ABSTRACT
PURPOSE: To investigate the impact of hip adduction and abduction on the femoro-epiphyseal acetabular roof (FEAR) index using conventional anteroposterior pelvic radiographs of patients with borderline hip dysplasia (BHD). METHODS: Patients with BHD (lateral center edge angle [LCEA] with values of 18° ≤ LCEA <25°) and who were subject to periacetabular osteotoym were selected from a consecutive retrospective cohort from January 2009 to January 2016. The hip ad-/abduction (adduction handled as negative values) and the FEAR index (lateral opening angles handled as positive values) were measured in the initially standardized acquired pelvic radiographs. Thereafter, using surgical planning software, different degrees of hip adduction and abduction were simulated from 20° of adduction to 30° of abduction, and the FEAR index was measured at each position. Pearson correlation was used to identify a potential correlation between the hip ad-/abduction and the FEAR index. RESULTS: Eighty-nine hips were included. Initial radiographs showed unintentional mean ad-/abduction of -5.3° ± 3.1° (range, -14° to 2°). The mean FEAR index at the initial ad-/abduction angle was -3.6° ± 7.2° (range, -18.0° to 16.0°) and was significantly different from neutral position (0° of ad-/abduction) -9.2° ± 7.0° (range, -27.0° to 10.0°) (P < .001). The FEAR index was negatively correlated to the hip ad-/abduction angle (r = 1, P < .001), with lower values for the FEAR index with the hip in abduction. CONCLUSIONS: Significant differences in FEAR index measurements were observed depending on hip ad-/abduction on pelvic radiographs. A linear change of the FEAR index of 5° per 5° of hip adduction or abduction could be demonstrated, with adduction resulting in increasing and abduction in decreasing values for the FEAR index. PURPOSE: An increased value of tibiofemoral rotation is frequently observed in patients with patellofemoral instability or maltracking. Nevertheless, the appropriate approach for addressing this parameter remains unclear so far. One potential approach for correcting tibiofemoral rotation is femoral rotational osteotomy. We hypothesized that femoral rotational osteotomy affects tibiofemoral rotation. METHODS: All patients who underwent femoral rotational osteotomy between January 2018 and May 2022 were included in this study. Pre- and postoperative tibiofemoral rotation and the degree of femoral rotation were measured using two-dimensional (2D) and three-dimensional (3D) measurements. The effect of femoral rotation on tibiofemoral rotation was assessed. RESULTS: Forty knees (18 right and 22 left) of 36 patients (28 females and 8 males) were included. Mean preoperative femoral torsion was 32.1 ± 10.1° in 2D and 30.8 ± 10.1° in 3D. Femoral rotation was performed by -14.1 ± 8.3° using 2D measurements and -15.0 ± 8.0° using 3D measurements. Tibiofemoral rotation changed from 9.9 ± 6.2° to 9.7 ± 6.0° (p = n.s.) in 2D, and from 10.2 ± 5.5° to 9.4 ± 5.4° (p = n.s.) in 3D. CONCLUSION: Tibiofemoral rotation showed no significant changes after femoral rotational osteotomy. Hence, femoral rotational osteotomy cannot be used to correct tibiofemoral rotation in addition to correcting the femoral version. Other surgical techniques need to be evaluated if correction of tibiofemoral rotation is required. LEVEL OF EVIDENCE: Level III.

PURPOSE: The aim of this study was to report the clinical and functional outcomes, complication rates, implant survivorship and the progression of tibiofemoral osteoarthritis (OA), after new inlay or onlay patellofemoral arthroplasty (PFA), for isolated patellofemoral OA. Comparison of different implant types and models, where it was possible, also represented one of the objectives. METHODS: A systematic literature search following PRISMA guidelines was conducted on PubMed, Scopus, Embase and Cochrane databases, to identify possible relevant studies, published from the inception of these databases until 11.11.2022. Randomized control trials (RCTs), case series, case control studies and cohort studies, written in English or German, and published in peer-reviewed journals after 2010, were included. Not original studies, case reports, simulation studies, systematic reviews, or studies that included patients who underwent TKA or unicompartmental arthroplasty (UKA) of the medial or lateral compartment of the knee, were excluded. Additionally, only articles that assessed functional and/or clinical outcomes, patient-reported outcomes (PROMs), radiographic progression of OA, complication rates, implant survival rates, pain, as well as conversion to TKA rates in patients treated with PFA, using inlay or onlay trochlea designs, were included. For quality assessment, the Methodological Index for Non-Randomized Studies (MINORS) for non-comparative and comparative clinical intervention studies was used. RESULTS: The literature search identified 404 articles. 29 of them met all the inclusion criteria following the selection process. Median MINORS for non-comparative studies value was 12.5 (range 11-14), and for comparative studies 20.1 (range 17-24). In terms of clinical and functional outcomes, no difference between onlay and inlay PFA has been described. Both designs yielded satisfactory results at short, medium and long-term follow-ups. Both designs improved pain postoperatively and no difference between them in terms of postoperative VAS has been noted, although the onlay groups presented a higher preoperative VAS. When comparing the inlay to onlay trochlea designs, the inlay group displayed a lower progression of OA rate. CONCLUSION: There is no difference in functional or clinical outcomes after PFA between the new inlay and the onlay designs, with both presenting an improvement in most of the scores that were used. A higher rate of OA progression was observed in the onlay design group. LEVEL OF EVIDENCE: III.

Inferior angle of scapula fractures are rare, and can lead to pain and impaired shoulder function. This report details the cases of two adult patients with inferior angle of scapula fractures who underwent acute surgical reduction and suture repair. In both patients, the avulsed fragment was dislocated and entrapped in the scapulothoracic joint resulting in noticeable crepitations and disturbed scapulothoracic rhythm during active range of motion. After surgery, both patients achieved significant improvements in shoulder function, enabling them to resume their daily activities without limitations. Fracture healing was uneventful in both cases. While the optimal treatment strategy for acute inferior angle of scapula fractures is still debated, early surgical intervention with suture fixation may offer favorable outcomes for patients with displaced fractures entrapped in the scapulothoracic joint, presenting with crepitus and disrupted scapulothoracic rhythm. Further research is needed to establish evidence-based guidelines for managing these rare fractures.
