OBJECTIVE: Research suggests families whose infants are admitted to the Neonatal Intensive Care Unit (NICU) experience elevated distress and may have pre-existing risk factors for maladjustment. This study sought to validate the newly developed Psychosocial Assessment Tool (PAT-NICU/Cardiac Intensive Care Unit [CICU]), a comprehensive screening measure for family psychosocial risk in the NICU. METHODS: The sample included 171 mothers, who completed the PAT-NICU/CICU and other related measures within 2 weeks of their infant's NICU admission at a level 4 unit within a large pediatric hospital. PAT-NICU/CICU scores were compared to a companion risk survey completed by NICU social workers. Test-retest reliability was assessed through repeated measures at 2-month follow-up. RESULTS: Analyses suggest the PAT-NICU/CICU is effective in classifying psychosocial risk. This is supported by statistically significant correlations between the PAT-NICU/CICU and validated measures, in addition to elevated scores on concurrent measures by risk classification. Internal consistency, test-retest reliability, and acceptability for the PAT-NICU/CICU were satisfactory. CONCLUSIONS: This preliminary study demonstrates the validity, reliability, and acceptability of the PAT-NICU/CICU as a psychosocial screening tool to aid identification of families who may benefit from supportive services during NICU admission. This new measure is a more comprehensive tool that assesses a wide variety of risk factors and stress responses. However, future studies of this measure are needed with more diverse samples. Prompt screening of NICU parents may facilitate earlier linkage with appropriate levels of resources or intervention. This research is crucial in improving risk assessment and psychosocial care for families in the NICU.

The objective of this study is to examine the incidence and clinical indicators for use of cardiorespiratory (CR) monitor use. We also examined parents' perceptions on the usefulness of home CR monitoring when their preterm infants were discharged from the hospital. A retrospective patients record review was conducted to assess CR monitor use at discharge in preterm infants from January 1, 2016, to December 31, 2017. Participants received a telephone survey that examined parents' CR monitor experiences and use. Over the study period, forty-four infants were discharged home on CR monitor with the incidence rate of 8.1%. Of those, 28 parents were surveyed between 1 and 2 years post discharge. Most parents reported feeling secure but anxious using the CR monitor. Parents who reported experiencing false alarms frequently also reported having feelings of anxiety while using the CR monitor. Findings from this preliminary study support the importance of understanding the role of parent perceptions of families with infants on CR monitors.

In the United States, ethnic minority status and low socioeconomic status both confer greater risk of premature birth. These sociodemographic factors also contribute to a greater risk of postpartum depression, as does giving birth prematurely. Considering the known adverse effects of postpartum depression on children's development, NICU-based mental health services for these high-risk mothers is an important public health intervention. Although counseling and educational interventions in the NICU have been shown to decrease maternal depressive symptoms, these interventions require parental presence on the unit. Mothers of both low socioeconomic and ethnic minority status face systemic barriers that may prevent them from visiting their infants, such as lack of paid leave, transportation, and childcare. We propose directions for future research with the aim of increasing access to services. Directions include brief individual therapy, telehealth, and increasing the psychosocial support skills of other health professionals. Potential barriers to implementation are discussed.

RESUMO

Several novel technologies have long been used in pediatric psychology. From using electronic pill-count bottles to track child adherence to pill-taking regimens to using bed alarms for night time enuresis, psychologists have relied on technology in their work with children who are ill. Much of the recent technology literature in pediatric psychology has focused on the use of the Internet and other Web-based technologies. This article thoroughly reviews the literature regarding telehealth in the field of pediatric psychology, more specifically the application of televideo or teleconferencing in various populations of children and adolescents with chronic illnesses, followed by the authors' clinical and research applications of telehealth in pediatric psychology. The review concludes with a summary of study findings and future directions in the field for clinicians and researchers alike.
