RESUMO

Hereby, we present a rare case of malignant transformation in a long-standing case of pseudoepitheliomatous keratotic and micaceous balanitis (PKMB), which typically affects older men. PKMB presents as whitish or silvery keratotic plaques on the glans and can remain stable for years, leading to potential confusion regarding its progression. The patient in this case experienced urinary obstruction due to tumorous ingrowth, prompting an investigation. Despite previous treatments, including fluorouracil (5-FU) and cryoablation, the lesion persisted, eventually growing in size and becoming malodorous. Initial biopsy showed PKMB without malignancy, but subsequent deeper biopsy revealed verrucous carcinoma. The patient underwent glansectomy and reconstruction with a full-thickness skin graft, achieving a disease-free state postoperatively. The paper underscores the importance of thorough investigation for malignancy in PKMB cases, the possibility of deeper malignancy missed by superficial biopsy, and the need for early diagnosis to enable organ-sparing treatments.

The environmental behavior of radioactive cesium (RCs) in contaminated areas is generally governed by soil and sediment components and natural weathering conditions. In this study, desorption tests and spectroscopic approaches were used to explore the interaction between the weathering of micaceous minerals (i.e., biotite and phlogopite) and the adsorption of Cs+ and the critical role of weathering in the environmental behavior of RCs. Results showed that the reaction sequence between weathering and Cs+ adsorption significantly affected the surface species of Cs+ and the structure of biotite and phlogopite. Regardless of whether it occurred before, after, or during Cs+ adsorption, weathering generated more high-affinity adsorption sites, namely, interlayer sites (ITs) and frayed edge sites (FESs), to different extents, and then facilitated the uptake of Cs+ at FESs and ITs on micaceous minerals in a poorly exchangeable state. Cs+ stabilized the micaceous mineral structure once it was absorbed within collapsed interlayers by hindering cation exchange and preventing further destruction during weathering. As important weathering factors, high temperature and Ca2+ content promoted the binding of Cs+ in the interlayers of biotite and phlogopite by enhancing interlayer cation exchange. These findings are beneficial for a better understanding of the environmental behaviors of RCs in the hydrosphere and pedosphere.

RESUMO

Pseudoepitheliomatous keratotic and micaceous balanitis (PKMB) is a rare nonvenereal penile condition with a risk of malignant transformation. We report a case of PKMB in a 45-year-old male who presented with multiple keratotic growths over his glans penis after circumcision for long-standing phimosis. Histopathology revealed pseudoepitheliomatous hyperplasia and atypical cells. He was successfully treated with wide local excision.

We report two cases of histologically documented pseudoepitheliomatous keratotic and micaceous balanitis in middle-aged male patients, which showed positivity for low-risk serotype human papillomavirus DNA. To our knowledge, only one other case has been documented. Further immunohistochemical proliferative markers were performed and compared to literature findings in penile epithelial proliferations. Evolution to invasive verrucous carcinoma has been associated with absence of HPV DNA. Thus, if confirmed by further studies, HPV testing should be included in pseudoepitheliomatous keratotic and micaceous balanitis assessment to address prognosis, and management.

Glans penis is an unusual site for horn. Only few cases are reported worldwide in English literature. Pseudoepitheliomatous, keratotic, and micaceous balanitis (PKMB) is a pyodermatitis with pseudoepitheliomatous response to chronic inflammation or infection. Rarely it can develop a horn. There is one case report of PKMB presenting as penile horn and one case with nail-like presentation in the literature. Mode of treatment of PKMB with horn ranged from topical 5-fluorouracil, electrosurgery, and cryosurgery to excision. Use of oral acitretin in PKMB or penile horn is unknown. We are reporting a 60-year-old circumcised male who presented with a penile horn. Histology was suggestive of PKMB. Treatment with topical 5-flurouracil did not work. He was successfully treated with oral acitretin.

The combined capacity of Jute Fibers (JF), the reinforcement, and Ground-Granulated Blast-Furnace Slag (GBFS), the binder, was examined as a sustainable solution towards ameliorating the inferior engineering properties of micaceous clays. A total of sixteen JF + GBFS mix designs, i.e., JF (% by total mass) = {0, 0.5, 1.0, 1.5} and GBFS (% by total mass) = {0, 3, 6, 9}, were tested for unconfined compression (UC) strength; for those mix designs containing GBFS, curing was allowed for 7 and 28 days prior to testing. Scanning electron microscopy (SEM) studies were also carried out to observe the evolution of fabric in response to JF, GBFS and JF + GBFS amendments. The greater the JF content the higher the developed strength and stiffness up to 1% JF, beyond of which the effect of JF-reinforcement led to some adverse results. The JF inclusions, however, consistently improved the ductility and toughness of the composite. The addition of GBFS to the JF-reinforced samples improved the soil⁻fiber connection interface, and thus led to further improvements in the composite's strength, stiffness and toughness. The mix design "1% JF + 9% GBFS" managed to satisfy ASTM's strength criterion and hence was deemed as the optimum choice in this investigation. Finally, a non-linear, multivariable regression model was developed and validated to quantify the peak UC strength as a function of the composite's index properties. The proposed model contained a limited number of fitting parameters, all of which can be calibrated by little experimental effort, and thus implemented for preliminary design assessments.

Relationships between Radiocesium Interception Potential (RIP) and mineralogical characteristics of the clay fraction isolated from 97 paddy soils (Hama-dori, n = 25; Naka-dori, n = 36; Aizu, n = 36) in Fukushima Prefecture, Japan were investigated to clarify the mineralogical factors controlling the (137)Cs retention ability of soils (half-life 30.1 y). Of all the fission products released by the Fukushima accident, (137)Cs is the most important long-term contributor to the environmental contamination. The RIP, a quantitative index of the (137)Cs retention ability, was determined for the soil clays. The composition of clay minerals in the soil clays was estimated from peak areas obtained using X-ray diffraction (XRD) analyses. The predominant clay mineral was smectite in soils from Hama-dori and Aizu, while this was variable for those from Naka-dori. Native K content of the soil clays was found to be an indicator of the amount of micaceous minerals. The average RIP for the 97 soil clays was 7.8 mol kg(-1), and ranged from 2.4 mol kg(-1) to 19.4 mol kg(-1). The RIP was significantly and positively correlated with native K content for each of the geographical regions, Hama-dori (r = 0.76, p < 0.001), Naka-dori (r = 0.43, p < 0.05), and Aizu (r = 0.76, P < 0.001), while it was not related to the relative abundance of smectite. The linear relationship between RIP and native K content not only indicate a large contribution of micaceous minerals to the (137)Cs retention ability of the soil clays, but also could be used to predict the (137)Cs retention ability of soil clays for other paddy fields in Fukushima and other areas.

Availability and mobility of radioactive cesium, (137)Cs, in soils are crucial for recovery from the accident in the Fukushima Dai-ichi Nuclear Power Plants. In this study we investigated the geochemical and grain-size distribution of (137)Cs in 11 soil samples collected in the eastern area of Fukushima Prefecture after the accident. Sequential extractions were performed to evaluate the distribution of (137)Cs having different geochemical interactions with soil components. The result was further compared with that of the stable cesium, (133)Cs, which had occurred in the soils before the accident. The distribution of (137)Cs in different grain-size fractions was also determined. Radioactive cesium was predominantly found in the extract obtained by strong-acid dissolution and the extraction residue and was more concentrated in silt and clay grains. X-ray diffraction analyses revealed that micaceous minerals as well as kaolin minerals were predominantly dissolved by the strong-acid treatment. Correlation between the fraction of (137)Cs and the content of micaceous minerals in different grain-size fractions of soil minerals suggests that micaceous minerals are responsible for the fixation of (137)Cs in the soils. The isotopic ratio of (137)Cs and (133)Cs in the extract by strong-acid dissolution was more than three times smaller than those in the extracts by water, ion exchange, and reductive dissolution. This indicates that the distribution of (137)Cs was not in the steady state in 2 y after the accident due to relatively slow fixation by the soil clay minerals.

RESUMO

A 50-year-old man presented with slow-growing dry, rough, micaceous scaly plaque over glans penis, which was compatible with clinical diagnosis of pseudoepitheliomatous keratotic and micaceous balanitis (PKMB) and histologically suggestive of PKMB without cellular atypia. He was treated successfully with topical 5-fluorouracil with complete clearance of lesion in 3 weeks without recurrence for 10 months. PKMB is an extremely rare and interesting condition; only a handful cases have been reported in world literature.

RESUMO

Pseudoepitheliomatous, keratotic and micaceous balanitis (PKMB) is a non-venereal and extremely rare pre-malignant condition characterized by silvery white plaque with micaceous scaling on glans seen in elderly uncircumcised men. Symptoms include phimosis, pain, and interference with sexual activity. Herein we present a young, 40-year-old male with PKMB of 5 years duration with deviation of urinary stream and histology showing acanthosis, hyperkeratosis with features of cellular atypia and abnormal mitosis suggestive of malignant transformation.

RESUMO

Many premalignant and malignant lesions affect the genitalia of elderly men. These include conditions like erythroplasia of Queyret, giant condyloma acuminata, verrucous carcinoma, invasive squamous cell carcinoma (SCC), which are relatively common. We report a case of a 65-year-old male with pseudoepitheliomatous keratotic and micaceous balanitis which is a rare penile condition affecting the elderly. It is considered as a distinct clinical entity by some, while others consider it as a condition that overlaps with verrucous carcinoma. It can also be considered a premalignant condition, as progression to invasive SCC has been noted.

RESUMO

A 51-year-old circumcised male presented with hard, thick, keratotic, nail-like covering of the skin of his glans penis of 2 year duration. Histology showed acanthosis, papillomatosis, and elongated rete ridges into the dermis suggestive of pseudoepitheliomatous, keratotic, and micaceous balanitis with features of cellular atypia. Partial penile amputation was done. There was no recurrence after 6 months of follow up.
