RESUMEN

Purpose: To determine the dynamic modification of the load exerted on the eye during air-puff testing by accounting for the deformation of the cornea. Methods: The effect of corneal load alteration with surface shape (CLASS) was characterized as an additional component of the load produced during the concave phase where the fluid outflow tangential to the corneal surface creates backward pressure. Concave phase duration (t CD ), maximum CLASS value (CLASS max ), and the area under CLASS-time curve (CLASS int ) are calculated for 26 keratoconic (KCN), 102 normal (NRL), and 29 ocular hypertensive (OHT) subjects. Tukey's HSD tests were performed to compare the three subject groups. A p-value less than 0.05 was considered statistically significant. Results: Accounting for CLASS increased the load by 34.6% ± 7.7% at maximum concavity; these differences were greater in KCN subjects (p < 0.0001) and lower in OHT subjects (p = 0.0028) than in NRL subjects. t CD and CLASS int were significantly longer and larger, respectively, for KCN subjects than those in the NRL and OHT groups (p < 0.0001). Conclusion: Load characterization is an essential step in assessing the cornea's biomechanical response to air-puff-induced deformation. The dynamic changes in the corneal surface shape significantly alter the load experienced by the corneal apex. This implies a subject-specific loading dynamic even if the air puff itself is identical. This is important when comparing the same eye after a surgical procedure or topical medication that alters corneal properties. Stiffer corneas are least sensitive to a change in load, while more compliant corneas show higher sensitivity.

Précis: Using a controlled experimental design with corneal phantoms, this study provides evidence of the lack of validity of a static air quality indicator, previously used to characterize aerosolization during dynamic noncontact tonometry. Purpose: To evaluate the accuracy of aerosol concentrations reported by an air quality indicator (AQI) following an air puff from a noncontact tonometer using non-aerosolizing corneal phantoms. Methods: Three rubber corneal phantoms of different stiffnesses were used to represent varying intraocular pressure (IOP) values. No liquid components and therefore no aerosol-generating potential was present. Reported concentrations of particulate matter (PM) having diameter less than 2.5 and 10µm, respectively PM2.5 and PM10, were recorded using an AQI before and during an air puff generated using noncontact tonometry. The effects of covariates IOP and sensor location on changes to air quality measurements from the baseline were evaluated using analysis of variance. Monte Carlo simulations were used to determine the likelihood of observing published trends by chance. The statistical significance threshold was p<0.05. Results: No correlations were found between PM2.5 and IOP or location. Reported concentrations of PM10 depended significantly on both IOP (p=0.0241) and location (p=0.0167). Monte Carlo simulations suggest the likelihood of finding a spurious positive correlation between IOP and PM at the upper same location are 53% and 92% for PM2.5 and PM10, respectively, indicating the AQI has systematic bias resulting from non-aerosol sources. Conclusions: We were able to reproduce the published correlation between reported aerosol concentration and IOP in non-contact tonometry using dry rubber phantoms in place of living corneas with tear films. In this study, we demonstrated that published correlations linking NCT to tear film aerosolization were artifacts of the measurement technique.

PURPOSE: The purpose of this study was to investigate the mechanism of potential droplet formation in response to air puff deformation with two noncontact tonometers (NCTs). METHODS: Twenty healthy volunteers were examined using two NCTs, Ocular Response Analyzer and Corvis ST, and two contact tonometers, iCare and Tono-Pen. High-speed videos of the tear film response were captured with at spatial resolution of 20 microns/pixel at 2400 fps. Droplet size, droplet velocity, distance between air puff impact location, and the tear meniscus-lid margin were characterized. RESULTS: One subject was excluded due to technical issues. Droplets were detected only in tests with instilled eye drop. Videos showed the tear film rolls away from the apex while remaining adherent to the ocular surface due to the tendency of the fluid to remain attached to a solid surface explained by the Coanda effect. Twelve out of 38 videos with an eye drop administration showed droplet formation. Only one resulted in droplets with predominantly forward motion, which had the shortest distance between air puff impact location and lower meniscus. This distance on average was 5.9 ± 1.1 mm. The average droplet size was 500 ± 200 µm. CONCLUSIONS: Results indicate no droplet formation under typical clinical setting. Hence, standard clinical use of NCT tests is not expected to cause droplets. NCT testing with eye drop administration showed droplet formation at the inferior eyelid boundary, which acts as a barrier and interrupts tear flow. TRANSLATIONAL RELEVANCE: Study of tear film interaction with NCT air puff shows that these tonometers are not expected to cause droplet formation in standard use and that if external drops are required, both eyelids should be held if patients need assistance to maintain open eyes to avoid droplets with predominantly forward motion.

BACKGROUND: This study aimed at assessment of post-transcatheter aortic valve (TAV) replacement hemodynamics and turbulence when a same-size SAPIEN 3 (Edwards Lifesciences Corp, Irvine, Calif) and Medtronic Evolut (Minneapolis, Minn) were implanted in a rigid aortic root with physiological dimensions and in a representative root with calcific leaflets obtained from patient computed tomography scans. METHODS: TAV hemodynamics were studied by placing a SAPIEN 3 26-mm and an Evolut 26-mm in rigid aortic roots and representative root with calcific leaflets under physiological conditions. Hemodynamics were assessed using high-fidelity particle image velocimetry and high-speed imaging. Transvalvular pressure gradients (PGs), pinwheeling indices, and Reynolds shear stress (RSS) were calculated. RESULTS: (1) PGs obtained with the Evolut and the SAPIEN 3 were comparable among the different models (10.5 ± 0.15 mm Hg vs 7.76 ± 0.083 mm Hg in the rigid model along with 13.9 ± 0.19 mm Hg vs 5.0 ± 0.09 mm Hg in representative root with calcific leaflets obtained from patient computed tomography scans respectively); (2) more pinwheeling was found in the SAPIEN 3 than the Evolut (0.231 ± 0.057 vs 0.201 ± 0.05 in the representative root with calcific leaflets and 0.366 ± 0.067 vs 0.122 ± 0.045 in the rigid model); (3) higher rates of RSS were found in the Evolut (161.27 ± 3.45 vs 122.84 ± 1.76 Pa in representative root with calcific leaflets and 337.22 ± 7.05 vs 157.91 ± 1.80 Pa in rigid models). More lateral fluctuations were found in representative root with calcific leaflets. CONCLUSIONS: (1) Comparable PGs were found among the TAVs in different models; (2) pinwheeling indices were found to be different between both TAVs; (3) turbulence patterns among both TAVs translated according to RSS were different. Rigid aortic models yield more conservative estimates of turbulence; (4) both TAVs exhibit peak maximal RSS that exceeds platelet activation 100 Pa threshold limit.

Polymeric heart valves (PHV) can be engineered to serve as alternatives for existing prosthetic valves due to higher durability and hemodynamics similar to bioprosthetic valves. The purpose of this study is to evaluate the effect of geometry on PHVs coaptation and hemodynamic performance. The two geometric factors considered are stent profile and leaflet arch length, which were varied across six valve configurations. Three models were created with height to diameter ratio of 0.6, 0.7, and 0.88. The other three models were designed by altering arch height to stent diameter ratio, to be 0, 0.081, and 0.116. Particle image velocimetry experiments were conducted on each PHV to characterize velocity, vorticity, turbulent characteristics, effective orifice area, and regurgitant fraction. This study revealed that the presence of arches as well as higher stent profile reduced regurgitant flow down to 5%, while peak systole downstream velocity reduced to 58% and Reynolds Shear Stress values reduced 40%. Further, earlier reattachment of the forward flow jet was observed in PHVs with leaflet arches. These findings indicate that although both geometric factors help diminish the commissural gap during diastole, leaflet arches induce a larger jet opening, yielding to earlier flow reattachment and lower energy dissipation.

Transcatheter aortic valve implantation (TAVI) has become a popular alternative technique to surgical valve replacement for critical patients. Biological valve tissue has been used in TAVI procedures for over a decade, with over 100,000 implantations to date. However, with only 6 years follow up, little is known about the long-term durability of biological tissue. Moreover, the high cost of tissue harvesting and chemical treatment procedures favor the development of alternative synthetic valve leaflet materials. Textile polyester is one such material which provides outstanding folding and strength properties combined with proven biocompatibility, and could therefore be considered as a candidate to replace the biological valve leaflets in TAVI procedures. For that purpose, in addition to the mechanical properties, the hemodynamic properties of the synthetic material should be comparable to the properties of biological tissue. An ideal replacement heart valve would provide low static and dynamic regurgitation, ensure laminar flow across the valve, and limit the turbidity of flow downstream of the valve. The purpose of the present work is to compare in vitro the mechanical and hemodynamic performances of textile woven polyester valves with biological ones. Testing results indicate that textile valves trade elasticity for superior mechanical strength, relative to biological tissue. Despite this, the dynamic flexibility of textile valve leaflets strongly resembled what was seen with biological leaflets. Regurgitation, as well as slightly modified turbulent patterns, in textile valves was higher than biological valves due to the increased porosity, but, rapid tissue ingrowth post-implantation would likely mitigate this effect. Together these findings provide additional evidence favoring the use of textile polyester as a synthetic heart valve leaflet material.

The most widely implanted prosthetic heart valves are either mechanical or bioprosthetic. While the former suffers from thrombotic risks, the latter suffers from a lack of durability. Textile valves, alternatively, can be designed with durability and to exhibit hemodynamics similar to the native valve, lowering the risk for thrombosis. Deviations from native valve hemodynamics can result in an increased Reynolds Shear Stress (RSS), which has the potential to instigate hemolysis or shear-induced thrombosis. This study is aimed at characterizing flow in multiple textile valve designs with an aim of developing a low profile valve. Valves were created using a shaping process based on heating a textile membrane and placed within a left heart simulator. Turbulence and bulk hemodynamics were assessed through particle imaging velocimetry, along with flow and pressure measurements. Overall, RSS was reduced for low profile valves relative to high profile valves, but was otherwise similar among low profile valves involving different fabric designs. However, leakage was found in 3 of the 4 low profile valve designs driving the fabric design for low profile valves. Through textile design, low profile valves can be created with favorable hemodynamics.
