BACKGROUND: Childhood cancer is one of the main causes of child mortality and its treatment has debilitating effects on the oral cavity. Several oral mucositis (SOM) is one of the most common and may cause undesirable symptoms such as pain and risk of systemic infection. MATERIAL AND METHODS: This was a longitudinal, retrospective, and observational study determining the incidence of severe oral mucositis (SOM) and its occurrence sites in pediatric oncologic patients, in João Pessoa, Brazil, between 2013 and 2018. Data from 56 patients aged 1 to 18 years were collected from their medical records and through an oral mucosa examination, from the 1st to 5th week of chemotherapy treatment (CT) using the modified Oral Assessment Guide, by previously calibrated examiners (Kappa index > 0.7). The data were analyzed by the Chi-square test, and Odds Ratios were calculated. RESULTS: Most patients were females (54.5%), aged 8.8 years (± 4.8), with hematologic tumors (73.2%), predominantly Acute Lymphoid Leukemia (50.0%). An increase in the occurrence of SOM was observed throughout the CT (P = 0.05), ranging from 12.5% in the 1st to 35.7% in the 5th CT week. In the 1st CT week, there was a predominance of alterations in the lips (5.5%) and saliva (5.5%), while in the 5th, the jugal / palate mucosa (21.4%) remained the most affected site by SOM. Differences in the severity of SOM in the jugal / palate mucosa (P = 0.01) and labial mucosa (P = 0.04) were observed over time. In the 5th CT week, the likelihood of developing SOM was 13.3-fold higher (95% CI: 1.5 - 105.6) in patients with hematologic tumors. CONCLUSIONS: The incidence of SOM was higher in the 5th CT week, most commonly affecting the jugal / palate mucosa, and patients with hematologic tumors were more prone to develop SOM.

The objective of this study was to assess worker exposure to mineral dust particles, and a metabolic model, based on the model adopted by ICRP, was applied to assess human exposure to Ta, and predicted values of Ta concentrations in excreta. The occupational exposure to Th, U, Nb, and Ta-bearing particles during routine tasks to obtain Fe-Nb alloys was estimated using air samplers and excreta samples. Ta concentrations in food samples and in drinking water were also determined. The results support that workers were occupationally exposed to Ta-bearing particles, and also indicate that a source of Ta exposure for both workers and the control group was the ingestion of drinking water containing soluble compounds of Ta. Therefore, some Ta compounds should be considered soluble compounds in gastrointestinal tract. Consequently, the metabolic model based on ICRP metabolic model and/or the transfer factor f 1 for Ta should be reviewed and the solubility of Ta compounds in gastrointestinal should be determined.

Ofloxacin is an antimicrobial agent frequently found in significant concentrations in wastewater and surface water. Its continuous introduction into the environment is a potential risk to non-target organisms or to human health. In this study, ofloxacin degradation by UV/TiO2 and UV/TiO2/H2O2, antimicrobial activity (E. coli) of samples subjected to these processes, and by-products formed were evaluated. For UV/TiO2, the degradation efficiency was 89.3% in 60 min of reaction when 128 mg L(-1) TiO2 were used. The addition of 1.68 mmol L(-1) hydrogen peroxide increased degradation to 97.8%. For UV/TiO2, increasing the catalyst concentration from 4 to 128 mg L(-1) led to an increase in degradation efficiency. For both processes, the antimicrobial activity was considerably reduced throughout the reaction time. The structures of two by-products are presented: m/z 291 (9-fluoro-3-methyl-10-(methyleneamino)-7-oxo-2,3-dihydro-7H-[1,4]oxazino[2,3,4-ij]quinoline-6-carboxylic acid) and m/z 157 ((Z)-2-formyl-3-((2-oxoethyl)imino)propanoic acid).

This paper presents a degradation study of the pesticide atrazine using photo-assisted electrochemical methods at a dimensionally stable anode (DSA(®)) of nominal composition Ti/Ru(0.3)Ti(0.7)O(2) in a prototype reactor. The effects of current density, electrolyte flow-rate, as well as the use of different atrazine concentrations are reported. The results indicate that the energy consumption is substantially reduced for the combined photochemical and electrochemical processes when compared to the isolated systems. It is observed that complete atrazine removal is achieved at low current densities when using the combined method, thus reducing the energy required to operate the electrochemical system. The results also include the investigation of the phytotoxicity of the treated solutions.

The present study assessed the application of [(3)H]Leucine incorporation into protein by periphytic bacteria associated with the roots of the floating aquatic macrophyte Eichornia crassipes. Basic assumptions underlying the method, such as linearity of leucine incorporation, saturation level of incorporation rates, incorporation into other macromolecules, specificity of incorporation for bacterial assemblages and [(3)H]Leucine degradation during samples storage were tested, and two procedures for extracting the incorporated leucine were compared. Both methods gave the same results, however, the hot TCA extraction method was less time consuming than the alkaline extraction method. Incorporation of [(3)H]Leucine was linear for up to 40 min. Saturation concentration of [(3)H]Leucine incorporation into protein was 1500 nM. An experiment with prokaryotic and eukaryotic inhibitors showed no significant [(3)H]Leucine incorporation into eukaryotes even in high leucine concentrations. No significant amounts of radiolabel were incorporated into other macromolecules. The maximum time of sample storage after the incubation is 15 days. The leucine incorporation method can be a reliable tool to measure bacterial production in the periphyton root-associated bacteria.

RESUMO

The extent to which pre-Columbian societies altered Amazonian landscapes is hotly debated. We performed a basin-wide analysis of pre-Columbian impacts on Amazonian forests by overlaying known archaeological sites in Amazonia with the distributions and abundances of 85 woody species domesticated by pre-Columbian peoples. Domesticated species are five times more likely than nondomesticated species to be hyperdominant. Across the basin, the relative abundance and richness of domesticated species increase in forests on and around archaeological sites. In southwestern and eastern Amazonia, distance to archaeological sites strongly influences the relative abundance and richness of domesticated species. Our analyses indicate that modern tree communities in Amazonia are structured to an important extent by a long history of plant domestication by Amazonian peoples.

As part of a project on the behaviour of radionuclides released by the Angra Nuclear Power Station in the marine environment, laboratory experiments were performed to study cesium and cobalt accumulation by juvenile groupers (Epinephelus sp.). The fishes were kept in aquariums equipped with gravel-bed filters spiked with 60Co (150 KBq/l) and 137Cs (75 KBq/l). Uptake was only via water since the fishes received only non-radioactive food during the 4 months of experiment. Cesium accumulation in muscle was higher than in any other organ, reaching concentration factors (CFs) of 5 (fresh weight basis). Cobalt, in contrast, was concentrated mostly in the liver, gut and skin and scales. Cobalt CF in muscle tissue was 8 x 10(-2), lower than any previously reported figure. Chromatographic analysis of the water at the beginning and end of the experiment showed that cobalt had been completely converted from 60CoCl2 to non-cationic forms, most probably through complexation with organic ligands, mediated by the intense microbial activity of the filter bed. This conversion apparently enhanced cobalt bioavailability since a second experiment performed with this aged seawater yielded CFs for muscle tissue higher by a factor of 2.5.

Levels and accumulation of MeHg were characterized in filtered water, suspended organo-mineral matter, phytoplankton, zooplankton and epiphyton during the dry season and the wet season. In open water of the lentic and lotic ecosystems, the MeHg in filtered water (< 0.2 microm) was near or below the detection limit (< 0.02-0.03 ng/l). These concentrations represent < 5% of the Hgtot. content in filtered water. Inundated forests (Igapó) and macrophyte floating mats were the only sites where MeHg was significantly detected (0.07-0.24 ng/l), representing 3-22% of the Hgtot. in filtered water. MeHg concentrations in organo-mineral suspended matter (2-26 ng/g dry wt. representing 0.6-7.3% of Hgtot.) were correlated with the N content but not with the C content. Data suggest that MeHg enrichment of suspended matter is strongly influenced by the presence of degraded planktonic remains relatively rich in N and MeHg. In zooplankton, MeHg concentrations (20-140 ng/g dry wt.) increased from the dry season to the end of the wet season. This increase was followed by higher proportions of MeHg during the wet season in comparison to the dry season (15-40 to 50-70% of the Hgtot.). The epiphytic material collected from the roots of macrophyte floating mats contained 2-8.5 ng/g dry wt. of MeHg. The proportion of MeHg to Hgtot. in epiphyton (1.5-8.3%) correlated with its C and N contents. The data suggest a greater bioavailability of MeHg in the Tapaj6s River ecosystems due to the seasonal increase in water level and the consequent inundation of the floodplain. Inundation favours the development of large macrophyte floating mats which increases the bioavailability of epiphyton to herbivorous/detritivorous fish. The root zone of floating macrophytes and the flooded organic horizon of the Igapó forest are the only sites along the Tapajós River where significant MeHg can be detected in the water column and sediment. This new study supports the hypothesis that MeHg production and transfer to the first link of the food chain in Amazonian river systems is closely related to organic matter biogeochemistry in the floodplain environment.

The seasonal and spatial variations of net methylmercury production in sediments, soils and other sites were evaluated by assays with 203Hg at different depths and locations along a lake-forest transect at lake Enseada Grande, Tapajós river. Soil and sediment samples were taken at the surface and at different depths up to 9 cm. Fresh samples and acidified controls (1-3 g dry wt.) were slurried with local water and incubated in the dark at 25-28 degrees C for 3 days with 0.5-1.6 microg Hg g(-1) (dry wt.) added as 203HgCl2. CH3 203Hg was extracted and measured in scintillation cocktail after acid leaching. Methylmercury production varied by orders of magnitude among sites and among sediment or soil layers. Seasonal variations were smaller than those with sample depth and location. In both seasons, MeHg formation in sediment and soil or flooded soil decreased with depth and was, in the top layers, one order of magnitude higher in the C-rich littoral macrophyte zone (2.3-8.9%) and flooded forest (3.2-4.5%) than in the center of the lake (0.2-0.56%). Similar MeHg production was found in slurried dry soils (dry season) and in soils already flooded for months. In the macrophyte zone soil (dry season), methylation was mainly associated with the thin Paspalum sp. rootlet layer. In the forest site, vertical variation in methylation was less pronounced in flooded than in dry soils and during the inundation the higher methylation rate was found in the flocculent sediment settled over the litter layer. The roots of floating Paspalum sp. were an important Hg methylation site, particularly those heavily colonized with periphyton (3.4-5.4%). Methylation in surface or near-bottom water was undetectable (< 3 x 10(-2)%) at all sites. Flooded forests and macrophyte mats are specific features of the Amazon and are important links between Hg inputs from natural and manmade sources and MeHg exposure of local populations through fish intake.

Mercury (Hg total) fluxes were calculated for rainwater, throughfall and stream water in a small catchment located in the northeastern region of the Brazilian Amazon (Serra do Navio, Amapá State), whose upper part is covered by a natural rainforest and lower part was altered due to deforestation and activities related to manganese mining. The catchment area is 200 km from the nearest gold mining (garimpo). Minimum and maximum Hg concentrations were measured monthly from October 1996 to September 1997 and were 3.5-23.4 ng l(-1) for rainwater, 16.5-82.7 ng l(-1) for throughfall (March-August 1997) and 1.2-6.1 and 4.2-18.8 ng l(-1) for stream water, in natural and disturbed areas, respectively. In the natural area, the inputs were 18.2 microg m 2 year(-1) in rainwater and 72 microg m(-2) year(-1) in throughfall. This enrichment was attributed to dry deposition. The stream output of 2.9 microg m(-2) year(-1) indicates that Hg is being recycled within the forest as other chemical species or is being retained by the soil system, as confirmed by the cumulative Hg burden in the 0-10 cm surface layer, which was 36480 microg m(-2). When the disturbed area of the catchment was included, the stream output was 9.3 microg m(-2), clearly indicating the impact of the deforestation of the lower part of the basin on the release of mercury. The Hg burden in the disturbed area was 7560 microg m(-2) for the 0-10 cm surface layer.

The tropical flood plain Pantanal is one of the world's largest wetlands and a wildlife sanctuary. Mercury (Hg) emissions from some upstream gold mining areas and recent findings of high natural Hg levels in tropical oxisols motivated studies on the Hg cycle in the Pantanal. A survey was made on total Hg in the most consumed piscivorous fish species from rivers and floodplain lakes in the north (Cáceres and Barão de Melgaço) and in the south part of Alto Pantanal (around the confluence of the Cuiabá and Paraguai rivers). Samples were collected in both the rainy and dry seasons (March and August 1998) and included piranha (Serrasalmus spp.), and catfish (Pseudoplatystoma coruscans, pintado, and Pseudoplatystoma fasciatum, cachara or surubim). There was only a small spatial variation in Hg concentration of the 185 analyzed fish samples from the 200 x 200 km large investigation area, and 90% contained total Hg concentration below the safety limit for regular fish consumption (500 ng g(-1)). Concentration above this limit was found in both Pseudoplatystoma and Serrasalmus samples from the Baia Siá Mariana, the only acid soft-water lake included in this study, during both the rainy and dry seasons. Concentration above this limit was also found in fish outside Baia Siá Mariana during the dry season, especially in Rio Cuiabá in the region of Barão de Melgaço. The seasonal effect may be connected with decreasing water volumes and changing habitat during the dry season. The results indicate that fertile women should restrict their consumption of piscivorous fishes from the Rio Cuiabá basin during the dry season. Measures should be implanted to avoid a further deterioration of fish Hg levels.

In aquatic systems, bottom sediments have often been considered as the main methylmercury (MeHg) production site. In tropical floodplain areas, however, floating meadows and flooded forests extend over large areas and can be important Hg methylating sites. We present here a cross-system comparison of the Hg net methylation capacity in surface sediments, flooded soils and roots of floating aquatic macrophytes, assayed by in situ incubation with 203Hg and extraction of formed Me203 Hg by acid leaching and toluene. The presence of mono-MeHg was confirmed by thin layer chromatography and other techniques. Study areas included floodplain lakes in the Amazon basin (Tapajós, Negro and Amazon rivers), the Pantanal floodplain (Paraguay river basin), freshwater coastal lagoons in Rio de Janeiro and oxbow lakes in the Mogi-Guaçú river, São Paulo state. Different Hg levels were added in assays performed in 1994-1998, but great care was taken to standardise all other test parameters, to allow data comparisons. Net MeHg production was one order of magnitude higher (mean 13.8%, range 0.28-35) in the living or decomposing roots of floating or rooted macrophyte mats (Eichhornia azurea, E. crassipes, Paspalum sp., Eleocharis sellowiana, Salvinia sp., S. rotundifolia and Scirpus cubensis) than in the surface layer of underlying lake sediments (mean 0.6%, range 0.022-2.5). Methylation in flooded soils presented a wide range and was in some cases similar to the one found in macrophyte roots but usually much lower. In a Tapajós floodplain lake, natural concentrations of MeHg in soil and sediment cores taken along a lake-forest transect agreed well with data on net methylation potentials in the same samples. E. azurea, E. crassipes and Salvinia presented the highest methylation potentials, up to 113 times higher than in sediments. Methylation in E. azurea from six lakes of the Paraguay and Cuiabá rivers, high Pantanal, was determined in the 1998 dry and wet seasons and ranged from 1.8 to 35%. Methylation was lower in washed roots than in untreated roots of E. azurea and methylation in solids isolated from the roots, was higher than in sediments but lower than in untreated roots. This indicates that the methylation in roots zones occurs mainly in the root-associated solids. Floating meadows are sites of intense production of biomass and of highly bioavailable MeHg and appear to be an essential link of the MeHg cycle in tropical aquatic systems.

Sediments from nine floodplain lakes in Pantanal were analyzed for a large-scale (300 km) survey of mercury (Hg) load in sediments and soils of the Alto Pantanal and to study the relationship between Hg and reactive aluminum, iron, and manganese oxy-hydroxides. The results were compared with the Hg content in river and stream sediments from the Poconé gold mining area, where Hg has been extensively used and still is in use. The results indicate that the Hg concentrations were elevated in river sediment close to the mining area in Bento Gomes river basin (average in the < 74-microm fraction 88.9 ng Hg g(-1) dry wt.; interquartile range 50.3-119.5), but there was no clear indication that the local Hg emissions have contaminated the remote floodplain lakes, where concentrations were surprisingly low (average in the < 74-microm fraction 33.2 ng Hg g(-1) dry wt. sediment; interquartile range 18.4-46.8), in particular when considering geochemical characteristics of the sediment. The sediment from the floodplain lakes contained less Hg-tot and more reactive iron oxy-hydroxides than soils from the Tapajós area in the Amazon basin. This resulted in a mass ratio between Hg and amorphous oxy-hydroxides of only 5 x 10(-6) for Hg-tot/Fe-oxa (interquartile range 3-7 x 10(-6).

In an oxisol-spodosol system developed on the terrestrial surface of the lower Tapajós Valley, the determination of total mercury (Hg), organic carbon (C), iron and aluminum oxy-hydroxide (Fe(cdb) and Al(cdb)) concentrations in the surface soil horizons are used to characterise the geochemical processes controlling the accumulation of Hg in soils under natural vegetation cover and in deforested and cultivated sites. Oxisols from the plateau have homogeneous and relatively high background Hg contents and burdens constituting an important natural reservoir of Hg for the region (90-210 ng/g dry wt. and 19-33 mg/m2 for the first 20 cm). The Fe(cdb) and Al(cdb) contents associated with the fine fraction (< 63 microns) of the soil suggest that oxy-hydroxides and, particularly Al-substituted Fe oxy-hydroxides, control the Hg concentrations observed in all of the soils of the study region. Consequently, the geochemistry of these minerals along the slopes governs the accumulation or the release of the Hg according to the natural evolution of the soil cover and/or following the degradation of soils by erosion after deforestation and cultivation. These observations have important implications for the interpretation of Hg contamination patterns observed in Amazonian aquatic systems that could be linked to different drainage sources of Hg from the terrestrial surface. The sandification and podzolisation that is characteristic of the evolution of numerous pedological systems in the equatorial Amazon could be responsible for exportation of the naturally accumulated Hg, as for other metals, by acidic complexation and migration to the black waters of the Amazon. In the central Amazon region, as a result of the fragility of the soil cover, deforestation and cultivation, affecting principally the superficial soil, promote the selective erosion of fine particles enriched in oxides and Hg. The erosion of soil could be responsible for an important release of Hg, transported in particulate form by drainage waters.

RESUMO

A 15 year old male patient was diagnosed as being infected with the nematode Lagochilascaris minor, presenting as abscesses over the left mastoid region, and invading the temporal bone, mastoid sinuses and possibly the CNS. Surgical drainage and administration of cambendazole and levamisole led to improvement, followed by an early relapse and poor tolerance to the antihelminthic drugs. Repeated doses of ivermectin (an animal preparation) were used for the first time to treat this condition and resulted in complete remission of signs of infection, maintained for 15 months after the end of drug therapy, indicating cure of the disease.

The main contamination areas in Goiânia were exposed soil surfaces, and with the first rainfalls, 137Cs quickly reached the Meia-Ponte River, the main local waterway. Regular measurements of bottom sediment, surface water, and fish were begun 3 wk after the accident. In the first survey, 137Cs was detectable in sediments up to 12 km downstream from the accident area. Maximum 137Cs concentrations found in sediments of the Meia-Ponte River were 1.3 kBq kg-1 dry weight and 200 Bq kg-1 fresh weight for fish. Cesium-137 was always less than 10 Bq L-1 in surface waters plus suspended particulates, even in the most contaminated creeks. Because it was the beginning of the rainy season, downstream sediment transport was enhanced. This resulted in a decrease of 137Cs concentrations in sediments of the urban area by a factor of approximately 20, 5 wk after the accident. The main sedimentation area for suspended sediments in this portion of the river is the Rochedo Reservoir, which is 80 km downstream from the accident area. In a survey performed 10 mo after the accident, 137Cs concentrations in the reservoir were up to 14 Bq kg-1 for fish, 1 Bq kg-1 for aquatic plants, and 100 Bq kg-1 dry weight for bottom sediments.

The authors studied the records of 108 cases of cancer of the larynx registered at the Department of Otolaryngology--Clinics Hospital--Faculty of Medicine of University of São Paulo, from 1985 to 1995. Dysphonia was the most common symptom observed (85.2%), independently of the site of the tumor. Dysphagia, dyspnea and weight loss had a similar incidence (32.4%; 34.3%; and 29.6%, respectively), with dysphagia more frequent in tumors which affected the supraglottis and dyspnea in glottic and subglottic tumors. As to staging, 45.8% presented at stage IV at the first consultation, and only 13.5% at stage I. No association was observed between tumor size (according to the TNM classification), presence of lymph node involvement, and delay, in diagnosis, taking the period between the beginning of symptoms and the first consultation at the hospital. In relation to the presenting symptom those with dysphonia sought medical help later. There was no correlation between histological invasion and tumor stage. It was concluded that the stage at presentation of tumors of the larynx is possibly more related to intrinsic differences in the tumor's aggressiveness and in host characteristics than to the diagnostic delay, and that it is necessary to warn the population about symptoms which may suggest the presence of cancer of the larynx.

The aim of this research was to evaluate the effect of sediment composition on methane (CH4) dynamics in sediments of different areas in the transition zone between a mangrove and the sea. This research was conducted in a mangrove at Coroa Grande, on the southern coast of Rio de Janeiro. Samples were collected at three stations: (1) region colonised by Rhizophora mangle L. on the edge of the mangrove, (2) region colonised by seagrasses and (3) infra-littoral region without vegetation. Samples were collected from the surface layer of the sediment to determine the concentrations of nutrients (C, N and P) and CH4 concentration and production. We observed that concentrations of CH4 and carbon (C) were significantly higher (p < 0.05) in station 1 than station 3. The molar ratios (C:N, C:P and N:P) suggest that the origin of the substrate is mainly autochthonous. Methanogenesis was initially low, possibly due to competition between methanogens and sulfate reducers, and increased significantly (p < 0.05) on the twenty-sixth day in the sediment of station 1, probably due to higher organic matter (OM) availability in this region. Results indicate that methanogenic activity observed herein is not regulated by the amount or quality of OM, but by other factors. The concentration of CH4 in the sea-land ecotone at Mangrove Coroa Grande is a function of available OM suggesting a possible inhibition of methanotrophy by intense oxygen consumption in the soil surface covered by detritus of Rhizophora mangle vegetation.

Laboratory radioassays were made to study mercury (Hg) methylation together with bacterial production in the periphyton of two aquatic macrophytes, the submerged Myriophyllum spicatum, from a constructed wetland in Sweden and the floating Eichhornia crassipes, from a eutrophied tropical lake in Brazil. Time course incubations were made by addition of (203)HgCl(2) and the methylmercury formed was extracted at pre-defined time intervals. Bacterial production ((14)C-leucine incorporation) was measured at the same time intervals, with plants removed from parallel incubations made with and without addition of cold HgCl(2). For E. crassipes, higher methylmercury production was observed at elevated bacterial production, whereas for M. spicatum, the bacterial production was significantly lower, and Hg methylation was below the detection limit. The combined results confirm the importance of microbial processes for Hg methylation, although other factors are known to influence this process in complex ways. The addition of Hg did not significantly influence bacterial production, while the incubation temperatures used (25 and 35 degrees C) resulted in different methylation rates. Radiotracer techniques for measurements of bacterial production such as (14)C-leucine uptake can provide useful insights into the Hg cycle in aquatic environments, and our data suggest that they may be used as a proxy of mercury methylation potentials.

Different aspects of mercury accumulation, retention, and elimination in temperate fish species have been reported, but few data are available for tropical species. In this work Hg accumulation in Trichomycterus zonatus, where experimental losses were estimated, was studied. Forty individuals (males and females) of T. zonatus were acclimatized for 30 days. In each experiment 10 aquaria were used, with 1 fish each (5 exposed to HgCl2 and the others as controls). The concentration tested was 15 microg Hg x liter-1 including 18 kBq x liter-1 203HgCl2 as tracer. Fish were dissected and measured for Hg by gamma scintillation spectrometry at regular intervals, and after 96 hr were transferred to mercury-free tap water. Approximately 40% of added mercury was lost by volatilization (20%) and adsorption (20%) in 48 hr. A faster absorption of inorganic mercury was observed initially in gills, a higher and lower retention in kidney and muscle, respectively. For T. zonatus the kidney, gills, and liver were considered the critical organs, but this order changed after 98 hr in mercury-free tap water. The distribution of inorganic mercury in T. zonatus seems quite similar to that found in temperate species, but the sensitivity of this species to Hg seems higher.
