In this study, an endophytic Bacillus sp. strain (K7) was isolated from the medicinally important ornamental plant, Jasminum officinale. Biochemical analyses were conducted to evaluate the nature of the extracted product, which displayed strong anticandidal activity against Candida albicans (CA) SC5314, as evident from the results obtained in agar-cup diffusion tests, phase-contrast microscopy, scanning electron microscopy and minimum inhibitory concentration assays. After confirming the presence of the gene clusters encoding the lipopeptides iturins and fengycin in the genome of K7, their corresponding molecular ions were identified using MALDI-TOF-MS. 3D structures of the lipopeptides were downloaded from specific databases and molecular docking was performed against a vital CA enzyme, exo-1,3-beta-glucanase, involved in cell wall remodelling, adhesion to polymer materials and biofilm formation. The docking score of iturins was found to be -8·6 and -8·2 kcal mol-1 and for fengycin it was -9·4 kcal mol-1 , indicating a strong affinity of these cyclic lipopeptides towards exo-1,3-beta-glucanase. The combined in vitro and in silico anticandidal studies suggested that these secreted lipopeptides from Bacillus sp. may be used as potential therapeutics against opportunistic and complicated infections of CA.

Cushing syndrome in the paediatric age group is very difficult to diagnose due to atypical presenting features in children. Primary pigmented nodular adrenocortical disease (PPNAD) is a rare cause of ACTH-independent Cushing syndrome in children and it has characteristic gross and microscopic pathologic features. We report a case of PPNAD in a 16-year-old boy who was evaluated in our hospital with chief complaints of poor height velocity and rapid weight gain for 2-3 years before presentation. Proper evaluation showed ACTH-independent Cushing syndrome with normal imaging. Total bilateral adrenalectomy was performed followed by hormones replacement. 6 months after surgery, significant acceleration of height velocity was noticed. Patient also lost body weight and developed secondary sexual characteristics.

Nanoscale power supply units will be indispensable for fabricating next generation smart nanoelectronic integrated circuits. Fabrication of nanoscale rectifier circuits on a Si platform is required for integrating nanoelectronic devices with on-chip power supply units. In the present study, a nanorectifier diode based on a single standalone In x Ga1-x As nanowire/p-Si (111) heterojunction fabricated by metal organic chemical vapor deposition technique has been studied. The nanoheterojunction diodes have shown good rectification and fast switching characteristics. The rectification characteristics of the nanoheterojunction have been demonstrated by different standard waveforms of sinusoidal, square, sawtooth and triangular for two different frequencies of 1 and 0.1 Hz. Reverse recovery time of around 150 ms has been observed in all wave response. A half wave rectifier circuit with a simple capacitor filter has been assembled with this nanoheterojunction diode which provides 12% output efficiency. The transport of carriers through the heterojunction is investigated. The interface states density of the nanoheterojunction has also been determined. Occurrence of output waveforms incommensurate with the input is attributed to higher series resistance of the diode which is further explained considering the dimension of p-side and n-side of the junction. The sudden change of ideality factor after 1.7 V bias is attributed to recombination through interface states in space charge region. Low interface states density as well as high rectification ratio makes this heterojunction diode a promising candidate for future nanoscale electronics.

Impaired burn wound healing in the elderly represents a major clinical problem. Hypoxia-inducible factor-1 (HIF-1) is a transcriptional activator that orchestrates the cellular response to hypoxia. Its actions in dermal wounds promote angiogenesis and improve healing. In a murine burn wound model, aged mice had impaired wound healing associated with reduced levels of HIF-1. When gene therapy with HIF-1 alone did not correct these deficits, we explored the potential benefit of HIF-1 gene therapy combined with the intravenous infusion of bone marrow-derived angiogenic cells (BMDACs) cultured with dimethyloxalylglycine (DMOG). DMOG is known to reduce oxidative degradation of HIF-1. The mice treated with a plasmid DNA construct expressing a stabilized mutant form of HIF-1α (CA5-HIF-1α)+BMDACs had more rapid wound closure. By day 17, there were more mice with completely closed wounds in the treated group (χ(2), P=0.05). The dermal blood flow measured by laser Doppler showed significantly increased wound perfusion on day 11. Homing of BMDACs to the burn wound was dramatically enhanced by CA5-HIF-1α gene therapy. HIF-1α mRNA expression in the burn wound was increased after transfection with CA5-HIF-1α plasmid. Our findings offer insight into the pathophysiology of burns in the elderly and point to potential targets for developing new therapeutic strategies.

Because of the excellent electronic properties, Si is a well-established semiconducting material for PV technology. However, slow kinetics and a fast corroding nature make Si inefficient for the hydrogen evolution reaction (HER) in photoelectrochemical (PEC) applications. Herein, we demonstrate a multifacet Si nanowire (SiNW) decorated with surface plasmon-enhanced carbon quantum dots (AuCQDs) as efficient, stable, economical, and scalable photocathodes (PCs) for HER. The PEC performance of SiNW_AuCQDs has more than a fourfold efficiency enhancement than the pristine SiNW, which we have attributed to the combined effect of enhanced solar absorption and efficient carrier transport. The optimized PC SiNW_AuCQDs results in the highest photocurrent ∼1.7 mA/cm2, an applied bias photon-to-current conversion efficiency of ∼0.8%, and H2 gas evolution rate of ∼182.93 µmol·h-1. Furthermore, these SiNW_AuCQDs PCs provide extraordinary stability under continuous operating conditions with 1 sun illumination (100 mW/cm2). The process-line compatible fabrication process of these PCs will open a new direction at the wafer-level designing of a heterostructure for large-scale solar-fuel conversion.

Solar-driven water splitting is considered as a futuristic sustainable way to generate hydrogen and chemical storage of solar energy. Further, considering the technological competence, silicon is one of the potential materials for developing large-scale and cost-effective photocathodes (PCs), but it lacks efficacy and stability. Here, we show that chlorophyll(a)/carbon quantum dots (Chl/CQDs) bio-nanocomposite (b-NC)-decorated Si-nanowires (SiNWs) as PC can surpass the reported efficiency for photoelectrochemical (PEC) hydrogen generation along with stability. The optimized heterojunction (Chl/CQDs_SiNW) significantly enhances broad-band solar absorption and protects Si surface from corrosion. Further, the appropriate band alignment enforces efficient photogenerated charge separation and possesses directional exciton transport property via the Förster resonance energy transfer (FRET) mechanism. This synergic effect demonstrates an ∼18 times increase in photocurrent density (26.36 mA/cm2) compared to pristine SiNW PC at 1.07 V vs reversible hydrogen electrode (RHE). The efficiency reaches ∼7.86%, which is comparably the highest reported for hybrid Si-based photocathodes. Hydrogen evaluation rate was measured to be ∼113 µmol/h at 0.8 V vs RHE under 1 sun illumination. With Si-process line compatibility, this new finding opens a new direction toward the development of Si-based efficient and stable PCs at a large scale for commercial applications.

Early detection is the cornerstone of hypertension management; still majority remains undetected until complications arise, especially in poor-resource settings. Paucity of information regarding undiagnosed and uncontrolled hypertension in eastern India thus called for a detailed investigation involving a representative sample of adults in Malda, one of the poorest districts in the region. In a cross-sectional study, between October 2013 and July 2014, using multistage random sampling with probability-proportional-to-size, 18 028 consenting adults were interviewed. Diagnosed cases were defined as uncontrolled if they still had hypertensive level of blood pressure (according to JNC-VIII criteria) while those detected during this study were defined as undiagnosed. Descriptive and regression analyses were performed using SAS version 9.3.2. Among 18 028 participants, 4695 (26.04% (95% confidence intervals: 95% CI=25.40-26.68)) had hypertension, of which 3937 (83.86% (82.80-84.91)) were undiagnosed and 548 (72.30 (69.10-75.49)) had uncontrolled hypertension. Relatively older subjects (adjusted Odds ratio (aOR)41-60 years=0.34 (95% CI=0.26-0.43) and aOR>60 years=0.29 (0.21-0.38)), who were divorced/separated/widowed/widower (aOR=0.76 (0.61-0.95)), had higher education (aOR=0.61 (0.43-0.88)), better socio-economic status (SES) (aORMiddle=0.77 (0.60-0.99) and aORUpper=0.64 (0.48-0.85)) and urban residence (aOR=0.44 (0.36-0.55)) were less likely while subjects who belonged to backward castes (aOR=1.37 (1.15-1.64)) were more likely to have undiagnosed hypertension. Odds of having uncontrolled hypertension were higher among participants aged >60 years (aOR=2.25 (1.27-3.99)). Burden of hypertension (diagnosed and undiagnosed) was high in Malda district of West Bengal. Significant predictors of undiagnosed hypertension were young age, backward caste, poor education and lower SES, while older subjects had poor control. Thus, appropriate surveillance targeting these at-risk groups might be effective in controlling hypertension in similar poor-resource settings.

A cross-sectional study was carried out during January-June 2004 to assess the status of HIV infection among brothel-based sex workers in Kolkata city, India. Six hundred and twenty-two sex workers, selected from six brothels, were included in the study to assess their HIV status, and 362 sex workers, a subset of the above population, were interviewed to study their risk behaviour and practices. Blood samples were collected from each sex worker for testing HIV by an unlinked anonymous method. The prevalence of HIV infection was 9.6%, but was much higher among younger sex workers aged 20 years or less (27.7%) compared to the older age group (8.4%). This difference was statistically significant, indicating an association of younger ages with HIV infection [p = 0.006 and odds ratio (OR) = 4.18; (95% confidence interval (CI) 1.2-13.8)]. HIV was not associated with duration of sex work, average number of clients entertained per day, condom use, practice of sex during menstruation, pre-coital examination of penis for visible ulcer/discharge, suffering from sexually transmitted infections, and entertaining clients outside the brothel. There was a big gap between the reported and the evaluated condom use by sex workers. The results suggest that there is a need to develop suitable HIV intervention strategies, considering the socioeconomic and cultural aspects of city sex workers, with a provision for continuous monitoring and evaluation.

Using percoll as the density gradient, a new single step method to isolate leukocytes from the peripheral blood of amphibians (B. himalayanus) has been described. Isolated leukocytes were photographed under the scanning electron microscope and an attempt has been made to characterize the leukocyte population on the basis of surface morphology. Apart from regular blood cell types, B. himalayanus have slender, elongated and slightly curved leukocyte type cells in their peripheral blood. Such slender elongated cells were absent in the blood of a related species B. stomnaticus and hence could not be categorized under the known blood cell types.

The scope of biomonitoring insecticide pollution in soil is discussed with the help of field and laboratory findings on the density, prey-predator ratio and fecundity of non-target microarthropod fauna. Field experiments were conducted in small plots with mustard, wheat and lady's finger crops and insecticides namely heptachlor 20EC (3.25 kg ai/ha = 16.25 lit/ha) and endosulfan 35EC (0.875 kg ai/ha = 2.5 lit/ha) applied at the seedling stages. Soil microarthropod population estimated at fortnightly intervals in the treated and untreated control plots revealed a general trend of adverse effect of the insecticides, prominently on the density and relative abundance of major prey groups like Collembola and Acari leading to notable decline in prey-predator ratio. Comparison of the percentage reductions of major taxonomic and trophic groups between pre-treatment and post-treatment intervals also demonstrated the ill effect of both heptachlor and endosulfan, notably on Collembola and the prey category. In the laboratory the survival success and fecundity of Cyphoderus javanus (Collembola) and Archegozetes longisetosus (Acari) were compared by exposing freshly emerged adults to sub-lethal concentrations of heptachlor and endosulfan for varying durations. The untreated control sets recorded high fecundity for both C. javanus and A. longisetosus, but chronic toxicity of the insecticides on adults confined to the treated soil resulted into very low fecundity. Even short duration exposure to heptachlor and endosulfan treated soil for 24 or 72 hours only was found to delay the egg-laying and decrease the fecundity of both the species. It is concluded that population responses and reproductive sensitivity in non-target soil microarthropods are potential eco-toxicological parameters for detecting pesticide pollution in soil and for ecological health assessment since the results are based on the bioactivity of toxicants.

PIP: Blood samples were obtained from six HIV-infected IV drug users attending the Indian Council for Medical Research Clinic at Imphal, the capital of Manipur state, as part of a study to analyze the sequences of the third variable region (V3) of the HIV envelope in that population. 18 sequences were obtained from the study participants, men aged 24-29 years. A fragment of the envelope gene encompassing the entire V3 loop and partial sequences from the third conserved region was sequenced, while amplifications were performed on DNA extracted directly from patient peripheral blood mononuclear cells to avoid the introduction of artifacts during cell culture. One set of viruses seems to be most closely related to a prototypical virus recovered from IV drug users in Thailand (Thai B), while the other sequences clustered more closely with a North American clade B virus (HXB). Sequences recovered from one person are rather diverse.^ieng

Eight renal oncocytomas are described. Clinical manifestations were present in 3 cases, with hematuria occurring in 2 and flank pain in 2. Tumors were between 2.5 and 10.0 cm. in diameter and showed considerable variation in histologic pattern. The tan coloration of some tumors may permit a naked-eye distinction from renal cell carcinoma. All tumors appeared to be benign.

Since 1989, injecting drug use (IDU) related HIV infection has affected thousands of young adults in Manipur, a north eastern state of India bordering Myanmar following a similar kind of epidemic in adjoining countries like Thailand and Myanmar. During a clinical surveillance of a group of HIV positive IDUs for a natural history study at Manipur, herpes zoster (HZ) emerged as the most specific early HIV related illness (positive predictive value of 100%) in patients belonging to the age group of 12-45 years. Data collected from the dermatology departments of the two main hospitals of the state revealed that there had been an epidemic of HZ since 1990 (rate of 1990 being 11.3/1000 compared to 6.5/1000 in 1989, P value < 0.0001) among males of 12-45 years. The epidemic of HZ has been attributed to the preceding epidemic of IDU related HIV in the same age and gender group occurring 1 year earlier. HZ should be recognised as a marker condition similar to tuberculosis indicating the necessity of screening for HIV in regions where the dual problem of IDU and HIV exist in young adults.

To determine the site of mineralization in calcifying tendinitis, we studied the ultrastructure of tendon fragments that were obtained during surgical interventions on ten patients with that condition. We found that calcification was occurring extracellularly and essentially in areas where the tendon had undergone cartilaginous transformation. Calcific crystals were present in matrix-vesicle-like structures that were seen either singly among collagen fibers or in aggregates. When they were in aggregates, the calcifying vesicles tended to fuse with each other. Thus, the process of mineralization in calcifying tendinitis resembled those calcifying conditions that have already been described in man and animals, both normal and pathological, in which the apatite crystals are initially localized in matrix vesicles.

We examined biopsy specimens obtained during surgery on 115 patients with complete rotator cuff rupture. The vascularised connective tissue covering the area of rupture and the proliferating cells in the fragmented tendons reflected more of the features of repair than of degeneration and necrosis. The main source of this fibrovascular tissue was the wall of the subacromial bursa. These features clearly indicated a vigorous reparative response which might play an important role in tendon reconstitution and remodelling. We therefore suggest that extensive debridement along with subtotal bursectomy, commonly practised during surgical repair of rotator cuff rupture, should be avoided. Although strong suture margins are essential for good operative results, debridement should be judicious and preserve as much as possible of the bursa and the associated fibrovascular tissue.

An acidic heteropolysaccharide has been isolated from the tropical angiosperm Feronia limonia syn. F. elephantum (family: Rutaceae). A partially carboxymethylated alpha-(1-4) polygalacturonan backbone structure with 2- and 2,4-O-alpha-L-rhamnopyranosyl, 2- and 2, 3-O-alpha-L-arabinofuranosyl and 3-, 2,4-and terminal alpha-D-galactopyranosyl bearing side chains has been tentatively assigned. The preliminary study in the murine model showed some significant in vivo Ehrlich ascites carcinoma cell growth inhibition.

In the course of experiments on the effects of anabolic steroids on the myocardium of rat conceptuses, we found that subcutaneous implantation of 10 mg of estradiol, Dianabol or testosterone to rats in mid pregnancy, resulted in embryo resorption. Placental tissue was identified only in estradiol-treated rats which also demonstrated a large amount of serosanguineous fluid that dilated the horns considerably. The yellow nodules of placental attachment sites were represented histologically by cellular and vascular proliferations between the inner and outer layers of the myometrium. The nodular aggregates of cells had variable features according to the steroid administered. Neither decidual cells nor metrial glands that are reported to be the constituents of placental attachment sites were seen in our material. We conclude that anabolic steroids are potent agents for embryo resorption, and that the cells in the nodules of placental attachment sites are likely to be derived from the myometrium.

BACKGROUND: The human immunodeficiency virus was first detected in young intravenous drug users in Manipur in 1989 and it quickly reached a high prevalence in this group. Diagnostic facilities are scarce and it is thus important to suspect the presence of the infection by its clinical features. METHODS: We did a cross-sectional survey for 13 months among residents of different detoxification centres of Imphal, Manipur, to study the sensitivity, specificity and positive predictive values of different signs and symptoms occurring at the early phase of the infection. RESULTS: Most of the young injectors in this survey were found to be in the early phases (stage I 43%; stage II 32%; stage III 15% and stage IV 9.9%) of the World Health Organization clinical staging of human immunodeficiency virus infection and disease. Herpes zoster, oral candidiasis, pruritic papular eruptions, jaundice and lymphadenopathy had positive predictive values of 100%, 100%, 93%, 93% and 88% respectively. Cryptosporidial diarrhoea and tuberculosis (pulmonary and extrapulmonary) were also encountered. CONCLUSION: Intravenous drug users in Manipur who have human immunodeficiency virus infection suffer from different opportunistic infections which give rise to clinical features that are easily recognizable. It is important to be aware of these in areas which lack diagnostic facilities for confirming the infection.


PIP: A cross-sectional study conducted among intravenous drug users in India's Manipur State suggests that certain clinical signs and symptoms can be used to detect human immunodeficiency virus (HIV) in areas with scarce diagnostic resources. From May 1992 to April 1993, 154 intravenous drug users recruited from drug detoxification centers in the capital city of Imphal were monitored for clinical manifestations of disease. 131 subjects were HIV-positive, but examining clinicians were not given data on HIV status. All subjects had started injecting within the last seven years, so the majority were in the early phases of HIV. The distribution, by clinical stage, was as follows: I, 43%, II, 32%, III, 15%, and IV, 9.9%). Clinical features most frequently encountered included herpes zoster (27 men), oral thrush (7), pruritic papular eruptions (15), lymphadenopathy (33), and jaundice (14). The positive predictive values of these signs were 100%, 100%, 93%, 88%, and 93%, respectively. Similar studies in other areas are urged to provide information on the sensitivity and specificity of major signs for defining clinical cases of HIV infection.

In rats with prolonged magnesium deficiency, paracrystalline inclusions in mitochondria were found frequently in the epithelial cells of the thick ascending limb of Henle, especially in the region of the upper medulla. The inclusions presented a variegated appearance. Some were filamentous aggregates, many had alternating dark and light bands, and others were elongated rectangular densities. They were not found in tubules with calcific deposits where the cells showed degenerative changes. This suggested that the inclusions were probably due to altered cell metabolism and not products of cell degeneration.
