RESUMO

Generally, the foods we usually eat are not only aqueous solutions, but also viscous solutions and solids. Therefore, it is interesting for us to explore how taste components are perceived in a viscous polymer solution. The relationship between the sensory evaluation of saltiness intensity, amount of added oil, and apparent viscosity was clarified in low-viscosity and high-viscosity polymer solutions. The study was conducted using samples containing corn oil, sodium chloride, and [xanthan gum] or [xanthan gum + locust bean gum] as a thickener. Oil was added to the viscous polymer solutions regardless of whether they were low- or high-viscosity, and saltiness intensity was evaluated as compared with a reference solution. The low-viscous polymer solutions with [xanthan gum] were perceived to be saltier than the high-viscous polymer solutions with [xanthan gum + LBG] as the amount of oil increased. The shear stress value gradually increased as the amount of oil increased in both the low-viscosity and the high-viscosity polymer solutions, as derived from the fluid constitutive equation. There was a correlation between saltiness intensity and apparent viscosity in both high- and low-viscosity polymer solutions. A coefficient of determination (R2 ) of 0.918 was obtained between saltiness intensity and "apparent viscosity" for the samples using [xanthan gum] and that of 0.683 between saltiness intensity and "apparent viscosity" for the samples using [xanthan gum+LBG]. The low-viscosity polymer solutions showed a saltier intensity as the amount of oil increased and a greater correlation with apparent viscosity, as compared with the high-viscosity polymer solutions.

OBJECTIVE: Although neoadjuvant chemotherapy(NAC)has been recognized as an important option for improving the clinical outcome of patients with advanced gastric carcinoma, convincing evidence that it prolongs life and brings about a good prognosis are both lacking. We retrospectively evaluated the efficacy and safety of NAC in ten patients with advanced gastric cancer. METHODS: A total of ten patients with advanced gastric cancer, who received NAC with the combination of S-1 and cisplatin in our hospital from April 2008 to March 2010, were retrospectively investigated. RESULTS: A total of 5 patients responded to neoadjuvant chemotherapy, and 2 patients showed a complete regression of the primary gastric carcinoma. Four of the 5 patients who responded had solid-type poorly-differentiated adenocarcinoma. CONCLUSION: NAC with the combination of S-1 and cisplatin was suggested to be effective for advanced gastric carcinoma, especially for solid-type poorly differentiated adenocarcinomas(por1).

RESUMO

The patient was a 77-year-old woman with multiple liver metastases of sigmoid colon cancer. She underwent low anterior resection for sigmoid colon cancer. After surgery, she selected oral administration of UFT and LV for liver metastases and multiple lymph node metastases. After two courses, the liver metastases had markedly diminished. Thirty-two months later, liver metastases had disappeared on computer tomography. This therapy was continued for five years, and recurrences are no longer shown. Severe adverse effects were not observed. Oral anti-cancer drugs can serve as effective therapy for advanced colorectal cancer of old patients.

Case 1 was a 58-year-old woman diagnosed with unresectable liver metastases from advanced rectal cancer. We performed laparoscopic low anterior resection for sigmoid cancer. As the first-line treatment, FOLFOX+bevacizumab(BV)was applied for 16 courses. The second-line treatment, FOLFIRI plus BV, was applied for four courses. However, the disease progressed with worsening liver metastases. The sequencing of K-RAS genes from the biopsy specimens of sigmoid colon cancer revealed an expression of a wild-type K-RAS. As the third-line treatment, panitumumab was applied. After 8 courses of this chemotherapy regimen, a significant reduction in the size of liver metastases was observed. Case 2 was an 81-year-old man diagnosed with unresectable liver metastases from advanced rectal cancer. We obliged the patient by performing laparoscopic rectal resection. As the first-line treatment, XELOX plus BV was applied for 10 courses. As the second-line treatment, IRIS was applied for 6 courses. However, this failed to prevent him from having a progressive disease. As the third-line treatment, panitumumab was applied for 2 courses, and a significant reduction in the size of liver metastases was observed. Our findings suggested that panitumumab has great potential for effective treatment of patients with unresectable stageIV colorectal cancer.

RESUMO

The patient was a 56-year-old female. At the age of 35 years, she had under gone left mastectomy and axillary lymph node dissection for breast cancer. After surgery, hormonal therapy was continued for 3 years. Then, no treatment was performed. In this study, single therapy with an AI agent was started to treatbilateral supraclavicular fossa/mediastinal lymphnode metastases. After 6 months, a partial response(PR)was achieved. However, progression of the disease(PD)was noted after 1 year. Thereafter,the regimen was switched to single high-dose(120mg/day)TOR therapy. CT revealed the disappearance of the bilateral supraclavicular fossa lymphnodes and a marked reduction of the other lymphnodes. Currently, the patient is being treated, with an interval of 10 months from the start of TOR therapy.

INTRODUCTION: The subvesical bile ducts are located in the peri-hepatic connective tissue of the gallbladder fossa. Injury of the subvesical bile ducts provokes the severe complication of bile leak. Until now, fluorescent cholangiography has been employed during hepatobiliary surgery. Herein, we report the detection of subvesical bile ducts by fluorescent cholangiography during laparoscopic cholecystectomy. PRESENTATION OF CASE: A 63-year-old female was admitted to our department for surgery for symptomatic cholelithiasis. The subvesical bile ducts were not observed on drip-infusion cholangiography with computed tomography. Immediately following induction of anesthesia, 2.5 mg of indocyanine green was intravenously injected. Fluorescent cholangiography demonstrated two thin aberrant bile ducts during dissection of Calot's triangle. We considered them to be subvesical bile ducts. We ligated them with clips, divided them, and then performed laparoscopic cholecystectomy using a standard procedure. The patient had a good post-operative recovery without bile leakage. Postoperative laboratory test results were all within normal limits. Computed tomography revealed no dilatation of the intrahepatic bile duct after laparoscopic cholecystectomy. The patient was discharged on postoperative day 4. DISCUSSION: Injury to the subvesical bile ducts is one of the most common causes of bile leakage associated with cholecystectomy. Fluorescent cholangiography enabled real-time identification of the thin subvesical bile ducts, which were undetectable by drip-infusion cholangiography with computed tomography. CONCLUSION: Fluorescent cholangiography during laparoscopic cholecystectomy may be useful for preventing postoperative bile leakage.

Pregnancy-associated breast carcinoma is generally defined as cancer that occurs during pregnancy or within 1 year of delivery, although treatment options are the most complicated when the disease is diagnosed during pregnancy. We report the case of a 30-year-old woman who was diagnosed with breast cancer at her 9th week of pregnancy. The patient initially had mastectomy with axillary lymph node dissection. She began adjuvant therapy with 3 courses of epirubicin/cyclophosphamide at 19 weeks of gestation. After delivery of a healthy child, she received one course of epirubicin/cyclophosphamide and 4 courses of docetaxel. Although the data are limited, pregnant patients with cancer can be treated with systemic chemotherapy with minimal risks to the fetus during the second or third trimester. Management of breast cancer during pregnancy requires an interdisciplinary care team and careful consideration of the patient's stage of disease, the gestational age of the fetus, and the preferences of the patient and her family.

We experienced a rare case of gallbladder metastasis from renal cell carcinoma. A 68-year-old man was admitted for further evaluation of a gallbladder tumor, which had been identified on follow-up computed tomography after partial nephrectomy for renal cell carcinoma. Enhanced computed tomography and magnetic resonance imaging showed an enhancing polypoid mass in the gallbladder lumen. Endoscopic ultrasonography demonstrated a homogenous hypo-echoic polypoid lesion, and the outer hyper-echoic layer of the adjacent wall was intact. Blood flow signals in the wall side of the mass were observed on color Doppler endoscopic ultrasonography images. Laparoscopic cholecystectomy was performed and he was uneventfully discharged. Macroscopic examination of the specimen revealed a 12 × 7 × 5 mm pedunculated tumor attached by a thin pedicle to the fundus of the gallbladder. Histology confirmed a metastasis of the renal cell carcinoma that had infiltrated the shallow subserosa but had mainly grown into cavity of the gallbladder. These imaging findings are considered characteristic and may assist preoperative diagnosis in patients with a history of renal cell carcinoma.

PURPOSE: This study assesses the possibility of preserving and transplanting non-heart-beating donor (NHBD) lungs under topical cooling for 6 h. METHODS: The donor dogs were killed and heparinized after cardiac arrest. After 10 min of cardiac massage, the bilateral lungs were deflated. In group 1 (n = 6), the recipient dogs were transplanted and the lungs preserved at 4 degrees C for 6 h. In group 2 (n = 2), the recipients received lungs preserved at room temperature for 4 h. In both groups the right main bronchus, pulmonary artery, and pulmonary vein were clamped and ventilated to examine the lung function. RESULTS: The mean total ischemic time before reperfusion was 7 h 48 min (group 1) and 5 h 55 min (group 2). All dogs in group 1 survived with excellent hemodynamics and gas exchange function. Both dogs in group 2 revealed congestion immediately after reperfusion, and consequently died. CONCLUSIONS: We concluded that it is possible to use NHBD lungs for clinical lung transplantation if topical cooling can be initiated soon after irreversible cardiac arrest. Topical cooling of NHBD lungs allows at least 6 h for preparation before transplantation.
