RESUMEN

OBJECTIVE: Describing how Kaingang seniors and their primary caregivers experience access to public health services. METHOD: A qualitative study guided by ethnography, conducted with 28 elderly and 19 caregivers. Data were collected between November 2010 and February 2013 through interviews and participative observation analyzed by ethnography. RESULTS: The study revealed the benefits and difficulties of the elderly access to health services, the facility to obtain health care resources such as appointments, medications and routine procedures, and the difficulties such as special assistance service problems and delays in the dispatching process between reference services. CONCLUSION: The importance of knowing and understanding the cultural specificities of the group in order to offer greater opportunities for the elderly access to health services was reinforced.

RESUMEN

Skeletal muscle regeneration after injury is a complex process involving inflammatory signaling and myoblast activation. Pro-inflammatory cytokines like tumor necrosis factor-alpha (TNF-α) are key mediators, but their effects on gene expression in proliferating myoblasts are unclear. We performed the RNA sequencing of TNF-α treated C2C12 myoblasts to elucidate the signaling pathways and gene networks regulated by TNF-α during myoblast proliferation. The TNF-α (10 ng/mL) treatment of C2C12 cells led to 958 differentially expressed genes compared to the controls. Pathway analysis revealed significant regulation of TNF-α signaling, along with the chemokine and IL-17 pathways. Key upregulated genes included cytokines (e.g., IL-6), chemokines (e.g., CCL7), and matrix metalloproteinases (MMPs). TNF-α increased myogenic factor 5 (Myf5) but decreased MyoD protein levels and stimulated the release of MMP-9, MMP-10, and MMP-13. TNF-α also upregulates versican and myostatin mRNA. Overall, our study demonstrates the TNF-α modulation of distinct gene expression patterns and signaling pathways that likely contribute to enhanced myoblast proliferation while suppressing premature differentiation after muscle injury. Elucidating the mechanisms involved in skeletal muscle regeneration can aid in the development of regeneration-enhancing therapeutics.

RESUMEN

The goal of this article is to analyze the development of gerontological nurses working in the Family Health Program in a municipal district in Belém, PA. Data have been collected through interviews with 14 nurses between 08/2009 and 02/2010 and have been analyzed using the content analysis method to generate topics. One such topic is the theme of this article: building gerontological work, and the sub-topics are the following: nursing consultation, home visits, family care, and seeking partnerships for integrated actions. According to the results, it may be inferred from nurses performing gerontological tasks that work is hampered primarily by urban violence, the inefficient functional structure of services and poor specific geriatric training. However, nurses have built a special "making of gerontology" by creating strategies of integrated actions according to each new situation that has been presented.

RESUMEN

OBJECTIVE: to map and analyze the scientific literature on nursing care aimed at people with Parkinson's disease in Primary Health Care. METHOD: this is a scoping review using the Joanna Briggs Institute methodology, carried out between July and October 2020, and updated in November 2021 in six databases from nursing care and Parkinson's disease descriptors, and their respective acronyms and synonyms in English, Portuguese and Spanish. RESULTS: a total of 44 publications were included in this review, which identified as nursing care in Primary Care: assessment of motor and non-motor functions; management of activities of daily living and instrumental activities of daily living; disease self-management education for people with Parkinson's and their care partners; supervised group approach; and personal factor management. CONCLUSION: Nursing care for people with Parkinson's at the primary level essentially involves actions that include providing focused care at an individual and group level, encompassing clinical assessment, patient education, patient involvement in the social context of care, and developing positive relationships with family members and caregivers.

RESUMEN

OBJECTIVE: To reflect on the role and initiatives of the Brazilian Nursing Association (ABEn) and the Federal Nursing Council (Cofen) in the development, recognition, and regulation necessary to implement advanced practice nursing (APN) in Brazil. METHOD: This is a theoretical-reflective essay resulting from the roundtable discussion entitled "Regulation and recognition of advanced nursing practices in Brazil: how are we and what is missing" held in May 2021, at the III International Symposium on Adult Health. RESULTS: ABEn and Cofen are strategic partners to define the areas of activity of this specialization in Brazil. Nursing organizations are responsible for defining professional profiles and curricula, advanced practice nursing conditions, and adequacy of existing graduate courses aiming at technical support, political leadership, regulation, and construction of an APN foundation in Brazil. CONCLUSION: Brazil must broaden the national debate on APN, define a training model for it, and articulate efforts with all its strategic partners to build a theoretical, political, and labor framework for advanced practice nurses' full professional practice.

RESUMEN

The pursuit of better therapies for disorders creating deficiencies in skeletal muscle regeneration is in progress, and several biotoxins are used in skeletal muscle research. Since recombinant proteins derived from Lonomia obliqua bristles, recombinant Lonomia obliqua Stuart-factor activator (rLosac) and recombinant Lonomia obliqua prothrombin activator protease (rLopap) act as cytoprotective agents and promote cell survival, we hypothesize that both rLosac and rLopap favour the skeletal muscle regeneration process. In the present work, we investigate the ability of these recombinant proteins rLosac and rLopap to modulate the production of key mediators of the myogenic process. The expression of myogenic regulatory factors (MRFs), cell proliferation, the production of prostaglandin E2 (PGE2) and the protein expression of cyclooxygenases COX-1 and COX-2 were evaluated in C2C12 mouse myoblasts pre-treated with rLosac and rLopap. We found an increased proliferation of myoblasts, stimulated by both recombinant proteins. Moreover, these proteins modulated PGE2 release and MRFs activities. We also found an increased expression of the EP4 receptor in the proliferative phase of C2C12 cells, suggesting the involvement of this receptor in the effects of PGE2 in these cells. Moreover, the recombinant proteins inhibited the release of IL-6 and PGE2, which is induced by an inflammatory stimulus by IL-1ß. This work reveals rLopap and rLosac as promising proteins to modulate processes involving tissue regeneration as occurs during skeletal muscle injury.

RESUMEN

This study was performed in six Long-Term Care Institutions for the Elderly (LTCIEs) that helped low-income senior citizens in three regions of the country. Its aim is to examine how LTCIEs' internal organizational system maintained structural coupling with surrounding systems. The data were collected through interviews and observation. The analysis was based on concepts of Luhmann's Social Systems Theory. As a result, the rules of belonging did not encourage aid proposals that stimulated independent life and the individuality of residents. The structural couplings with the external environment generated negative resonance within LTCIEs, such as the lack of links to programmatic actions of public primary health care, inability to maintain a multiprofessional staff, inability to fully adapt the infrastructure, and inability to bring reçatives closer to the institution's daily routine. As a positive aspect, the staff was empowered by the presence of students and their professors.

RESUMEN

OBJECTIVE: to identify scientific productions on nursing care related to fall risk prevention among hospitalized elderly people. METHODS: an integrative literature review from 2015 to 2019 in the PubMed/MEDLINE, Scopus, Web of Science, LILACS, BDENF, SciELO and CINAHL databases, in Portuguese, English and Spanish. The keywords were elderly, hospitalization, accidents due to falls, nursing care. RESULTS: thirty-three publications were analyzed. The synthesis of the studies resulted in the categories: Clinical nursing assessments to prevent falls among hospitalized elderly people; Fall risk factors for elderly people; Fall risk prevention strategies for elderly people. FINAL CONSIDERATIONS: it was found that the scientific knowledge produced on nursing care related to fall risk prevention for hospitalized elderly people evidences the clinical assessment, risk factors and strategies such as nursing care, contributing to foster self-care behavior and promotion security for elderly people.

This is a case report of women with pregnancy morbidity (PM), some of them associated with antiphospholipid syndrome (APS), in which the glycan patterns of immunoglobulin G (IgG) were investigated based on the theory of alteration of glycosylation in autoimmunity. We used lectin blot to determine changes in terminal glycosylation of polyclonal IgG from women with antiphospholipid (aPL) antibodies and PM plus vascular thrombosis (PM/VT) and seronegative-obstetric APS (SN-OAPS). In addition, we analyzed IgG from women with PM without aPL (PM/aPL-) and healthy women, as controls. Even though the SN-OAPS and PM/VT groups share the PM, only the SN-OAPS group showed a decreased expression of galactose compared to the healthy group. We also found the presence of mannosylated oligosaccharides in IgG from all patients being significantly higher in IgG from women of the PM/aPL- group. The differences in glycans presented here could relate to pathological mechanisms of PM associated with APS.

OBJECTIVE: to analyze in available scientific literature what problems are caused to the musculoskeletal system of caregivers of older adults in the home environment. METHODS: an integrative literature review indexed in the PubMed, CINAHL, Web of Science, SCOPUS, BDENF, LILACS and SciELO databases. RESULTS: the sample totaled 17 studies that showed three thematic categories: Care overload for caregivers' health; Caregivers' physical illness; Caregiver self-neglect. CONCLUSION: the evidence identified health problems for caregivers of older adults, regarding the musculoskeletal system, demonstrating the importance of considering intervention strategies that provide guidelines for self-care guidelines, in order to prevent musculoskeletal complications resulting from older adults' care activities at home.

OBJECTIVE: To assess the contribution of gerontotechnologies in nursing care to older adults with Parkinson's disease, aiming at the prevention of falls. METHOD: Convergent Assistance Research, through the development and evaluation of gerontechnologies focused on the prevention of falls in older adults with Parkinson's Disease. Clinical evaluation, interviews and workshops with older adults were carried out, as well as analysis and judgment of the materials developed by ten judges certified in gerontology by the Brazilian Society of Geriatrics and Gerontology. RESULTS: Older adults need at least six years of study to understand the educational gerontotechnology development. The objectives related to the content, understanding of the text, illustration, presentation, motivation and cultural adaptation were achieved. CONCLUSION: The educational gerontotechnology developed has relevant content, which can be used by older adults, family members and caregivers, to generate clarification of questions on the theme of fall prevention for older adults with Parkinson's Disease.

RESUMEN

As a tribute to Butantan Institute in its 120th anniversary, this review describes some of the scientific research efforts carried out in the study of Lonomia envenoming in Brazil, a country where accidents with caterpillars reach over 42,000 individuals per year (especially in South and Southeast Brazil). Thus, the promising data regarding the studies with Lonomia's toxins contributed to the creation of new research centers specialized in toxinology based at Butantan Institute, as well as to the production of the antilonomic serum (ALS), actions which are in line with the Butantan Institute mission "to research, develop, manufacture, and provide products and services for the health of the population". In addition, the study of the components of the Lonomia obliqua bristle extract led to the discovery of new molecules with peculiar properties, opening a field of knowledge that could lead to the development and innovation of new drugs aimed at cell regeneration and inflammatory diseases.

RESUMEN

This study aimed to systematize a basic model of nursing care based on the taxonomy from the North American Nursing Diagnosis Association (NANDA), tested in elderly patients with dementia affection, living in a Long-Stay Institution (ILPIs) in the countryside of Bahia, Brazil. Nine elder residents were selected at the Institution by the application of the Mini Test on Mental State in two stages, with an interval of six months, which confirmed dementia affection. Data were collected through multidimensional assessment tools for the elder provided by the Ministry of Health. In order to elaborate a caring plan, in accordance with the steps of the nursing process, nursing diagnoses were grouped into four categories: motor behavior cognition and memory, communication and other health physical problems. The caring plans have been implemented and evaluated. Testing and systematization of the basic model from nursing care based on NANDA has been possible among institutionalized demented elder inferring its applicability to other ILPIs.

OBJECTIVE: To ascertain the characteristics of the nursing care provided to institutionalized elderly people with dementia. METHODS: Integrative review of studies published between 2013 and 2017, in English, Spanish, and Portuguese, from the databases BDENF, LILACS, CINAHL, PubMed, SciELO, TRIP, and National Guideline Clearing House. Thematic data analysis was used. RESULTS: From the data collected in the 41 included studies, three themes emerged: Care with a focus on the needs of elderly people in LTCIEs; Care based on the work process of the nursing team; and Shared care. Final considerations: Elderly care can take place from different perspectives: that of the elderly; the nursing team; and it can be shared among the different parties involved. The importance of communication must be stressed, as do the development of skills and attitudes of the team, as well as proper training and support, good environment in the care process, and an approach centered on the institutionalized elderly.

This descriptive research aims to identify the profile of the family caregiver using the World Health Organization's QPFC and WHOQOL-Bref (instrument application methods) in a sample of 120 family caregivers of dependent elderly patients who represented the population of three Healthcare Centers in the area of the Great Porto, Portugal. Data were collected in the period between January 2005 and March 2005. Results indicated that female caregivers with 55 years of average age, who had other family care giving duties besides caring for the elderly family member, were the majority. The circumstances surrounding care giving, in the perspective of the elderly patient and the other family demands, generated negative consequences to the caregiver's life and health. However, at the same time, caregivers expressed positive perceptions and rewarding feelings when caring for the elderly family member. In a nutshell, it is essential for nurses to consider the binomial caregiver and dependent elderly patient, since both demand special healthcare attention from healthcare services.

OBJECTIVE: To analyze the trends in hospitalization and mortality rates for ambulatory care sensitive conditions (ACSC) among older adults in the state of Santa Catarina, Brazil, from 2008 to 2015. METHOD: Ecological time-series study based on data from the Hospital Information System (Sistema de Informações Hospitalares - SIH) of the public health system (Sistema Único de Saúde - SUS), using hospital admission authorization forms as data source, from 2008 to 2015, in Santa Catarina. Data were analyzed by gender and age, which was stratified into two age groups: 60 to 79 years and 80 years or older. We standardized age rates using the direct method and statistical analysis using segmented linear regression (joinpoint regression). RESULTS: Sensitive conditions led to 41% of total hospitalizations of older adults in 2008 and 32% in 2015. The annual rate variation decreased [-4.6^ 95%CI (-5.7; -3.6)]. The most prevalent causes were: heart failure, chronic obstructive pulmonary disease, and cerebrovascular diseases. Hospitalization mortality rates dropped, regardless of gender and in the age group up to 80 years, representing an annual variation of -2.4%, with -2.1% for males and -2.7% for females. CONCLUSION: Analyzing the ACSC indicator, which evaluates the quality and effectiveness of primary care, the results of this research revealed a reduction in hospitalization rates among older adults, regardless of gender and age groups established in this study.

MÉTODO: Estudo ecológico de série temporal, com dados do Sistema de Informações Hospitalares (SIH) do Sistema Único de Saúde (SUS), utilizando as autorizações de internações hospitalares como fonte de dados, no período de 2008 a 2015, em Santa Catarina. Os dados foram analisados por sexo e idade, estratificada em duas faixas etárias: 60a 79 anos e 80 anos ou mais. Foram realizadas a padronização das taxas por idade através do método direto e a análise estatística por meio de regressão linear segmentada (joinpoint regression). Introduction: The antiphospholipid syndrome is characterized by the persistent presence of antiphospholipid antibodies and clinical manifestations of thrombosis or gestational morbidity that are associated with oxidative stress and endothelial dysfunction. Objective: To evaluate markers of oxidative stress in endothelial cells induced by the serum from women with different clinical manifestations of the antiphospholipid syndrome, and to analyze the antioxidant capacity of the sera. Materials and methods: We included 48 women who were classified as follows: presence of antiphospholipid antibodies and clinical criteria of gestational morbidity alone, vascular thrombosis only, and gestational morbidity/vascular thrombosis. Control groups included antiphospholipid antibodies negative women. In an in vitro model of endothelial cells stimulated with sera from women included in the groups, some markers of oxidative stress were determined by flow cytometry. The antioxidant capacity in the sera of these women was analyzed. Results: The sera from the groups of women with antiphospholipid syndrome that presented thrombosis, with or without gestational morbidity, generated a significant increase (p<0.05 and p<0.001) in endothelial oxidative stress markers in contrast to the control of normal human serum. There were no differences in the effect of the sera from the different study groups on endothelial lipid peroxidation. Also, there was also no difference in the antioxidant activity of the sera. Conclusion: Mitochondrial oxidative stress in the endothelium is associated with the presence of thrombosis; instead, its association with gestational morbidity generates intracellular oxidative stress.


Introducción. El síndrome antifosfolípido se caracteriza por la presencia persistente de anticuerpos antifosfolípidos y manifestaciones clínicas de trombosis o morbilidad gestacional, las cuales se asocian con estrés oxidativo y disfunción endotelial. Objetivo. Evaluar los marcadores de estrés oxidativo en células endoteliales, inducidos por el suero de mujeres con diferentes manifestaciones clínicas del síndrome antifosfolípido y analizar la capacidad antioxidante de los sueros. Materiales y métodos. Se incluyeron 48 mujeres que fueron clasificadas así: presencia de anticuerpos antifosfolípidos y criterios clínicos de morbilidad gestacional, trombosis vascular o ambas. Como grupos control se incluyeron mujeres negativas para anticuerpos antifosfolípidos. En un modelo in vitro de células endoteliales estimuladas con los sueros de las mujeres del estudio, se determinaron algunos marcadores de estrés oxidativo por citometría de flujo. También, se analizó la capacidad antioxidante de los sueros incluidos. Resultados. Los sueros de los grupos de mujeres con síndrome antifosfolípido que presentaban trombosis, con morbilidad gestacional o sin ella, generaron un incremento significativo (p<0,05 y p<0,001) en los marcadores de estrés oxidativo endotelial, en contraste con el control de suero humano normal. No se observaron diferencias en el efecto de los sueros de los diferentes grupos de estudio sobre la lipoperoxidación endotelial. Tampoco se encontró diferencia en la actividad antioxidante de los sueros. Conclusión. El estrés oxidativo mitocondrial en el endotelio se asocia con la presencia de trombosis. Sin embargo, cuando esta se asocia con morbilidad gestacional, también se genera estrés oxidativo intracelular.

RESUMEN

OBJECTIVE: To understand how nursing workers perceive care of hypertension (HBP) in older adult within the scope of the Family Health Strategy. METHOD: A qualitative descriptive study carried out in the city of Natal/RN, with 20 nursing workers providing care to older adults with HBP. The data were obtained through a semi-structured interview and analysed by the Thematic Analysis, based on the theoretical support of the integrality, using Atlas.ti 7.0 software. RESULTS: The elements found as facilitators were: territorialization, partnerships, professional proactivity and the user's bond with the team. Among those found as barriers were: disease-centered care; academic education based on the biomedical model; lack of inter-sectorality and discontinuity of care in the care network. FINAL CONSIDERATIONS: Nursing workers perceive that health institutions lack articulated and innovative practices that incorporate new paradigms focused on integrality of care.

RESUMEN

OBJETIVO: Descrever os aplicativos voltados para a segurança do paciente idoso cirúrgico nos períodos pré e pós-operatório. MÉTODO: Prospecção tecnológica com abordagem qualitativa realizada nas lojas virtuais Apple Store, Google Play e aplicativo web. Como estratégia de busca utilizou-se os termos: 'autocuidado idoso', 'pré-operatório', 'pós-operatório', 'segurança do paciente', 'segurança do paciente idoso'. RESULTADOS: Foram identificados 12 aplicativos na loja virtual Apple Store, 17 na Google Play e um web app, os quais estavam voltados a gamificação de profissionais da saúde e da população; aplicativos educacionais aos profissionais de saúde e usuários do sistema de saúde; aplicativos relacionados como ferramenta de trabalho para os profissionais de saúde; aplicativos como ferramenta de autocuidado para usuários do sistema de saúde; e, aplicativos voltados para pós-operatório de usuários do sistema de saúde. CONCLUSÃO: Não foram encontradas tecnologias voltadas para o autocuidado e segurança do paciente idoso nos períodos pré e pós-operatório


OBJECTIVE: To describe applications focused on the safety of elderly surgical patients in the pre- and postoperative periods. METHOD: Technology prospecting with a qualitative approach in the Apple Store, Google Play virtual stores, and web application. As a search strategy, the following terms were used: 'elderly self-care', 'preoperative', 'postoperative', 'patient safety', 'elderly patient safety'. RESULTS: 12 applications were identified in the Apple Store, 17 in Google Play, and a web app, which focused on the gamification of health and population professionals; educational applications to health professionals and health system users; related applications as a work tool for health professionals; applications as a self-care tool for health system users; and, post-operative applications for health system users. CONCLUSION: Technologies aimed at self-care and the safety of elderly patients were not found in the pre- and postoperative periods.

In this research, the unrefined non-centrifugal sugar (UNCS) quality obtained from two sugarcane varieties (RD 7511 and CC 8475) and using two types of technologies (traditional and Ward-Cimpa production facilities) were evaluated. The parameters monitored through the process were impurities, total soluble solids, acidity, pH, and temperature profile. Microbiological analyses were carried out on beating, molding, packing, and storage operations; and finally, an organoleptic analysis was carried out on the final UNCS product. Results showed that the UNCS obtained from variety CC 8475 had higher consumer acceptance; meanwhile, the technologies assessed did not show significant differences in final product quality. However, these technologies showed significant differences in the highest temperature, syrup, and juice properties. Microbiological analyses highlighted beating and molding as the critical points in UNCS production safety. Finally, it was evident that the implementation of new technologies or the improvement of the furnace, as in the Ward-Cimpa production facility, is not enough to achieve food safety requirements, as many other conditions affect the microbiological quality of the product. Although the temperatures reached on the Ward-Cimpa furnace are higher than those reached with the traditional furnace and thus, enough to kill all the harmful microorganisms, contamination in downstream operations still occurs.
