OBJECTIVES: Computed tomography (CT)-derived fractional flow reserve (FFRCT) decreases continuously from proximal to distal segments of the vessel due to the influence of various factors even in non-obstructive coronary artery disease (NOCAD). It is known that FFRCT is dependent on vessel-length, but the relationship with other vessel morphologies remains to be explained. PURPOSE: To investigate morphological aspects of the vessels that influence FFRCT in NOCAD in the right coronary artery (RCA). METHODS: A total of 443 patients who underwent both FFRCT and invasive coronary angiography, with < 50% RCA stenosis, were evaluated. Enrolled RCA vessels were classified into two groups according to distal FFRCT: FFRCT ≤ 0.80 (n = 60) and FFRCT > 0.80 (n = 383). Vessel morphology (vessel length, lumen diameter, lumen volume, and plaque volume) and left-ventricular mass were assessed. The ratio of lumen volume and vessel length was defined as V/L ratio. RESULTS: Whereas vessel-length was almost the same between FFRCT ≤ 0.80 and > 0.80, lumen volume and V/L ratio were significantly lower in FFRCT ≤ 0.80. Distal FFRCT correlated with plaque-related parameters (low-attenuation plaque, intermediate-attenuation plaque, and calcified plaque) and vessel-related parameters (proximal and distal vessel diameter, vessel length, lumen volume, and V/L ratio). Among all vessel-related parameters, V/L ratio showed the highest correlation with distal FFRCT (r = 0.61, p < 0.0001). Multivariable analysis showed that calcified plaque volume was the strongest predictor of distal FFRCT, followed by V/L ratio (ß-coefficient = 0.48, p = 0.03). V/L ratio was the strongest predictor of a distal FFRCT ≤ 0.80 (cut-off 8.1 mm3/mm, AUC 0.88, sensitivity 90.0%, specificity 76.7%, 95% CI 0.84-0.93, p < 0.0001). CONCLUSIONS: Our study suggests that V/L ratio can be a measure to predict subclinical coronary perfusion disturbance. CLINICAL RELEVANCE STATEMENT: A novel marker of the ratio of lumen volume to vessel length (V/L ratio) is the strongest predictor of a distal CT-derived fractional flow reserve (FFRCT) and may have the potential to improve the diagnostic accuracy of FFRCT. KEY POINTS: • Physiological FFRCT decline depends not only on vessel length but also on the lumen volume in non-obstructive coronary artery disease in the right coronary artery. • FFRCT correlates with plaque-related parameters (low-attenuation plaque, intermediate-attenuation plaque, and calcified plaque) and vessel-related parameters (proximal and distal vessel diameter, vessel length, lumen volume, and V/L ratio). • Of vessel-related parameters, V/L ratio is the strongest predictor of a distal FFRCT and an optimal cut-off value of 8.1 mm3/mm.

BACKGROUND: Early neurorehabilitation can enhance neurocognitive outcomes in very preterm infants (<32 weeks), and conventional magnetic resonance imaging (MRI) is commonly used to assess neonatal brain injury; however, the predictive value for neurodevelopmental delay is limited. Timely predictive quantitative biomarkers are needed to improve early identification and management of infants at risk of neurodevelopmental delay. OBJECTIVE: To evaluate the potential of quantitative synthetic MRI measurements at term-equivalent age as predictive biomarkers of neurodevelopmental impairment and establish practical cutoff values to guide clinical decision-making. MATERIALS AND METHODS: This retrospective study included 93 very preterm infants who underwent synthetic MRI at term-equivalent age between January 2017 and September 2020. Clinical outcomes were assessed using the Bayley-III scale of infant development (mean age 2.1 years). The predictive value for impaired development was analyzed using receiver operating characteristic curves for synthetic MRI-based volumetry and T1 and T2 relaxation measurements. RESULTS: The T1 relaxation time in the posterior limb of the internal capsule was a potent predictor of severe (sensitivity, 92%; specificity, 80%; area under the curve (AUC), 0.91) and mild or severe (AUC, 0.75) developmental impairment. T2 relaxation time in the posterior limb of the internal capsule was a significant predictor of severe impairment (AUC, 0.76), whereas the brain parenchymal volume was a significant predictor of severe (AUC, 0.72) and mild or severe impairment (AUC, 0.71) outperforming the reported qualitative MRI scores (AUC, 0.66). CONCLUSION: The proposed cutoff values for T1 relaxation time in the posterior limb of the internal capsule and for total brain volume measurements, derived from synthetic MRI, show promise as predictors of both mild and severe neurodevelopmental impairment in very preterm infants.

OBJECTIVES: Computed tomography (CT) derived fractional flow reserve (FFRCT) decreases from the proximal to the distal part due to a variety of factors. The energy loss due to the bifurcation angle may potentially contribute to a progressive decline in FFRCT. However, the association of the bifurcation angle with FFRCT is still not entirely understood. This study aimed to investigate the impact of various bifurcation angles on FFRCT decline below the clinically crucial relevance of 0.80 in vessels with no apparent coronary artery disease (CAD). METHODS: A total of 83 patients who underwent both CT angiography including FFRCT and invasive coronary angiography, exhibiting no apparent CAD were evaluated. ΔFFRCT was defined as the change in FFRCT from the proximal to the distal in the left anterior descending artery (LAD) and left circumflex artery (LCX). The bifurcation angle was calculated from three-dimensional volume rendered images. Vessel morphology and plaque characteristics were also assessed. RESULTS: ΔFFRCT significantly correlated with the bifurcation angle (LAD angle, r = 0.35, p = 0.001; LCX angle, r = 0.26, p = 0.02) and vessel length (LAD angle, r = 0.30, p = 0.005; LCX angle, r = 0.49, p < 0.0001). In LAD, vessel length was the strongest predictor for distal FFRCT of ≤ 0.80 (ß-coefficient = 0.55, p = 0.0003), immediately followed by the bifurcation angle (ß-coefficient = 0.24, p = 0.02). The bifurcation angle was a good predictor for a distal FFRCT ≤ 0.80 (LAD angle, cut-off 31.0°, AUC 0.70, sensitivity 74%, specificity 68%; LCX angle, cut-off 52.6°, AUC 0.86, sensitivity 88%, specificity 85%). CONCLUSIONS: In vessels with no apparent CAD, vessel length was the most influential factor on FFRCT, directly followed by the bifurcation angle. KEY POINTS: • Both LAD and LCX bifurcation angles are factors influencing FFR CT. • Bifurcation angle is one of the predictors of a distal FFRCT of ≤ 0.80 and an optimal cut-off value of 31.0° for the LAD and 52.6° for the LCX. • Bifurcation angle should be taken into consideration when interpreting numerical values of FFRCT.

BACKGROUND: Iodine contrast agent (CA) is widely used in cardiac computed tomography (CT). The CA can increase the organ radiation doses due to the photoelectric effect. PURPOSE: To investigate the impact of CA on radiation dose in cardiac CT by comparing the radiation dose between contrast coronary CT angiography (CCTA) and non-contrast calcium scoring CT (CSCT). MATERIAL AND METHODS: Radiation doses were computationally calculated for 30 individual patients who received CSCT and CCTA in the same exam session. The geometry and acquisition parameters were modeled in the simulations based on individual patient CT images and acquisitions. Doses in the presence and absence of CA were obtained in the aorta, left ventricle (LV), right ventricle (RV), and myocardial tissue (MT). The dose values were normalized by size-specific dose estimate (SSDE). The dose enhancement factors (DEFSSDE) were calculated as the ratio of doses in CCTA over doses in CSCT. RESULTS: Compared to the CSCT scans, doses increase in the CCTA scans in the aorta (DEFSSDE = 2.14 ± 0.20), LV (DEFSSDE = 1.78 ± 0.26), and RV (DEFSSDE = 1.31 ± 0.22). A linear relation is observed between the local CA concentrations and the dose increase in the heart; DEFSSDE = 0.07*I(mg/mL) + 0.80 (R2 = 0.8; p < 0.01). The DEFSSDE in the MT (DEFSSDE = 0.96 ± 0.08) showed no noticeable impact of CA on the dose in this tissue. In addition, patient variability in the dose distributions was observed. CONCLUSION: A linear causal relation exists between local CA concentration and increase in radiation dose in cardiac CT. For the same CT exposure, dose to the heart is on average 55% higher in contrast cardiac CT.

BACKGROUND: Unoptimized coronary CT angiography (CTA) exams typically result in a highly variable arterial enhancement (HUa ) across patients. This study aimed at harmonizing arterial enhancement by implementing a patient-, contrast- and kV-tailored injection protocol. METHODS: First, the optimal body size metric to predict HUa was identified by retrospectively analysing images of 76 patients, acquired with 70 ml contrast media (G1). Second, using phantom experiments, correction factors for the effect of kV and contrast concentration on HUa were determined. Third, a model was developed, prescribing the optimal contrast dose to be injected to obtain a diagnostically appropriate arterial target enhancement HUtarget . The model was then validated on 278 prospectively collected patients, in two groups with two different HUtarget : 525 HU (207 patients, G2A) and 425 HU (71 patients, G2B). The HUa histograms were compared among groups and to the target enhancement through their mean and standard deviation (SD) at 100 kVp reference level. Also, signal-to-noise ratio was obtained and compared among the groups. RESULTS: Fat free mass (FFM) showed the highest correlation with HUa (r = 0.69). KVp correction factors ranged from 0.65 at 70 kVp to 1.22 at 140 kVp. The obtained model reduced the group heterogeneity (SD) from 101HU for reference G1 to 75HU (p < 0.001) for G2A and 68HU (p < 0.001) for G2B. The mean HUa of 506HU in G2A was slightly below HUtarget  = 525HU (p = 0.01) whereas in G2B, the mean HUa of 414HU was not significantly different from HUtarget  = 425HU (p = 0.54). The total iodine dose was lowered from 19.5 g-I to 17.6 g-I and 14.2 g-I from G1 to G2A and G2B, on average. CONCLUSION: A contrast injection model, based on patient's fat free mass and accounting for the contrast agent concentration and the planned CT-scan tube voltage, harmonized arterial enhancement among patients towards a predefined target enhancement in coronary CTA scanning, without affecting the bolus timing.

OBJECTIVES: To compare the amount of fluid in synovial sheaths of the ankle before and after running. Our hypothesis was that this amount would increase and that the threshold for what is normally acceptable should be adjusted after physical activity. METHODS: Twenty-one healthy volunteers (n = 42 ankles) ran for 40 min on a treadmill. They underwent 3 T MRI before and immediately after running using a dedicated ankle coil. The images were stored and subsequently measured in a standardized way and independently read by two readers for fluid in the tendon sheaths in the retro and inframalleolar area. Statistics were performed for each tendon (Wilcoxon signed rank test), and also for the pooled data. Intraclass correlation coefficients were calculated. RESULTS: For reader 1, for all tendons the values after running increased without reaching statistical significance. For reader 2 this was not the case for all tendons but for most. When all the data were pooled (n = 800 measurements), the statistical difference before and after running was significant (p < 0.001). CONCLUSION: Data pre and post-running show a trend of increasing synovial fluid, however, not significant for each individual tendon. The pooled data for all tendons, (n = 800) show a statistically significant increase after running (p < 0.001). The clinical implication is that the threshold for normally acceptable fluid should be adjusted if the patient undergoes an MR study after recent physical activity.

PURPOSE: (1) to assess the influence of medial or lateral imaging plane inclination on the measurement of sulcus angle, trochlear depth, and facet asymmetry on transverse cross-sectional images. (2) to assess the effect of measurement level (height) on these respective parameters. MATERIALS AND METHODS: Twenty dry femurs (9 left, 11 right) were imaged with CT. A 3D dataset was obtained from which axial images were reconstructed in the ideal plane without inclination as well as with 8° of medial and lateral inclination. Sulcus angle, trochlear depth, and facet asymmetry were measured on the 3 image sets. In addition, the measurements were performed at 5 mm and 10 mm from the superior margin of the medial trochlear facet. Statistical analysis consisted of the Wilcoxon test and calculation of measurement variation. RESULTS: There were no statistically significant differences between the indicated measurements on the reference set compared to medial or lateral inclination. All measurements were significantly different depending on measurement height. CONCLUSION: Medial or lateral inclination in the transverse imaging plane of 8° does not influence the values of typical parameters used for the assessment of trochlear dysplasia. The measurement height has a significant influence, and a consensus should be found as to which is the optimal measurement height.

BACKGROUND: Dry powder inhaler (DPI) use requires sufficient peak inspiratory flow over the DPI internal resistance (PIFR). OBJECTIVES: We examined whether spirometric peak inspiratory flow (PIFspiro) could serve to predict PIFR in patients with obstructive lung disease. METHOD: Thirty healthy nonsmokers and 140 stable outpatients (70 COPD, 70 asthma) performed spirometry according to the 2019 ERS/ATS spirometry update, yielding PIFspiro. Using a PIFR measurement device with varying orifices, all subjects' PIFR values were recorded for 5 predefined resistance levels, characterized by 5 orifice cross sections (SR). A test group including all healthy subjects, 30 of the asthma, and 30 of the COPD patients was used to establish the relationship between PIFR and both PIFspiro and SR by multiple regression. A validation group including the remaining 40 asthma and 40 COPD patients, served to verify whether their predicted PIFR value corresponded to the measured PIFR for each resistance level. RESULTS: The asthma (FEV1 = 78 ± 17 [SD] %pred) and COPD (FEV1 = 46 ± 17 [SD] %pred) patients under study had varying airway obstruction. In the test group, PIFR could be predicted by ln[PIFspiro] (p < 0.0001), SR (p < 0.0001), and SR2 (p = 0.006), with an adjusted R2 = 0.71. In the validation group, estimated PIFR did not significantly differ from measured PIFR (p > 0.05 for the 5 resistance levels). CONCLUSIONS: We propose a simple method to predict PIFR for a range of common DPI resistances, based on the device characteristics and on the patient's characteristics reflected in PIFspiro. As such, routine spirometry can serve to estimate a patient's specific PIFR without the need for additional testing.

BACKGROUND: Contrast leakage after arthrography is common. We sought to investigate if immobilization could prevent it. PURPOSE: The purpose of this study was to determine the effects on contrast extravasation and image quality produced by strict immobilization of the shoulder between arthrography puncture and subsequent MR imaging. MATERIAL AND METHODS: Fifty patients underwent shoulder MR arthrography using a standard shoulder puncture in the anteroinferior quadrant. Ten milliliters of contrast mixture of saline, iodinated contrast, and gadolinium contrast was injected by a senior musculoskeletal (MSK) radiologist using a 21G needle. Half of the patients were immediately immobilized using a shoulder sling, and the other half were allowed to move their shoulder and arm freely during the time before MR imaging. MR arthrography was performed with a 3 T system using standard T1 and PD weighted sequences. The MR images were reviewed independently by 2 MSK radiologists and graded for extravasation using a five-point scale (1: none, 2: less than 2 cm, 3: 2-5 cm, 4: 5-10 cm, 5: more than 10 cm) and for image quality using a 5 point scale (1: poor, 5: good). The Pearson correlation was calculated to assess the correlation between leakage and image quality. RESULTS: There was no significant difference in amount of leakage between both groups, and global image quality was found equal in both groups. A negative correlation was found between leakage and quality assessment. CONCLUSION: This study shows that it cannot be avoided by strict shoulder immobilization and that it negatively affects image quality and interpretation.

The lens of the eye can be damaged by ionising radiation, so individuals whose eyes are exposed to radiation during their work may need to protect their eyes from exposure. Lead glasses are widely available, but there are questions about their efficiency in providing eye protection. In this study, Monte Carlo simulations are used to assess the efficiency of lead glasses in protecting the sensitive volume of the eye lens. Two designs currently available for interventional cardiologists are a wraparound (WA) style and ones with flat frontal lenses with side shielding. These designs were considered together with four modifications that would impact upon their efficiency: changing the lead equivalent thickness, adding lead to the frames, elongating the frontal lenses, and adding a closing shield to the bottom rim. For the eye closest to the source, standard models of lead glasses only decrease the radiation reaching the most sensitive region of the eye lens by 22% or less. Varying the lead thickness between 0.4 mm and 0.75 mm had little influence on the protection provided in the simulation of clinical use, neither did adding lead to the frames. Improved shielding was obtained by elongating the frontal lens, which could reduce radiation reaching the eye lens by up to 76%. Glasses with lenses that had a rim at the base, extending towards the face of the user, also provided better shielding than current models, decreasing the dose by up to 80%. In conclusion, elongating the frontal lens of lead glasses, especially of the WA design, could provide a three-fold increase in shielding efficiency and this is still valid for lenses with 0.4 mm lead equivalence.

OBJECTIVES: Routine dosimetry calculations do not account for the presence of iodine in organs and tissues during CT acquisition. This study aims to investigate the impact of contrast agent (CA) on radiation dose. METHODS: First, relation between absorbed radiation dose and iodine concentrations was investigated using a cylindrical water phantom with iodine-saline dilution insertions. Subsequently, a retrospective study on abdominal dual-energy CT (DECT) patient data was performed to assess the increase of the local absorbed radiation dose compared to a non-contrast scan. Absorbed doses were estimated with Monte Carlo simulations using the individual CT voxel data of phantom and patients. Further, organ segmentations were performed to obtain the dose in liver, liver parenchyma, left kidney, right kidney, aorta, and spleen. RESULTS: In the phantom study, a linear relation was observed between the radiation dose normalized by computed tomography dose index (CTDI) and CA concentrations Iconc (mg/ml) for three tube voltages; [Formula: see text] = 0.14 × Iconc + 1.02, [Formula: see text] = 0.16 × Iconc + 1.21, [Formula: see text] = 0.16 × Iconc + 1.24, and for DECT acquisition; [Formula: see text] = 0.15 × Iconc + 1.09. Similarly, a linear relation was observed between the dose increase and the organ iodine contents (R2 = 0.86 and pvalue < 0.01) in the patient study. The relative doses increased in the liver (21 ± 5%), liver parenchyma (20 ± 5%), right kidney (37 ± 7%), left kidney (39 ± 7%), aorta (34 ± 6%) and spleen (26 ± 4%). In addition, the local dose distributions changed based on patient's anatomy and physiology. CONCLUSIONS: Compared to a non-contrast scan, the organ doses increase by 30% in contrast-enhanced abdominal CT. This study suggests considering CA in dosimetry calculations, epidemiological studies, and organ dose estimations while developing new CT protocols. KEY POINTS: • The presence of contrast media increases radiation absorption in CT, and this increase is related to the iodine content in the organs. • The increased radiation absorption due to contrast media can lead to an average 30% increase in absorbed organ dose. • Iodine should be considered in CT radiation safety studies.

BACKGROUND: Four-dimensional computed tomography (4DCT) is a commonly performed examination in the management of primary hyperparathyroidism, combining three-dimensional imaging with enhancement over time as the fourth dimension. We propose a novel technique consisting of 16 different contrast phases instead of three or four different phases. The main aim of this study was to ascertain whether this protocol allows the detection of parathyroid adenomas within dose limits. Our secondary aim was to examine the enhancement of parathyroid lesions over time. METHODS: For this prospective study, we included 15 patients with primary hyperparathyroidism and a positive ultrasound prior to surgery. We performed 4DCT with 16 different phases: an unenhanced phase followed by 11 consecutive arterial phases and 4 venous phases. Continuous axial scanning centered on the thyroid was performed over a fixed 8 cm or 16 cm coverage volume after the start of contrast administration. RESULTS: In all patients, an enlarged parathyroid lesion was demonstrated, and the mean lesion size was 13.6 mm. The mean peak arterial enhancement for parathyroid lesions was 384 Hounsfield units (HU) compared to 333 HU for the normal thyroid. No significant difference could be found. The time to peak (TTP) was significantly earlier for parathyroid adenomas than for normal thyroid tissue: 30.8 s versus 32.3 s (p value 0.008). The mean slope of increase (MSI) of the enhancement curve was significantly steeper than that of normal thyroid tissue: 29.8% versus 22.2% (p value 0.012). The mean dose length product was 890.7 mGy cm with a calculated effective dose of 6.7 mSv. CONCLUSION: Our 4DCT protocol may allow better visualization of the pattern of enhancement of parathyroid lesions, as enhancement over time curves can be drawn. In this way, wash-in and wash-out of contrast in suspected lesions can be readily demonstrated. Motion artifacts are less problematic as multiple phases are available. Exposure to our proposed 4DCT technique is comparable to that for classic helical 4DCT. Careful selection of parameters (lowering kV and SNR) can help to further reduce the dose.

PURPOSE: The aim of this study was to confirm our clinical observation that a pseudo-tear appearance of the Achilles tendon is commonly seen on MRI and is of no clinical relevance. MATERIALS AND METHODS: Forty-one ankles were imaged on a 3 T MR system, and PD weighted images with fat saturation were obtained in three orthogonal planes (TR, 2969 ms; TE, 30 ms; NA, 2; slice thickness, 2.5 mm). Volunteer exclusion criteria were symptoms of Achilles tendon pathology (such as acute or chronic posterior heel pain), history of trauma or surgery of the Achilles tendon. Internal signal of the Achilles tendon on axial and sagittal images was assessed independently by two observers. Internal signal of the Achilles tendon was classified from homogenously dark to different degree of hyperintense signal, where 0 means no internal hyperintensity, 1-minimal hyperintensity, 2-moderate and 3-marked. Descriptive statistics were calculated. Correlation between the two readers was also assessed. Two fresh cadavers were used in this study, one specimen being sliced in the sagittal plane and one specimen being dissected by an experienced anatomist. RESULTS: Twenty one volunteers (8 men, 13 women), mean age of 24.7 years (19-43 years) were included in the study. On sagittal images both raters appreciated any degree of hyperintense signal in 59% of tendons. On axial images any degree of hyperintensity was seen in almost half of the cases (46 vs. 49%). Minimal hyperintensities were seen most commonly. Cohen's kappa coefficient for sagittal images was 0.964 (almost perfect agreement); for axial images 0.764 (substantial agreement). The anatomical studies demonstrated that the Achilles tendon is made up of different components that are partially separated and twist around each other explaining the pseudo-tear appearance. CONCLUSION: The Achilles tendon is frequently not homogenously dark in normal volunteers as would be expected. Hyperintense signal is common in the long and short axis and related to the underlying anatomical features.

OBJECTIVES: To evaluate at which sensitivity digital breast tomosynthesis (DBT) would become cost-effective compared to digital mammography (DM) in a population breast cancer screening program, given a constant estimate of specificity. METHODS: In a microsimulation model, the cost-effectiveness of biennial screening for women aged 50-75 was simulated for three scenarios: DBT for women with dense breasts and DM for women with fatty breasts (scenario 1), DBT for the whole population (scenario 2) or maintaining DM screening (reference). For DM, sensitivity was varied depending on breast density from 65 to 87%, and for DBT from 65 to 100%. The specificity was set at 96.5% for both DM and DBT. Direct medical costs were considered, including screening, biopsy and treatment costs. Scenarios were considered to be cost-effective if the incremental cost-effectiveness ratio (ICER) was below €20,000 per life year gain (LYG). RESULTS: For both scenarios, the ICER was more favourable at increasing DBT sensitivity. Compared with DM screening, 0.8-10.2% more LYGs were found when DBT sensitivity was at least 75% for scenario 1, and 4.7-18.7% when DBT sensitivity was at least 80% for scenario 2. At €96 per DBT, scenario 1 was cost-effective at a DBT sensitivity of at least 90%, and at least 95% for scenario 2. At €80 per DBT, these values decreased to 80% and 90%, respectively. CONCLUSION: DBT is more likely to be a cost-effective alternative to mammography in women with dense breasts. Whether DBT could be cost-effective in a general population highly depends on DBT costs. KEY POINTS: • DBT could be a cost-effective screening modality for women with dense breasts when its sensitivity is at least 90% at a maximum cost per screen of €96. • DBT has the potential to be cost-effective for screening all women when sensitivity is at least 90% at a maximum cost per screen of €80. • Whether DBT could be used as an alternative to mammography for screening all women is highly dependent on the cost of DBT per screen.

The reduction in the occupational dose limit of the eye lens has created the need for optimising eye protection and dose assessment, in particular for interventional clinicians. Lead glasses are one of the protection tools for shielding the eyes, but assessing the eye lens dose when these are in place remains challenging. In this study, we evaluated the impact of the position of H p (3) dosemeters on the estimated eye lens dose when lead glasses are used in interventional settings. Using the Monte Carlo method (MCNPX), an interventional cardiology setup was simulated for two models of lead glasses, five beam projections and two patient access routes. H p (3) dosemeters were placed at several positions on the operator and the obtained dose was compared to the dose to the sensitive part of the eye lens (H lens). Furthermore, to reproduce an experimental setup, a reference dosemeter, H p (3)ref, was placed on the surface of the eye. The dose measured by H p (3)ref was, on average, only 60% of H lens. Dosemeters placed on the glasses, under their shielding, underestimated H lens for all parameters considered, by from 10% up to 90%. Conversely, dosemeters placed on the head or on the glasses, over their shielding, overestimated H lens, on average, up to 60%. The presence or lack of side shielding in lead glasses affected mostly dosemeters placed on the forehead, at the left side. Results suggest that both use of a correction factor of 0.5 to account for the presence of lead glasses in doses measured outside their shielding and placing an eye lens dosemeter immediately beneath the lenses of lead glasses may lead to the underestimation of the eye lens dose. Most suitable positions for eye lens dose assessment were on the skin, unshielded by the glasses or close to the eye, with no correction to the dose measured.

OBJECTIVE. The purpose of this study is to investigate the contributing effect of contrast media (CM) iodine dose on radiation-induced DNA damage in blood lymphocytes during a cardiac CT scan. MATERIALS AND METHODS. The minipigs were exposed 12 times in total to a fixed cardiac CT scan protocol. An unenhanced and two CM injection protocols were considered, the latter with 50% saline diluted (160 mg I/mL) and standard iodixanol. Blood samples were collected before and after CT, and radiation-induced DNA double-strand breaks were assessed using γ-H2AX (H2A histone family member X) immunofluorescent staining of the blood lymphocytes. Significant differences in foci numbers were investigated with an independent sample t test. In addition, a numeric dosimetry model was applied that simulates the cardiac CT scan, with the heart represented by a blood volume containing a mixture of six iodine concentrations (0, 10, 20, 30, 40, and 50 mg I/mL). RESULTS. Compared with the unenhanced (0 mg I/mL) protocol, the number of γ-H2AX foci per cell increased significantly (p < 0.038), by 56.1% for the reduced iodine dose (160 mg I/mL) and by 141.1% for the standard iodine dose (320 mg I/mL) protocols. These in vivo results are confirmed by the dosimetry simulation model, in which 78.8% and 133.7% increases in locally absorbed blood dose in the left ventricle were observed for the reduced and standard iodine dose protocols, respectively. CONCLUSION. Administration of CM during a cardiac CT examination significantly increases radiation-induced DNA damage in blood lymphocytes. Moreover, a lower CM iodine dose results in a reduced level of DNA damage, at constant radiation exposure.

INTRODUCTION: Bone mineral density (BMD) analysis by Dual-Energy x-ray Absorptiometry (DXA) can have some false negatives due to overlapping structures in the projections. Spectral Detector CT (SDCT) can overcome these limitations by providing volumetric information. We investigated its performance for BMD assessment and compared it to DXA and phantomless volumetric bone mineral density (PLvBMD), the latter known to systematically underestimate BMD. DXA is the current standard for BMD assessment, while PLvBMD is an established alternative for opportunistic BMD analysis using CT. Similarly to PLvBMD, spectral data could allow BMD screening opportunistically, without additional phantom calibration. METHODOLOGY: Ten concentrations of dipotassium phosphate (K2HPO4) ranging from 0 to 600 mg/ml, in an acrylic phantom were scanned using SDCT in four different, clinically-relevant scan conditions. Images were processed to estimate the K2HPO4 concentrations. A model representing a human lumbar spine (European Spine Phantom) was scanned and used for calibration via linear regression analysis. After calibration, our method was retrospectively applied to abdominal SDCT scans of 20 patients for BMD assessment, who also had PLvBMD and DXA. Performance of PLvBMD, DXA and our SDCT method were compared by sensitivity, specificity, negative predictive value and positive predictive value for decreased BMD. RESULTS: There was excellent correlation (R2 >0.99, p < 0.01) between true and measured K2HPO4 concentrations for all scan conditions. Overall mean measurement error ranged from -11.5 ± 4.7 mg/ml (-2.8 ± 6.0%) to -12.3 ± 6.3 mg/ml (-4.8 ± 3.0%) depending on scan conditions. Using DXA as a reference standard, sensitivity/specificity for detecting decreased BMD in the scanned patients were 100%/73% using SDCT, 100%/40% using PLvBMD provided T-scores, and 90-100%/40-53% using PLvBMD hydroxyapatite density classifications, respectively. CONCLUSIONS: Our results show excellent sensitivity and high specificity of SDCT for detecting decreased BMD, demonstrating clinical feasibility. Further validation in prospective clinical trials will be required.

PURPOSE: This study aimed at studying the MR imaging appearance of the tibiotalar ligament in asymptomatic volunteers. MATERIALS AND METHODS: Fourty-two ankles were imaged on a 3T MR system using proton density weighted images with fat saturation (TR, 2969 ms; TE 30 ms; NA, 2; slice thickness, 2.5 mm). Subjects with acute ankle conditions or history of previous trauma were not included in the study group. Images were obtained in the three orthogonal planes. The posterior tibiotalar ligament was assessed on coronal imaging, by consensus of two radiologists. The signal intensity was recorded as isointense, hypointense, or hyperintense relative to muscle. The morphology of the ligament was classified as homogenous or striated. Descriptive statistics were obtained. RESULTS: There were 8 men and 14 women with a mean age of 24.7 years (range 19-43 years). The ligaments were classified as hyperintense in 30/42 (70%) of ankles and isointense in 9/42 (21%) of ankles. A striated appearance was seen in 34/42 (80%) of ankles. CONCLUSION: The posterior deep deltoid ligament is commonly hyperintense. It is usually striated although it can be homogeneously hyperintense. This appearance simulates a tear.

PURPOSE: To assess normal distribution of fluid in the tendon sheaths of the ankle. METHODS: 21 healthy volunteers were evaluated. Bilateral ankle MRI was performed on a 3T unit with PD-weighted images with fat saturation. The images were interpreted by two radiologists separately, and the short-axis dimension of fluid amount was measured. Bland-Altman plots and correlation plots were used to assess consistency between readers. RESULTS: There were 13 men and 8 women. The mean age was 24.7 years. Fluid in the retromalleolar part of the peroneus longus was seen in three ankles of three volunteers and in the inframalleolar part in three ankles of three volunteers. Fluid in the retromalleolar part of the peroneus brevis was seen in four ankles of three volunteers and in the inframalleolar part in three ankles of two volunteers. Fluid in the retromalleolar part of the tibialis posterior was seen in 37 ankles of 20 volunteers and in the inframalleolar part in 38 ankles of 21 volunteers Fluid in the retromalleolar part of the flexor digitorum was seen in 14 ankles of eight volunteers and in the inframalleolar part in 11 ankles of eight volunteers Fluid in the retromalleolar part of the flexor hallucis longus was seen in 23 ankles of 16 volunteers and in the inframalleolar part in 17 ankles of 11 volunteers. CONCLUSION: Fluid is common in the retro- and inframalleolar parts of the medial tendons. Fluid is virtually absent in the peroneal tendons and anterior tendon sheaths in normal volunteers.

PURPOSE: To optimize our standard full-spine radiography with respect to diagnostic quality and dose. METHODS: A phantom study was performed to establish an optimal posterior-anterior view (PA) full spine protocol having the lowest dose with non-inferior quality compared to standard. We then applied this protocol in 40 pediatric patients (group B). The radiographs were scored on six criteria by a pediatric radiologist and orthopedist and compared to the scores of 40 PA full spine radiographs performed in 2013 with standard protocol (group A). Radiation dose was assessed by dose area product (DAP) and effective dose (E). Statistical analysis included independent samples t test, Mann-Whitney U test and intra-class correlation coefficient (ICC). RESULTS: An optimized protocol was defined (0.2 mm Cu filter, 0.87 relative exposure, with grid). Mean age was 13.3 ± 1.6 years for group A and 13.4 ± 1.7 years for group B. For group B, the mean DAP was 47.0 µGy m2 with an E of 0.13 mSv. For group A, the mean DAP was 85.3 µGy m2 with an E of 0.24 mSv. This represents a dose reduction of 45%. Mean image quality scores for group A (27.9 ± 2.4) and group B (28.1 ± 2.3) were similar (p = 0.612). Interobserver agreement was observed to be excellent (ICC 0.92). CONCLUSION: This study demonstrates that a low-dose full-spine radiograph can be performed in patients with idiopathic scoliosis without loss of image quality.
