Objectives: To identify the front-of-package scheme-Multiple Traffic Light (MTL), Nutri-Score (NS), and black octagon Warning System (WS)-most effective in reducing purchase intention and perceived product healthfulness of drinking yogurts, cookies and cheese spreads, and to assess the joint influence of nutrient claims on the effects. Methods: Randomized-controlled experiment. A within-subjects factorial design was used to evaluate the influence of three independent factors on perceived product healthfulness and purchase intention. A total of 704 adults in Argentina were shown three mock-up products and asked to indicate which product they would buy choosing between pairs of products from different categories and to rate product healthfulness on a 7-point Likert scale. Results: The WS was the most effective in reducing purchase intention in drinking yogurts (OR:0.16, 95%CI: 0.09;0.28), cookies (OR:0.10, 95%CI: 0.05;0.18) and cheese spreads (OR:0.10, 95%CI: 0.05;0.18), and the perception of healthfulness (WS Mean score 3,63 vs No label 4,24, p<0.001), regardless of the participants' gender, age, and level of education. NS was inefficacious in reducing product healthfulness perception, while MTL had significantly increased how healthful the product was perceived (p<0.001). Nutrient claims increased purchase intention and perception of healthfulness, thus reducing the effectiveness of front-of-package labels (p<0.001). Conclusions: In line with growing evidence, our findings support that WS perform better than NS and MTL in reducing purchase intention and healthfulness perception of products with excessive amounts of critical nutrients associated with the greatest burden of diseases. Front-of-package WS are expected to facilitate the population in Argentina to make healthier decisions.

INTRODUCTION: Wheat flour enrichment is a public health strategy recommended to prevent micronutrient deficiencies, including iron deficiency. The objective of this study was to determine iron content in enriched wheat flour and flour products and their contribution to nutritional recommendations for children. POPULATION AND METHODS: Observational, analytical, cross-sectional study based on the total diet study method. Enriched wheat flour (as per Law no. 25630) and the most frequently consumed flour products were analyzed. Products were selected using a questionnaire on the frequency of food intake and a 24-hour recall interview with the parents of children aged 6 months to 7 years. Food iron levels were determined based on atomic absorption spectrometry and their contribution to the estimated average requirement was assessed. RESULTS: Enriched flour and flour products showed the expected iron amount, except for French bread and breadcrumbs. The contribution of studied products to iron requirements was 7 % in children aged 6 months to 1 year, 81 % in those aged 1-3 years, and 45 % in those older than 4 years. CONCLUSIONS: Except for French bread and breadcrumbs, studied flour and flour products showed the expected enrichment level. The consumption of this type of food does not provide significant amounts of iron to infants, but it does to children older than 1 year.


Introducción. El enriquecimiento de la harina de trigo es una de las estrategias de salud pública recomendadas para prevenir las deficiencias de micronutrientes, entre ellos el hierro. El objetivo de este trabajo fue determinar el contenido de hierro en harina de trigo enriquecida y sus derivados, y la contribución de estos a las recomendaciones nutricionales en niños. Población y métodos. Estudio observacional, analítico, de corte transversal, basado en la metodología de estudio de dieta total. Se analizaron la harina de trigo enriquecida (según la ley 25630) y sus derivados más consumidos. Para seleccionar los alimentos, se realizó un cuestionario de frecuencia de consumo y recordatorio de 24 horas a los padres de niños con edades entre 6 meses y 7 años. Se determinó la concentración de hierro en los alimentos mediante absorción atómica y se evaluó su contribución al requerimiento promedio estimado. Resultados. La harina enriquecida y sus derivados tuvieron la cantidad de hierro esperada, excepto el pan francés y el pan rallado. La contribución a los requerimientos de hierro de los alimentos evaluados fue el 7 % en niños de 6 meses a 1 año, el 81 % en el grupo de 1 a 3 años y el 45 % en los mayores de 4 años. Conclusiones. Excepto el pan francés y el pan rallado, las harinas y sus derivados analizados presentaron el nivel de enriquecimiento esperado. El consumo de estos alimentos no aporta cantidades significativas de hierro en los lactantes, pero sí en los niños mayores de 1 año.

Sodium intake in Argentina has been estimated to be at least double the dose of 2000 mg/day recommended by WHO, mostly coming from processed foods. Argentina is one of the few countries in the world that have regulated sodium content in certain food products. This study presents an assessment of sodium content in a selection of food groups and categories as reported in the nutrient information panels. We surveyed 3674 food products, and the sodium content of 864 and 1375 of them was compared to the maximum levels according to the Argentinean law and the regional targets, respectively. All food categories presented high variability of sodium content. Over 90% of the products included in the national sodium reduction law were found to be compliant. Food groups with high median sodium, such as condiments, sauces and spreads, and fish and fish products, are not included in the national law. In turn, comparisons with the lower regional targets indicated that almost 50% of the products analyzed had sodium contents above the recommended values. This evidence suggests that enhancing sodium reduction in processed foods may be a necessity for public health objectives and it is also technically feasible in Argentina.

OBJECTIVE: To test the hypothesis that persistent villi hypoperfusion explains intramucosal acidosis after endotoxemic shock resuscitation. DESIGN: Controlled experimental study. SETTING: University-based research laboratory. SUBJECTS: A total of 14 anesthetized, mechanically ventilated sheep. INTERVENTIONS: Sheep were randomly assigned to endotoxin (n = 7) or control groups (n = 7). The endotoxin group received 5 microg/kg endotoxin, followed by 4 microg x kg(-1) x hr(-1) for 150 mins. After 60 mins of shock, hydroxyethylstarch resuscitation was given to normalize oxygen transport for an additional 90 mins. MEASUREMENTS AND MAIN RESULTS: Endotoxin infusion decreased mean arterial blood pressure, cardiac output, and superior mesenteric artery blood flow (96 +/- 10 vs. 51 +/- 20 mm Hg, 145 +/- 30 vs. 90 +/- 30 mL x min(-1) x kg(-1), and 643 +/- 203 vs. 317 +/- 93 mL x min(-1) x kg(-1), respectively; p < .05 vs. basal), whereas it increased intramucosal-arterial PCO2 (deltaPCO2) and arterial lactate (3 +/- 3 vs. 14 +/- 8 mm Hg, and 1.5 +/- 0.5 vs. 3.7 +/- 1.3 mmol/L; p < .05). Sublingual, and serosal and mucosal intestinal microvascular flow indexes, and the percentage of perfused ileal villi were reduced (3.0 +/- 0.1 vs. 2.3 +/- 0.4, 3.2 +/- 0.2 vs. 2.4 +/- 0.6, 3.0 +/- 0.0 vs. 2.0 +/- 0.2, and 98% +/- 3% vs. 76% +/- 10%; p < .05). Resuscitation normalized mean arterial blood pressure (92 +/- 13 mm Hg), cardiac output (165 +/- 32 mL x min(-1) x kg(-1)), superior mesenteric artery blood flow (683 +/- 192 mL x min(-1) x kg(-1)), and sublingual and serosal intestinal microvascular flow indexes (2.8 +/- 0.5 and 3.5 +/- 0.7). Nevertheless, deltaPCO2, lactate, mucosal intestinal microvascular flow indexes, and percentage of perfused ileal villi remained altered (10 +/- 6 mm Hg, 3.7 +/- 0.9 mmol/L, 2.3 +/- 0.4, and 78% +/- 11%; p < .05). CONCLUSIONS: In this model of endotoxemia, fluid resuscitation corrected both serosal intestinal and sublingual microcirculation but was unable to restore intestinal mucosal perfusion. Intramucosal acidosis might be due to persistent villi hypoperfusion.

[RESUMEN]. Las plantas de cannabis se clasifican en quimiotipos, según las concentraciones relativas de tetrahidrocannabinol (THC) y cannabidiol (CBD). El quimiotipo I tiene alto contenido de THC (sativa dominante); el quimiotipo II tiene un contenido aproximado 1:1 de THC:CBD (híbrida) y el quimiotipo III con CBD como constituyente predominante (indica dominante). El objetivo fue clasificar las muestras recibidas en el Laboratorio de con trol de calidad de la ENyS según su quimiotipo. El análisis de cannabinoides se realizó mediante cromatografía líquida de ultra alta resolución acoplada a espectrometría de masas y se calculó la razón THC/CBD. Se analizaron 121 muestras de tejido vegetal, que tuvieron un fuerte predominio de quimiotipo I (75%) y 139 muestras de aceite, con 41% de quimiotipo I, 40% de quimiotipo II y 19% de quimiotipo III.


[ABSTRACT]. Cannabis plants are classified into chemotypes, based on the relative concentrations of tetrahydrocannabinol (THC) and cannabidiol (CBD). Chemotype I has high THC content (sativa dominant); chemotype II has an approximate 1:1 content of THC:CBD (hybrid) and chemotype III has CBD as the predominant constituent (indica dominant). Our aim was to classify the samples received at the ENyS Quality Control Laboratory according to their chemotype. Cannabinoid analysis was performed using ultra-high-performance liquid chromatography coupled to mass spectrometry and the THC/CBD ratio was calculated. 121 plant tissue samples were analyzed, which had a strong predominance of chemotype I (75%) as well as 139 oil samples, wich rendered 41% chemotype I, 40% chemotype II and 19% chemotype III.

INTRODUCTION: Lead is a toxic metal which, even at low blood levels, can alter normal neurodevelopment in children, so no blood lead level is acceptable. Lead absorption from diet accounts for the highest contribution to blood lead levels in the population who is not exposed to contaminated environments or because of their occupation. The objective of this study was to determine the contribution of diet to lead exposure among children aged 1 to 7 years who attended Hospital de Niños de La Plata for health check-ups. POPULATION AND METHODS: The study was conducted between June 2015 and May 2016. A questionnaire on the frequency of food intake was administered to 91 children whose average age ± standard deviation was 3.0 ± 1.7 years, and foods included in the analysis were selected based on this questionnaire. Selected foods were purchased from different regional stores. Composite samples were made up of different food groups. Lead levels corresponding to each food group were determined and, finally, the daily intake of lead was estimated for the studied population. RESULTS: The daily intake of lead was 138 µg/day. The food groups with the higher intake rates were processed meat products (15.4%), bakery products (14.8%), milk (12.5%), and meat (11.7%). CONCLUSIONS: Children aged 1 to 7 years attending a public hospital in La Plata have a lead burden from dietary intake of 138 µg/day.


INTRODUCCIÓN: El plomo es un metal tóxico, que, aun en bajas concentraciones sanguíneas, es capaz de alterar el normal desarrollo neurológico de los niños, por lo que no existe un nivel aceptable de plombemia. La absorción de plomo de los alimentos es la mayor contribución a los niveles de plombemia de la población no expuesta laboralmente o a ambientes contaminados. El objetivo fue determinar la contribución dietaria a la exposición al plomo en niños de 1 a 7 años de edad que concurrían por controles de salud al Hospital de Niños de La Plata. POBLACIÓN Y MÉTODOS: El estudio se realizó entre junio de 2015 y mayo de 2016. Se realizó un cuestionario de frecuencia de consumo de alimentos sobre 91 niños cuya edad promedio ± desvío estándar fue de 3,0 ± 1,7 años, sobre la cual se seleccionaron los alimentos por analizar. Se compraron los alimentos seleccionados en diversos comercios de la región. Se armaron muestras compuestas de diversos grupos de alimentos. Se determinó la concentración de plomo en los grupos de alimentos y, finalmente, se estimó la ingesta diaria de plomo de la población estudiada. RESULTADOS: La ingesta diaria de plomo fue de 138 µg/día. Los grupos de alimentos que tuvieron mayor contribución fueron los productos cárnicos procesados (15,4%), los panificados (14,8%), la leche (12,5%) y la carne (11,7%). CONCLUSIONES: Los niños de 1 a 7 años que concurren a un hospital público de La Plata tienen una carga de plomo en la dieta de 138 µg/día.

Determinar el sistema de etiquetado frontal -semáforo múltiple, Nutri-Score (NS) y sistema de advertencia con forma de octágono negro- más efectivo para reducir la intención de compra y la percepción saludable de yogures bebibles, galletitas y quesos para untar, y evaluar la influencia conjunta de las declaraciones nutricionales sobre los efectos del etiquetado frontal. Métodos. En el estudio, un ensayo controlado aleatorizado, se empleó un diseño factorial entre los sujetos para evaluar la influencia de tres factores independientes en la percepción saludable y la intención de compra. A un total de 704 adultos en Argentina se les mostraron tres prototipos de productos; se les pidió que indicaran qué producto comprarían eligiendo entre pares de diferentes categorías y que calificaran qué tan saludable era cada producto en una escala Likert de 7 puntos. Resultados. El sistema de advertencias con octágonos negros fue el más efectivo para reducir la intención de compra en yogures bebibles (OR: 0,16, IC 95%: 0,09; 0,28), galletitas (OR: 0,10, IC 95%: 0,05; 0,18) y quesos para untar (OR: 0,10, IC 95%: 0,05; 0,18) y para reducir la percepción saludable (puntuación promedio del sistema de advertencias: 3,63; ninguna etiqueta: 4,24, p<0,001), independientemente del sexo, la edad y el nivel de educación de los participantes. El Nutri-Score fue ineficaz para reducir la percepción saludable, mientras que el sistema de semáforo múltiple aumentó significativamente cuán saludable se percibió el producto (p<0,001). Las declaraciones nutricionales aumentaron la intención de compra y la percepción saludable, lo que redujo la eficacia de las etiquetas frontales en el envase (p<0,001). Conclusiones. En consonancia con la evidencia disponible, nuestros resultados respaldan que el sistema de advertencia con forma de octágono negro funciona mejor que el Nutri-Score y el semáforo múltiple en la reducción de la intención de compra y la percepción de saludable de productos con cantidades excesivas de nutrientes críticos asociados con la mayor carga de enfermedades. Identificar o modelo de rotulagem nutricional frontal - tipo semáforo (Multiple Traffic Light, MTL), Nutri-Score (NS) e octógonos pretos (Warning System, WS) - mais eficaz para reduzir a intenção de compra e a percepção de saudabilidade de iogurtes líquidos, biscoitos doces e pastas de queijo, bem como avaliar a influência conjunta das alegações nutricionais sobre tais efeitos. Métodos. Estudo experimental randomizado e controlado. Foi utilizado um delineamento fatorial de sujeito único para avaliar a influência de três fatores independentes sobre a percepção de saudabilidade e a intenção de compra dos produtos. A uma amostra de 704 adultos na Argentina foram mostrados três mock-ups de produtos, e foi solicitado aos participantes que indicassem qual produto eles comprariam (escolhendo entre pares de produtos de diferentes categorias) e que classificassem a saudabilidade do produto em uma escala tipo Likert de 7 pontos. Resultados. O sistema WS foi o mais eficaz em reduzir a intenção de compra dos iogurtes líquidos (odds ratio [OR]: 0,16, intervalo de confiança [IC] 95%): 0,09; 0,28), biscoitos doces (OR: 0,10, IC95%: 0,05; 0,18) e pastas de queijo (OR: 0,10, IC95%: 0,05; 0,18), bem como a percepção de saudabilidade (pontuação média: 3,63 com rotulagem WS x 4,24 sem rotulagem, p<0,001), independentemente do sexo, idade e escolaridade dos participantes. O sistema NS foi ineficaz em reduzir a percepção de saudabilidade dos produtos, e o MTL aumentou significativamente essa percepção (p<0,001). As alegações nutricionais aumentaram a intenção de compra e a percepção de saudabilidade, reduzindo assim a eficácia da rotulagem frontal (p<0,001). Conclusões. Corroborando um corpo crescente de evidências, nossas constatações indicam que o sistema WS de rotulagem frontal tem melhor desempenho do que os modelos NS e MTL na redução da intenção de compra e da percepção de saudabilidade de produtos com teor excessivo de nutrientes críticos associados a uma maior carga de morbidade. Espera-se que a rotulagem nutricional frontal do tipo WS favoreça a tomada de decisões mais saudáveis pela população da Argentina.

BACKGROUND: The identification of anaerobic metabolism in critically ill patients is a challenging task. Observational studies have suggested that the ratio of venoarterial PCO2 (Pv-aCO2) to arteriovenous oxygen content difference (Ca-vO2) might be a good surrogate for respiratory quotient (RQ). Yet Pv-aCO2/Ca-vO2 might be increased by other factors, regardless of anaerobic metabolism. At present, comparisons between Pv-aCO2/Ca-vO2 and RQ have not been performed. We sought to compare these variables during stepwise hemorrhage and hemodilution. Since anemia predictably produces augmented Pv-aCO2 and decreased Ca-vO2, our hypothesis was that Pv-aCO2/Ca-vO2 might be an inadequate surrogate for RQ. METHODS: This is a subanalysis of a previously published study. In anesthetized and mechanically ventilated sheep (n = 16), we compared the effects of progressive hemodilution and hemorrhage by means of expired gases analysis. RESULTS: There were comparable reductions in oxygen consumption and increases in RQ in the last step of hemodilution and hemorrhage. The increase in Pv-aCO2/Ca-vO2 was higher in hemodilution than in hemorrhage (1.9 ± 0.2 to 10.0 ± 0.9 vs. 1.7 ± 0.2 to 2.5 ± 0.1, P < 0.0001). The increase in Pv-aCO2 was lower in hemodilution (6 ± 0 to 10 ± 1 vs. 6 ± 0 to 17 ± 1 mmHg, P < 0.0001). Venoarterial CO2 content difference and Ca-vO2 decreased in hemodilution and increased in hemorrhage (2.6 ± 0.3 to 1.2 ± 0.1 vs. 2.8 ± 0.2 to 6.9 ± 0.5, and 3.4 ± 0.3 to 1.0 ± 0.3 vs. 3.6 ± 0.3 to 6.8 ± 0.3 mL/dL, P < 0.0001 for both). In hemodilution, Pv-aCO2/Ca-vO2 increased before the fall in oxygen consumption and the increase in RQ. Pv-aCO2/Ca-vO2 was strongly correlated with Hb (R 2 = 0.79, P < 0.00001) and moderately with RQ (R 2 = 0.41, P < 0.0001). A multiple linear regression model found Hb, RQ, base excess, and mixed venous oxygen saturation and PCO2 as Pv-aCO2/Ca-vO2 determinants (adjusted R 2 = 0.86, P < 0.000001). CONCLUSIONS: In hemodilution, Pv-aCO2/Ca-vO2 was considerably increased, irrespective of the presence of anaerobic metabolism. Pv-aCO2/Ca-vO2 is a complex variable, which depends on several factors. As such, it was a misleading indicator of anaerobic metabolism in hemodilution.

BACKGROUND: The microvascular reperfusion injury after retransfusion has not been completely characterized. Specifically, the question of heterogeneity among different microvascular beds needs to be addressed. In addition, the identification of anaerobic metabolism is elusive. The venoarterial PCO2 to arteriovenous oxygen content difference ratio (Pv-aCO2/Ca-vO2) might be a surrogate for respiratory quotient, but this has not been validated. Therefore, our goal was to characterize sublingual and intestinal (mucosal and serosal) microvascular injury after blood resuscitation in hemorrhagic shock and its relation with O2 and CO2 metabolism. METHODS: Anesthetized and mechanically ventilated sheep were assigned to stepwise bleeding and blood retransfusion (n = 10) and sham (n = 7) groups. We performed analysis of expired gases, arterial and mixed venous blood gases, and intestinal and sublingual videomicroscopy. RESULTS: In the bleeding group during the last step of hemorrhage, and compared to the sham group, there were decreases in oxygen consumption (3.7 [2.8-4.6] vs. 6.8 [5.8-8.0] mL min-1 kg-1, P < 0.001) and increases in respiratory quotient (0.96 [0.91-1.06] vs. 0.72 [0.69-0.77], P < 0.001). Retransfusion normalized these variables. The Pv-aCO2/Ca-vO2 increased in the last step of bleeding (2.4 [2.0-2.8] vs. 1.1 [1.0-1.3], P < 0.001) and remained elevated after retransfusion, compared to the sham group (1.8 [1.5-2.0] vs. 1.1 [0.9-1.3], P < 0.001). Pv-aCO2/Ca-vO2 had a weak correlation with respiratory quotient (Spearman R = 0.42, P < 0.001). All the intestinal and sublingual microcirculatory variables were affected during hemorrhage and improved after retransfusion. The recovery was only complete for intestinal red blood cell velocity and sublingual total and perfused vascular densities. CONCLUSIONS: Although there were some minor differences, intestinal and sublingual microcirculation behaved similarly. Therefore, sublingual mucosa might be an adequate window to track intestinal microvascular reperfusion injury. Additionally, Pv-aCO2/Ca-vO2 was poorly correlated with respiratory quotient, and its physiologic behavior was different. Thus, it might be a misleading surrogate for anaerobic metabolism.

INTRODUCTION: Lead has neurotoxic effects in children, even at a very low level in blood. The risk factors (RFs) for lead exposure have not been adequately identified in La Plata. The objectives of this study were to determine mean blood lead levels and identify RFs in children aged 1 to 6 years old living in La Plata and the outskirts. POPULATION AND METHODS: A cross-sectional study was conducted in children who attended primary health care centers for a health check-up. Blood lead levels were determined by atomic absorption spectroscopy, and a socioenvironmental survey was administered to outline RFs. The Mann-Whitney test was used to compare measurements. A multivariate statistical analysis was done to establish the most relevant RFs. RESULTS: A total of 319 children participated (51% were boys); the median (interquartile range) blood lead level was 2.2 pg/dL (1.1-3.6 pg/dL). Significant mean differences in blood lead levels were observed for age≤ 3years old, anemia, pica behavior, overcrowding, dirt floors, and maternal education < 7 years. Age≤ 3years old and pica behavior were both RFs with significant odds ratios (ORs). The OR as adjusted by logistic regression was significant only for age≤ 3years old. CONCLUSIONS: The median blood lead level in the studied population was 2.2 pg/dL. The main RFs identified for lead exposure were age≤ 3years old and pica behavior. Other less relevant RFs included anemia, maternal education < 7 years, overcrowding, and dirt floors.

Los objetivos fueron determinar la media de plombemia e identificar los FR en niños de 1 a 6 años de la ciudad de La Plata y alrededores. POBLACIÓN Y MÉTODOS: Se realizó un estudio de corte transversal en niños que concurrieron a controles de salud en centros de atención primaria. Se determinó la plombemia por absorción atómica y se realizó una encuesta socioambiental para relevar los FR. Se usó la prueba de Mann-Whitney para comparar medias. Se realizó un análisis estadístico multivariable para determinar los FR más relevantes. RESULTADOS: Participaron 319 niños (51% de varones); la mediana y el rango intercuartílico de plombemia fue 2,2 (1,1-3,6) pg/dL. Se hallaron diferencias de media de plombemia significativas para edad ≤ 3 años, anemia, hábito de pica, hacinamiento, piso de tierra y escolaridad materna menor de 7 años. Los FR edad≤ 3años y hábito de pica tuvieron OR significativos. El OR ajustado por regresión logística fue significativo solo para edad ≤ 3 años. CONCLUSIONES: La mediana de plombemia de la población estudiada fue de 2,2 pg/dL. Los principales FR de exposición al plomo identificados fueron edad≤ 3años y hábito de pica. Otros factores menos relevantes fueron anemia, escolaridad materna menor de 7 años, hacinamiento y piso de tierra.

Introducción. El enriquecimiento de la harina de trigo es una de las estrategias de salud pública recomendadas para prevenir las deficiencias de micronutrientes, entre ellos el hierro. El objetivo de este trabajo fue determinar el contenido de hierro en harina de trigo enriquecida y sus derivados, y la contribución de estos a las recomendaciones nutricionales en niños. Población y métodos. Estudio observacional, analítico, de corte transversal, basado en la metodología de estudio de dieta total. Se analizaron la harina de trigo enriquecida (según la ley 25630) y sus derivados más consumidos. Para seleccionar los alimentos, se realizó un cuestionario de frecuencia de consumo y recordatorio de 24 horas a los padres de niños con edades entre 6 meses y 7 años. Se determinó la concentración de hierro en los alimentos mediante absorción atómica y se evaluó su contribución al requerimiento promedio estimado. Resultados. La harina enriquecida y sus derivados tuvieron la cantidad de hierro esperada, excepto el pan francés y el pan rallado. La contribución a los requerimientos de hierro de los alimentos evaluados fue el 7 % en niños de 6 meses a 1 año, el 81 % en el grupo de 1 a 3 años y el 45 % en los mayores de 4 años. Conclusiones. Excepto el pan francés y el pan rallado, las harinas y sus derivados analizados presentaron el nivel de enriquecimiento esperado. El consumo de estos alimentos no aporta cantidades significativas de hierro en los lactantes, pero sí en los niños mayores de 1 año.


Introduction. Wheat flour enrichment is a public health strategy recommended to prevent micronutrient deficiencies, including iron deficiency. The objective of this study was to determine iron content in enriched wheat flour and flour products and their contribution to nutritional recommendations for children. Population and methods. Observational, analytical, cross-sectional study based on the total diet study method. Enriched wheat flour (as per Law no. 25630) and the most frequently consumed flour products were analyzed. Products were selected using a questionnaire on the frequency of food intake and a 24-hour recall interview with the parents of children aged 6 months to 7 years. Food iron levels were determined based on atomic absorption spectrometry and their contribution to the estimated average requirement was assessed. Results. Enriched flour and flour products showed the expected iron amount, except for French bread and breadcrumbs. The contribution of studied products to iron requirements was 7 % in children aged 6 months to 1 year, 81 % in those aged 1-3 years, and 45 % in those older than 4 years. Conclusions. Except for French bread and breadcrumbs, studied flour and flour products showed the expected enrichment level. The consumption of this type of food does not provide significant amounts of iron to infants, but it does to children older than 1 year.

The alterations in O2 extraction in hemodilution have been linked to fast red blood cell (RBC) velocity, which might affect the complete release of O2 from Hb. Fast RBC velocity might also explain the normal mucosal-arterial Pco2 (ΔPco2). Yet sublingual and intestinal microcirculation have not been completely characterized in extreme hemodilution. Our hypothesis was that the unchanged ΔPco2 in hemodilution depends on the preservation of villi microcirculation. For this purpose, pentobarbital-anesthetized and mechanically ventilated sheep were submitted to stepwise hemodilution (n = 8), hemorrhage (n = 8), or no intervention (sham, n = 8). In both hypoxic groups, equivalent reductions in O2 consumption (V̇o2) were targeted. Microcirculation was assessed by videomicroscopy, intestinal ΔPco2 by air tonometry, and V̇o2 by expired gases analysis. Although cardiac output and superior mesenteric flow increased in hemodilution, from the very first step (Hb = 5.0 g/dl), villi functional vascular density and RBC velocity decreased (21.7 ± 0.9 vs. 15.9 ± 1.0 mm/mm(2) and 1,033 ± 75 vs. 850 ± 79 µm/s, P < 0.01). In the last stage (Hb = 1.2 g/dl), these variables were lower in hemodiution than in hemorrhage (11.1 ± 0.5 vs. 15.4 ± 0.9 mm/mm(2) and 544 ± 26 vs. 686 ± 70 µm/s, P < 0.01), and were associated with lower intestinal fractional O2 extraction (0.61 ± 0.04 vs. 0.79 ± 0.02, P < 0.01) but preserved ΔPco2 (5 ± 2 vs. 25 ± 4 mmHg, P < 0.01). Therefore, alterations in O2 extraction in hemodilution seemed related to microvascular shunting, not to fast RBC velocity. The severe microvascular abnormalities suggest that normal ΔPco2 was not dependent on CO2 washout by the villi microcirculation. Increased perfusion in deeper intestinal layers might be an alternative explanation.

INTRODUCCIÓN: La Organización Mundial de la Salud (OMS) recomienda validar los sistemas de perfiles nutricionales (SPN) tomando como referencia las Guías Alimentarias del país donde se aplican. El objetivo fue evaluar el nivel de acuerdo y exigencia de los SPN latinoamericanos respecto a las Guías Alimentarias para la Población Argentina (GAPA). MÉTODOS: Se evaluó la calidad nutricional de un listado de alimentos según los SPN de la región diseñados para políticas de etiquetado frontal (EF). Se analizó el nivel de acuerdo (índice kappa y Gwet's) y de exigencia (porcentaje de alimentos a limitar) entre las clasificaciones de los SPN y los mensajes de las GAPA. RESULTADOS: Se evaluó un total de 1703 alimentos clasificados en 8 grupos y 48 categorías. Los SPN con mayor nivel de acuerdo con las GAPA fueron el SPN de la Organización Panamericana de la Salud (OPS) (Gwet's=0,663) y el SPN Uruguay (Gwet's=0,651), seguidos por el SPN Chile (Gwet's=0,558) y el SPN Ecuador (Gwet's=0,410) y, por último, el SPN Perú (Gwet's=0,373) y el SPN Bolivia (Gwet's=0,350). En el análisis del nivel de exigencia se observó que al menos uno de los SPN (OPS, Uruguay o Chile) era más exigente que las GAPA en 14 categorías y menos en 4. DISCUSIÓN: Los SPN de OPS y Uruguay son los que más se adecúan a los estándares nutricionales de las GAPA. Este estudio constituye un insumo clave para desarrollar un SPN nacional basado en evidencia científica, que asegure la efectividad de una política de EF

BACKGROUND: The aim of the study was to describe the effects of intra-abdominal hypertension (IAH) on regional and microcirculatory intestinal blood flow, renal blood flow, and urine output, as well as their response to increases in blood pressure induced by norepinephrine. METHODS: This was a pilot, controlled study, performed in an animal research laboratory. Twenty-four anesthetized and mechanically ventilated sheep were studied. We measured systemic hemodynamics, superior mesenteric and renal blood flow, villi microcirculation, intramucosal-arterial PCO2, urine output, and intra-abdominal pressure. IAH (20 mm Hg) was generated by intraperitoneal instillation of warmed saline. After 1 h of IAH, sheep were randomized to IAH control (n = 8) or IAH norepinephrine (n = 8) groups, for 1 h. In this last group, mean arterial pressure was increased about 20 mm Hg with norepinephrine. A sham group (n = 8) was also studied. Fluids were administered to prevent decreases in cardiac output. Differences between groups were analyzed with two-way repeated measures of analysis of variance (ANOVA). RESULTS: After 2 h of IAH, abdominal perfusion pressure decreased in IAH control group compared to IAH norepinephrine and sham groups (49 ± 11, 73 ± 11, and 86 ± 15 mm Hg, P < 0.0001). There were no differences in superior mesenteric artery blood flow, intramucosal-arterial PCO2, and villi microcirculation among groups. Renal blood flow (49 ± 30, 32 ± 24, and 102 ± 45 mL.min(-1).kg(-1), P < 0.0001) and urinary output (0.3 ± 0.1, 0.2 ± 0.2, and 1.0 ± 0.6 mL.h(-1).kg(-1), P < 0.0001) were decreased in IAH control and IAH norepinephrine groups, compared to the sham group. CONCLUSIONS: In this experimental model of IAH, the gut and the kidney had contrasting responses: While intestinal blood flow and villi microcirculation remained unchanged, renal perfusion and urine output were severely compromised.

Prenatal lead exposure is a health hazard that may cause cognitive development impairments and other adverse effects in children. We conducted a cross sectional study analyzing cord blood lead levels (CBLL) of newborns and their relationship with maternal determinants of lead exposure. Mothers answered a questionnaire about socio-demographic, lifestyle habits and environmental characteristics. We used Mann-Whitney's test to compare CBLL geometrical means (GM) corresponding to the presence or absence of each lead exposure determinant, and Chi square test to study the relationship between CBLL and maternal lead exposure determinants. A total of 159 newborns participated in the study. CBLL GM was 2.1 µg/dL; and 25% of the participants had a measurable CBLL (LOQ=3.3 µg/dl). Although the participants had several determinants of lead exposure, we only found a significant relationship with inside household determinants, such as presence of lead piping (p=0.026), unplastered walls (p=0.046) and peeling paint (p=0.048). Our results show that CBLL GM was similar to that reported in several studies conducted around the world. However, 25% of the participants might have some degree of risk for lead poisoning.

Introducción. El plomo es un metal tóxico, que, aun en bajas concentraciones sanguíneas, es capaz de alterar el normal desarrollo neurológico de los niños, por lo que no existe un nivel aceptable de plombemia. La absorción de plomo de los alimentos es la mayor contribución a los niveles de plombemia de la población no expuesta laboralmente o a ambientes contaminados. El objetivo fue determinar la contribución dietaria a la exposición al plomo en niños de 1 a 7 años de edad que concurrían por controles de salud al Hospital de Niños de La Plata. Población y métodos. El estudio se realizó entre junio de 2015 y mayo de 2016. Se realizó un cuestionario de frecuencia de consumo de alimentos sobre 91 niños cuya edad promedio ± desvío estándar fue de 3,0 ± 1,7 años, sobre la cual se seleccionaron los alimentos por analizar. Se compraron los alimentos seleccionados en diversos comercios de la región. Se armaron muestras compuestas de diversos grupos de alimentos. Se determinó la concentración de plomo en los grupos de alimentos y, finalmente, se estimó la ingesta diaria de plomo de la población estudiada. Resultados. La ingesta diaria de plomo fue de 138 µg/día.Los grupos de alimentos que tuvieron mayor contribución fueron los productos cárnicos procesados (15,4%), los panificados (14,8%), la leche (12,5%) y la carne (11,7%). Conclusiones. Los niños de 1 a 7 años que concurren a un hospital público de La Plata tienen una carga de plomo en la dieta de 138 µg/día.


Introduction. Lead is a toxic metal which, even at low blood levels, can alter normal neurodevelopment in children, so no blood lead level is acceptable. Lead absorption from diet accounts for the highest contribution to blood lead levels in the population who is not exposed to contaminated environments or because of their occupation. The objective of this study was to determine the contribution of diet to lead exposure among children aged 1 to 7 years who attended Hospital de Niños de La Plata for health check-ups. Population and methods. The study was conducted between June 2015 and May 2016. A questionnaire on the frequency of food intake was administered to 91 children whose average age ± standard deviation was 3.0 ± 1.7 years, and foods included in the analysis were selected based on this questionnaire. Selected foods were purchased from different regional stores. Composite samples were made up of different food groups. Lead levels corresponding to each food group were determined and, finally, the daily intake of lead was estimated for the studied population. Results. The daily intake of lead was 138 µg/day. The food groups with the higher intake rates were processed meat products (15.4%), bakery products (14.8%), milk (12.5%), and meat (11.7%). Conclusions. Children aged 1 to 7 years attending a public hospital in La Plata have a lead burden from dietary intake of 138 µg/day.

OBJECTIVE: To evaluate the effects of nitroglycerin (glyceryl trinitrate) on intestinal microcirculation during endotoxaemic shock. DESIGN: Controlled experimental study. SETTING: Research laboratory. SUBJECTS: 20 anaesthetised, mechanically ventilated sheep. INTERVENTIONS: Septic shock was induced by endotoxin infusion. After 60 minutes without resuscitation, sheep received fluid resuscitation and were randomised to control or nitroglycerin groups. Nitroglycerin was infused at a rate of 0.2 µg/kg/min for 90 minutes. MAIN OUTCOME MEASURE: Improved villi microcirculation. RESULTS: Endotoxin lowered arterial blood pressure, cardiac output and intestinal blood flow, which were improved by fluid resuscitation. Mean (SD) ileal intramucosal-arterial PCO2 gradient increased during shock and remained elevated after resuscitation in control and nitroglycerin groups (8 [8], 15 [9] and 17 [9], and 6 [6], 13 [11] and 14 [9]mmHg, respectively; P < 0.05, baseline v shock and resuscitation for both groups). Villi microvascular flow index was reduced during shock and remained lower than baseline after the resuscitation in both groups (3.0 [0.0], 2.5 [0.2] and 2.7 [0.2], and 3.0 [0.0], 2.3 [0.3] and 2.6 [0.3], respectively; P < 0.05, baseline v shock and resuscitation for both groups). The red blood cell velocity behaved similarly (859 [443], 553 [236] and 670 [276], and 886 [440], 447 [124] and 606 [235] µm/s, respectively; P < 0.05, baseline v shock and resuscitation for both groups). CONCLUSIONS: In endotoxaemic sheep, low doses of nitroglycerin failed to improve the subtle but persistent villi hypoperfusion that remains present after fluid resuscitation.

INTRODUCCION: La leche materna proveniente de los bancos de leche humana (BLH) es el mejor sustituto cuando la lactancia no es posible. Sin embargo, la pasteurización puededisminuir el contenido de vitamina A. OBJETIVO: Comparar el contenido de vitamina A en leche madura de BLH antes y después de la pasteurización, y analizar si la leche pasteurizada se adecua a los requerimientos nutricionales del lactante. METODOS:Las muestras de leche fueron recogidas por las madres donantes. Se analizó la vitamina A por cromatografía líquida, y se calculó la media de concentración antes y después de la pasteurización. Secompararon los resultados según el test de Student. Se analizó la adecuación de la vitamina a los requerimientos nutricionales del lactante. RESULTADOS: Se analizaron 53 muestras de leche de madres donantes cuya edad promedio era de 31,3±4,4 años. La media del tiempo de lactancia fue de 91±59 días. La concentraciónmedia de vitamina A en leche antes y después de la pasteurización fue de 36,6±13,5 μg/dl y 30,5±12,8 μg/dl, respectivamente (p<0,001). Se observó una disminución media de concentraciónde 6,1 μg/dl (16,7%) de vitamina A luego de la pasteurización. El porcentaje de adecuación de la vitamina A en la leche pasteurizada fue del 59,6%. CONCLUSIONES: La disminución de vitamina A en leche madura de BLH luego de la pasteurización fue del 16,7%. La concentración de vitamina A hallada en la leche pasteurizada fue insuficiente para cubrir los requerimientos del lactante.


INTRODUCTION: Breast milk from human milk banks (HMB) is the best substitute when breastfeeding is not possible, although pasteurization maydecrease vitamin A content. OBJECTIVE: To compare vitamin A content in mature milk from HMB before and after pasteurization, and to analyze whether pasteurized milk meets the infant nutritional requirements. METHODS:Milk samples were collected by donor mothers. Vitamin A was assessed by liquid chromatography, and mean concentration before and after pasteurization was calculated. Results were compared with StudentÆs t-test.Vitamin A adequacy to the infantÆs nutritional requirements was analyzed. RESULTS: A total of 53 human milksamples from donor mothers were analyzed (average age: 31.3±4.4 years; mean breastfeeding duration:91±59 days). Mean vitamin A concentration in milk before and after pasteurization was 36.6±13.5 μg/dland 30.5±12.8 μg/dl, respectively (p<0.001). The mean decrease of vitamin A concentration after pasteurization was 6.1 μg/dl (16.7%). Vitamin A adequacy percentagein milk after pasteurization was 59.6%. CONCLUSIONS: Vitamin A decrease in mature milk from HMB after pasteurization was 16.7%. Vitamin A concentration in milk after pasteurization was not sufficient to meet the infantÆs needs.
