INTRODUCTION: Colorectal cancer is the fourth leading cause of cancer-related death in both men and women in our population. In this regard, rectal cancer accounts for more than half of colorectal cancer deaths, and its incidence is expected to increase in the coming years. There have been significant changes in neoadjuvant therapy regimens, with promising results, as demonstrated by the recent RAPIDO and PRODIGE23 studies. Around 40% of patients diagnosed with locally advanced rectal cancer show some degree of response to neoadjuvant treatment, with complete tumor regression observed in up to one in five patients. MATERIALS AND METHODS: Retrospective observational study. A total of 181 patients with locally advanced rectal cancer treated with neoadjuvant chemoradiotherapy followed by surgery were analyzed. Clinical and pathological data were collected from the patients, including assessment of tumor regression through histopathological studies after surgery. The Mandard tumor regression grading system was used to categorize tumor response into different grades. RESULTS: The results showed a significant association between the degree of tumor regression and several important clinical outcomes. Specifically, patients with higher tumor regression had significantly better disease-free survival than those with less regression (p = 0.004). In addition, tumor regression was also correlated with the incidence of local recurrence (p = 0.018) and distant metastasis (p = 0.032). These associations suggest that tumor responsiveness to neoadjuvant therapy may influence the long-term progression of the disease. Regarding tumor deposits and the presence of lymphadenopathy, these factors were also found to be significantly associated with clinical outcomes. Patients with tumor deposits had a higher incidence of local recurrence (p = 0.025) and distant metastases (p = 0.041), while the presence of lymphadenopathy increased the risk of local recurrence (p = 0.013). These findings highlight the importance of evaluating not only tumor regression but also other pathological markers to predict prognosis and guide clinical management. CONCLUSIONS: The degree of tumor regression was not an independent predictor of survival compared to other variables such as nodal stage and presence of tumor deposits. This indicates that while tumor regression is an important factor, other elements also play a crucial role in determining the prognosis of patients with locally advanced rectal cancer. This study provides additional evidence for the importance of tumor regression, tumor deposits, and lymphadenopathy as predictors of clinical outcomes in patients with rectal cancer treated with neoadjuvant chemoradiotherapy.

Spontaneous cervical hematoma usually occurs as a consequence of extracapsular bleeding from a parathyroid gland, generally due to the presence of an adenoma (giant adenoma), glandular hyperplasia, cystic component, or, less frequently, due to the existence of a carcinoma. The hematoma can be confined to the cervical compartment or extend to the mediastinum, potentially causing airway compression. Despite this, the recommended management in hemodynamically stable patients consists of surveillance and hospital monitoring with delayed surgery after a few weeks. On the other hand, in those patients with airway compromise and instability, emergency surgery, consisting of cervicotomy and drainage, is mandatory. The present study describes the case of a 78-year-old patient with a medical history of high blood pressure, non-insulin-dependent diabetes mellitus, dyslipidemia, moderate aortic stenosis, chronic kidney disease and sarcoidosis under pharmacological treatment who attended the emergency department due to symptoms of neck pain, an increase in soft tissue, and dyspnea on moderate exertion with an evolution leading to respiratory failure. This was secondary to a diagnosis of spontaneous cervical hematoma that required urgent surgical intervention. The results of histopathological analysis revealed that a giant parathyroid adenoma was responsible for the bleeding. The patient had a complicated post-operative period with a prolonged admission to the intensive care unit.

Colorectal cancer (CRC) ranks as the third leading cause of cancer-related mortality in developed countries. While its incidence in early stages has increased due to screening programs, a significant number of patients experience the development of metastases either at the time of diagnosis or during follow-ups. Unlike certain other types of cancer, such as breast, prostate, or lung cancer, where bone tissue is a common site for secondary dissemination, CRC primarily spreads to the lymph nodes, liver and lungs. The occurrence of bone metastases from CRC is rare and usually coincides with tumor involvement in other locations. Risk factors for bone metastases include the location of the primary tumor, the age of the patients, KRAS mutations and the degree of tumor differentiation. Unlike metastases to the liver and lungs, bone metastases tend to be symptomatic, affecting the patient's quality of life and resulting in a poorer prognosis with shorter survival rates. The approach to patient management needs to be personalized. The present study describes the of a patient who underwent surgery for stage IV rectal adenocarcinoma and later developed a metastasis in the costal wall 79 months post-intervention, with no evidence of recurrence at other sites.

Thyroid abscess is a rare entity, commonly experienced by immunocompromised patients, or those who have anatomical abnormalities or a pre-existing thyroid disease. An early diagnosis continued by treatment with antibiotics and drainage of the abscess is the recommended therapeutic strategy for such cases. The present study describes a clinical case of this rare event, and also provides a brief literature review. The present study describes the case of a 48-year-old healthy male with no medical antecedents, apart from acute prostatitis treated with antibiotics for 6 days prior, who visited the Emergency Department of the authors' hospital with neck pain and progressive swelling of the mass. Diagnostic imaging confirmed the authors' suspicion of an abscess and revealed the lesion displacing the airway to the contralateral side. This restricted the mobility of the neck of the patient. As an emergency measure, the patient was then taken to the operating room for a neck examination. A hemithyroidectomy was finally performed. Following a prolonged hospital duration, he was discharged from the hospital and his recovery was uneventful without any voice alterations, hypocalcemia or recurrence.

INTRODUCCIÓN: El hematoma espontáneo del músculo recto del abdomen es una causa infrecuente de dolor abdominal que sucede con mayor frecuencia en pacientes anticoagulados. El objetivo de nuestro trabajo fue analizar la forma de presentación, el diagnóstico y los resultados del tratamiento en este grupo de pacientes. MÉTODOS: Análisis retrospectivo con una base de datos prospectiva de todos los casos de hematoma espontáneo del músculo recto del abdomen tratados en nuestro centro entre marzo de 2003 y diciembre de 2014. RESULTADOS: Se incluyó a 34 pacientes (25 mujeres) con una edad media de 80 años. En todos los casos el hematoma fue unilateral e infraumbilical. Veintiocho pacientes recibían tratamiento anticoagulante como parte de su tratamiento habitual (26 de ellos acenocumarol y 2 pacientes heparinas de bajo peso molecular a dosis terapéuticas). Seis pacientes recibían heparina de bajo peso molecular a dosis profilácticas. El diagnóstico se realizó mediante ecografía en 7 pacientes, tomografía axial computarizada con contraste intravenoso en fase arterial (angio-TC) en 27 y, con ambos métodos, en 6. El tratamiento consistió en la suspensión de la anticoagulación, corrección de la hemostasia y reposición hemática. En 10 pacientes se evidenció hemorragia activa en angio-TC y en 8 se realizó embolización selectiva. La evolución fue favorable en 32 pacientes. Dos pacientes fueron intervenidos debido a hemorragia grave persistente y fallecieron. CONCLUSIONES: El hematoma espontáneo del músculo recto del abdomen es más frecuente en mujeres de edad avanzada y en tratamiento con anticoagulantes orales. La mayoría evolucionan favorablemente con tratamiento no operatorio. La angio-TC es útil para indicar la embolización arterial selectiva en caso de hemorragia activa


INTRODUCTION: Spontaneous haematoma of the rectus abdominis muscle is an uncommon cause of abdominal pain. It occurs mostly in anticoagulated patients. The objective of this paper is to analyse the onset, diagnosis and treatment in patients under anticoagulant therapy. METHODS: A retrospective analysis of a prospectively maintained database of all patients with a diagnosis of spontaneous hematoma of the abdominal rectus muscle between March 2003 and December 2014. RESULTS: The study included 34 patients, of whom 28 were women, with an average age of 80 years old. All the patients showed a unilateral infraumbilical haematoma. Twenty- 8 patients had received long-term anticoagulant treatment (26 with acenocumarol and 2 low molecular weight heparin); and 6 patients were under anticoagulant prophylaxis with low molecular weight heparin. The diagnosis was performed with ultrasound in 7 cases, computed tomography angiography in 27 patients, and with both methods in 6 cases. The treatment consisted of stopping the anticoagulant drug, correcting haemostasis parameters and blood transfusion when required. Ten patients displayed active bleeding in the computed tomography angiography, and 8 underwent selective arterial embolization. The evolution was successful in 34 patients, however, 2 patients required surgery and, finally, died due to persistent haemorrhage. CONCLUSION: Spontaneous haematoma of the rectus abdominis muscle is more frequent in elderly women under oral anticoagulant treatment. Non-operative treatment is successful in most cases. Computed tomography angiography is useful to determine which patients could benefit from selective arterial embolization

Introducción: se analiza el valor de la determinación postoperatoria de paratirina como indicador de riesgo de hipocalcemia tras tiroidectomía total. Material y método: estudio prospectivo de 67 pacientes sometidos a tiroidectomía total por enfermedad benigna. Se determinó la concentración de paratirina y calcio iónico a las 20h de postoperatorio. Se calculó la sensibilidad, la especificidad y los valores predictivos positivo (VPP) y negativo (VPN) de las concentraciones de paratirina y calcio iónico para predecir la aparición de hipocalcemia sintomática o no. Resultados: presentaron hipocalcemia (Ca iónico<0,95 mmol/l) 42 pacientes (62,7%) pacientes, pero únicamente 20 (29,9%) mostraron síntomas. La concentración de paratirina a las 20h de la intervención fue inferior en el grupo con hipocalcemia sintomática (5,57±6,4pg/ml) que en el grupo de hipocalcemia sin síntomas (21,5±15,3pg/ml) y que entre los pacientes normocalcémicos (26,8±24,9pg/ml) (p=0,001). Con un punto de corte para la paratirina en 13pg/ml, la sensibilidad, la especificidad, el VPP y el VPN de paratirina para predecir la aparición de hipocalcemia analítica fueron del 54, el 72, el 76 y el 48%, respectivamente. Por otro lado, la sensibilidad para predecir hipocalcemia sintomática fue del 95% y la especificidad, el 76%. El test presentó alta incidencia de falsos positivos (11/30) (36%). El VPN fue del 97% y el VPP, el 65%. Paratirina y calcio iónico en el análisis multivariable fueron los únicos factores con valor predictivo como indicadores de riesgo de hipocalcemia sintomática. Conclusiones: una concentración normal de paratirina a las 20h de la intervención prácticamente descarta la aparición posterior de síntomas de hipocalcemia. Por contra, cifras de paratirina bajas no se acompañan necesariamente de síntomas debido al elevado número de falsos positivos (AU)


Introduction: Postoperative parathyroid hormone (PTH) levels as a predictor of hypocalcaemia in patients subjected to total thyroidectomy is analyzed. Matherial and method: Prospective study involving 67 patients who underwent total thyroidectomy due to a benign disease. Serum PTH and ionised calcium were measured 20h after surgery. Sensitivity, specificity and predictive values of PTH and ionised calcium levels were calculated to predict clinical and analytical hypocalcaemia. Results: A total of 42 (62.7%) patients developed hypocalcaemia (ionized calcium<0.95mmol/l), but only 20 (29.9%) presented with symptoms. PTH concentration the day after surgery was significantly lower in the group that developed symptomatic hypocalcaemia (5.57±6.4pg/ml) than in the asymptomatic (21.5±15.3pg/ml) or normocalcaemic (26.8±24.9pg/ml) groups (p=0.001). Taking the value of 13pg/ml as a cut-off point of PTH levels, sensitivity, specificity, positive predictive value and negative predictive value were 54%, 72%, 76% and 48%, respectively. On the other hand, sensitivity for predicting symptomatic hypocalcaemia was 95% and specificity was 76%. The test showed a high incidence of false positives (11/30, 36%). Negative predictive value was 97% and positive predictive value was 65%. In multivariate analysis, PTH and ionised calcium were the only perioperative factors that showed an independent predictive value as risk indicators of symptomatic hypocalcaemia. Conclusions: Normal PTH levels 20h after surgery practically rule out the subsequent appearance of hypocalcaemia symptoms. On the other hand, low PTH levels are not necessarily associated to symptomatic hypocalcaemia due to the high number of false positives (AU)

Los cistoadenomas hepatobiliares son tumores quísticos infrecuentes. Su diagnóstico preoperatorio es difícil ya que con frecuencia se confunden con otras lesiones quísticas más comunes. Son lesiones con potencial maligno, de ahí que su tratamiento consista en la extirpación quirúrgica. Los datos de laboratorio, la historia clínica y las imágenes radiológicas deben orientarnos hacia su diagnóstico. Se presentan dos de estos infrecuentes casos y se discuten tanto los modos de presentación como los tipos histológicos y el tratamiento que precisan estas lesiones (AU)


Cystoadenomas of the liver are rare cystic tumors that have seldom been reported. Accurate preoperative diagnosis is difficult because they are usually mistaken for more frequent lesions. Due to their malignant potential, complete surgical removal of the lesion is required. Correct diagnosis is guided by clinical history, radiological imaging, and laboratory parameters. We report two of these rare cases and discuss the spectrum of presentation, pathological features and treatment of these tumors (AU)
