RESUMO

A study of burn thresholds from superficially penetrating radio-frequency (RF) energy at 8.2 and 95 GHz for swine skin was conducted. The study determined the thresholds for superficial, partial-thickness, and full-thickness burn severities after 5 seconds of exposure at power densities of 4-30 W/cm2and 2-15 W/cm2at 8.2 and 95 GHz, respectively. There were significant differences in he burn thresholds at the different severities between the two frequencies due to the large difference in energy penetration depths. Biopsies were collected from each burn site at 1, 24, 72, and 168 hr post exposure. Each sample was assessed by a burn pathologist against 20 histological factors to characterize the damage resulting from these RF overexposures. A one-dimensional, layered digital phantom that utilized realistic values for dielectric and thermal properties was used to explain some observed thresholds. The results of the heating and cooling response of the animal model and histology scores of each exposure are provided to enhance future efforts at simulation of RF overexposures and to establish damage thresholds.

Recent advances in the molecular characterization of specific isoforms of the Na-K-Cl cotransporter have allowed rapid progress in the study of the structure, function, and regulation of these members of a family of Cl-dependent cation cotransporters. Two distinct isoforms have been identified, one from Cl(-)-secretory epithelia and another found specifically in the diluting segment of the vertebrate kidney, a Cl(-)-absorptive epithelium. The discovery of three alternatively spliced variants of the absorptive isoform, which differ only by 31 amino acids and which appear to be differentially distributed within the mammalian thick ascending limb of the loop of Henle, highlight this spliced region as an important functional component of the protein.

GABA gating an anion channel primarily permeable to chloride can hyperpolarize or depolarize, depending on whether the chloride equilibrium potential (E(Cl)) is negative or positive, respectively, to the resting membrane potential (E(rest)). If the transmembrane Cl(-) gradient is set by active transport, those neurons or neuronal regions that exhibit opposite responses to GABA should express different chloride transporters. To test this, we immunostained retina for the K-Cl cotransporter (KCC2) that normally extrudes chloride and for the Na-K-Cl cotransporter (NKCC) that normally accumulates chloride. KCC2 was expressed wherever E(Cl) is either known or predicted to be negative to E(rest) (ganglion cells, bipolar axons, and OFF bipolar dendrites), whereas NKCC was expressed wherever E(Cl) is either known or predicted to be positive to E(rest) (horizontal cells and ON bipolar dendrites). Thus, in the retina, the opposite effects of GABA on different cell types and on different cellular regions are probably primarily determined by the differential targeting of these two chloride transporters.

A 59-year-old woman was initially seen with anemia, purpura, gastrointestinal tract and vaginal bleeding, pronounced hyperglobulinemia, and an increased serum viscosity, all suggestive of Waldenström's macroglobulinemia. Subsequent investigation, however, indicated that these abnormalities were more likely due to chronic active hepatitis with cirrhosis. The striking similarity of this patient's symptoms to those of Waldenström's macroglobulinemia and the pathophysiologic characteristics of polyclonal hypergammaglobulinemia in chronic liver disease are discussed.

There was a report of spongiform encephalopathy transmitted to Syrian hamsters by intracerebral inoculation with the blood buffy coat of patients with Alzheimer's disease (AD) and their unaffected first-degree relatives. We attempted to verify that report, taking measures to reduce the risk of contaminating samples with agents causing spongiform encephalopathies. We obtained blood from 50 subjects, including six patients with familial AD, 21 unaffected first-degree relatives (siblings and offspring) of patients with familial AD, and 20 control subjects. We inoculated the buffy coats intracerebrally into Syrian LVG hamsters, observed them for signs of neurologic disease, examined their brains for neuropathologic changes at time of death, and performed serial (blind) passages by inoculating suspensions of all recovered brains into fresh LVG hamsters. We discerned no clinical illness or histopathologic changes resembling experimental spongiform encephalopathy in any hamster inoculated with human buffy coat nor in blind-passage hamsters, nor were the life spans of those hamsters shortened. We conclude that AD is not caused by an agent that transmits spongiform encephalopathy to hamsters.

The seasonal variation of CuCl2-mediated low density lipoprotein (LDL) oxidation (10 microM Cu2+, lag phase, rate of oxidation and maximum absorbance at 234 nm) were measured in 43 men and women on 4-6 occasions (mean 5.7 +/- 0.5) over a 12-month period. The lag phase averaged 52.7 +/- 0.6 min and did not differ by gender. Lag phase and rate of the rapid propagation phase of LDL oxidation showed a sinusoidal pattern over the year (increased and reduced oxidative susceptibility during January and June-July, respectively; both p < 0.001). Changes in plasma alpha-tocopherol, ascorbic acid, lycopene or beta-carotene concentrations did not explain seasonal differences in oxidative susceptibility of LDL in vitro. Nor did plasma lipid content of linoleic acid, the main substrate of lipid peroxidation, vary. However, the amount of hydroperoxy- plus hydroxy-fatty acids in plasma lipids varied according to season (p < 0.024) and was related to the lag phase (r = -0.26, p < 0.001). Seasonal variation in oxidative susceptibility was not significant after adjusting for hydroperoxy- plus hydroxy-fatty acids (p = 0.506). Isolated LDL is more vulnerable to Cu2+-induced lipid peroxidation during the winter and this may be due to the higher amount of oxidised lipids during that period.

RESUMO

Three patients with four or more follow-up magnetic resonance imaging (MRI) examinations over a 15-22 month period are described to illustrate the differing patterns of follow-up seen with MRI in multiple sclerosis (MS). These cases illustrate patterns of remission, exacerbation and remission, and rapid progression. The value of MRI in the follow-up of MS is discussed.

RESUMO

Fulminant liver failure can be caused by many agents, and has a high mortality. Death is usually due to complications of poor liver function, rather than to parenchymal loss. Recovery from fulminant hepatic failure can be achieved by careful metabolic and nutritional support, reduction of cerebral edema, and prompt treatment of infection and coagulopathy.

Because reporting of complications by physicians is not reliable, monitoring of the hospital course of surgical patients by nurses can improve effectiveness of identifying complications. Monitoring the course of only those patients who remain in the hospital more than four postoperative days will disclose 90 per cent of complications expected in all patients undergoing operation. Twenty hours per week of a nurse's time is sufficient to monitor 1,300 such patients annually. Identification of patients with large hospital bills will include most patients with complicated hospital courses but does not discriminate them from the large number of uncomplicated courses which also generate large hospital charges.

Initial clinical experience with magnetic resonance imaging (MRI) of the abdomen using short TI inversion-recovery (STIR) pulse sequences is described and compared with X-ray CT in a variety of abdominal disease. The extent of abnormality shown with MRI was greater than that with CT in 21 of 30 cases and equal in 9 cases. Lesion contrast was greater with MRI in 15 cases, equal in 14 and less in 1. The level of artefact was equal in 27 cases and greater with MRI in 3 cases. The STIR pulse sequence has significant advantages in producing high soft-tissue contrast, controlling respiratory artefact, avoiding confusion with intra-abdominal fat and identifying bowel loops.

Gas chromatography (GC) is used for the analysis of trans-fatty acids in partially hydrogenated vegetable oils. Although trans-isomers of C18 carbon length predominate in partially hydrogenated vegetable oils, trans-isomers of C20 and C22 carbon length occur in partially hydrogenated fish oil. We report a simple silver ion chromatographic combined with capillary GC technique for quantitative analysis of trans-monoenes derived from partially hydrogenated fish oil. Silver nitrate thin-layer chromatographic (TLC) plates are developed in toluene/hexane (50:50, vol/vol). Fatty acid methyl esters are separated into saturates (Rf 0.79), trans-monoenes (Rf 0.49), cis-monoenes (Rf 0.27), dienes (Rf 0.10), and polyunsaturated fatty acids with three or more double bonds remaining at the origin. The isolated trans-monoenes are quantitatively analyzed by capillary GC. The technique of argentation TLC with GC analysis of isolated methyl esters is highly reproducible with 4.8% variation (i.e., coefficient of variation, CV%) in R. values and 4.3 and 6.9% CV% in quantification within batch and between batch, respectively. Furthermore, the combined technique revealed that direct GC analysis underestimated the trans-content of margarines by at least 30%. In this study, C20 and C22 trans-monoenes were found in relatively large quantities; 13.9% (range 10.3-19.6%) and 7.5% (range 5.3-11.5%), respectively, in margarine purchased in 1995, but these C20 and C22 trans-monoenes were much reduced (0.1%) in a fresh selection of margarine purchased in 1998. Compositional data from labels underestimated the trans-content of margarines, especially those derived from hydrogenated marine oil. Low levels of C20 transmonoenes (range 0.1-0.3%) and C22 trans-monoenes (range 0.0-0.1%) were identified in adipose tissue obtained from healthy volunteers in 1995, presumably indicating consumption of partially hydrogenated fish oil.

Current therapies for chronic viral hepatitis, autoimmune "lupoid" chronic active hepatitis, and drug-induced chronic hepatitis are discussed in the context of recent advances in our understanding of the pathophysiology of chronic active liver disease. Accurate diagnosis is the cornerstone of proper treatment; the limitations and pitfalls of conventional techniques are discussed. Current theories of the pathogenesis of chronic hepatitis B are reviewed to provide a framework for the use of antiviral drugs. Data from the early results of therapy with adenine arabinoside, acyclovir, and immunomodulatory agents are reviewed, and the theoretical basis for the use of alpha-interferon as well as preliminary data supporting its efficacy is presented. Strategies for the treatment of chronic delta hepatitis and chronic non-A, non-B viral hepatitis are discussed as well. The immunological changes associated with autoimmune chronic active hepatitis are described to help define those patients with chronic active hepatitis who are likely to respond to immunosuppressive therapy. The recognized hazards of long-term corticosteroid therapy are indicated and guidelines for the management of these patients are suggested. Chronic drug-induced liver disease will usually improve with cessation of the offending agent. An approach to the patient with suspected drug-induced chronic hepatitis is indicated. Finally, the role of liver transplantation is mentioned as the ultimate treatment modality available for endstage liver disease.

Affinity matured murine monoclonal antibody producing cell lines can now be rapidly generated using a novel repetitive, multiple site immunization strategy designated RIMMS. RIMMS capitalizes on rapid hypermutation and affinity maturation events which occur in B cell populations localized within secondary lymphatic tissue early in response to antigenic challenges. A murine myeloma cell line, P3XBcl-2-13, stably transfected with Bcl-2, enhances the outgrowth of hybridomas following somatic fusion with immune lymphocytes isolated from pooled peripheral lymph nodes (PLN) 8-14 days after the initial immunization. Immunizations somatic fusion, screening and isolation of affinity matured IgG secreting monoclonal antibody cell lines occur within a one month time period. By using RIMMS, we have been able to expedite the isolation of affinity matured monoclonal antibodies to numerous antigens, including a drug hapten.

This in vitro study compared physical properties of root canal posts made of carbon fiber-reinforced epoxy resin with those of stainless steel posts. Three-point bending tests were used to derive the transverse modulus of elasticity of the posts. Resin composite cores on the posts were subjected to tensile forces to test the bonds between the cores and posts. Carbon fiber posts appeared to have adequate rigidity for their designed purpose. The bond strength of the resin composite cores to the carbon fiber posts was significantly less than that to the stainless steel posts.

Complete-denture therapy involves a complex interplay between biologic demands and technical limitations. Success with complete-denture therapy is reportedly high. Dissatisfied denture patients, although few, do exist. This paper reviews some of the common faults reported with complete dentures.

This paper presents the technical background and research relevant to the electroplating technique. Electroforming is a process whereby a metallized layer is applied to an object via a chemical process referred to as electrolysis. Electrodeposition of dental elastomeric impressions enables a metallized die to be fabricated. Different die materials and techniques exist for the fabrication of dental prostheses via the indirect technique. Electrodeposition of elastomeric impressions offers many advantages, provided an understanding and application of principles is adhered to.

Electroplating of non-aqueous elastomeric impression materials is a well-documented technique. Electroplated silver or copper plated dies have many clinical advantages compared to other die systems. Silver electroplating appears to be used more frequently than copper electroplating. Utilization of silver for electroplating requires an appreciation of financial and biohazard considerations. Research on compatibility of copper electroplating with current non aqueous elastomeric impression materials is lacking. Samples of currently used non aqueous elastomeric impression materials were used to register impressions of conventional crown preparations on a typodont. These impressions were electroplated to form copper plated dies. The surface qualities of the dies were evaluated. Not all impression materials produced visually acceptable or workable copper plated dies.

RESUMO

A questionnaire survey was carried out in 1995 amongst university trained general dental practitioners in the city state of Singapore to assess the use of removable partial dentures (RPD). 37% of the original sample of 469 completed and returned the questionnaire. The results of this survey indicate that RPDs are a common treatment modality in Singapore. Acrylic partial dentures appear to be the preferred choice for RPD treatment. The work profile of those who had a postgraduate qualification in removable prosthodontics did not differ from that of the general dental practitioner.

RESUMO

In some facilities, group learning has been used to teach adults who have various disabilities or chronic diseases (e.g., diabetes, cardiac problems, cancer, Parkinson's disease, spinal cord injury). The following literature review discusses the advantages of group learning, which include peer support, individuals' learning and gaining motivation from each other, reduced feelings of isolation, and opportunities to share problems and goals. It appears likely that greater use of group learning could enhance rehabilitation efforts for adults.
