ABSTRACT

In a series of seventeen patients with unilateral osteoarthritis of the hip a scintiscanning follow-up study was made before and after total hip replacement for the assessment of the normal course of the 87mSr-scintiscan. In another series of twenty-eight patients with total hip replacement a photoscan was made as a supplement for the diagnosis of loosening of one or both components of a total hip implant. In most of these cases it proved to be a useful method, especially when clinical and raidological examination was inconclusive. It is concluded that up to six months after operation increased osteoblastic activity exists; the scintiscan became normal after that time. 87mSr scintiscanning offers a safe and simple technique for the assessment of the success and stability of total hip arthroplasty. It is also a useful aid for the early detection of loosening and infection. The procedure can help in the differential diagnosis of complaints after total hip replacement.

ABSTRACT

INTRODUCTION: The most common treatment of enchondromas is curettage (with or without adjuvant therapy) followed by cancellous bone grafting. To avoid donor-site morbidity of the iliac crest, we applied plaster of Paris as a bone defect filler after curettage of enchondromas. MATERIALS AND METHODS: We treated 19 enchondromas of the hand and foot in 19 patients (mean age 40 years, range 21-79 years) with curettage and filling of the cavity with sterile plaster of Paris (calcium sulphate/phosphate) tablets. RESULTS: The diagnosis was histologically confirmed in all cases. After a mean follow-up of 53 months (range 15-139 months), the mean functional Muskuloskeletal Tumor Society Score was 29.1 points (97%; range 28-30). There were no local recurrences although adjuvant therapy was not utilized. CONCLUSION: Plaster of Paris appears safe and effective as a bone-filling substance after curettage of enchondroma.

ABSTRACT

We followed 26 Richards type II patellofemoral arthroplasties in 24 patients (19 women) for a mean of 11 (1-20) years. Their mean age was 59 (22-90) years. The preoperative diagnoses were primary patellofemoral arthrosis in 17 cases and secondary arthrosis in 9 (8 malalignment, 1 patellar fracture). Patellectomy was later performed for persistent pain or patellar malalignment in 3 cases and a conversion to a total knee arthroplasty for progressive tibio-femoral degeneration or patella malalignment in 2. The mean Knee Society knee score for 21 knees at follow-up was 90 (65-100) points. The patients rated the results of surgery in 9 knees as excellent, 7 good, 4 improved, and 1 unimproved at follow-up. None of the implants showed signs of loosening or infection. The Richards type II patellofemoral arthroplasty yields acceptable long-term results in patients with isolated end-stage patellofemoral osteoarthrosis. Patient selection and patella alignment are important.
