RESUMO

The purpose of this study is to describe the technical modalities and to evaluate the results of percutaneous renal surgery for ureteropelvic stenosis, pyelocaliceal diverticula, renal cysts and urothelial tumors of the kidney. Based on a review of literature and although the number of the reported cases is limited, the results indicate a great deal of variability in the success of these technics depending on the pathology.

RESUMO

Pyelocaliceal diverticula sometimes give rise to complications such as lithiasis or infection requiring surgical treatment. In addition to various open surgical techniques there is the possibility of percutaneous surgery. This short series of nine cases collected in 2 years covers a complete review of the different therapeutic possibilities.

The authors report 3 cases of haematic cysts of the adrenal, discovered in young adults (3 female), presenting with lumbar or lumbo-abdominal pain, sometimes violent, and accompanied by fever. In each cases there was a palpable swelling in the hypochondrium and lumbar fossa. In two cases severe anaemia reflected the amount of haemorrhage which had taken place. IVU with a prolonged vascular nephrotomography time with several views and echotomography confirmed the diagnosis, avoiding the need for aortography whenever possible. The size of the cysts was such that adrenalectomy was necessary in two cases and the extent of adhesions required simultaneous nephrectomy. This is followed by a review of the literature, the dominant features being: the prevalence of haematic cysts which account for 40% of adrenal cysts, the absence of specificity of clinical symptoms and signs and the importance of radiological examinations, and finally the usual absence of any cause even though trauma may sometimes be possible.

RESUMO

Renal angiomyolipomas are benign mesenchymal tumours. Whether isolated or associated with phacomatosis, they are histologically identical, but it is often difficult to distinguish an isolated angiomyolipoma from renal cancer. When it is combined with phacomatosis, it is frequently bilateral and can provoke retroperitoneal haemorrhages. These peculiarities qualify the therapeutical management which would a priori be conservative in the case of a benign tumour.

RESUMO

Remembering that the diagnosis of stress urinary incontinence is essentially clinical, the authors describe the fundamental stages of their technique, i.e. the musculo-aponeurotic suspension of the bladder neck (Goebell-Stoeckel technique). This operation has enabled them to have good results in 88% of their cases. The long-term study of these results shows slight deterioration of the success rate which becomes stable after 2 years.

RESUMO

155 children presenting with lithiasis have been studied. The study confirms other recent analyses: (1) Urinary lithiasis in the child is far more frequent than was previously thought. It is not always easy to diagnose because of the limited opacity of the calculi. (2) Its secondary nature is shown in 25% of the cases. (3) Its cause is still debated in spite of the evident part played by Proteus infection. (4) The prognosis is generally favourable.
