RESUMO

BACKGROUND: The adverse health effects of occupational exposure to asbestos dust may occur several years after first exposure. The objective of the study was to assess the relationship between lesions in the respiratory system and the factors contributing to occupational exposure to asbestos described in the first medical examination as well as to analyze the factors responsible for the progression of these changes in further medical tests. MATERIAL AND METHODS: The study group comprised 591 former workers of asbestos processing plant "Gambit" in Lubawka. The results of medical examinations carried out in 2001-2012 were assessed. Statistical inference was performed based on bilateral significance tests at the 0.05 level of significance. RESULTS: A higher risk of interstitial lung changes along with an increase in the cumulative concentration of asbestos was indicated; for the employees with the highest exposure, the adjusted odds ratio (OR) was 1.63 (95% confidence interval (CI): 0.99-2.71), while for changes with the severity degree qualifying for asbestosis diagnosis, the risk was significantly increased, over fivefold higher, compared to subjects employed in the lowest exposure. The analysis of the relationship between the progression of interstitial changes and the exposure to asbestos dust showed a fourfold higher risk of the progression in workers employed in the highest exposure. Mean values of FEV1 (forced expiratory volume in 1 s), FVC (forced vital capacity), FEV1/FVC (forced expiratory volume in 1 s to forced vital capacity) were significantly lower in the subjects working in a higher asbestos exposure. The effect of tobacco smoking on the occurrence of interstitial lung changes and their progression was also confirmed. CONCLUSIONS: The results of prophylactic medical examinations of the health status of workers formerly employed in the plants using chrysotile indicate the importance andthe need for a long-term clinical follow-up and the promotion of anti-smoking prevention in this group of former employees. RESUMO

BACKGROUND: Occupational exposure of workers to mechanical hand transmitted vibrations may result in the development of nonspecific lesions. Balanced diet is one of factors that protect human organism against abnormalities resulting from occupational exposure. The aim of this study was to assess the supply of energy and macronutrients in daily food rations (DFR) of people exposed to hand transmitted vibration. MATERIALS AND METHODS: Eighty workers, including 37 men (mean age 44.6 years) and 43 women (mean age 44 years) exposed to hand transmitted vibrations were recruited in this study. Of those, 72 people worked in crystal glassworks as glass cutters, 5 people were physiotherapists and 3 were woodcutters. For all workers, there was measured exposure to hand transmitted vibration. Nutritional status was assessed using 24-hour dietary recall; the procedure was repeated 3 times. The results of the nutritional assessment were compared with recommended daily allowances (RDA) for energy, protein, minerals and vitamins, total carbohydrates, total fat, saturated fatty acids (SFA), monounsaturated fatty acids (MUFA), polyunsaturated fatty acids (PUFA), cholesterol and dietary fibre. RESULTS: The average intake of energy in daily food rations of the women was too low while men received the appropriate amount of energy. Average protein intake by men and women was higher than the relevant RDA. The average amount of carbohydrates received by the surveyed women and men were too low, just as the consumption of PUFA, dietary fibre, potassium and calcium. We also found an excessive, average supply of total fat, MUFA, SFA, cholesterol and most of the vitamins with the DFR of the women and men. CONCLUSIONS: The nutrition in the group of workers exposed to hand transmitted vibration was found to be imbalanced due to excessive intakes of total fat, saturated fatty acids and cholesterol combined with insufficient consumption of carbohydrates, dietary fibres, potassium and calcium. The co-occurrence of vibration and imbalanced diet may increase the susceptibility of the subjects to cardiovascular and bone diseases.

RESUMO

Road accident belongs to one of the categories of traumatic events, and can cause posttraumatic stress disorder (PTSD). The most common psychological consequences of road traffic accidents are the emotional disorders, cognitive deficits (disorders concentration of attention and memory function), impaired social relationships, troubles with performing work duties and physical symp-toms of stress. The article discusses the program of psychological support given to the participants of road accidents, conducted in Wroclaw, its theoretical assumptions and the mode of its implementation. Basic theoretical assumptions of the program are inter alia based on the theories of H. Selye and R. Lazarus. The authors of this article also refer to the so-called therapeutic process of education designed by Everly and Rosenfeld, who recommend that the process of developing self-responsibility be used for therapeutic purposes. This requires clarifying the exact nature of the problem, and then looking for possible remedies. The program is open to all road accident participants (victims, perpetrators, witnesses) and their families. Classes are designed to combine theory (lectures) and practice (exercises). Anxiety and cognitive processes, as well as relaxation training, interpersonal training (eg, assertiveness) and kinesiology are the major areas of activities. Psychological support provided for road accident participants is of intervention--and preventive nature; intervention, as it relates to the consequences ofa specific stressogenic event, namely a road accident; preventive, as it serves the overriding purpose--the improvement of road safety. This article presents the main findings of the program, the results of the survey evaluation, and proposals to develop psychological operations aimed at the road accidents participants.

RESUMO

Importance: Although Pyemotes species have been known to cause dermatitis, recent reports are rare. During the past 30 years, only 3 outbreaks of dermatitis caused by Pyemotes ventricosus have been reported. Objective: To analyze the causative agent of skin changes in employees of a company that produced herbal medicines. Design, Setting, and Participants: This case series includes 18 patients (8 men, 10 women; mean age, 44 years) who contracted unusual dermatitis after an occupational exposure in July and August of 2012 while working for a company that produced herbal medicines. The patients were examined at the Lower Silesia Regional Centre of Occupational Medicine in Wroclaw, Poland. Exposures: Workers weighed and packed 1 part of the Helichrysum arenarium herb. Main Outcomes and Measures: We hypothesized the causative agent to be P ventricosus. An examination of the herbal specimens for ectoparasites confirmed the diagnosis. Results: Initially 16 employees developed pruritic skin changes. Skin lesions with pruritic vesicles on an erythematous base with surrounding swelling and edema were observed. Several employees also developed a flulike illness. After 44 days, 2 new employees presented with the same skin changes. The analysis of working conditions showed that the same part of the H arenarium herb was weighed and packed at that time. Conclusions and Relevance: We found that changing the time when the herbs were weighed and abandoning gas fumigation containing methyl bromide resulted in the recurrence of an almost forgotten disease.

RESUMO

The overload of the osteoarticular system resulting from standing and stooping position of the body is the main health problem of dentists. This may cause vertebral pain, symptoms of sciatica and foot valgo-planus. Symptoms of carpal tunnel syndrome are induced by repeated carpus movements. Frequent numbness associated with the peripheral nerves changes result from using drills by dentists. Menstruation disturbances observed in dental assistants could be related to the increased levels of mercury in serum and urine. Allergy is also a frequent medical problem, particularly allergy to latex. manifested by contact eczema or allergic urticaria, asthma and shock. There also is an increased risk for infectious diseases through the contact with biological material, mostly saliva and blood.
