RESUMO

Chorioangiomas are benign placental tumours derived from fetal vessels, that occur with a frequency of approximately 1%. Often they are associated with hydramnios and intra-uterine fetal death. Based on a case of multiple chorioangiomas with a fatal course for the baby, aspects of the pathogenesis of hydramnios are discussed. Future development in ultrasounc technology may facilitate antenatal diagnosis.

Urinary albumin concentration (UA) and albumin/creatinine ratio (UA/UC) in early morning specimens were assayed in 225 consecutive pregnant women at 20, 26, 28 and 30 weeks of gestation. 193 did not develop preeclampsia (control group), 14 developed preeclampsia later (preeclamptic group), 9 were excluded and 9 dropped out. Reference intervals of UA and UA/UC of healthy pregnant women (wk 20-30) was obtained. A statistically significant increase in urinary albumin excretion was observed with increasing gestational age as a normal phenomenon. There was no significant difference in the values of UA and UA/UC in the preeclamptic group when compared with the control group at the same stage of gestation. This indicates that microalbuminuria cannot be used as a predictor of preeclampsia.

In a randomized trial, three alternative schemes of treatment of preterm labour with ritodrine (long infusion, short infusion, and intramuscular injection) were compared with standard treatment by bed rest and sedatives. None of the ritodrine treatments were found to be better in postponing delivery than the standard treatment. However, the Bishop scores of the patients in the standard treatment group tended to be lower than of the patients treated with ritodrine. Moreover, all cases of congenital malformation and polyhydramnios occurred in the patients treated with ritodrine. The incidence of respiratory insufficiency was significantly higher in infants of mothers treated with ritodrine. The mean birth weight was highest in the standard treatment group.
