RESUMO

This article presents the results of a comparative prospective study of 276 subjects at the age between 40 and 65 years presenting with risk factors (RF) of atherosclerosis and its preclinical stages (APC). All of them were reckoned to be at a moderate risk of developing APC based on the results of evaluation with the use of the SCORE scale which implied the necessity of their differential non-pharmacological treatment during a two-year period with the application of a variety of preventive therapeutic strategies. The patients were allocated to three different (initially comparable) groups. The main group (group 1) was comprised of 98 subjects who received the optimized combined treatment including climatotherapy, dietary therapy, psychotherapy, hydrotherapy, physiotherapy, and hypoxytherapy; in addition, they were involved in the program of therapeutic physical exercises and in the educational program. The group of comparison (group 2) consisted of 88 subjects who were advised to withstand pernicious habits, to keep to the prescribed diet, and intensify the physical activity. The subjects included in the control group 3 were given no preventive treatment whatsoever. The optimized system for the early diagnostics and identification of the risk factors and/or ACD was employed for examining the members of group 1. By the end of the study, risk factors of atherosclerosis and APC were documented in 86.2% and 76.6% of the patients of group 1, respectively. The number of disability days averaged 7.2 per person versus 16.3 and 22.6 in comparison and control groups, respectively. There were no newly diagnosed cases of cardiovascular diseases (CVD) over the study period among the patients of group 1, in contrast to 11.4% and 25.6% such cases in groups 2 and 3, respectively. The results of this study give evidence that the optimized strategy of non-pharmacological correction of the risk factors and the preclinical stages of atherosclerosis at the level of the health resort-based primary care has advantages over the standard therapy because it makes it possible to reduce the frequency of development of new cases of cardiovascular diseases and to decrease the disability rate. It is concluded that the proposed approach enhances the effectiveness of primary prophylaxis of cardiovascular diseases associated with atherosclerosis and relieves the burden of this pathology. It can therefore be recommended for the more extensive application at the level of the health resort-based primary care settings.

RESUMO

Administration of preparations of potassium and magnesium to patients with arterial hypertension is an accepted therapeutic practice. In this paper we examine relationships between the content of potassium and magnesium in the body and levels of arterial pressure. Results of relevant fundamental and clinical studies are also presented. In hypertension and hypotension modulating effects of potassium and magnesium develop with the use of their small doses compensating deficit of these elements.

The aim of the study was to examine anthropometry, physical fitness, metabolism at the level of the microcirculation and adaptive processes of the organism as a whole under the inclusion of enzyme-hydrolyzed whey in the diet of 27 weightlifters 17-23 (21.7±2.7) years old, with the level of sports skills of 1 category of up to masters of sports. The body composition was determined by bioimpedance method, functioning of microcirculation system and oxygen transport were determined using the multifunction laser diagnostic complex. For analysis of heart rate variability we used complex "Varicard 2.51", physical fitness was assessed by generally accepted pedagogical tests. The products inclusion in the diet in the increasing dose of 0.5 to 1.5 g/kg of body weight within 21 days increases physical fitness. The growth of physical efficiency provided change of body composition with slight increase in muscle mass by 2% and decrease in fat component by 3.6% (p <0.05). Increasing muscle mass is the functional stimulus for the revitalization of the microcirculation system for the exchange of nutrients and energy. Under the consumption of serum the intensity of microcirculation increased by 131%, elevating dominance of active mechanisms of modulation of the flow over passive. As a result of intensive diffusion of oxygen from the blood into tissues, the saturation rate of mixed blood reduced by 7.7% and consumption of oxygen by cells of work tissues increased by 23.6%. Assimilation of nutrients and energy, adequate to physical exercises, improved functional capacity of the organism in general and extended its adaptive capacity, as evidenced by the increase of the balance of vegetative regulation of cardiac rhythm.

Cardiovascular diseases remain the leading cause of death in most countries, including Russia. Non-pharmacological-modalities intended for the primary prevention of cardiovascular disease including hypertension and atherosclerosis currently acquire special significance. The objective of the present*study was to develop a system of methods for early diagnostics, prevention and treatment of the preclinical stages of atherosclerosis and hypertension Another objective was to estimate the effectiveness of these methods. A total of 310 patients at risk-of arterial hypertension and atherosclerosis or with pre-clinical stages of these conditions were examined during a two year observation period. The system of early diagnosis and non-pharmacological primary prevention of cardiovascular disease was developed and evaluated using climatotherapeutic and physiotherapeutic methods in combination with and personalized dietary therapy, hypoxic-hypercapnic factors and educational programs. The study has demonstrated that the proposed system allowed to significantly (p < 0.05) reduce cardiovascular morbidity. The patients of the main group experienced the reduction of abdominal obesity and blood pressure along with the improvement of the blood lipid profile and other indicators. These parameters remained unaltered in the patients comprising the control group. In the main group, the incidence of coronary heart disease and hypertension was 62.5% and 81.25% lower respectively than in the control group.

A total of 162 patients with risk factors of cardiovascular diseases were examined in our health centre. 102 of them were enrolled in the optimized program of non-medicamental prophylaxis applied under health resort and outpatient conditions during four years. The program included rational nutrition, psychotherapy, adequate locomotor activity, hydro-, gaso-, and thermotherapeutic procedures, and educational curricula. The patients of the control group were given the traditional health resort treatment. The comparison of the clinical and functional conditions of the patients in both groups revealed a significant advantage of the proposed therapeutic strategy.

Th present study was designed to examine 409 practically healthy men at the age of 50 to 79 years differing in terms of daily locomotor activities. They were divided into 4 groups each comprising subjects whose age differed within 10 years. Group 1 included former high-class athletes continuing physical exercises. Group 2 consisted of former high-class athletes practicing sedentary lifestyle. Group 3 contained subjects regularly doing health-improving physical exercises and group 4 those who had never been engaged in sports activities. The main parameters measured in all the subjects included the heart rate, arterial pressure, stroke volume, pulse pressure, minute blood volume, cardiac index, total peripheral vascular resistance, and mean dynamic arterial pressure. It was shown that both the routine regime of motor activity and its history in the preceding period have significant influence on hemodynamic characteristics in late ontogenesis. The high level of routine motor activity in the former high-class athletes enabled them to maintain for a long time good functional state of the cadiovascular system compared with the former athletes practicing sedentary lifestyle and the subjects who had never been engaged in sports activities. The former athletes practicing sedentary lifestyle developed the hypokinetic type of hemodynamics by the age of 60-69 years.

Different variants of sports massage were tested on 8 fencers (masters and candidate masters of sports) aged between 18 and 22 years. Variant 1 was stimulating massage with the predominant impact on the selected groups of muscles while variant II included relaxation massage with the selective impact on the connective-tissue structures. Blood glucose, cortisol, thyroxin, triiodo-thyronine levels and concentrations of lactic, uric, and non-esterified fatty acids were measured before and after the massage. The results of the study indicate that different technological regimens of sports massage may cause multidirectional changes in the metabolism of selected biologically important compounds. The stimulating massage improves the functional state of the neuromuscular apparatus and activates energy production (in the first place, through enhanced utilization of the substances required for maintaining this process) which suggests formation of the mechanisms necessary for "emergency" adaptation. The relaxation massage suppresses the functional activity of the neuromuscular apparatus and energy consumption which results in the transition of the organism into the state dominated by accumulation of selected substances characteristic of "long-term" adaptation.

The authors report the results of the study on the hormonal and psychovegetative status, functional activity of the vegetative nervous system, the level of activity of the protective and adaptive hemostatic mechanisms, and the clinical features of concomitant extragenital pathology in 90 adolescent girls suffering uterine bleeding. All the patients had gone through in-patient and out-patient phases of therapy, sixty of them were selected for the spa and resort rehabilitative treatment. The use of rehabilitative technologies on an individual basis at a spa and resort facility made it possible to ensure rather high efficacy of the combined therapeutic and preventive treatment of reproductive system pathology and concomitant psycho-vegetative disorders. It helped to decrease the frequency of relapses of uterine bleeding and exacerbation of the accompanying extragenital diseases.

The present study involved 720 patients with metabolic syndrome, diabetes mellitus, coronary heart disease, hypertensive disease, and various combinations of these conditions examined in the course of sanatorium and spa treatment. The patients received either traditional or modified therapy. The latter modality included climato-balneotherapy, hypoxytherapy, bioresonance therapy, and hypercapnotherapy supplemented by the intake of Arfazetin herbal tea, Magnerot tablets, and Chvizhepse mineral water. Results of the study indicate that the proposed combined treatment had more pronounced curative effect compared with traditional therapy in patients of all groups included in this study.

The present study included 229 healthy men having different habitual patterns of daily locomotor activity. They were allocated to 4 groups encompassing subjects of different age. Group 1 consisted of medium-level athletes and subjects regularly engaged in physical exercises, group 2 included former high-level athletes continuing active physical training, group 3 was comprised of former high-level athletes living a sedentary life style, and group 4 included subjects who were never engaged in physical exercises. All the participants of the study were examined using an AMSAT-COVERT automated diagnostic system designed for rapid topical evaluation of the human functional state. The results of the study indicate that the regime of daily locomotor activity and previous experience with physical training have marked effect on the age-specific dynamics of the main functional systems of the organism. Continuation of physical activity in former athletes permits them to preserve a higher functional status of the main functional systems of the organism compared with former high-level athletes living a sedentary life style and subject who were never engaged in physical exercises.

A total of 55 patients with angina of effort (functional classes I-II) were treated by magneto-peloidotherapy and hydrogen sulfide baths. Effectiveness of he treatment was evaluated based on the lipid profile (total cholesterol, triglycerides, high and low density lipoproteides), atherogenicity index, lipid peroxidation, reactivity of the antioxidative defense system, and immune characteristics. Results of the study indicate that combination of magneto-peloidotherapy and hydrogen sulfide baths has hypolipidemic effect and reduces lipid peroxidation in the absence of activation of the antioxidative defense system and correction of the disbalanced immune system. Taken together, these effects decrease severity of the systemic inflammatory reaction and facilitate remission of the atherosclerotic process.

Prolonged series of specially selected contrast fragments of classic music has a positive effects on heart rate variability and circadian dynamics of heart rhythm. The benefit depends on the status of heart rate control mechanisms before the music influence.
