RESUMO

Melnick-Needles syndrome is an X-linked dominant skeletal dysplasia in which patients often succumb at an early age to chronic pulmonary disease. Radiographic findings of Melnick-Needles syndrome consist of characteristic bony abnormalities and interstitial lung disease. We present the high-resolution computed tomographic (CT) findings in a 39-year-old survivor of Melnick-Needles syndrome. The clinical and physical exam findings also demonstrate an association between Melnick-Needles syndrome and obstructive sleep apnea.

RESUMO

RATIONALE AND OBJECTIVES: Evaluate proton magnetic resonance spectroscopy ((1)H-MRS) for assessing and grading brain tumors. MATERIALS AND METHODS: The research was done at Detroit Medical Center in a 1.5-T Siemens MR magnet using single-voxel or multivoxel MRS. This study consisted of 27 patients: 10 females and 17 males ages 22-83 years (average age 43.8). The data were recorded for three peaks-N-acetyl aspartate (NAA), choline (Cho), creatine (Cr)-which were used to calculate the ratios Cho/NAA and Cho/Cr. RESULTS: Abnormal spectra were seen in 25 patients and normal spectra in 2. In 16 patients with brain astrocytoma of various grades, the pathology grading was correlated with Cho/NAA and Cho/Cr. These values were 6.53 and 3.35 for nine patients with Grade 4 astrocytoma; 1.85 and 1.62 for three patients with Grade 3 astrocytoma; 2.21 and 1.50 for three patients with Grade 2 astrocytoma; and 1.45 and 1.49 for one patient with Grade 1 astrocytoma. The remaining nine patients with abnormal spectra were also correlated with pathology. CONCLUSION: MRS ratios can be used to differentiate malignant and nonmalignant lesions from normal brain tissue. In general, high-grade astrocytoma have higher Cho/NAA and Cho/Cr ratios compared with low-grade astrocytoma.

RATIONALE AND OBJECTIVE: Intentional deception (ie, lying) is a complex cognitive act, with important legal, moral, political, and economic implications. Prior studies have identified activation of discrete anterior frontal regions, such as the ventrolateral prefrontal cortex (VLPFC), dorsolateral prefrontal cortex (DLPFC), dorsal medial prefrontal cortex (DMPFC), and anterior cingulate cortex (ACC) during deception. To extend these findings, we used novel real-time functional magnetic resonance imaging (fMRI) technology to simulate a polygraph experience in order to evoke performance anxiety about generating lies, and sought to ascertain the neural correlates of deception. MATERIALS AND METHODS: In this investigational fMRI study done with a 4-T scanner, we examined the neural correlates of lying in 14 healthy adult volunteers while they performed a modified card version of the Guilty Knowledge Test (GKT), with the understanding that their brain activity was being monitored in real time by the investigators conducting the study. The volunteers were instructed to attempt to generate Lies that would not evoke changes in their brain activity, and were told that their performance and brain responses were being closely monitored. RESULTS: Subjects reported performance anxiety during the task. Deceptive responses were specifically associated with activation of the VLPFC, DLPFC, DMPFC, and superior temporal sulcus. DISCUSSION: These findings suggest the involvement of discrete regions of the frontal cortex during lying, and that the neural substrates responsible for cognitive control of behavior may also be engaged during deception.

Viswanathan's Preference for Numerical Information scale was used to investigate the attitudes of 236 African-American college students toward using numerical information in their thinking and problem solving. The results showed that the participants expressed a positive preference for numerical information, statistically similar in size to the preferences reported in prior research for other groups of students. Also, participants' scores correlated positively (r=.27) with their academic achievement (fraction answered correctly) on examination questions requiring the use of numerical information but were nonsignificantly related to their academic achievement on questions not requiring the use of numerical information. This pattern of correlations persisted, at reduced levels, when the effects of variations in SAT scores were partialled out. Moreover, for the men correlations between Preference for Numerical Information scores and achievement on examination questions involving numerical information significantly exceeded the correlations between the test scores and academic achievement on questions not involving numerical information. This contrasted with results obtained for women. For them, the correlations did not vary significantly by question type. Thus, for reasons yet to be uncovered, Preference for Numerical Information scores of the African-American men appear to reflect an aspect of cognitive style related to proficiency in using and manipulating numerical information.

RESUMO

Retinal hemorrhage is a well-recognized manifestation of child abuse found in many babies with shaken baby syndrome. The presence of retinal hemorrhage is generally associated with more severe neurological damage and a worse clinical outcome. MR imaging findings of retinal hemorrhages are not well described in the pediatric literature. We present a 6-month-old boy with new-onset seizures, subdural hemorrhage and bilateral retinal hemorrhages that were detected by MRI and confirmed by indirect ophthalmoscopy. This case demonstrates the MR imaging findings of retinal hemorrhages and the importance of radiologists being able to recognize these specific imaging features.

MRI in a 9-year-old boy with left testicular polyorchidism and prior right orchiectomy showed single epididymis-ductus deferens complex. This anatomical arrangement suggests the possibility of reproductive potential and alters the patient's management.

RESUMO

Manpower shortages have led many radiologists to consider using radiologist extenders, and the American Society of Radiological Technologists and the ACR have responded by formulating the radiology assistant (RA) program. There is no doubt that the RA program is becoming a reality or that it can be a boon to radiologists and physician radiologist trainees in freeing up faculty members' time, freeing trainees from repetitive tasks, and providing limited instruction in RAs' areas of expertise. Patients will likely accept RAs and trainee physicians equally. A financial analysis of using residents and physician extenders in anesthesia suggests that residents may be a cheaper labor source in those institutions not at the cap for resident positions.
