RESUMO

The main objective of the German Environmental Specimen Bank (ESB) is the long-term storage of environmental and human samples under stable deep-freeze conditions for future research. The ESB is unique in providing a continuous historical record of environmental and human exposure to chemicals in Germany. ESB was started parallel to the development of the first German Chemicals Legislation in the late 1970s. In 1979, the ESB test operation began. After the Chemicals Law came into force in 1982, the ESB was established as a permanent facility in 1985. With the new European Chemicals Legislation, REACH, in 2007 responsibility for the safety of commercial chemicals and risk assessment was assigned to the industry. Since then, the ESB has become even more important in verifying the self-assessment of the industry, in evaluating the effectiveness of regulations, thus ensuring the protection of humans and the environment against adverse effects caused by exposure to chemicals. These objectives are pursued by the regular monitoring of contaminations and the assessment of temporal trends. Demonstrating the necessity of deriving exposure reduction measures, ESB results serve as key information for policy-makers. Information on preventing exposure to chemicals is available to the general public and to the public health services. The ESB is thus an important monitoring instrument of the Federal Ministry for the Environment, Nature Conservation, Building and Nuclear Safety. The Federal Environment Agency operates the ESB based on its own concepts, heads the scientific data evaluation and transfers results into the environmental policy arena and to the general public.

The German Environmental Specimen Bank for Human Tissues (ESBHum) as part of the German Environmental Specimen Bank (ESB) focuses on documenting and assessing trends of human exposure via real-time monitoring of body burden and long-term storage of samples under stable deep freezing conditions (-150 degrees C) for later retrospective analyses. Real-time monitoring is performed after completing sampling processes of one year and covers actually 20 inorganic and 5 organic substances. While concentrations of several substances, e.g., arsenic, cadmium and mercury, are remained unchanged over time, other substances, e.g., lead and pentachlorophenol (PCP), show a clearly perceptible decrease. Substances which are not routinely analyzed in real-time monitoring are retrospectively measured by indication in the stored human specimens. Indications of retrospective monitoring are availability of valid analytical methods, e.g., in case of PCDF and PCDD, or assessment of concentration trends of substances with actual interest of toxicology and/or environmental medicine, e.g., polybrominated diphenyl ethers (PBDE), perfluorooctane sulfonate (PFOS) and perfluorooctanoic acid (PFOA). While over time the body burden of dioxins as well as PFOS and PFOA decreased, the PBDE concentrations in human blood increase. The observed decrease of blood lead and PCP levels over time is a consequence of legal prohibition and restriction. The time-dependent concentrations of the aforementioned substances agree with results of other national studies. So it can be concluded that the German ESBHum is an important instrument for health-related environmental observation and protection in Germany.

Human health risk assessments for dioxins and dioxin-like PCBs (with the exception of the one by US-EPA) recommend health based exposure limits within the range of 1-4 pg WHO-TEQ/kg bw per day. As all humans are exposed to measurable levels of dioxins and related substances, the determination of the tolerated daily intake is a very significant decision and may influence limit values guiding risk reduction measures and target levels. The proposed TDI has to protect all human subpopulations. In the case of dioxin this is particularly important as the exposure of infants through breast-feeding may exceed the exposure of adults by one or two orders of magnitude. An overview of recently recommended limit values (WHO, SCF, JECFA) for PCDDs, PCDFs and dioxin-like PCBs using WHO-TEFs shows the common feature that the values were derived only from non carcinogenic endpoints. In November 2000 the Scientific Committee on Food of the European Commission published an 'Opinion of the SCF on the Risk Assessment of Dioxins and Dioxin-like PCBs in Food' [SCF, Scientific Committee on Food 2000. On the basis of this extensive review of data and experimental results the Committee recommended a temporary tolerable weekly intake (t-TWI) of 7 pg WHO-TEQ/kg bw. Only six months later the SCF carried out a re-evaluation of its t-TWI from November 2000. The reconsideration of 'pivotal studies' led to the situation that the re-assessment is now based only on rat studies which investigated only reproductive effects only on male offspring and, in addition, three of these studies are single dose studies at gestational day 15. Applying an overall uncertainty factor of 10 to the LOAEL derived estimated human daily intakes (EHDI) the SCF concluded that 14 pg/kg bw per week should be considered as a tolerable intake for 2,3,7,8-TCDD. The SCF stated that on a body weight basis, the dioxin intake of breast-fed infants has been estimated to be one to two orders of magnitude higher than the average adult intake. Recent German data suggest that the body burden of formerly breast-fed children aged 9-11 is still about 30% higher than those of their formula-fed age-mates. As breast-feeding has measurable benefits for neurological and immunological development, formula feeding cannot be recommended as an alternative to lower dioxin intake. So the only remaining way to lower the dioxin uptake is to drastically reduce the background exposure of the general population. It is acknowledged that any recommendation of a precise number for a TDI is flawed by uncertainties and the possibility of different weight being given to the studies of relevance. The determination of the TDI has influence on all regulatory limit values that are based on the TDI value. A higher TDI lowers the level of protection for humans. It is proposed by the German Federal Environmental Agency that the TDI should be reassessed in a process transparent to the public and on the basis of all relevant endpoints from animal experiments and human epidemiology, including the assessment of cancer risks.

Health-relevant exposures to environmental pollutants, fungi, bacteria, noise, and air pollution have to be identified at an early stage. At the same time, impacts on health and their potential environmental causes need to be investigated and documented. The German Environmental Survey for Children and Adolescents 2014-2017 (GerES V) is the environmental module of KiGGS Wave 2 of the Robert Koch Institute and takes a deeper look at the sections living conditions and health status of the KiGGS study. GerES V collects up-to-date information on the exposure of children and adolescents in Germany aged 3 to 17 to chemicals and investigates chemical and physical environmental pollutants in their living environments. The survey contributes to identifying environmental hazards and measures that effectively reduce or prevent such hazards in order to protect and promote the health of the young generation.

RESUMO

A workshop was held in Berlin September 12-14th 2012 to assess the state of the science of the data supporting low dose effects and non-monotonic dose responses ("low dose hypothesis") for chemicals with endocrine activity (endocrine disrupting chemicals or EDCs). This workshop consisted of lectures to present the current state of the science of EDC action and also the risk assessment process. These lectures were followed by breakout sessions to integrate scientists from various backgrounds to discuss in an open and unbiased manner the data supporting the "low dose hypothesis". While no consensus was reached the robust discussions were helpful to inform both basic scientists and risk assessors on all the issues. There were a number of important ideas developed to help continue the discussion and improve communication over the next few years.
