Glass ionomer cement (GIC) is a self-adhesive dental restorative material composed of a polyacrylic acid liquid and fluoro-aluminosilicate glass powder. It is commonly used for cementation during dental restoration. This study aimed to systematically review the existing literature regarding the clinical performance of GIC in load-bearing dental restorations. A comprehensive literature search was conducted in EBSCO, PubMed, Embrace, and Cochrane databases. Only randomized controlled trials (RCTs) were included in the search, and a broad search technique was used, where inclusion and exclusion criteria were applied. After a thorough evaluation, 12 RCTs were extensively reviewed, and whether GIC is suitable for load-bearing restorations was determined. Significant variations in staining surface or margin, color match, translucency, esthetic anatomical form, retention, material fracture, marginal adaptation, surface luster, occlusal contour, wear, and approximal anatomical form indicated the unsuitability of GIC. By contrast, significance differences in patient view and periodontal response indicated that GIC is suitable. No significant differences in postoperative sensitivity, recurrence of caries, or tooth integrity were observed. Nevertheless, the results of the review demonstrated that the clinical performance of GIC is comparable to that of traditional restorative materials with regard to the parameters analyzed. GIC is a suitable restorative material for load-bearing restorations regarding surface margin, esthetic anatomical form, material retention and fracture, marginal adaptation, occlusal contour, wear, and approximal anatomical form. It reduces other parameters, such as postoperative sensitivity, recurrence of caries, and tooth integrity.

The mammalian and avian auditory brainstem likely arose by independent evolution. To compare the underlying molecular mechanisms, we focused on Atoh7, as its expression pattern in the mammalian hindbrain is restricted to bushy cells in the ventral cochlear nucleus. We thereby took advantage of an Atoh7 centered gene regulatory network (GRN) in the retina including upstream regulators, Hes1 and Pax6, and downstream targets, Ebf3 and Eya2. In situ hybridization demonstrated for the latter four genes broad expression in all three murine cochlear nuclei at postnatal days (P) 4 and P30, contrasting the restricted expression of Atoh7. In chicken, all five transcription factors were expressed in all auditory hindbrain nuclei at embryonic day (E) 13 and P14. Notably, all five genes showed graded expression in the embryonic nucleus magnocellularis (NM). Atoh7 was highly expressed in caudally located neurons, whereas the other four transcription factors were highly expressed in rostrally located neurons. Thus, Atoh7 shows a strikingly different expression between the mammalian and avian auditory hindbrain. This together with the consistent absence of graded expression of GRN components in developing mammalian nuclei provide the first molecular support to the current view of convergent evolution as a major mechanism in the amniote auditory hindbrain. The graded expression of five transcription factors specifically in the developing NM confirms this nucleus as a central organizer of tonotopic features in birds. Finally, the expression of all five retinal GRN components in the auditory system suggests co-options of genes for development of sensory systems of distinct modalities.

Immunity to malaria has a major role in controlling disease and pathogenesis with cytokine production being involved in almost every phase of the immune response. The present study aimed to assess hematological variables and to measure plasma levels of TNFα, IFNγ and IL10, their ratios, and their relation to parasitemia among patients with uncomplicated falciparum malaria in Hodeidah, Yemen. Forty patients with uncomplicated P. falciparum monoinfection and 40 healthy age and sex-matched controls were enrolled in the study. Parasitological diagnosis was confirmed, and parasite density was estimated. Plasma cytokine levels, hematologic parameters, and the presence of gametocytes were determined. Results revealed higher TNFα, IFNγ and IL10 in patients than in controls. A relatively higher IL10 production was demonstrated by the significantly lower TNFα/IL10 and IFNγ/IL10 ratios in patients than in controls. TNFα and IL10 correlated positively with parasite density. Lower Hb levels, RBC, lymphocyte and platelet counts, and higher neutrophil and reticulocyte counts were observed in patients compared to controls. Reticulocyte count was higher and IFNγ level was lower in the presence of gametocytes. Conclusively, uncomplicated falciparum malaria is associated with the ability to regulate the production of pro-inflammatory and anti-inflammatory cytokines. This mediates parasite clearance while simultaneously avoiding severe pathology.

Unmanned aerial vehicle (UAV) networks offer a wide range of applications in an overload situation, broadcasting and advertising, public safety, disaster management, etc. Providing robust communication services to mobile users (MUs) is a challenging task because of the dynamic characteristics of MUs. Resource allocation, including subchannels, transmit power, and serving users, is a critical transmission problem; further, it is also crucial to improve the coverage and energy efficacy of UAV-assisted transmission networks. This paper presents an Enhanced Slime Mould Optimization with Deep-Learning-based Resource Allocation Approach (ESMOML-RAA) in UAV-enabled wireless networks. The presented ESMOML-RAA technique aims to efficiently accomplish computationally and energy-effective decisions. In addition, the ESMOML-RAA technique considers a UAV as a learning agent with the formation of a resource assignment decision as an action and designs a reward function with the intention of the minimization of the weighted resource consumption. For resource allocation, the presented ESMOML-RAA technique employs a highly parallelized long short-term memory (HP-LSTM) model with an ESMO algorithm as a hyperparameter optimizer. Using the ESMO algorithm helps properly tune the hyperparameters related to the HP-LSTM model. The performance validation of the ESMOML-RAA technique is tested using a series of simulations. This comparison study reports the enhanced performance of the ESMOML-RAA technique over other ML models.

The oxygen evolution reaction (OER) can provide electrons for reducing water, carbon dioxide, and ammonia. On the other hand, copper compounds are among the most interesting OER catalysts. In this study, water oxidation of a Cu foil in the presence of K2FeO4, a soluble Fe source, under alkaline conditions (pH ≈ 13) is investigated using electrochemical methods, X-ray diffraction, X-ray photoelectron spectroscopy, in situ visible spectroelectrochemistry, Raman spectroscopy, and scanning electron microscopy. After the reaction of the Fe salt with the Cu foil, a remarkable improvement for OER is recorded, which indicates that either the Fe ions on the copper foil directly participate in OER or these ions are critical for activating copper ions on the surface toward OER. Indeed, a remarkable decrease (130 mV) in the overpotential is recorded for the Cu foil in the presence of [FeO4]2-. Tafel slopes for the Cu foil in the absence and presence of K2FeO4 are 113.2 and 46.4 mV/decade, respectively. X-ray photoelectron spectroscopy shows that there is a strong interaction between Cu(II) and Fe(III) on the surface of the Cu foil. During OER in the presence of Cu(II) (hydr)oxide, Cu(III) is detected. In situ visible spectroelectrochemistry shows that Cu and Fe ions are dynamically active and precipitate on the surface of the counter electrode during cyclic voltammetry (CV). The isotopic experimental data using H218O based on Raman spectroscopy show that there is no change in the lattice oxygen. All of these experiments adopt a new perspective on the role of Fe in OER in the presence of a Cu foil under alkaline conditions.

The design of molecular-based catalysts for oxygen-evolution reaction (OER) requires more investigations for the true catalyst to be found. First-row transition metal complexes are extensively investigated for OER, but the role of these metal complexes as a true catalyst is doubtful. Some doubts have been expressed about the role of first-row transition metal complexes for OER at high overpotentials (η > 450). Generally, the detection of the true catalyst has so far been focused on high overpotentials (η > 450) because at low overpotentials (η < 450), many methods are not sensitive enough to detect small amounts of heterogeneous catalysts on the electrode surface during the first seconds of the reaction. Ni(II) phthalocyanine-tetra sulfonate tetrasodium (1) is in moderate conditions (at 20-50 °C and pH 5-13) in the absence of electrochemical driving forces, which could make it noteworthy for OER. Herein, the results of OER in the presence of 1 at low overpotentials under alkaline conditions are presented. In addition, in the presence of Ni complexes, using an Fe ion is introduced as a new method for detecting Ni (hydr)oxide under OER. Our experiments indicate that in the presence of a homogeneous OER (pre)catalyst, a deep investigation is necessary to rule out the heterogeneous catalysts formed. Our approach is a roadmap in the field of catalysis to understand the OER mechanism in the presence of a molecular Ni-based catalyst design. Our results shown in this study are likely to open up new perspectives and discussion on many molecular catalysts in a considerable part of the chemistry community.

In vehicular ad hoc networks (VANETs), content pre-caching is a significant technology that improves network performance and lowers network response delay. VANET faces network congestion when multiple requests for the same content are generated. Location-based dependency requirements make the system more congested. Content pre-caching is an existing challenge in VANET; pre-caching involves the content's early delivery to the requested vehicles to avoid network delays and control network congestion. Early content prediction saves vehicles from accidents and road disasters in urban environments. Periodic data dissemination without considering the state of the road and surrounding vehicles are considered in this research. The content available at a specified time poses considerable challenges in VANET for content delivery. To address these challenges, we propose a machine learning-based, zonal/context-aware-equipped content pre-caching strategy in this research. The proposed model improves content placement and content management in the pre-caching mode for VANET. Content caching is achieved through machine learning, which significantly improves content prediction by pre-caching the content early to the desired vehicles that are part of the zone. In this paper, three algorithms are presented, the first is zone selection using the customized algorithm, the second is the content dissemination algorithm, and the third is the content pre-caching decision algorithm using supervised machine learning that improves the early content prediction accuracy by 99.6%. The cache hit ratio for the proposed technique improves by 13% from the previous techniques. The prediction accuracy of the proposed technique is compared with CCMP, MLCP, and PCZS+PCNS on the number of vehicles from 10 to 150, with an improved average of 16%. Finally, the average delay reduces over time compared with the state-of-the-art techniques of RPSS, MLCP, CCMP, and PCZS+PCNS. Finally, the average delay shows that the proposed method effectively reduces the delay when the number of nodes increases. The proposed solution improves the content delivery request while comparing it with existing techniques. The results show improved pre-caching in VANET to avoid network congestion.

BACKGROUND: Although many studies have discussed nurses' caring performance, few have focussed on critical care nurses. Also, none have studied this among newly graduated nurses despite their very different circumstances. AIM: The purpose of this study was to examine factors affecting the caring performance of newly graduated nurses in critical care units. METHOD: A descriptive cross-sectional design, using self-reported questionnaires, was used among a sample of 209 nurses interviewed in Jordanian hospitals. The Mental Health Professionals Stress Scale was used for measuring work-related stress and the Caring Behaviours Assessment tool to assess caring performance. Data collection was conducted from March to May 2018. RESULTS: The participants had moderate levels of caring performance (M = 182.6/315, SD = 15.46). The highest score was reported for 'Supportive/Protective/Corrective Environment' subscale, while the lowest for 'Teaching/Learning' subscale. Many significant differences were noticed in caring performance based on participants' demographics. Only working in private hospitals was a significant predictor for caring performance (P < .05) predicting 14% of variance. CONCLUSION: Caring performance of newly graduated nurses in critical care units was affected by many factors including workload, self-doubt, income and area of working. The significance was flagged of developing nursing policies concerned with stress relief and improvement of caring performance.

Ankle fractures are extremely common, with isolated distal fibular fractures being the most common variant. The current gold standard in treating unstable distal fibular fractures is open reduction internal fixation. However, with potential risk of wound complications, minimally invasive techniques have been introduced. This systematic review was performed to evaluate the clinical and functional outcomes of varying minimally invasive techniques including minimally invasive plate osteosynthesis, intramedullary (IM) nailing, and IM screw fixation. A comprehensive English literature search on PubMed was performed yielding 543 studies. With specific study selection criteria, a total of 13 articles were selected. After studying the reference of each of the 13 studies, an additional 7 articles were included, resulting in a total of 20 studies reviewed. A total of 8 articles reviewed used IM nailing as the fixation of choice with a total of 211 patients. Of the 211 patients, 33 experienced complications. Six articles using minimally invasive plate osteosynthesis were reviewed with a total of 264 patients. Of the 264 patients, 39 experienced complications. IM screw fixation was used in 6 articles reviewed with a total of 219 patients. There were 30 cases of complications from the 132 patients. Additionally, mean American Orthopedic Foot and Ankle Society hindfoot-ankle scores among all fixation types was 88.4 ± 3.40 whereas the mean Olerud and Molander Score among all fixation types was 76.7 ± 16.58. The results of this study indicate that minimally invasive techniques for fixation of distal fibular fractures can provide excellent functional results with low complication rates compared with traditional open reduction internal fixation.

How cognitive neural systems process information is largely unknown, in part because of how difficult it is to accurately follow the flow of information from sensors via neurons to actuators. Measuring the flow of information is different from measuring correlations between firing neurons, for which several measures are available, foremost among them the Shannon information, which is an undirected measure. Several information-theoretic notions of "directed information" have been used to successfully detect the flow of information in some systems, in particular in the neuroscience community. However, recent work has shown that directed information measures such as transfer entropy can sometimes inadequately estimate information flow, or even fail to identify manifest directed influences, especially if neurons contribute in a cryptographic manner to influence the effector neuron. Because it is unclear how often such cryptic influences emerge in cognitive systems, the usefulness of transfer entropy measures to reconstruct information flow is unknown. Here, we test how often cryptographic logic emerges in an evolutionary process that generates artificial neural circuits for two fundamental cognitive tasks (motion detection and sound localization). Besides counting the frequency of problematic logic gates, we also test whether transfer entropy applied to an activity time-series recorded from behaving digital brains can infer information flow, compared to a ground-truth model of direct influence constructed from connectivity and circuit logic. Our results suggest that transfer entropy will sometimes fail to infer directed information when it exists, and sometimes suggest a causal connection when there is none. However, the extent of incorrect inference strongly depends on the cognitive task considered. These results emphasize the importance of understanding the fundamental logic processes that contribute to information flow in cognitive processing, and quantifying their relevance in any given nervous system.

OBJECTIVE: The aim of the article is a theoretical and methodological justification for strengthening the role of international trade in medicines and equipment during the COVID-19 pandemic, to establish the benefits of free trade, to develop important international trade policy priorities in this field. PATIENTS AND METHODS: Materials and methods: Legislation of foreign countries and international organizations, statistical data published by international organizations are the materials of the research. Methods of analysis, synthesis, generalization were applied during the research. CONCLUSION: Conclusions: It has been proved that the legal regulation of world trade activity by medical supplies are increasingly playing more important roles in the implementation of international development goals. The authors have offered three priorities to all countries: to cooperate globally around the deployment of life-saving medical supplies, publicly commit all countries not to implement export bans or limits on relevant medical supplies; to control all trade measures countries have taken in response to the COVID-19 outbreak; to bring together in a global form measures and procedures to eliminate this phenomenon.

OBJECTIVES: The present study aimed to evaluate the outcome and potential limitations of a repeated MitraClip procedure (ReClip). BACKGROUND: The MitraClip procedure has emerged as a treatment option in high surgical risk patients suffering from severe mitral regurgitation (MR). However, despite successful initial repair a significant number of patients develops severe recurrent MR. METHODS: Patients undergoing a ReClip procedure in our institution were retrospectively identified. Baseline data and the procedural outcome were assessed to identify potential limitations of such procedures. RESULTS: Fifteen out of 234 patients undergoing a mitral-valve repair with the MitraClip device (Abbott Vascular) underwent a ReClip due to recurrent MR. In 11 patients, a MR reduction of at least one degree without causing mitral valve stenosis (trans-mitral mean gradient ≥5 mmHg) was achieved by performing a ReClip. After 1 year, two patients developed severe recurrent MR again. Pulmonary artery pressures significantly decreased after the procedure in individuals with successful repair (MR reduction of at least one degree and mitral valve mean gradient <5 mmHg). CONCLUSION: A ReClip procedure may be feasible in patients with recurrent MR but the risk benefit ratio should be carefully balanced against other treatment options.

PI3K/AKT signaling is involved in cell survival, proliferation, and migration. In this pathway, PI3Kα enzyme is composed of a regulatory protein encoded by p85 gene and a catalytic protein encoded by PIK3CA gene. Human PIK3CA locus is amplified in several cancers including lung and colorectal cancer (CRC). Therefore, microRNAs (miRNAs) that are encoded within the PIK3CA gene might have a role in cancer development. Here, we report a novel microRNA named PIK3CA-miR1 (EBI accession no. LN626315), which is located within PIK3CA gene. A DNA segment corresponding to PIK3CA-premir1 sequence was transfected in human cell lines that resulted in generation of mature exogenous PIK3CA-miR1. Following the overexpression of PIK3CA-miR1, its predicted target genes (APPL1 and TrkC) were significantly downregulated in the CRC-originated HCT116 and SW480 cell lines, detected by qRT-PCR. Then, dual luciferase assay supported the interaction of PIK3CA-miR1 with APPL1 and TrkC transcripts. Endogenous PIK3CA-miR1 expression was also detected in several cell lines (highly in HCT116 and SW480) and highly in CRC specimens. Consistently, overexpression of PIK3CA-premir1 in HCT116 and SW480 cells resulted in significant reduction of the sub-G1 cell distribution and apoptotic cell rate, as detected by flowcytometry, and resulted in increased cell proliferation, as detected by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay. PIK3CA-miR1 overexpression also resulted in Wnt signaling upregulation detected by Top/Fop assay. Overall, accumulative evidences indicated the presence of a bona fide novel onco-miRNA encoded within the PIK3CA oncogene, which is highly expressed in colorectal cancer and has a survival effect in CRC-originated cells.

Our previous studies in an experimental model of rabies showed neuronal process degeneration in association with severe clinical disease. Cultured adult rodent dorsal root ganglion neurons infected with challenge virus standard (CVS)-11 strain of rabies virus (RABV) showed axonal swellings and reduced axonal growth with evidence of oxidative stress. We have shown that CVS infection alters a variety of mitochondrial parameters and increases reactive oxygen species (ROS) production and mitochondrial Complex I activity vs. mock infection. We have hypothesized that a RABV protein targets mitochondria and triggers dysfunction. Mitochondrial extracts of mouse neuroblastoma cells were analyzed with a proteomics approach. We have identified peptides belonging to the RABV nucleocapsid protein (N), phosphoprotein (P), and glycoprotein (G), and our data indicate that the extract was most highly enriched with P. P was also detected by immunoblotting in RABV-infected purified mitochondrial extracts and also in Complex I immunoprecipitates from the extracts but not in mock-infected extracts. A plasmid expressing P in cells increased Complex I activity and increased ROS generation, whereas expression of other RABV proteins did not. We have analyzed recombinant plasmids encoding various P gene segments. Expression of a peptide from amino acid 139-172 increased Complex I activity and ROS generation similar to expression of the entire P protein, whereas peptides that did not contain this region did not increase Complex I activity or induce ROS generation. These results indicate that a region of the RABV P interacts with Complex I in mitochondria causing mitochondrial dysfunction, increased generation of ROS, and oxidative stress.

The objectives of this study were to examine nurses' perceptions of the hospital safety culture in Jordan and to identify the relationships between aspects of hospital safety culture and selected safety outcomes. Data from 242 registered nurses in five Jordanian hospitals were analyzed. Aspects of hospital safety culture and outcomes were measured using the Hospital Survey on Patient Safety Culture. Among various aspects of hospital safety culture, teamwork within units had the highest average percentage of positive responses (49.8%). Additionally, participants reported deficits in other aspects of safety culture, particularly in staffing and nonpunitive response to errors, with average percentages of positive responses of 30.4% and 30.7%, respectively. Pearson correlation analysis revealed that 9 of 10 subscales of hospital safety culture were significantly correlated to one or more of the hospital safety outcomes. The findings of this study can help policymakers and healthcare administrators identify the weaknesses and strengths of hospital safety issues in order to propose effective strategies to improve patient safety and quality of care.

BACKGROUND: Nurses from various setting lack sufficient knowledge about diabetes and diabetes management. Better understand of evidence-based practices by nurses who are involved in caring for hospitalized individuals with diabetes can positively influence care outcomes. METHOD: A pretest design was used to evaluate the effectiveness of a diabetes education program for RNs working voluntarily participated. A 1-day education program was developed and delivered to the participating nurses. Knowledge regarding diabetes was tested before and after the education program. RESULTS: a significant difference was noted in the modified diabetes basic knowledge mean test scores before and after implementation of the education program (t[128] = 17.95, P < 0.001). CONCLUSIONS: The diabetes education program had a positive on nurses' knowledge. This finding has implications for developing diabetes education content within nursing curricula, as well as continuing education courses for practicing nurses.

PURPOSE: (1) To assess Jordanian ICU patients' pain characteristics (intensity and interference) and levels of pain management satisfaction; and (2) to determine potential predictors of pain management satisfaction among ICU patients. METHODS: A descriptive cross-sectional design was utilised using the American Pain Society-Patient Outcome Questionnaire to survey 139 Jordanian ICU patients from different health care sectors in Jordan. RESULTS: High levels of pain and pain interferences were reported by participants, which were higher than those reported by previous studies in other countries. However, participants were relatively satisfied with pain management approaches. Also, the results showed a predictive model of three potential predictors, which accounted for 36% of the variance in participants' satisfaction with pain management (adjusted R(2)=0.36, F=12.14, df=7129, p<0.005). The strongest predictor to participants' satisfaction with pain management was time needed to get analgesia (beta=-0.480, p<0.001), followed by average pain interference (beta=0.218, p=0.02), and being told about importance of reporting pain (beta=0.198, p=0.006). CONCLUSION: Jordanian ICU patients reported high pain levels, which supports the need for applying a caring attitude in managing patients' pain reports. Also, such a study is among the first pain management studies in Jordan aiming at setting the stage for future research studies. Finally, results can be included in planning pain management strategies and protocols within hospitals.

Mitochondrial dysfunction occurs in sensory neurons and contributes to diabetic neuropathy. Ciliary neurotrophic factor (CNTF) stimulates axon regeneration in type 1 diabetic rodents and prevents deficits in axonal caliber, nerve conduction, and thermal sensation. We tested the hypothesis that CNTF enhances sensory neuron function in diabetes through JAK/STAT (Janus kinase/signal transducers and activators of transcription) signaling to normalize impaired mitochondrial bioenergetics. The effect of CNTF on gene expression and neurite outgrowth of cultured adult dorsal root ganglia (DRG) sensory neurons derived from control and streptozotocin (STZ)-induced diabetic rodents was quantified. Polarization status and bioenergetics profile of mitochondria from cultured sensory neurons were determined. CNTF treatment prevented reduced STAT3 phosphorylation (Tyr 705) in DRG of STZ-diabetic mice and also enhanced STAT3 phosphorylation in rat DRG cultures. CNTF normalized polarization status of the mitochondrial inner membrane and corrected the aberrant oligomycin-induced mitochondrial hyperpolarization in axons of diabetic neurons. The mitochondrial bioenergetics profile demonstrated that spare respiratory capacity and respiratory control ratio were significantly depressed in sensory neurons cultured from STZ-diabetic rats and were corrected by acute CNTF treatment. The positive effects of CNTF on neuronal mitochondrial function were significantly inhibited by the specific JAK inhibitor, AG490. Neurite outgrowth of sensory neurons from age-matched control and STZ-induced diabetic rats was elevated by CNTF and blocked by AG490. We propose that CNTF's ability to enhance axon regeneration and protect from fiber degeneration in diabetes is associated with its targeting of mitochondrial function and improvement of cellular bioenergetics, in part, through JAK/STAT signaling.

Pain is still undertreated among hospitalized patients. Recently, patient satisfaction with pain management has received significant attention. This field has not yet been explored among Jordanian patients. The purpose of this study was to determine the knowledge regarding pain characteristics, beliefs, and satisfaction that can be included in planning pain management strategies and protocols within Jordanian hospitals. Using descriptive cross-sectional methodology, the American Pain Society Patient Outcome Questionnaire (APS-POQ) was used to survey 375 inpatients from Jordanian hospitals. Participants reported relatively severe pain and pain interferences while being hospitalized and seemed to be well informed regarding pain and pain management. Participants reported high levels of pain management satisfaction. Also, the Arabic version of the APS-POQ was found to be reliable among the Jordanian population. Findings of this study are similar to those reported by earlier studies in other countries and support the need for applying the caring attitude in managing patients' reports of having pain. This study is the first in Jordan, opening the door for future studies to be conducted in this important field.

RESUMO

The purpose of this paper is to examine the impact of demographic variables, organizational commitment levels, perception of health and quality of work on turnover intentions. A self-reported cross-sectional survey design was used to collect data from Jordanian registered nurses who were working between June 2011 and November 2011. The findings showed strong effects of the quality of work, perception of health and normative organizational commitments on turnover intentions. This study sheds the light on the important work outcomes in health-care organizations. Increasing nursing quality of work and normative organizational commitment are good strategies for reducing turnover intentions.
