ABSTRACT

Our objective was to systematically evaluate the efficacy and safety of proton pump inhibitors combined with clopidogrel in patients undergoing percutaneous coronary intervention and to provide an evidence basis for clinical treatment decision-making. The database EMBASE, PubMed/Medline, Web of Science, the Cochrane Library and CNKI records from establishment of each database until August 2020 were included. Articles were evaluated for quality. Meta-analysis of selected articles was conducted by RevMan5.3 software. Three RCTs and 4 cohort studies were included, with a total of 9932 patients. Four studies reported gastrointestinal (GI) bleeding events, 3 of which were RCT studies. Overall, there was a significantly lower risk of GI bleeding events in the PPI group compared to the no PPI group [OR = 3.06, 95% CI: 1.89 to 4.95] (P < 0.00001). In 3 RCT studies, there was also a significantly lower risk of GI bleeding events in the PPI group compared to the no PPI group [OR = 3.06, 95% CI: 1.80 to 5.21] (P < 0.0001). Seven studies including 3 RCTs and 4 cohort studies reported MACE. Overall, there was no significant difference in MACE events between PPI group and no PPI group [OR = 1.05, 95% CI: 0.91 to 1.21] (P = 0.50). Both in RCT and cohort studies subgroups, there also was no significant difference in MACE events between the PPI group and the no PPI group [OR = 1.16, 95% CI: 0.87 to 1.53] (P = 0.32), [OR = 1.02, 95% CI: 0.87 to 1.19] (P = 0.84), respectively. For PCI patients taking clopidogrel and PPI therapy, PPI reduced the risk of GI bleeding while having no impact on MACE.

ABSTRACT

Malaria data in 2011-2013 were collected and statistically analyzed using Microsoft Excel 2003 software. A total of 501 malaria cases were reported, and the annual incidence was 0.2510/100,000, 0.2486/100,000, and 0.3223/100,000, in 2011, 2012, and 2013, respectively, with an average of 0.2740/100,000 in Hunan Province. All these cases were imported and mainly reported from Changsha (44.3%, 222/501), Shaoyang (16.6%, 83/501), Huaihua (8.4%, 42/501), and Yiyang (8.0%, 40/501). 97.0% (486/501) of the cases were laboratory confirmed cases, while the other 3.0% (15/501) were clinically diagnosed. Among those lab confirmed, 41.3% (207/501) were vivax malaria cases, 47.9% (240/501) falciparum malaria cases, 1.4% (7/501) ovale malaria cases, 0.8% (4/501) malariae cases, 6.6% (33/501) mixed infection, and 2.0% (10/501) were unclassified cases. Most cases (202/501) occurred among persons aged 40-49 years. These patients were mainly farmers, workers, migrant workers, and cadres. 47.7% (239/501) were from Africa and 50.1% (251/501) from Southeast Asia.

ABSTRACT

Periodic assembly and break-up of supercontinents since at least two billion years ago (Ga), dubbed the supercontinent cycle, provides the first-order tectonic control on the evolution of the Earth System including episodic orogenic events, mineralization, the formation and closure of oceans and superoceans, and even the evolution of life. However, the lifespan of the supercontinents appears to decrease with time, from ∼300 million years (Myr) for Nuna/Columbia, to 200-250 Myr for Rodinia and ∼150 Myr for the youngest supercontinent Pangaea. To understand what caused such a secular decrease in supercontinental lifespan, we conduct 3-D geodynamic modeling using realistic tectonic settings. The results show that the yield stress of newly formed orogens during the assembly of a supercontinent provides the dominant control on the lifespan of the supercontinent, implying that the yield stress of young orogens becomes lower with time. We hypothesize that the decreasing mantle temperature due to Earth's secular cooling might have caused new orogens to become weaker.

ABSTRACT

Argyle is the world's largest source of natural diamonds, yet one of only a few economic deposits hosted in a Paleoproterozoic orogen. The geodynamic triggers responsible for its alkaline ultramafic volcanic host are unknown. Here we show, using U-Pb and (U-Th)/He geochronology of detrital apatite and detrital zircon, and U-Pb dating of hydrothermal titanite, that emplacement of the Argyle lamproite is bracketed between 1311 ± 9 Ma and 1257 ± 15 Ma (2σ), older than previously known. To form the Argyle lamproite diatreme complex, emplacement was likely driven by lithospheric extension related to the breakup of the supercontinent Nuna. Extension facilitated production of low-degree partial melts and their migration through transcrustal corridors in the Paleoproterozoic Halls Creek Orogen, a rheologically-weak rift zone adjacent to the Kimberley Craton. Diamondiferous diatreme emplacement during (super)continental breakup may be prevalent but hitherto under-recognized in rift zones at the edges of ancient continental blocks.

ABSTRACT

Earth's known supercontinents are believed to have formed in vastly different ways, with two endmembers being introversion and extroversion. The former involves the closure of the internal oceans formed during the break-up of the previous supercontinent, whereas the latter involves the closure of the previous external superocean. However, it is unclear what caused such diverging behavior of supercontinent cycles that involved first-order interaction between subducting tectonic plates and the mantle. Here we address this question through 4D geodynamic modeling using realistic tectonic set-ups. Our results show that the strength of the oceanic lithosphere plays a critical role in determining the assembly path of a supercontinent. We found that high oceanic lithospheric strength leads to introversion assembly, whereas lower strength leads to extroversion assembly. A theoretically estimated reduction in oceanic crustal thickness, and thus its strength, during Earth's secular cooling indicates that introversion was only possible for the Precambrian time when the oceanic lithosphere was stronger, thus predicting the assembling of the next supercontinent Amasia through the closure of the Pacific Ocean instead of the Indian-Atlantic oceans. Our work provides a new understanding of the secular evolution of plate tectonics and geodynamics as the Earth cooled.

ABSTRACT

Rare oceanic diamonds are believed to have a mantle transition zone origin like super-deep continental diamonds. However, oceanic diamonds have a homogeneous and organic-like light carbon isotope signature (δ13C - 28 to - 20‰) instead of the extremely variable organic to lithospheric mantle signature of super-deep continental diamonds (δ13C - 25‰ to + 3.5‰). Here, we show that with rare exceptions, oceanic diamonds and the isotopically lighter cores of super-deep continental diamonds share a common organic δ13C composition reflecting carbon brought down to the transition zone by subduction, whereas the rims of such super-deep continental diamonds have the same δ13C as peridotitic diamonds from the lithospheric mantle. Like lithospheric continental diamonds, almost all the known occurrences of oceanic diamonds are linked to plume-induced large igneous provinces or ocean islands, suggesting a common connection to mantle plumes. We argue that mantle plumes bring the transition zone diamonds to shallower levels, where only those emplaced at the base of the continental lithosphere might grow rims with lithospheric mantle carbon isotope signatures.

OBJECTIVE: This study aims to investigate the infection of Clonorchis sinensis ( C. sinensis) in high-incidence areas of Hunan Province, China. The phylogenetic analysis of the C. sinensis species in the highly infected areas was carried out. METHOD: Infection of the definitive human host and intermediate fish host by C. sinensis was investigated, and the mitochondrial genes cox1 and Nad1were used as genetic markers for phylogenetic analysis. RESULTS: In 2016-2020, the average population infection rate of Hunan was 1.38%, while in Tongdao County the rate was up to 26.90%, and the highest fish infection rate was detected in Qiyang County (99.44% in the dorsal fin of crucian carp). High genetic sequence similarity was observed in the samples from Qiyang and Lengshuitan which exhibited high homology with those from Guangdong and Gansu, whereas the parasitic species from Tongdao was highly homologous with those located in high-latitude areas. Moreover, no significant difference was found in the gene sequence of the parasitic species in definitive hosts dogs and cats. CONCLUSION: The systematically study of C. sinensis infection in the high-incidence areas will contribute greatly to the prevention and effectively controlling the spread of Clonorchis sinensis in Hunan Province The endemic of C. sinensis infection in Hunan Province is the result of co-action of local and foreign parasite species.

ABSTRACT

Clonorchiasis is an important food-borne parasitic disease in China because of the popularity in ingesting raw freshwater fish. To explore the epidemiology and determinants of clonorchiasis in children, a cross-sectional survey was implemented in two middle schools in Qiyang county, Hunan province, in southeastern China. Questionnaire survey and fecal examination were implemented. Questionnaires were fed back by 627 students, while stool samples were collected from 557 students, out of which 545 ones also provided questionnaire information. The percentage of students ingesting raw freshwater fish was 40.5% (254/627), while the prevalence with Clonorchis sinensis infection was 18.9% (105/557). Such factors contributed significantly to the students' practice in eating raw freshwater fish including boys, fathers' eating raw freshwater fish, mothers' eating raw freshwater fish, and preparation of raw freshwater fish at home, with an adjusted odds ratio of 1.9 (95% confidence intervals (95% CI): 1.3-2.8), 3.9 (95% CI: 2.3-6.5), 3.0 (95% CI: 1.8-4.8) and 2.8 (95% CI: 1.8-4.5), correspondingly. Ingestion of raw freshwater fish, fathers' eating raw freshwater fish and preparation of raw freshwater fish at home were risk factors of C. sinensis infection in students, and the adjusted odds ratio was 3.2 (95% CI: 1.9-5.5), 2.1 (95% CI: 1.1-3.9) and 1.8 (95% CI: 1.0-3.2), respectively. Thus, clonorchiasis was endemic in the surveyed schools due to the ingestion of raw freshwater fish, which is influenced by family environment. Education should be implemented in schools to promote behavioral change of eating raw freshwater fish.

ABSTRACT

Progressive mantle melting during the Earth's earliest evolution led to the formation of a depleted mantle and a continental crust enriched in highly incompatible elements. Re-enrichment of Earth's mantle can occur when continental crustal materials begin to founder into the mantle by either subduction or, to a lesser degree, by delamination processes, profoundly affecting the mantle's trace element and volatile compositions. Deciphering when mantle re-enrichment/refertilization became a global-scale process would reveal the onset of efficient mass transfer of crust to the mantle and potentially when plate tectonic processes became operative on a global-scale. Here we document the onset of mantle re-enrichment/refertilization by comparing the abundances of petrogenetically significant isotopic values and key ratios of highly incompatible elements compared to lithophile elements in Archean to Early-Proterozoic mantle-derived melts (i.e., basalts and komatiites). Basalts and komatiites both record a rapid-change in mantle chemistry around 3.2 billion years ago (Ga) signifying a fundamental change in Earth geodynamics. This rapid-change is recorded in Nd isotopes and in key trace element ratios that reflect a fundamental shift in the balance between fluid-mobile and incompatible elements (i.e., Ba/La, Ba/Nb, U/Nb, Pb/Nd and Pb/Ce) in basaltic and komatiitic rocks. These geochemical proxies display a significant increase in magnitude and variability after ~3.2 Ga. We hypothesize that rapid increases in mantle heterogeneity indicate the recycling of supracrustal materials back into Earth's mantle via subduction. Our new observations thus point to a ≥ 3.2 Ga onset of global subduction processes via plate tectonics.

ABSTRACT

OBJECTIVE: To make an epidemiological investigation on clonorchiasis sinensis and relevant factors in the south of Hunan Province. METHODS: One village from each of Lengshuitan District and Qiyang County was selected for the survey from November to December in 2006. Stool samples from villagers were collected and examined by modified Kato-Katz method. Questionnairing was performed for relevant knowledge and attitude among residents. The infection rate in animal reservoirs and intermediate hosts were detected. RESULTS: A total of 586 cases with Clonorchis sinensis infection were found from 777 people with a prevalence of 75.4%. The average egg density was 451 eggs per gram (EPG) feces. Light, moderate and heavy infections occupied 85.5% (501/586), 14.0% (82/586), and 0.5% (3/586) respectively. Prevalence in males and females was 76.9% (316/411) and 73.8% (270/366) respectively with no significant difference (chi2 = 1.013, P > 0.05). Infections were found in all age groups, with the highest prevalence in the group of 70 to 79 years (85.7%, 30/35). By occupations, the prevalence was 82.5% (447/542) in peasants, 79.3% (42/53) in doctors, 73.7% (28/38) in teachers, and 73.5% (25/34) in local cadres. The infection rate was 17.4% (29/167) and 7.4% (2/27) in Parafossarulus seriatulus and Alocinma longicornis, and 69.2% (9/13) and 5.3% (1/19) in Carassius auratus and Cyprinus carpios respectively. Adult worms were found in all 3 dogs dissected. Over 80% inhabitants did not know that this disease can be acquired by eating raw fish. 95.6% (153/160) of the farmers and 56.7% (349/616) of the students had a history of eating raw fish. The water was contaminated with C. sinensis eggs by using untreated feces as fertilizer for farming and by scrubbing pail latrines in the ponds. CONCLUSION: The prevalence of clonorchiasis in human population is high in Lengshuitan District and Qiyang County of Hunan Province. Eating raw fish and using untreated feces as fertilizer are the most important epidemiological factors of the disease.

ABSTRACT

OBJECTIVE: To reveal the natural distribution of Angiostrongylus cantonensis in the mainland of China. METHODS: The potential areas of A. cantonensis and its main intermediate host Pomacea canaliculata were predicted based on degree-day models using GIS technique. A grid sampling was performed on the prediction map and 5% grids were randomly sampled. A total of 55 sampled sites were selected for the survey on A. cantonensis and its hosts in September-October of 2006. RESULTS: Nineteen Provinces/Municipalities/Autonomous Regions were found as potential habitats for P. canaliculata in the mainland of China. It was then confirmed that the snails distributed in the provinces of Fujian, Jiangxi, Zhejiang, Hunan, Guangdong, Guangxi, Hainan, and Yunnan. Higher prevalence of A. cantonensis in P. canaliculata was detected in Jianou of Fujian (36.6%), Xingguo of Jiangxi (19.9%), Rui'an of Zhejiang (16.0%), Rucheng of Hunan (5.0%), Huazhou of Guangdong (6.3%), Shangsi of Guangxi (39.1%) and Wuzhishan of Hainan (25.0%). CONCLUSION: Natural nidi of A. cantonensis have been found in seven provinces where natural infection in P. canaliculata has been detected.

ABSTRACT

Plate tectonics and mantle plumes are two of the most fundamental solid-Earth processes that have operated through much of Earth history. For the past 300 million years, mantle plumes are known to derive mostly from two large low shear velocity provinces (LLSVPs) above the core-mantle boundary, referred to as the African and Pacific superplumes, but their possible connection with plate tectonics is debated. Here, we demonstrate that transition elements (Ni, Cr, and Fe/Mn) in basaltic rocks can be used to trace plume-related magmatism through Earth history. Our analysis indicates the presence of a direct relationship between the intensity of plume magmatism and the supercontinent cycle, suggesting a possible dynamic coupling between supercontinent and superplume events. In addition, our analysis shows a consistent sudden drop in MgO, Ni and Cr at ~3.2-3.0 billion years ago, possibly indicating an abrupt change in mantle temperature at the start of global plate tectonics.

ABSTRACT

Serpentinite is a major carrier of fluid-mobile elements in subduction zones, which influences the geochemical signature of arc magmatism (e.g. high abundances of Li, Ba, Sr, B, As, Mo and Pb). Based on results from Neoproterozoic serpentinites in the Arabian-Nubian Shield, we herein report the role of antigorite in the transportation of fluid-mobile elements (FME) and light rare earth elements (LREE) from the subducted slab to arc-related magma during subduction. The serpentinites contain two generations of antigorites: the older generation is coarse-grained, formed at a temperature range of 165-250 °C and is enriched in Li, Rb, Ba and Cs, whereas the younger generation is finer-grained, formed at higher temperature conditions (425-475 °C) and has high concentrations of B, As, Sb, Mo, Pb, Sr and LREE. Magnesite, on the other hand, remains stable at sub-arc depths beyond the stability field of both antigorites, and represents a potential reservoir of FME and LREE for deeper mantle melts. Magnesite has high FME and LREE absorbing capacity (over 50-60%) higher than serpentine phases. Temperature is the main controlling factor for stability of these minerals and therefore the release of these elements from subducted slabs into arc magmatism. As the liberation of these elements varies along the length of the slab, the resulting cross-arc geochemical variation trend can help to determine the subduction polarity of ancient arcs.

BACKGROUND: Clonorchiasis is caused by eating of raw or undercooked freshwater fish containing the larvae of Clonorchis sinensis; the Kato-Katz method is widely applied in diagnosis. The improvement of repeated Kato-Katz smears from multiple stool samples has been well illuminated in many helminths other than C. sinensis. METHODS: A cross-sectional investigation was implemented to capture the epidemiology and risk factors of clonorchiasis among middle school students in Qiyang county, China. Students with complete data of six Kato-Katz thick smears from two stool samples were included in this analysis. Data on the habits of eating raw freshwater fish were also collected and compared. RESULTS: Altogether, 397 students had complete information of six smears, out of which 394 reported the information on eating habits. According to the 'gold' standard by six smears, 77 students (19.4%) were detected with C. sinensis. However, only 45 (11.3%) were detected using a single smear, with an underestimation of 41.6% compared to the 'gold' standard. However, the geometric mean of eggs per gram of feces in detected cases was 126.4 in a single smear, overestimated by 105.2% compared to 61.6 by the 'gold' standard. The linear relationship between prevalence and infection intensity of detected cases based on different smears was significantly negative. The habits of eating raw freshwater fish in the false negative cases was similar to those in the detected cases, but these two groups had significantly higher levels for habits of eating raw freshwater fish than negative individuals. CONCLUSIONS: In low endemicity situations, underestimation of C. sinensis infection could not be avoided based on a limited number of Kato-Katz smears. Thus, repeated smears from at least two stool samples should be considered when an individual eats raw freshwater fish, drug efficacy is evaluated or elimination of C. sinensis is verified. Additionally, when logistics are insufficient for multiple samples to be taken for diagnosis for survey and surveillance in the areas or populations of low endemicity, prevalence accuracy needs to be corrected.

ABSTRACT

Mantle melts provide a window on processes related to global plate tectonics. The composition of chromian spinel (Cr-spinel) from mafic-ultramafic rocks has been widely used for tracing the geotectonic environments, the degree of mantle melting and the rate of mid-ocean ridge spreading. The assumption is that Cr-spinel's core composition (Cr# = Cr/(Cr + Al)) is homogenous, insensitive to post-formation modification and therefore a robust petrogenetic indicator. However, we demonstrate that the composition of Cr-spinel can be modified by fluid/melt-rock interactions in both sub-arc and sub-mid oceanic mantle. Metasomatism can produce Al-Cr heterogeneity in Cr-spinel that lowers the Cr/Al ratio, and therefore modifies the Cr#, making Cr# ineffective as a geotectonic and mantle melting indicator. Our analysis also demonstrates that Cr-spinel is a potential sink for fluid-mobile elements, especially in subduction zone environments. The heterogeneity of Cr# in Cr-spinel can, therefore, be used as an excellent tracer for metasomatic processes.

ABSTRACT

Background: The growth rate of centenarians was unusually rapid in recent decades, ushering in an era of longevity. This study aims to explore the difference between centenarians and non-centenarians using quantitative research, and to scientifically guide residents to develop the correct lifestyle and health care ways. Methods: From October 2013 to August 2017. A cross-sectional survey was conducted on 271 centenarians and 570 non-centenarians by using a questionnaire to assess longevity and health issues which was developed for the needs of the study, who came from 29 counties and districts in 11 cities of Zhejiang province, China. Two hundred and fifty-five valid questionnaires were returned, with an effective response rate of 94.1%. Meanwhile, data of 526 non-centenarians from Zhejiang province was collected as a control group, with an effective response rate of 92.3%. Results: The prevalence rates of tumor, stomach and duodenal ulcer, diabetes, bronchial asthma, and chronic obstructive pulmonary disease, tuberculosis among centenarians were all lower than those among non-centenarians. The oral health of centenarians is better than that of non-centenarians. The consumption of coarse cereals, pasta, other staple foods and fruits among centenarians was higher than that of non-centenarians. The percentage of centenarians who smoke or engage in recreational activities every day was lower than that of non-centenarians. Conclusions: We should give full play to the role of preventive medicine and health management to safeguard the health of residents. ABSTRACT

BACKGROUND: There is increasing evidence demonstrating the highly inadequate reporting of preclinical research in multiple scientific publications. The purpose of this study is to systematically investigate the reporting quality of acupuncture for neurogenesis in animal models of acute ischemic stroke. METHODS: We searched eight databases, including PubMed, EMBASE, CINAHL, AMED, Chinese National Knowledge Infrastructure, VIP information database, Wanfang data Information Site, and Chinese Biomedical Literature Database. The methodological quality of included studies was assessed by using the CAMARADES 10-item checklist. The STRICTA statement was modified to gear to animal acupuncture research. The reporting quality was assessed according to the ARRIVE guidelines and the modified STRICTA statement. Data were analyzed with descriptive statistics. RESULTS: Ultimately, 44 studies containing 2,411 subjects were identified. The overall compliance with the CAMARADES 10-item checklist has a mean of 4.3. The reporting quality indicated that the overall compliance with ARRIVE guidelines has a mean of 59.9% and with the modified STRICTA statement a mean of 71.8%. The findings suggest that the reporting quality of acupuncture for preclinical stroke was generally poor. CONCLUSIONS: Full compliance with ARRIVE guidelines and/or modified STRICTA statement in designing, conducting and reporting preclinical acupuncture research is urgently needed in the future.

ABSTRACT

OBJECTIVE: To evaluate the clinical effects of manipulative reduction of cervical vertebrae combined with Pi needle release for the treatment of temporomandibular joint dysfunction. METHODS: From March 2012 to May 2017, 60 patients with temporomandibular joint dysfunction were treated by manipulative reduction of cervical vertebrae with Pi needle release, including 26 males and 34 females, ranging in age from 18 to 60 years old, with an average of 32.5 years old. The courses of the disease ranged from 1 week to 5 years, with a mean duration of 3 months. The patients were followed up before and after treatment by maximum active opening of temporomandibular joint, joint clicking and comparison of visual analogue scale (VAS) pain scores. RESULTS: All 60 patients were followed up, and the duration ranged from 6 to 24 months, with an average of 12 months. The maximum active opening of temporomandibular joint increased from(1.99±0.47) cm before treatment to(3.17±0.19) cm. The joint clicking decreased from 100% before treatment to(27.33±13.51)% after treatment. The VAS score decreased from 4.73±0.67 before treatment to 1.80±0.53 after treatment. CONCLUSIONS: Treatment of temporomandibular joint dysfunction by manipulative reduction of cervical vertebrae combined with Pi needle release has such advantages as easy operation, obvious curative effect, short course of treatment, which is an effective method for the treatment of temporomandibular joint dysfunction.
