RESUMO

In this study, the lifetime prevalence of stressful events and current posttraumatic stress disorder (PTSD) in the general adult population in the Netherlands were examined, and risk groups for PTSD were determined. A representative sample of 2,238 adults (>or=18 years) in the Netherlands completed digital questionnaires by computer-assisted self-interviewing. In total, 52.2% of the population reported at least one stressful event throughout their life. The estimated prevalence of current PTSD in the total population was 3.8%. Rape and physical assault were the stressful events most likely to be associated with PTSD, witness of injury the least likely. Stressful medical events were moderately associated with PTSD. Prevalence of PTSD was elevated among single women and middle-aged men.

In winter 2016-2017, Highly Pathogenic Avian Influenza (HPAI) H5N8 virus spread across Europe, causing unprecedented epizootics. France was massively affected, resulting in the culling of over 6 million poultry. Boosted regression tree (BRT) models were used to quantify the association between spatial risk factors and HPAI H5N8 infection in poultry holdings and to generate predictive maps for HPAI infection. Three datasets were combined to build the model: a dataset of the reported outbreaks in poultry, a dataset of the poultry holdings where the virus has not been reported and a set of relevant spatial risk factors, including poultry production and trade, and water bird habitat. Results identified key associations between the 'foie gras' production systems and HPAI H5N8 risk of occurrence and indicate that strengthening surveillance of fattening duck production systems and making the transportation of fattening ducks more secure would be key priority options for HPAI prevention and control.

After its introduction in Turkey in November 2013 and subsequent spread in this country, lumpy skin disease (LSD) was first reported in the western Turkey in May 2015. It was observed in cattle in Greece and reported to the World Organization for Animal Health (OIE) in August 2015. From May 2015 to August 2016, 1,092 outbreaks of lumpy skin disease were reported in cattle from western Turkey and eight Balkan countries: Greece, Bulgaria, The Former Yugoslav Republic of Macedonia, Serbia, Kosovo, and Albania. During this period, the median LSD spread rate was 7.3 km/week. The frequency of outbreaks was highly seasonal, with little or no transmission reported during the winter. Also, the skewed distribution of spread rates suggested two distinct underlying epidemiological processes, associating local and distant spread possibly related to vectors and cattle trade movements, respectively.

Our objective was to study the ability of a syndromic surveillance system to identify spatio-temporal clusters of drops in the number of calvings among beef cows during the Bluetongue epizootic of 2007 and 2008, based on calving seasons. France was partitioned into 300 iso-populated units, i.e. units with quite the same number of beef cattle. Only 1% of clusters were unlikely to be related to Bluetongue. Clusters were detected during the calving season of primary infection by Bluetongue in 28% (n = 23) of the units first infected in 2007, and in 87% (n = 184) of the units first infected in 2008. In units in which a first cluster was detected over their calving season of primary infection, Bluetongue was detected more rapidly after the start of the calving season and its prevalence was higher than in other units. We believe that this type of syndromic surveillance system could improve the surveillance of abortive events in French cattle. Besides, our approach should be used to develop syndromic surveillance systems for other diseases and purposes, and in other settings, to avoid "false" alarms due to isolated events and homogenize the ability to detect abnormal variations of indicator amongst iso-populated units.

Pseudo-Kaposi's sarcoma has been associated, in most cases, with an underlying congenital arteriovenous (AV) fistula. A patient with chronic renal failure and an acquired, iatrogenic AV fistula in his left wrist for hemodialysis developed pseudo-Kaposi's sarcoma on his left hand three years after placement of the AV fistula. Histologic findings included a proliferation of superficial dermal vessels and fibroblasts, extravasated red blood cells, and occasional fibrin thrombi in vessels. To our knowledge, this is the third case of an association between pseudo-Kaposi's sarcoma and an acquired, iatrogenic AV fistula, and the first to involve the hand. Pseudo-Kaposi's sarcoma occurs in association with underlying congenital and acquired AV fistulas.

An experimental study of "annular scotoma" was conducted. Variations in prismatic effects at the boundary of the lens with respect to incidence were evaluated with the aid of a laser beam. The use of an arc-perimeter and an artificial aphakic eye allowed reconstruction of light beam paths from the outside to the inside of the eye. The results obtained enabled a more rigorous analysis of this optical phenomenon. A straightforward formula obtained from the experiment (tan theta = b/a) provided better understanding of the variations generated by certain parameters. This same formula can be applied to the mechanism of the "Jack-in-the-box" phenomenon.

We describe the principles of the measure of visual acuity by using the laser interference retinometer. Interference fringes are projected on the retina, with variation of direction and thickness varied, to study the functional capacity of the macula. This method is particularly useful in patients with cataracts, because it is much less influenced by the clarity of the media than Snellen chart listing. We give the results of a series of 122 eyes with cataract, comparing pre-operative laser acuity and post-operative Snellen visual acuity. The pre-operative interfermometric measure is possible in 77% of the cases, but in most cases of mature cataract or dense posterior subcapsular opacities, the patients do not discern any interference fringes. In immature cataracts according to other results of the literature, post-operative Snellen visual acuity reaches preoperative laser acuity in 60% of the cases, and is better in 29% of the cases. Clinical conditions such as macular degeneration, cystoid macular oedema, amblyopia, visual field cuts through fixation or myopic choroïdopathy, may make the interferometer test unreliable.

RESUMO

Two patients with unilateral painful ophthalmoplegia are presented, a woman of 26 and a man of 28. In both cases the phlebography of the orbit shows an alteration of the orbital veinous blood flow, specially of the third segment of the superior orbital vein. Another characteristic feature of this syndrom is efficiency of corticotherapy and heparinotherapy, which may suggest an inflammatory and thrombophlebitic origin.

RESUMO

The authors report a case of a 14 years old girl with a bilateral optic neuropathy especially an optic atrophy on the left eye. They show the clinical and radiological diagnostic difficulty of these symptoms before coming to the bilateral optic nerve meningioma. The authors have compared this rare case to those described in literature and insist on the importance of the CT Scan and eventually the nerve surgical exploration.

The effectiveness of thrombolytic agents used in local infusion has been demonstrated in the treatment of acute myocardial infarctions by several randomized trials. Thrombolytic therapy seems also to improve the prognosis of certain acute life-threatening brain stem strokes. Two cases of central retinal artery occlusion were treated with intracarotid urokinase injections. Visual acuity was improved in both cases in less than 24 hours, repermeation of the occluded vessels was observed 48 hours after the beginning of treatment. The limits of this kind of therapy are emphasized: there must be no contra-indication to the fibrinolytic agents, a delay shorter than 6 hours after the beginning of symptoms, a lack of arguments in favour of an embolism, and integrity of the carotid bifurcation. Surveillance in an intensive cares unit during the first days is required.

Over the last 20 years, oral vaccination implementing a live attenuated vaccine has been experimented in Europe in order to control classical swine fever (CSF) in Wild Boar (Sus scrofa sp.). This has generally led to an enhanced seroprevalence and a decreased viroprevalence at the scale of the whole vaccinated populations, but no quantitative analysis has demonstrated the protective effect of preventive vaccination or intensive baiting. In the present paper we conducted a retrospective analysis at the scale of the municipality, taking into account the local dynamics and possible covariates of infection to test the effect of preventive vaccination and of the baiting effort. To be efficient, vaccination was expected to increase seroprevalence above the level considered as suitable for preventing disease invasion (40-60%) independently of infection, to protect free areas from disease invasion or contribute to control subsequent disease intensity and duration. We also hypothesized that a better baiting effort would be correlated with an improvement of immunisation and disease control. In uninfected municipalities, seroprevalence increased up to 40% after 1 year, i.e., three vaccination campaigns. We observed a significant protective effect of preventive vaccination, especially within municipalities that had been vaccinated at least 1 year before disease emergence and where virus detection did not last more than one quarter. On the other hand, we did not detect a significant effect of the baiting effort on local seroprevalence or disease dynamics, suggesting that the baiting system could be improved. We discuss these results regarding the improvement of management measures and further research perspective.

Increasing numbers of chemotherapeutic agents are being used to treat patients with cancer and various immunologically mediated and inflammatory disorders. Many of the drugs used have distinctive cutaneous side effects that range from relatively common ones, such as alopecia, stomatitis, and hyperpigmentation, to more unusual ones, such as radiation enhancement and recall phenomena, photosensitivity and hypersensitivity reactions, and phlebitis or chemical cellulitis. In addition, there are some rare complications such as diffuse sclerosis of the hands and feet, Raynaud's phenomenon, sterile folliculitis, and flushing reactions. By being aware of which drug may have caused a particular cutaneous reaction, dermatologists will be able to contribute to the care of patients with complex problems in a meaningful way.

There now exist protocols for specific scanning which make it possible to obtain tranverse sections of a few millimetres in thickness of the orbits and their contents, with minimum sweep and with a very low dose of radiation for the lens. Normal scanner anatomical findings in the orbit may thus be defined for the eyeball (outer coats, vitreous, lens) as well as retrobulbar structures (optic nerve, oculomotor muscles in particular). Intraocular tumours (e.g. retinoblastoma) are visible on enlarged scan films. The value of scannings lies above all in the determination of the site, size and shape of orbital lesions and, where applicable, their relations with neighbouring extra- and intra-cerebral structures. In addition, certain conditions such as exophthalmos during hyperthyroidism have characteristic scan findings. It nevertheless remains difficult to attempt to predict a histological diagnosis on the basis of scan findings, the risk of a false positive being particularly significant. Particular emphasis must be placed upon the contribution of techniques of treatment of the films obtained (enlargement, contrast media, histograms, lateral views) as well as the complementary use of other methods of radiological investigation (in particular arteriography and phlebography) which scanning cannot replace but which it should usually precede since it is atraumatic and non-invase.

In a 15-year retrospective study the spontaneous evolution of serous detachment of the pigment epithelium was investigated functionally, by perimetry, biomicroscopically, and angiofluorographically, in order to identify the manifestations of this syndrome and determine its prognosis. Serous detachments of the retinal pigment epithelium are generally associated with senile macular degeneration, central serous chorioretinopathy, diffuse pigment epitheliopathy, or are idiopathic. The patients studied were divided into two groups: 45 patients under 55 years old (64 eyes), kept under observation for an average of 70 months, and 42 patients over 55 years old kept under observation for an average of 38 months. These two groups were compared with a group of cases of isolated colloidal degeneration. The good prognosis for the first group is emphasized. Stabilization or improvement of visual acuity was normal. In the second group, complications included subretinal neovascularization in 29% and tears in the retinal pigment epithelium in 3.5% of the cases. Visual acuity deteriorated in one-third of the cases. Factors which adversely affected prognosis were an initial detachment larger than the diameter of the optic disk, macular location, and contralateral neovascularization. However, the results with spontaneous evolution were not worse than those in laser-treated cases reported in the literature.

Two anatomo-clinical cases of neuro-ophthalmic sarcoidosis are reported where recognition of systemic involvement was delayed because unusual and quite isolate visual symptoms were present. The first patient had granulomas involving the intracranial optic nerve, presenting as optic nerve glioma. The second case presented as a chiasmal syndrome sharing unusual perimetric evolution. This binasal hemianopsia was probably the consequence of pre-chiasmal circulatory changes. Neuro-ophthalmic sarcoidosis should be included in the differential diagnosis of any involvement of the anterior visual pathways since these localizations are not as rare as was formerly thought. Improved awareness of this condition is a result of better neurodiagnostic studies by computed tomography which was not accurate enough a few years ago.

After penetrating keratoplasties for different congenital or acquired corneal diseases, the presence of characteristic cataracta glaucomatosa lens opacities was noted in 9 cases of fixed, dilated pupil iris atrophy of Urrets-Zavalia. This association--rarely mentioned in the literature--is an opportunity to study again the different physiopathological theories on this syndrome (pupillary block by mydriasis with hypertony, upsetting of iris neurovegetative balance, particular sensitivity of ocular tissues to the surgical aggression in keratoconus), to analyze their arguments, and the preventive measures that proceed from them. The almost complete extinction of this type of complication of the corneal graft surgery, since the routine use of these preventive measures, proves the multifactorial origin of this syndrome.

RESUMO

A standardised method with an original reference line in C.A.T. exploration of the orbital region permits to visualize the entire orbital region with four tomographic scannings when using 6 mm tomographic sections. Some exemples of orbital lesions are demonstrated as optic nerve tumor, dysthyroïd exopthalmos, maxillary sinus tumor with extension into the orbit.
