ABSTRACT

An ocean-ice-acoustic coupled model is configured for the Beaufort Sea. The model uses outputs from a data assimilating global scale ice-ocean-atmosphere forecast to drive a bimodal roughness algorithm for generating a realistic ice canopy. The resulting range-dependent ice cover obeys observed roughness, keel number density, depth, and slope, and floe size statistics. The ice is inserted into a parabolic equation acoustic propagation model as a near-zero impedance fluid layer along with a model defined range-dependent sound speed profile. Year-long observations of transmissions at 35 Hz from the Coordinated Arctic Acoustic Thermometry Experiment and 925 Hz from the Arctic Mobile Observing System source were recorded over the winter of 2019-2020 on a free-drifting, eight-element vertical line array designed to vertically span the Beaufort duct. The ocean-ice-acoustic coupled model predicts receive levels that reasonably agree with the measurements over propagation ranges of 30-800 km. At 925 Hz, seasonal and sub-seasonal ocean and ice driven variations of propagation loss are captured in the data and reproduced in the model.

ABSTRACT

A pseudoaneurysm (PA) is a collection of blood caused by an incomplete tear in the vessel wall. PA can be arterial or venous in origin. In the maxillofacial region, arterial PA can result from surgical interventions. Venous PAs in the maxillofacial region have never been described. A standardized protocol for management of post-traumatic PAs in the maxillofacial region would help clinicians make treatment decisions. On the basis of the available literature and our institutional experience, we present an algorithm for management of post-traumatic maxillofacial PAs. We also present patients from our institution who illustrate some of the management options in the algorithm.

ABSTRACT

Penetrating injury to the head and neck accounts for a minority of trauma but significant morbidity in the US civilian population. The 3-zone anatomical framework has historically guided evaluation and management; however, the most current evidence-based protocols favor a no-zone, systems-based approach. In stable patients, a thorough physical examination and noninvasive imaging should be prioritized, with surgical exploration of the head and neck reserved for certain circumstances. Diagnostic and management decisions should be tailored to the mechanism of injury, history, physical examination, experience of personnel, availability of equipment, and clinical judgment.

ABSTRACT

Complementary and integrative medicine is particularly popular among patients undergoing facial cosmetic enhancement. This article reviews the efficacy of the most commonly practiced integrative therapies among this surgical population, with a focus on the most current literature regarding the application or potential for application of these treatments to benefit patients undergoing facial aesthetic surgery. Adverse effects of the most popular herbal and dietary supplements are also reviewed. Finally, the potential for interaction among integrative treatments as well as with conventional pharmacologic therapy is discussed.
