Patients suffering from head trauma or hemorrhagic cardiovascular accident can be cared for in special facilities. Located near Grenoble, the Fondation santé des étudiants de France Grenoble La Tronche (ex-clinique du Grésivaudan) (38) provides post-resuscitation care for brain-damaged patients. This article presents the department, its specific features and their daily routine, as shared with us by Mélanie, Leslie and her fellow nurses in the neurological rehabilitation department.

Relying on the remaining abilities of elderly people at home, in hospital or in residential care facilities for the dependent elderly, allows them to maintain their autonomy and avoid restraints. If geriatric caregivers observe an elderly person who is agitated, at risk of falling, or putting themselves in danger, they suggest strategies to calm the person. As a last resort, physicians may prescribe an appropriate restraint. This is a deprivation of liberty. The multidisciplinary evaluation every twenty-four hours of this care is based on the principle of beneficence by re-evaluating the prescribed device.

In the hospital, the geriatric missions of nurses and paramedical and medical teams are essential in order to benefit elderly patients and to fight against the immobilization syndrome through responsiveness and quality care. This care takes place in parallel with the specific medical care of the medical pathologies treated. In 2017, the High Authority for Health advocated good professional practices to avoid iatrogenic dependence of hospitalized elderly people. Through this article, we carry out a focus on the immobilization syndrome and its deleterious consequences: we must not do in the place of the elderly subject!

Today in France, one in three people live with an illness or disability throughout their lives. Due to the successive confinements linked to the health crisis, therapeutic patient education (TPE) programmes have often had to be adapted, notably through telephone follow-up and videoconferences. At the same time, the legislative framework has evolved, with the law of January 1st, 2021, which replaces the authorisation scheme for TPE programmes with a declaration scheme.

Elderly people are the main victims of discontinuities in their treatment because they require appropriate care. The current system is no longer able to satisfactorily cover all these complex and increasingly important demands due to the lengthening of life expectancy and the profound crisis affecting health professionals. Feedback from the experience of nurses with a Master's degree in Advanced Practice in Gerontology since 2012 shows that in this new and critical context, they are a serious solution to be adopted by the legislator to meet these public health challenges.

Nursing clinical approach, listening to the body. In their practice working closely with patients and their families, nurses base their clinical approach on the gathering and analysis of the multiple signals sent by the body of the patient they are caring for. In collaboration with the doctor, the monitoring of vital signs and biological data helps to guide the care and the choice and implementation of therapies. A concrete case illustrates the contribution of nursing sciences to the clinical approach led by the nurse.
