. Efficient, targeted surveillance of CD may accelerate diagnosis and identify candidates for early treatment.

Worldwide, over 6 million people are infected with Trypanosoma cruzi, the pathogen that causes Chagas disease (CD). In the Americas, CD creates the greatest burden in disability-adjusted life years of any parasitic infection. In Colombia, 437 000 people are infected with T. cruzi, of whom 131 000 suffer from cardiomyopathy. Colombia's annual costs for treating patients with advanced CD reach US$ 175 016 000. Although timely etiological treatment can significantly delay or prevent development of cardiomyopathy-and costs just US$ 30 per patient-fewer than 1% of people with CD in Colombia and elsewhere receive it. This represents a missed opportunity for increasing patients' healthy, productive years of life while significantly reducing the economic burden on the health care system. Key barriers are complexities and delays in the diagnosis and treatment process, lack of awareness of CD among both patients and health care professionals, and administrative barriers at the primary care level. In 2015, stakeholders from government, academia, nongovernmental organizations, and patient associations participated in a seminar in the city of Bogotá on eliminating barriers to diagnosis and treatment for CD. The seminar gave birth to a model of care for increasing patient access, including a patient road map that simplifies diagnostic and treatment processes, shifting them from specialists to primary care facilities. The patient road map was implemented in a pilot project in four endemic communities beginning in 2016, with the goal of testing and refining the model so it can be implemented nationally. This article describes key components in the development of a new, recently implemented model of care for CD in Colombia.


En todo el mundo, hay más de 6 millones de personas infectadas por el Trypanosoma cruzi, el agente patógeno causante de la enfermedad de Chagas. En la Región de las Américas, esta es la infección parasitaria que tiene la mayor carga en cuanto a años de vida ajustados en función de la discapacidad. En Colombia, 437 000 personas están infectadas por el T. cruzi; de ese total, 131 000 sufren de miocardiopatía. En ese país, el costo anual de tratar a los pacientes que tienen la enfermedad de Chagas se ubica en US$ 175 016 000. A pesar de que un tratamiento etiológico oportuno puede retrasar o prevenir significativamente la aparición de una miocardiopatía ­a un costo de apenas US$ 30 por paciente­ menos de 1% de las personas con enfermedad de Chagas en Colombia y otros países lo reciben. Esto implica que se pierde la oportunidad de incrementar el número de años de vida saludables y productivos de los pacientes y, al mismo tiempo, reducir significativamente la carga económica que soporta el sistema de atención de salud. Los obstáculos clave son la complejidad y las demoras en los procesos de diagnóstico y tratamiento, la falta de conocimiento sobre la enfermedad de Chagas por parte de los pacientes y de los profesionales de la salud, y las barreras administrativas que existen a nivel de la atención primaria.En el 2015, representantes del gobierno, la comunidad académica, organizaciones no gubernamentales y asociaciones de pacientes participaron en un seminario en Bogotá sobre la eliminación de las barreras al diagnóstico y el tratamiento de la enfermedad de Chagas. En este seminario se elaboró un modelo de atención para aumentar el acceso de los pacientes, incluida una hoja de ruta centrada en el paciente que simplifica los procesos de diagnóstico y tratamiento al trasladarlos de los especialistas a los establecimientos de atención primaria. La hoja de ruta centrada en el paciente se aplicó a principios del 2016 como parte de un proyecto piloto que se puso en marcha en cuatro comunidades endémicas con el objetivo de poner a prueba y perfeccionar el modelo para luego poder aplicarlo en todo el país. En este artículo se describen los componentes clave que se usaron para crear un modelo de atención de la enfermedad de Chagas puesto en marcha recientemente en Colombia.

Na Colômbia, estima-se que 437 mil pessoas são infectadas pelo T. cruzi, das quais 131 mil têm miocardiopatia. O custo anual para tratar os pacientes com doença de Chagas em estágio avançado chega a US$ 175.016.000. Embora o tratamento oportuno mirando o agente etiológico possa postergar significativamente, ou prevenir, a ocorrência de miocardiopatia, ao custo de apenas US$ 30 por paciente, menos de 1% dos portadores da doença de Chagas é tratado na Colômbia e em outros lugares. Representa uma perda de oportunidade de prolongar os anos de vida saudável e produtiva dos pacientes e de reduzir consideravelmente o ônus econômico ao sistema de saúde. Os principais entraves são a complexidade e a demora do processo de diagnóstico e tratamento, a falta de conhecimento sobre a doença por parte dos pacientes e dos profissionais da saúde e os obstáculos administrativos ao nível da atenção primária. Em 2015, interessados diretos de setores do governo, comunidade acadêmica, organizações não governamentais e associações de pacientes participaram de um seminário realizado na cidade de Bogotá em que discutiram como eliminar as barreiras ao diagnóstico e ao tratamento da doença de Chagas. Deste seminário nasceu um modelo para aumentar o acesso dos pacientes à atenção à saúde, com a preparação de um guia simplificado para o diagnóstico e o tratamento e a transição do atendimento dos serviços especializados aos serviços de atenção primária. Como parte de um projeto-piloto, a partir de 2016, o guia do paciente foi implantado em quatro comunidades endêmicas com o propósito de testar e aprimorar o modelo para que possa ser implantado em todo o país. O presente artigo descreve os principais componentes deste modelo de atenção para a doença de Chagas recém-implantado na Colômbia.

BACKGROUND: Nifurtimox is 1 of only 2 medications available for treating Chagas disease (CD) and currently the only drug available in the United States, but its safety and tolerance have not been extensively studied. This is the first study to evaluate tolerance of nifurtimox in US patients with CD. METHODS: This investigation assessed side effects in a sample of 53 patients with CD, all Latin American immigrants, who underwent treatment with nifurtimox (8-10 mg/kg in 3 daily doses for 12 weeks) from March 2008 to July 2012. The frequency and severity of adverse events (AEs) was recorded. RESULTS: A total of 435 AEs were recorded; 93.8% were mild, 3.0% moderate, and 3.2% severe. Patients experienced a mean of 8.2 AEs; the most frequent were anorexia (79.2%), nausea (75.5%), headache (60.4%), amnesia (58.5%), and >5% weight loss (52.8%). Eleven patients (20.8%) were unable to complete treatment. Experiencing a moderate or severe AE (odds ratio [OR], 3.82; P < .05) and Mexican nationality (OR, 2.29; P < .05) were significant predictors of treatment discontinuation, but sex and cardiac progression at baseline were not. Patients who did not complete treatment experienced nearly 3 times more AEs per 30-day period (P = .05). CONCLUSIONS: Nifurtimox produces frequent side effects, but the majority are mild and can be managed with dose reduction and/or temporary suspension of medication. The high frequency of gastrointestinal symptoms and weight loss mirrors results from prior investigations. Special attention should be paid during the early stages of treatment to potentially severe symptoms including depression, rash, and anxiety.

Chagas disease, caused by the protozoan Trypanosoma cruzi, is a highly overlooked parasitic infection within the United States. It affects an estimated 300,000 individuals, often remaining asymptomatic for years before triggering severe complications such as cardiomyopathy in 30-40% of cases. While many contract the disease in Latin America, its transmission by local vectors in the southern U.S. presents a significant challenge. Unfortunately, limited access to diagnosis and treatment persists, alongside unresolved gaps in healthcare systems and disease pathogenesis. In this viewpoint, we discuss the need for focused research and public health initiatives, with U.S. research institutions playing a crucial role in developing new treatments and identifying biomarkers. Furthermore, investigating the genetic variations of T. cruzi between North and South America is vital for improving diagnostic and treatment strategies. BACKGROUND: More than six million people worldwide, particularly in vulnerable communities in Latin America, are infected with Trypanosoma cruzi, the causative agent of Chagas disease. Only a small portion have access to diagnosis and treatment. Both drugs used to treat this chronic, neglected infection, benznidazole and nifurtimox, were developed more than 50 years ago, and adverse drug reactions during treatment pose a major barrier, causing 20% of patients to discontinue therapy. Fexinidazole proved efficacious in an earlier, interrupted clinical trial, but the doses evaluated were not well tolerated. The present study evaluated fexinidazole at lower doses and for shorter treatment durations. METHODS: In this randomised, double-blind, phase 2 trial, we included adult patients (18-60 years old) with confirmed T cruzi infection by serology and PCR and without signs of organ involvement. We evaluated three regimens of fexinidazole-600 mg once daily for 10 days (6·0 g total dose), 1200 mg daily for 3 days (3·6 g), and 600 mg daily for 3 days followed by 1200 mg daily for 4 days (6·6 g)-and compared them with a historical placebo control group (n=47). The primary endpoint was sustained negative results by PCR at end of treatment and on each visit up to four months of follow-up. This study is registered with ClinicalTrials.gov, NCT03587766, and EudraCT, 2016-004905-15. FINDINGS: Between Oct 16, 2017, and Aug 7, 2018, we enrolled 45 patients (n=15 for each group), of whom 43 completed the study. Eight (19%) of 43 fexinidazole-treated patients reached the primary endpoint, compared with six (13%) of 46 in the historical control group. Mean parasite load decreased sharply following treatment but rebounded beginning 10 weeks after treatment. Five participants had seven grade 3 adverse events: carpal tunnel, sciatica, device infection, pneumonia, staphylococcal infection, and joint and device dislocation. Two participants discontinued treatment due to adverse events unrelated to fexinidazole. INTERPRETATION: The fexinidazole regimens in this study had an acceptable safety profile but did not prove effective against T cruzi infection. Development of fexinidazole monotherapy for treating T cruzi infection has been stopped. FUNDING: The Drugs for Neglected Diseases initiative.

BACKGROUND: Chagas disease is a public health challenge in Colombia, where only an estimated 1.2% of people at risk have accessed diagnosis, while less than 0.5% of affected people have obtained treatment. The development of simplified diagnostic algorithms would enable progress in access to diagnosis; however, the current diagnostic algorithm relies on at least two laboratory-based tests that require qualified personnel, processing equipment, and infrastructure, which are still generally lacking at the primary care level. Rapid diagnostic tests (RDTs) for Chagas disease could simplify diagnosis, but their performance in the epidemiological context of Colombia is not well known. METHODOLOGY: A retrospective analytical observational study of RDTs was performed to estimate the operational characteristics of 11 commercially available RDTs designed for in vitro detection of anti-T. cruzi IgG antibodies. The study was performed under controlled laboratory conditions using human serum samples. PRINCIPAL FINDINGS: Eleven RDTs were assessed, ten using 585 serum samples and one using 551 serum samples. Employing the current national diagnostic algorithm as a reference standard for serological diagnosis of chronic infection, the sensitivity of the assessed RDTs ranged from 75.5% to 99.0% (95% CI 70.5-100), while specificity ranged from 70.9% to 100% (95% CI 65.3-100). Most tests (7/11, 63.6%) had sensitivity above 90%, and almost all (10/11, 90.9%) had specificity above 90%. Five RDTs had both sensitivity and specificity above 90%. CONCLUSIONS/SIGNIFICANCE: The evaluation of these 11 commercially available RDTs under controlled laboratory conditions is a first step in the assessment of the diagnostic performance of RDTs in Colombia. As a next step, field studies will be conducted on available RDTs with sensitivity and specificity greater than 90% in this study, to evaluate performance in real world conditions, with the final goal to allow simplified diagnostic algorithms.

Travelers to Chagas disease endemic regions of Latin America may be at risk for Trypanosoma cruzi infection. We report a 67-year-old woman who screened positive for T. cruzi infection while donating blood. The patient had a history of an unusual febrile illness and marked swelling of the face sustained at age 10 after camping in northern Mexico that led to a 3-week hospitalization without a diagnosis. More than 4 decades later, rapid diagnostic tests and commercial and confirmatory serology for Chagas disease were all positive for T. cruzi infection. On evaluation, the patient described a progressive chronic cough, gastroesophageal reflux, and dysphagia for > 10 years. There was no evidence of any cardiac complications. However, esophageal manometry demonstrated significant dysmotility, with 90% of swallows being ineffective with evidence of esophageal pressurization and retrograde peristalsis in several swallows, suggesting early autonomic disruption due to Chagas disease esophagopathy. In this report, we highlight the importance of travel-related Chagas disease among travelers to endemic regions and the need to further identify potential risks of transmission among this at-risk population.

Triatomines (Hemiptera: Reduviidae: Triatominae), commonly called "kissing bugs", are blood-sucking pests and vectors of the protozoan parasite Trypanosoma cruzi, the causative agent of Chagas disease (CD). Eleven species of kissing bugs occur throughout the southern half of the USA, four of which are well known to invade human dwellings. Certain kissing bugs in the USA are known to transmit T. cruzi to humans and other animals and their bites can also lead to serious allergic reactions, including anaphylaxis. In Florida, the kissing bug Triatoma sanguisuga frequently invades homes, bites residents, and has been found infected with T. cruzi, placing humans and companion animals at risk for CD. This review outlines integrated pest management (IPM) strategies for minimizing human exposure to T. sanguisuga and CD. A comprehensive IPM plan for kissing bugs includes detailed inspections, removal of vertebrate host nesting areas, and kissing bug harborage, home improvements to exclude kissing bugs from entering structures, pest removal, and judicious use of pesticides. This approach can limit or eliminate kissing bug entry into residential structures, thereby preventing kissing bug bites, and CD infections in humans and companion animals.

ACC/AHA guidelines recommend STEMI patients receive percutaneous coronary intervention (PCI) at high volume hospitals performing ≥400 procedures/year. The objective of this study was to evaluate changes in the organization and implementation of care for STEMI patients in Florida. We assessed trends and predictors of STEMI patients first hospitalized at high PCI volume hospitals in Florida from 2001-2009. This is the first study to examine statewide trends in hospital admission for all STEMI patients. We classified Florida hospitals by PCI volume (high, medium, low, non-PCI) for each quarter from January, 2001 through June, 2009. Using hospital discharge data, we determined the percent of STEMI patients who went to each type of hospital and analyzed multiple predictors. From 2001-2009 the proportion of STEMI patients first hospitalized at high PCI volume hospitals rose from 62.4 to 89.7%, while admissions to non-PCI hospitals declined from 31% to 4.9%. Persistent barriers to high PCI volume hospital admission were age ≥85 years (OR 0.56, 95% CI 0.50-0.62), female gender (OR 0.85, 95% CI 0.79-0.91), and residence in a major metropolitan county. Through the efforts of local coalitions throughout Florida, by 2009 almost 90% of Florida STEMI patients were first admitted to high PCI volume hospitals. Greater hospital competition may explain lower admission rates to high PCI volume hospitals in major metropolitan counties. The age and gender disadvantage we observed requires further research to determine potential causes.

Chagas disease is considered one of the most important neglected tropical diseases in the Western Hemisphere, given its morbidity, mortality, and societal and economic burden. The United States has the fifth highest global burden of Chagas disease. Every year, thousands of migrant people from Latin America and throughout the globe travel to the U.S.- Mexico border searching for asylum. The U.S. CDC's Guidance for the U.S. Domestic Medical Examination for Newly Arriving Refugees provides recommendations to safeguard the health of individuals who enter the United States with a humanitarian-based immigration status as defined by the CDC's guidance under Key Considerations and Best Practices. We encourage the inclusion of Trypanosoma cruzi infection screening recommendations in this guidance as an important step toward understanding the risk and burden of Chagas disease in this vulnerable population, strengthening their access to care and contributing to the 2030 objectives of the WHO's neglected tropical diseases road map.

Colombia has one of the largest burdens of Chagas disease globally, with about 438,000 people affected according to 2015 estimates. Despite this, < 1% of the population has had access to diagnosis and treatment. A patient-centered roadmap for Chagas disease was developed from 2015 onwards to address access barriers and increase diagnostic and therapeutic coverage and was implemented in five municipalities where Chagas disease is endemic. The mean number of people tested per year increased from 37 before the project to 262 following implementation, and the average days between medical order and diagnostic confirmation results decreased from 258 to 19. The mean days from diagnostic confirmation to treatment initiation decreased from 354 before the project to 135 after implementation. The 5,654 people tested included 3,467 women of childbearing age. The prevalence of T. cruzi infection was 11.5%, and thus far 266 people have received antitrypanosomal treatment. Collaborative creation and implementation of a patient-centered roadmap can address access barriers in specific contexts, helping to reduce the invisibility and burden of this neglected disease.

Aurora, commonly seen in the polar sky, is a ubiquitous phenomenon occurring on Earth and other solar system planets. The colorful emissions are caused by electron beams hitting the upper atmosphere, after being accelerated by quasistatic electric fields at 1-2 R(E) altitudes, or by wave electric fields. Although aurora was studied by many past satellite missions, Cluster is the first to explore the auroral acceleration region with multiprobes. Here, Cluster data are used to determine the acceleration potential above the aurora and to address its stability in space and time. The derived potential comprises two upper, broad U-shaped potentials and a narrower S-shaped potential below, and is stable on a 5 min time scale. The scale size of the electric field relative to that of the current is shown to depend strongly on altitude within the acceleration region. To reveal these features was possible only by combining data from the two satellites.

STUDY OBJECTIVE: Current guidelines recommend that ST-elevation myocardial infarction (STEMI) patients receive percutaneous coronary intervention less than or equal to 90 minutes from first medical contact, preferably at high-volume percutaneous coronary intervention centers (≥400 percutaneous coronary interventions annually). Because many patients present to low-volume or non-percutaneous coronary intervention-capable STEMI referral hospitals, timely percutaneous coronary intervention treatment requires effective transfer systems, which include interfacility transport times of less than 30 minutes. We investigate the geographic feasibility of achieving timely interfacility transport from STEMI referral hospitals to percutaneous coronary intervention hospitals in Florida. METHODS: Using 2006 Florida hospital discharge data, we calculated driving times between STEMI referral hospitals and the nearest medium-/high-volume percutaneous coronary intervention centers. We plotted transfer travel time cumulative proportion survival curves for hospitals and patients to assess the feasibility of transfer within 30 minutes to higher-volume facilities. Differences by geographic location (rural versus urban) and patient race/ethnicity were examined. RESULTS: In 2006, 77% of STEMI referral hospitals had transfer travel times within 30 minutes; 90th percentile for interhospital driving time was 56 minutes. For patients at STEMI referral hospitals, 85.6% were at facilities within a 30-minute drive of a high-/medium-volume percutaneous coronary intervention center; 90th percentile was 31 minutes. We found marked rural/urban disparities, with longer average driving times for patients in rural and small metropolitan counties. Significant racial/ethnic disparities in transfer travel times were not observed, although 90th percentile driving times were highest for blacks. CONCLUSION: Driving times do not pose a major geographic barrier to transfer of STEMI patients in Florida. A majority of STEMI patients could be transferred from STEMI referral hospitals to high-volume percutaneous coronary intervention centers within 30 minutes.

BACKGROUND: Chagas disease (CD), caused by the parasite Trypanosoma cruzi, affects ~6-7 million people worldwide. Significant limitations still exist in our understanding of CD. Harnessing individual participant data (IPD) from studies could support more in-depth analyses to address the many outstanding research questions. This systematic review aims to describe the characteristics and treatment practices of clinical studies in CD and assess the breadth and availability of research data for the potential establishment of a data-sharing platform. METHODOLOGY/PRINCIPAL FINDINGS: This review includes prospective CD clinical studies published after 1997 with patients receiving a trypanocidal treatment. The following electronic databases and clinical trial registry platforms were searched: Cochrane Library, PubMed, Embase, LILACS, Scielo, Clintrials.gov, and WHO ICTRP. Of the 11,966 unique citations screened, 109 (0.9%) studies (31 observational and 78 interventional) representing 23,116 patients were included. Diagnosis for patient enrolment required 1 positive test result in 5 (4.6%) studies (2 used molecular method, 1 used molecular and serology, 2 used serology and parasitological methods), 2 in 60 (55.0%), 3 in 14 (12.8%) and 4 or more in 4 (3.7%) studies. A description of treatment regimen was available for 19,199 (83.1%) patients, of whom 14,605 (76.1%) received an active treatment and 4,594 (23.9%) were assigned to a placebo/no-treatment. Of the 14,605 patients who received an active treatment, benznidazole was administered in 12,467 (85.4%), nifurtimox in 825 (5.6%), itraconazole in 284 (1.9%), allopurinol in 251 (1.7%) and other drugs in 286 (1.9%). Assessment of efficacy varied largely and was based primarily on biological outcome; parasitological efficacy relied on serology in 67/85 (78.8%) studies, molecular methods in 52/85 (61.2%), parasitological in 34/85 (40.0%), microscopy in 3/85 (3.5%) and immunohistochemistry in 1/85 (1.2%). The median time at which parasitological assessment was carried out was 79 days [interquartile range (IQR): 30-180] for the first assessment, 180 days [IQR: 60-500] for second, and 270 days [IQR: 18-545] for the third assessment. CONCLUSIONS/SIGNIFICANCE: This review demonstrates the heterogeneity of clinical practice in CD treatment and in the conduct of clinical studies. The sheer volume of potential IPD identified demonstrates the potential for development of an IPD platform for CD and that such efforts would enable in-depth analyses to optimise the limited pharmacopoeia of CD and inform prospective data collection.

Chagas disease is the neglected tropical disease of greatest public health impact in the United States, where it affects over 300,000 people. Diverse barriers limit healthcare access for affected people; fewer than 1% have obtained testing or treatment. We interviewed 50 people with Chagas disease in Los Angeles, California, and administered a cultural consensus analysis questionnaire. Participants were asked about their experiences and perceptions of Chagas disease, access to healthcare, and strategies for coping with the disease. In participants' narratives, the physical and emotional impacts of the disease were closely interwoven. Participant explanatory models highlight difficulties in accessing care, despite a desire for biomedical treatment. Obtaining testing and treatment for Chagas disease poses substantial challenges for US patients.

Chagas disease (CD) poses a major public health challenge for the Americas and non endemic regions around the world. This study discusses the legal framework surrounding access to healthcare for CD for Bolivian migrants living in São Paulo, Brazil. While recent guidelines stipulating care for CD exist, there is a lack of legal provisions to ensure they are regularly implemented. Bolivian migrants in SP have specific needs, including language differences and a high level of mobility. Interviews were conducted with ten participants representing public health institutions or organizations working with the Bolivian migrant community. Additionally, a review was conducted of legal, official, and health policy documents pertaining to rights of Bolivian migrants in SP. Although the right to healthcare is constitutionally guaranteed for all, in practice, immigrants, especially those without documentation, encounter barriers to initiating treatment for CD. Providing the primary health care system (SUS) card would not only improve access to healthcare for Bolivian migrants, but also provide a potential pathway toward regularization of status. The approval of clinical protocols and therapeutic guidelines for CD (2018) represents an opportunity to improve care for all Brazilians with CD. Programs with multidisciplinary teams should be developed taking into account the specific social and cultural needs of this population.
