ABSTRACT

BACKGROUND: The mechanisms linking mild behavioral impairment (MBI) and Alzheimer's disease (AD) have been insufficiently explored, with conflicting results regarding tau protein and few data on other metabolic markers. We aimed to evaluate the longitudinal association of the MBI domains and a spectrum of plasma biomarkers. METHODS: Our study is a secondary analysis of data from NOLAN. The longitudinal association of the MBI domains with plasma biomarkers, including pTau181, was tested using adjusted linear mixed-effects models. RESULTS: The sample comprised 359 participants (60% female, mean age: 78.3, standard deviation: 0.3 years). After 1 year, the MBI domain of abnormal perception was associated with steeper increases in plasma pTau181. Abnormal perception, decreased motivation, and impulse dyscontrol were associated with homocysteine or insulin dysregulation. DISCUSSION: Apart from the association with plasma pTau181, our results suggest that MBI might also represent metabolic dysregulation, probably contributing to dementia transition among older adults with subjective cognitive decline or mild cognitive impairment. HIGHLIGHTS: Mild behavioral impairment (MBI) psychosis was associated with steeper increases in plasma p. pTau could be a pharmacological target to treat agitation and psychosis symptoms. MBI domains were linked to metabolic dysregulation involving insulin and homocysteine.

ABSTRACT

BACKGROUND: Recent evidence has linked air pollution with frailty, yet little is known about the role of NO2 in this association. Our aim was to assess the association between frailty and NO2 air concentrations in Mexican older adults. METHODS: We used georeferenced data from the population-based Nutrition and Health Survey in Mexico (NHNS) 2021, representative of national and subnational regions, to measure a frailty index based on 31 health deficits in adults aged 50 and older. Air pollution due to NO2 concentrations was estimated from satellite images validated with data from surface-level stations. Maps were produced using Jensen's Natural break method. The association of frailty and NO2 concentrations was measured using the frailty index (multivariate fractional response logit regression) and a frailty binary variable (frailty index [FI] ≥0.36, multivariate logit regression). RESULTS: There was a positive and significant association of the frailty index with the NO2 concentrations, adjusting for age, sex, urban and rural area, years of education, socioeconomic status, living arrangement, particulate matter smaller than 2.5 microns, and indoor pollution. For each standard deviation increase in NO2 concentrations measured 10 years before the survey, the odds of being frail were 15% higher, and the frailty index was 14.5% higher. The fraction of frailty attributable to NO2 exposure ranged from 1.8% to 23.5% according to different scenarios. CONCLUSIONS: Frailty was positively associated with exposure to NO2 concentrations. Mapping frailty and its associated factors like NO2 air concentrations can contribute to the design of targeted pro-healthy aging policies.

ABSTRACT

Aging is characterized by several major changes, including altered body composition, which is associated with numerous negative clinical consequences such as sarcopenia, osteoporosis, and frailty. The study is to evaluate body composition parameters depending on age and sex in a population ranging from the young adult to the very old, and to identify break points in the association between body composition and age. In this cross-sectional study, we included the enrolment population of the French INSPIRE-T prospective cohort, accounting for 915 subjects (62% female). Age ranged from 20 to 93 years, median age (years) was 63 (IQR 27). Body composition (lean mass, fat mass, and bone mineral content) was assessed with dual-X-ray absorptiometry (DXA). Different break points in the relationship between age and body composition variables in males and females were identified using a segmented regression analysis adjusted on physical activity, nutritional status, educational level, and comorbidities. Lean mass decreased from the age of 55 years for males (CI 95% 44-66) and 31 years for females (CI 95% 23-39). For fat mass, we observed a trend towards an increase with age for males. For females, we observed an increase with age up to age 75 (CI 95% 62-86), followed by a decreasing trend. In this study, we described the relationship between body composition and age as a function of sex, establishing a foundation for further studies on predictive biomarkers of age-related body composition alteration.

ABSTRACT

The Integrated Care for Older People (ICOPE) program is a healthcare pathway that uses a screening test for intrinsic capacity (IC) as its entry point. However, real-life data informing on how IC domains cluster and change over time, as well as their clinical utility, are lacking. Using primary healthcare screening data from more than 20,000 French adults 60 years of age or older, this study identified four clusters of IC impairment: 'Low impairment' (most prevalent), 'Cognition+Locomotion+Hearing+Vision', 'All IC impaired' and 'Psychology+Vitality+Vision'. Compared to individuals with 'Low impairment', those in the other clusters had higher likelihood of having frailty and limitations in both activities of daily living (ADL) and instrumental activities of daily living (IADL), with the strongest associations being observed for 'All IC impaired'. This study found that ICOPE screening might be a useful tool for patient risk stratification in clinical practice, with a higher number of IC domains impaired at screening indicating a higher probability of functional decline.

Resumen: Objetivo: Evaluar los factores sociales asociados con el consumo de tabaco y alcohol en adolescentes mexicanos de poblaciones menores a 100 000 habitantes. Material y métodos: Estudio transversal a partir de resultados de la Encuesta Nacional de Salud y Nutrición (Ensanut 100k) que se enfocó en adolescentes de entre 10 y 19 años. Se estimaron modelos de regresión logística ajustados y se obtuvieron razones de momios (RM) e intervalos de confianza al 95% (IC95%). Resultados: La prevalencia de consumo de tabaco en el último mes fue de 5.5% (10-14 años=1.0%, 15-19 años= 11.2%); de consumo de alcohol, 9.3%, y de consumo excesivo de alcohol, 2.8%. Factores asociados: consumo de tabaco, sexo (hombre, RM= 6.00; IC95%: 2.89-12.46), edad (RM=1.73; IC95%: 1.46-2.04), trabajo remunerado (RM= 2.87; IC95%: 1.12-7.34) y afiliación a servicios de salud (RM= 0.26; IC95%: 0.08-0.82); consumo de alcohol, (hombre, RM= 5.11; IC95%: 2.38-11.0), edad (RM= 1.70; IC95%: 1.51-1.92) y trabajo remunerado (RM= 2.51; IC95%: 1.01-6.24). Conclusiones: Para reducir el consumo de alcohol y tabaco en adolescentes deben implementarse estrategias de prevención integrales que tomen en cuenta el contexto individual y social.


Abstract: Objective: To evaluate the social factors associated with tobacco and alcohol consumption in Mexican adolescents from populations under 100 000 inhabitants. Materials and methods: Cross-sectional study based on the National Health and Nutrition Survey (Ensanut100k). It included adolescents between 10-19 years. Adjusted logistic regression models were estimated, obtaining Odds Ratio (OR) and confidence intervals 95% (CI95%). Results: Prevalence of tobacco consumption in the last month was 5.5% (10-14 years= 1.0%, 15-19 years= 11.2%); alcohol consumption 9.3%, and excessive alcohol consumption, 2.8%. Associated factors: smoking, sex (male, OR=6.00, CI95%: 2.89-12.46), age (OR=1.73, CI95%: 1.46-2.04), remunerated work (OR=2.87, CI95%: 1.12-7.34), affiliation to health services (OR=0.26, CI95%: 0.08-0.82); alcohol consumption, (male, OR=5.11, CI95%: 2.38-11.00), age (OR=1.70, CI95%: 1.51-1.92), remunerated work (OR=2.51, CI95%: 1.01-6.24). Conclusions: To reduce the consumption of alcohol and tobacco in adolescents, comprehensive prevention strategies must be implemented that consider the individual and social context.

ABSTRACT

Abstract: Objective: To examine the longitudinal association between the social determinants of health (SDH) and frailty status with all-cause mortality in older Mexican adults. Materials and methods: Longitudinal study with a sample of adults aged 60 and over of Study on Global AGEing and Adult Health (SAGE) in Mexico. A Cox proportional hazard model was used to estimate the SDH and frailty-related hazard ratios (HR) for mortality over the study period. Results: Overall mortality rate was 16.9%. Higher education, having a higher frequency of inter-personal contacts (HR=0.96;p<0.01) reduced the risk of dying, after adjusting for potential confounders. While, not counting on someone to trust (HR= 1.59;p<0.03) and having a sense a lack of control over important decisions in life increased the mortality risk. Conclusions: Given that frailty and the SDH affect health using independent pathways, public health systems in Mexico could benefit from increasing the capacity of identifying frail and isolated older adults and providing a risk-stratified health care accordingly.


Resumen: Objetivo: Examinar la asociación longitudinal entre los determinantes sociales en salud (DSS) y la fragilidad con la mortalidad por todas las causas en adultos mayores mexicanos. Material y métodos: Estudio longitudinal con una muestra de adultos mayores de 60 años o más del estudio Envejecimiento Global y Salud de los Adultos (SAGE, por sus siglas en inglés) en México. Se utilizó el modelo riesgos proporcionales de Cox para estimar la asociación entre DSS y la fragilidad en la mortalidad. Resultados: La tasa de mortalidad general fue 16.9%. Tener mayor educación y una mayor frecuencia de contactos interpersonales (HR= 0.96,p<0.01) reducen el riesgo de morir, después de ajustar por covariables. Mientras tanto, no contar con alguien en quien confiar (HR= 1.59;p<0.03) y tener una sensación de falta de control sobre las decisiones importantes en la vida aumentan el riesgo de mortalidad. Conclusiones: Dado que la fragilidad y los DSS inciden sobre la salud usando vías independientes, el sistema de salud de México se beneficiaría al incrementar su capacidad para detectar a los adultos mayores frágiles y con aislamiento social, para proveer cuidados a la salud.
