ABSTRACT

BACKGROUND: Progressive oxidation of highly cross-linked ultra-high molecular weight (UHMPWE-X) liners is considered to be a risk factor for material failure in THA. Antioxidants such as vitamin E (alpha-tocopherol) (UHMWPE-XE) were supplemented into the latest generation of polyethylene liners. To prevent inhomogenous vitamin E distribution within the polymer, blending was established as an alternative manufacturing process to diffusion. The purpose of the present study was to investigate the in vivo wear behavior of UHMWPE-XE in comparison with conventional UHMWPE-X liners using virtual CAD-based radiographs. METHODS: Until now, 94 patients from a prospective, randomized, controlled, multicenter study were reviewed at 5-year follow-up. Of these, 51 (54%) received UHMWPE-XE and 43 (46%) UHMWPE-X liners. Anteroposterior pelvic radiographs were made immediately after surgery and at 1 and 5 years postoperatively. The radiographs were analyzed using the observer-independent analysis software RayMatch® (Raylytic GmbH, Leipzig, Germany). RESULTS: The mean wear rate was measured to be 23.6 µm/year (SD 13.7; range 0.7-71.8 µm). There were no significant differences between the two cohorts (UHMWPE-X: 23.2 µm/year vs. UHMWPE-XE: 24.0 µm/year, p = 0.73). Cup anteversion significantly changed within the 1st year after implantation independent from the type of polyethylene liner [UHMWPE-X: 18.2-23.9° (p = 0.0001); UHMWPE-XE: 21.0-25.5° (p = 0.002)]. No further significant changes of cup anteversion in both groups were found between year 1 and 5 after implantation [UHMWPE-X (p = 0.46); UHMWPE-XE (p = 0.56)]. CONCLUSION: The present study demonstrates that the addition of vitamin E does not adversely affect the midterm wear behavior of UHMWPE-X. The antioxidative benefit of vitamin E is expected to become evident in long-term follow-up. Cup anteversion increment by 5° within the 1st year is likely a result of the released hip flexion contracture resulting in an enhanced posterior pelvic tilt. Therefore, a reassessment of target values in acetabular cup placement might be considered.

ABSTRACT

The precise and accurate measurement of implant wear, acetabular cup anteversion and inclination from routine anterior-posterior radiographs still poses a challenge. Current approaches suffer from time-consuming procedures accompanied by low and observer-dependent accuracy and precision. We present and validate a novel, automated method for determining total hip arthroplasty parameters by comparing its accuracy and precision with methods in contemporary scientific literature. The algorithm uses CAD-model-based two dimensional-three dimensional (2D-3D)-registration supported by convolutional neural networks. Two in-vitro experimental set-ups were designed to validate the proposed 2D-3D-method. The set-ups provided 84 predefined wear values and 24 configurations of anteversion and inclination in 114 radiographs. Accuracy and precision were evaluated by systematically comparing the predefined ground truth and the automatically calculated values from in-vitro X-rays. In addition, an algorithm was developed and validated against physician's measurements on clinical X-rays to determine the inclination of the interteardrop (ITL) and biischial line (BL) to account for the individual patient's pelvic rotation in the frontal plane. Using X-rays from experimental set-ups, the determined mean error was 0.014 mm (standard deviation: 0.020 mm; root-mean-square-error: 0.024 mm) for wear in pelvic position, -0.01° (0.24°; 0.23°) for radiographic cup anteversion, and 0.11° (0.38°; 0.39°) for radiographic cup inclination. The inclination of ITL and BL was automatically determined in all clinical X-rays with excellent interclass correlation coefficients of 0.95 and 0.91, respectively. The presented algorithm allows the accurate and precise evaluation of total hip arthroplasty parameters without additional equipment. The method might help to investigate different implant designs, biomaterials, and surgical techniques with greater objectivity.

ABSTRACT

The quaternary coastal Collo aquifer in northeast Algeria (NE Algeria) marks an important local water resource supporting domestic, industrial and agricultural activities. The aquifer shows signs of contamination due to the existence of various pollution sources, especially nitrogen compounds. Focusing the local identification of key vulnerable zones and related main hazard types for wise future water management, the present study highlights results from a coupled analysis of the well-established Geographical Information System (GIS)-based GOD (groundwater occurrence, overall aquifer class, depth to groundwater) hazard index analysis and the COST Action 620 plan. Most prevalent hazard types in the study area were identified as the urban/residential areas without public sewage systems, landfill and agricultural/pasturing areas. Regarding the vulnerability analysis particularly the northern aquifer region is endangered, dominated by high (22.4%) and moderate (27.4%) vulnerability classes. Central, western and southern aquifer regions are characterized by low (23.3%) and very low (26.9%) vulnerability classes. Overall, these GOD-derived results are in good agreement with earlier results obtained by the more complex DRASTIC approach. Final risk assessment and validation related to 2014/2015 nitrate sampling campaigns indicate that "high risk" and "very high risk" classes only apply to a small part of the study area in the northern sector (8%), whereas the main part (>60%) broadly affecting the central, western and southern sector only bears a low to very low risk of water pollution. Apart from a future-oriented groundwater abstraction strategy it is recommended to update the evaluation regularly to effectively consider dynamic changes of local anthropogenic activities and hazards.
