RESUMO

This review deals with the pharmacological and psychological means to help in smoking cessation and compares the efficacy of the different methods. Pharmacological support results in a smoking cessation rate reaching at best 20-25%. The efficacy of behavioural and cognitive therapy have been much less validated so far. Multidisciplinary specialized centres for helping smokers have been raised under the care of FARES in the French Community of Belgium.

BACKGROUND: Because of the expected high performances of scintigraphic scans with [18F]-fluorodeoxyglucose (FDG) not only in diagnostics and but also in therapeutic impact, especially in thoracic oncology, there are a lot of French nuclear medicine departments which will soon be equipped with a positron emission tomograph (PET). MATERIAL AND METHODS: The Nuclear Medicine Department of the Hôpital d'Instructions des Armées du Val-de-Grâce, Paris, led a retrospective study among physicians interested in 338 FDG-PET exams performed between may 2000 and march 2002 in order to compare its own results with international literature concerning four indications for lung cancer: pulmonary nodule or mass malignancy diagnostic, lung carcinoma extension evaluation, therapeutic efficiency, recurrence suspicion. RESULTS: There seems to be no divergence, regarding limitation induced by the not exhaustive analysis of the retrospective study: more than every two FDG-PET exam highly influenced the effective therapy. CONCLUSION: That is why clinical FDG-PET has to be widely developed to investigate lung cancer.

RESUMO

We report the case of a 50-yr-old man who experienced exertional dyspnea 5 mo after a left pneumonectomy for carcinoma. As the clinical features pointed toward a pulmonary embolism, we performed a ventilation plus perfusion radionuclide lung scan. It showed no evidence of pulmonary embolism, but it did show a systemic uptake of the isotope, suggesting a right-to-left shunt that was confirmed by contrast echocardiography, which revealed an atrial septal defect. Right-to-left shunts after pneumonectomy have already been reported and can be diagnosed by lung scintigraphy. Usually, a patent foramen ovale is encountered, but the underlying physiopathology remains under discussion. Clinically, right-to-left shunts are often related to platypnea-orthodeoxia.

RESUMO

Myeloid metaplasia with myelofibrosis develops in about 10% of patients with polycythemia vera. We report a case of a 48-year-old female with postpolycythemic myelofibrosis successfully treated with allogeneic HLA-matched bone marrow transplantation.

A sterilisable radiation probe of small dimensions was designed to locate the lesions at orthopaedic surgical sites according to the procedure of intraoperative bone scintigraphy. The probe has a collimated opening 2 mm in diameter. It is connected to a portable radioactivity counter which converts the disintegration rates detected at surgical sites into an acoustic signal that increases steeply with increasing disintegration rate. The acoustic signal enables the surgeons and isotope specialists to readily monitor radioactivity in the region of interest without attention being distracted from the surgical site. Dimethylaminodiphosphonate (designated SF44) was the osteotropic radiopharmaceutical chosen for carrying out intraoperative bone scintigraphy, since the available data show that this chemical increases the pathological: normal bone uptake ratio of the lesion by 25% compared to the usual diphosphonates. Forty-seven orthopaedic interventions were carried out according to the intraoperative bone scintigraphy procedure. They showed that this procedure facilitated the rapid location of the lesion, the objective termination of the operation, less frequently the reduction in dimension of the excised areas, and rarely the simplification of the surgical technique. Practice of intraoperative bone scintigraphy requires proper training and caution.

OBJECTIVE: Improvement of neurosurgical techniques with a more detailed description of brain tumors and their functional environment. METHODS: We performed: (1) anatomical magnetic resonance imaging (MRI) for reference, (2) functional sequences dedicated to the adjacent cortical structures (sensorimotor, visual, language paradigms), and (3) thallium 201 cerebral tomoscintigraphy to visualize active tumor invasion. Data were transferred to a workstation for automatic registration. RESULTS: All data were combined into one synthetic image showing the foci of high proliferative activity, which have to be completely resected, and the peritumoral functional structures, which have to be spared in order to minimize postoperative sequelae. This trimodal image is entered into a surgical neuronavigation computer for preoperative planning in order to outline tumoral target and functional risk areas. All this information is displayed in the operative microscope (Zeiss MKM) optically linked to MR images. This multimodality technique diminishes operative time by reducing electrocorticography and improves the operative short-term outcome. CONCLUSION: Multimodal imaging is useful for optimization of neurosurgical tumor resection.

The optimisation of the field of irradiation is the aim of the radiotherapist. Pet-Scan provides information revealing a better cartography of node involvement and allowing a better delineation of the tumour zone. In the follow-up of irradiated patients, PET-Scan provides information on the nature of residual lesions and a possible recurrence.

This study was performed to evaluate the incidence ant the practical consequences of left ventricular diastolic dysfunction in hypertensive. In 70 mild to moderate hypertensive subjects group [systolic 161 +/- 16 and diastolic blood pressure 104 +/- 9 mmHg 18 women, 52 men, 51 +/- 7 years old] and in a 15 normal subjects control group, the peak filling rate (PFR) and the time to peak filling rate (TPFR) were measured with the time/activity curve of the rest equilibrium blood pool scintigraphy. The ejection fraction and the stress test were normal in all patients [EF 0.66 +/- 0.05, ranging from 0.59 to 0.88]. The PFR was not significantly different in the hypertensive group but 59/70 patients [84 p. 100] showed an individual value lower than the theoretical age and heart rate expected value. The TPFR was not significantly different (183 +/- 33 ms-vs 180, p = ns). In a Holter-defined sub-group of patients (n = 22) exhibiting a high prevalence of supra-ventricular premature beats or a paroxysmal atrial fibrillation, the PFR was significantly slower than in the total hypertensive group [1.92 +/- 0.33 EDV/s-1, p = 0.02]. Early indices of diastolic function give some instantaneous information on left ventricular filling. Determining the exact significance of individual values of PFR and TPFR requires a better knowledge of physiologic and pathologic determinants of LV filling.

Because of discrepancy in interpretation of early diastolic filling indices in normal subjects and hypertensive, we studied the correlations between age and radionuclide angiographic peak filling rate (PFR), doppler echocardiographic early E and late A waves, left ventricular mass (LVM), blood pressure (BP) and ejection fraction (EF) in cautiously screened 30 untreated hypertensive and 30 age paired normal subjects (mean of age 52 +/- 17 ranging from 34 to 78 years). No patient had gross obesity nor coronary artery disease. Univariate analysis revealed strong correlations between LV filling and age in normal (r = -0.82 p less than 0.0001) and hypertensive (r = -0.61 p less than 0.001), with a very significant difference in y intercepts (t = 0.61 p = 10(-6)). LVM correlated poorly with age (r = 0.35 p less than 0.05) but with none of the LV filling indexes. BP correlated with PFR (r = 0.33 p less than 0.05) and A wave (r = 0.44 p less than 0.02) in hypertensive only. After multivariate analysis, significant dependencies of PFR, age, LV mass were more accurate if BP was in a higher range. The variability of the values of LV filling indexes was wider in hypertensive than in normotensive. Normotensive aging and hypertension have similar effects on the cardiovascular system. In the most aged people even without apparent cardiac disease, it is not possible to identify the specific effects of hypertension on diastolic function.

In order to establish which indices of early diastolic function obtained by Doppler echocardiography and the time/activity curve of 99mTc radionuclide angiography are the most useful clinically, the two investigations were performed in 41 subjects aged 52 +/- 15 years. The maximum velocity of LV filling and mitral E wave provided the best correlation (r = 0.48); the Doppler A wave did not correlate with its scintigraphic homologue. The correlations with the morphological data (mass, thickness/radius ratio) were better with radionuclide angiography. Variance analysis of E and maximum velocity of filling showed that gamma angiography was significantly more accurate (FE LVF greater than 10 p = 10 - 4); on the other hand, the A wave best illustrated the transfer of filling to late diastole. In conclusion, left ventricular filling may be assessed clinically by Doppler echocardiography; the greater accuracy of the radionuclide measurements with respect to left ventricular volumes makes this a more appropriate method when quantitative information is required.

In some patients with coronary disease hyperactivity of the lungs is commonly observed when the myocardial scintigraphic images are recorded during exercise. Pulmonary activity of radionuclide tracers has been reported in the literature and quantified in different ways. The present study contributes to this quantification by suggesting a new index: the pulmonary activity to cardiac activity percentages ratio during exercise and blood redistribution. The value of the new index has been evaluated comparatively in 11 subjects with normal coronary arteries (0.76 +/- 0.09) and in 39 patients with angiographically proven coronary disease (0.92 +/- 0.10; p less than 0.001). Remarkably, this index hardly varies in normal subjects and therefore is a highly specific aid to diagnosis. In coronary patients, multivariate analysis has shown that it correlates in decreasing order of intensity with: (1) a left ventricular end-diastolic pressure of 14 mmHg or more; (2) a moderate rise in heart rate at exercise; (3) a moderate heart work; (4) an ST depression of 2 mm or more; (5) a multilocular myocardial ischaemia at scintigraphy. Our index did not prove capable of discriminating between one-, two- or three- vessel diseases. In view of the data obtained in this study and those found in the literature, this index should be regarded as a reliable marker of left ventricular dysfunction during stress. Being relatively inexpensive and easy to obtain, it complements myocardial scintigraphy and may be useful in clinical practice.

UNLABELLED: Cardiac autonomic neuropathy is a frequent complication of diabetes leading to resting tachycardia, postural hypotension, painless myocardial ischaemia, rhythm disturbances and sudden cardiac death. The aim of the study was to evaluate in a diabetic population the sensitivity of two exploration modes of autonomic neuropathy in diabetics: the Ewing tests which are, at present time, the reference method and the (123-I) meta-iodo-benzyl-guanidine (MIBG) single photon emission computed tomography (SPECT) which evaluates the cardiac sympathetic innervation. PATIENTS AND METHODS: 9 male insulin-dependent diabetes mellitus patients were studied. Mean age was 40.7 +/- 15 years and diabetes duration was 10.8 +/- 6 years. None had hypertension or macroangiography as demonstrated by patient's history, clinical examination, rest and exercise electrocardiography and ambulatory blood pressure monitoring. The complications observed were background retinopathy in 2 patients, incipient nephropathy in 3 and a peripheral neuropathy in 1 patient. Ewing tests, i.e. Valsalva maneuver, beat to beat heart rate variation during deep breathing and standing, blood pressure response to standing and to sustained handgrip, were performed. The results were considered as pathologic when the score was over 2. After injection of 10 mCi (123-I) MIBG, planar images were realized at times 1, 2 and 4 hours and SPECT images after 2 hours. The heart/mediastinum uptake ratio was calculated. RESULTS: We noted abnormalities of planar images in 3 patients, SPECT images in 1, and both in 1 patient. None was positive for Ewing tests. CONCLUSION: Although MIBG SPECT will explore only the sympathetic innervation, these preliminary findings suggest that this technique could be more sensitive for the evaluation of cardiac autonomic neuropathy. Nevertheless cost and lack of disponibility of this technique should limit its use.

CT and MRI are not consistently able to differentiate central nervous system (CNS) lymphoma from focal cerebral infection such as toxoplasmosis in the human immunodeficiency virus (HIV) involvement. In this study thallium 201 (and/or technetium-MIBI) SPECT was performed in 6 patients suffering from AIDS and CNS lesions. An index based on the ratio of thallium uptake in the lesion vs the contralateral scalp was calculated. In 4 out of 5 patients with lymphoma (3 confirmed by biopsy, 2 highly suspected on CT and resistance to antitoxoplasmosis treatment) focal lesions showed high uptake of thallium. On the other hand one markedly necrotic lymphoma and all infectious lesions did not take up thallium. This suggests a role for thallium 201 brain SPECT in the workup of focal CNS lesions in AIDS.

RESUMO

The authors present a case of tricuspid regurgitation which reveals a carcinoid syndrome in an 68 year-old woman discovered in view of right heart failure. This carcinoid heart disease is early without symptoms of the primitive or the liver metastatic tumors. We see a good evolution with somatostatin's analogue. We review the diagnosis features in trans-thoracic and trans-oesophageal echocardiography, and the possibility of surgical treatment of these lesions.

PURPOSE: To report magnetic resonance imaging abnormalities in reflex sympathetic dystrophy of the foot. METHODS: Retrospective study of 22 algodystrophies of the foot, in warm phase in 17 cases, in cold phase in 5. RESULTS: Algodystrophy in warm phase: Bone medullary abnormalities were noted in 17 cases (decrease of signal intensity was found in T1 weighted images, increase of signal intensity in T2 weighted images, in T1 and T2 with fat-saturation, in T1 with gadolinium), located at the increased uptake technetium site in 16 cases. T1 and T2 weighted images with fat-saturation and T1 with fat-saturation after injection of gadolinium were pathological in all cases, T1 was normal in 2 cases. Soft tissues abnormalities were noted in 11 cases, joint effusion in 8 cases, synovial hypertrophy enhanced by gadolinium in 2 cases and a subchondral linear area of hypointense signal on T1 and T2 images was not present after gadolinium injection in 1 case. Six fractures were detected. Algodystrophy in cold phase: no bone edema, no synovial hypertrophy, no joint effusion, no soft tissues abnormalities, no fractures are detected. CONCLUSION: The results suggest that the MRI has a considerable value in diagnosis during the warm phase of reflex sympatetic dystrophy of the foot. The normal MRI findings during the cold phase could be important to understand its etiology.

To increase our understanding of the interaction between anti-hypertensive drug therapy and left ventricular filling, 23 hypertensive patients (DBP 100 +/- 4 mmHg) had their myocardial mass measured and underwent gamma-angiography (determination of maximum volume of filling, MVF, end-diastolic volume/s-1, and the maximum time for filling, MTF, ms) before and after therapy with propranolol (100 mg/d), captopril (50 mg/d), nitrendipine (28 mg/day) or xipamide (20 mg/d). Blood pressure was recorded in the four treatment groups, and myocardial mass regressed only in those treated with propranolol or nitrendipine. Only the latter drug acts simultaneously on MVF (2.19 +/- 0.22 vs 2.47 +/- 0.43 EDV/s -1) and MTF (221 +/- 21 vs 192 +/- 20 ms) significantly (p less than 0.05). Xipamide only improved MTF (228 +/- 28 vs 209 +/- 20 ms; p less than 0.05). These data compared to those of the literature demonstrate the variability in the antihypertensive effects on ventricular filling, while its improvement may be proposed as a usual objective in treatment of hypertensive disease.

In order to evaluate the action of captopril on left ventricular filling in hypertension, 14 hypertension (158 +/- 10/101 +/- 5 mmHg) patients aged 51 +/- 6 years were investigated by Technetium 99m gamma-angiography. The time/activity curve was used to determine the maximum filling rate (MFR) and maximum filling time (MFT) of the ventricle before and after treatment with captopril (mean dose : 44 +/- 26 mg/day for 7 months). Blood pressure was significantly lowered by treatment and there was a decrease in left ventricular mass from 128 +/- 17 to 118 +/- 15 g/m2 (p = 0.07). Maximum filling rate was accelerated by treatment from 2.27 +/- 0.57 to 2.57 +/- 0.43 VTD . s-1, p = 0.005). This variation was due essentially to half of the patients, suggesting an "all or nothing" type response. Maximum filling time did not vary. The basic question raised by this type of study is to know whether the improvement in the available relaxation parameter, MFR, was associated with actual improvement in filling, which is the true aim. Although the explanations offered for the observed findings are hypothetical, taking into account of all the trial data together with the morphological data provided by echocardiography suggests that captopril does have an actual and hemodynamically significant action on filling.

The effects of a nitroglycerin transdermal therapeutic system (TTS NG 10 mg/24 h) 24 hours after application were evaluated by exercise left ventricular radionuclide cineangiography. 20 patients with ischemic heart disease were included in a double-blind, within patient, acute study comparing TTS NG to placebo. Were measured: exercise-test parameters and global (EF) and regional (rEF) ejection fractions at rest and during exercise. At maximum load, the double product rose from 18,680 +/- 4,118 with placebo to 21,538 +/- 3,988 with TTs NG (p less than 0.01) and ST-segment depression at same load decreased from 2.6 +/- 0.7 mm with placebo to 1.0 +/- 0.7 mm with TTS NG (p less than 0.001). Exercise EF increased from 0.57 +/- 0.15 with placebo to 0.63 +/- 0.12 with TTS NG (p less than 0.001) and was higher at exercise than at rest. Two hypokinetic territories improved during exercise with TTS NG: basal rEF increased to 0.45 +/- 0.07 with TTS NG versus 0.31 +/- 0.13 with placebo (p less than 0.01); anteroseptal rEF increased to 0.49 +/- 0.14 versus 0.43 +/- 0.14 with placebo (p 0.05); no significant improvement was observed in anterolateral and inferior territories were previous myocardial infarction occurred predominantly. These data demonstrate the 24 hours efficacy of this new galenic form of TTS NG in ischemic heart disease on exercise test parameters. TTS NG improving predominantly regional kinetics of ischemic but non infarcted territories and thus improving EF at exercise.

RESUMO

A NEW FORM OF MEDICAL IMAGING: Positron emission tomography (PET) is used for the non-invasive in vivo visualisation of biochemical cell processes. It reveals the metabolic characteristics of neoplastic lesions and hence their identification by compensating the lack of lesion specificity of radiological techniques. VARIOUS INDICATIONS: Using the current oncology marker, 18F-fluorodeoxyglucose (FDG), excellent results with PET have been established at all stages of neoplasia, notably for the diagnosis of initial malignancy and the identification of residual lesions and early detection of relapses. Moreover, the fact that the whole of the body can be explored makes PET the tool of choice in the control of the extension and operability of cancers. With the close correlation between imaging and the metabolism of the lesions, PET is the earliest and most precise for assessing the effects of treatment. LIMITS AND PERSPECTIVES: The existence of benign inflammatory FDG binding should lead to the development of markers of other metabolisms directly linked to cell proliferation. The lack of anatomical reference points characteristic of PET does not permit the precise localisation of the lesions detected and could be corrected by combining, in a single apparatus, the PET camera and an X scan, the anatomical resolution of which is irreplaceable. This type of equipment represents the development of a new branch of medical imaging, oncological imaging.
