ABSTRACT

The present study was conducted to identify the effect of vasopressin (AVP) on electrocardiographic changes produced by ischemia-reperfusion. Male rats were divided into seven groups (n=8-13) subjected to 30min ischemia and 120 min reperfusion. In protocol I (control group), saline was administered before ischemia. In protocol II, different doses of AVP (0.015, 0.03, 0.06 and 0.12µg/rat) were infused 10 min before ischemia. In protocol III SR49059 (1 mg/kg), was injected 20 min prior to ischemia with and without the effective dose of AVP (0.03 g/rat). Ischemia-induced arrhythmia and myocardial infarct size (IS) were measured. Different doses of vasopressin decreased IS. There were no significant differences in PR, QRS duration and &DGR;T/amp;DGR;ST ratio between control and intervention groups in ischemia. ST elevation was significantly increased in control and AVP 0.015, 0.03, 0.06 groups during ischemia. In AVP 0.12 group there was no significant difference in ST deviation between the baseline and ischemia phase. JT interval was significantly increased in control and antagonist group during ischemia. AVP 0.12µ/rat prevented the increase of JT interval in ischemia compared to the baseline. In summary, AVP mediated preconditioning improved ST resolution, prevented prolongation of JT interval and decreased the likelihood of subsequently ventricular arrhythmia.

ABSTRACT

Objective The aim of this study is to evaluate the laparoscopic method in gastric cancer staging before surgery in patients undergoing Neo-adjuvant chemotherapy. Methods This was a cross-sectional study on patients with T3-T4 and node positive non-metastatic gastric adenocarcinoma who was candidates for neo-adjuvant chemotherapy. The patients were excluded from the study when they were not in the condition to continue the study. All eligible patients underwent metastases work up by abdominal, pelvic and chest CT scans and then received neo-adjuvant chemotherapy. Pre-operative laparoscopy was performed before definitive surgery. The collected data was analyzed using SPSS software. The statistical significance, sensitivity, specificity and predictive values were calculated from this data. Results In this study, out of 50 patients, 26 (52%) were male. The mean age of the patients was 62.44 ± 12.136 years. Most tumors were located in one-third distal of stomach (21 patients, 42%). The accuracy of the laparoscopic method, to determine the degree of involvement of the lymph nodes, revealed that the method had a sensitivity of 84%, a specificity of 47.62%, a positive likelihood ratio (PLR) of 1.6, a negative likelihood ratio (NLR) of 0.33%, a positive predictive value of 65.62% and a negative predictive value of 71.43%. The accuracy of the laparoscopic method, regarding determining the presence or absence of metastasis, revealed that the method had a sensitivity, specificity, positive predictive value, and a negative predictive value of 100%. Conclusion Based on the evidence of this study, and other studies in this field, it seems that the use of a step-by-step combination methods in gastric cancer staging would be logical. Alone, no singular method would be sufficient in providing the physician with sufficient information to successfully stage the tumor and thus determine the appropriate treatment. Therefore, the combination of methods should be used which, in view of the economic constraints of health systems, would be recommended for high-risk patients for metastasis (N+/ T3-T4), diagnostic pre-operative laparoscopy.

ABSTRACT

OBJECTIVE: To identify Leishmania using PCR. METHODS: This study was conducted from April 2009 to March 2011 in order to identify Leishmania species in a new endemic area of CL in Lorestan, Iran. Samples were taken from 62 patients that referred to the health centers in different cities of Lorestan province, the presence of Leishmania was confirmed using direct smear and then grown in NNN media and mass cultured in RPMI 1 640 medium supplemented with 10% heat-inactivated fetal bovine serum. DNA was extracted from cultured promastigotes and used in ITS-PCR. RESULTS: 45(72.6%) samples out of 62 showed a band in the range of 485 bp and 17 (27.4%) with a band in the range of 626 bp which were similar to standard strains of Leishmania tropica(L. tropica) and Leishmania major(L. major), respectively. 50 (65.80%) of samples were collected from people with no history of travel in at least a year prior to the onset which shows that indigenous source of infection. CONCLUSIONS: Since the vector and reservoir of the two species are different, so precise and extensive control and prevention methods should be designed and carried out.

ABSTRACT

BACKGROUND: Leishmaniasis is a group of diseases that are created by intracellular parasites of Leishmania. Cutaneous leishmaniasis is considered as one of the health problems in some provinces of Iran. METHODS: In this study, a total of 178 Giemsa-stained slides from confirmed cases of cutaneous leishmaniasis were examined. The slides were prepared from the patients with cutaneous leishmaniasis that referred to health centers and infected during the epidemic of cutaneous leishmaniasis in Poldokhtar city, Lorestan Province, Iran in 2006.Genomic DNA from each slide was extracted. After DNA extraction, ITS-PCR was used. RESULTS: Out of 178 slides, 129 (72.47%) samples had a band in the range of 485 bp and 49 (27.53%) samples 626 bp that matched L. tropica and L. major standard samples, respectively. CONCLUSION: This study showed that Leishmania DNA could be efficiently extracted and amplified even from old Giemsa-stained microscopic slides that were stored more than 6 yr. In this study was shown that both L. tropica and L. major species exist in Lorestan Province.
