BACKGROUND: Women seem to be more susceptible to psychosocial stress than men, and stress is associated with worse outcomes after acute myocardial infarction (AMI). OBJECTIVES: To investigate whether the female gender is an independent predictor of risk for stress and to compare stress levels between women and men after AMI. METHODS: Cross-sectional study of a case series. Patients aged 18 to 65 years who were treated for AMI at the study facility between January 2017 and June 2018 were eligible. The presence of stress was assessed using Lipp's Stress Symptoms Inventory for Adults (ISSL), which categorizes stress into four phases (alertness, resistance, near-exhaustion, and exhaustion), through a list of physical and psychological symptoms. Data were analyzed using SPSS Version 24.0. The significance level was set at p<0.05. RESULTS: Of the 330 respondents, 89% of women and 70% of men experienced stress. The female gender was associated with nearly threefold higher odds of experiencing stress (EXP (B)2.79, p = 0.02). Regarding the phases of stress, women were more often in the near-exhaustion and exhaustion phases, while men were more often in the resistance phase. CONCLUSIONS: This study showed that women are most often in the third and fourth phases of stress, i.e., in situations of long-standing psychosocial stress. These findings can assist in the development of gender-specific strategies for health promotion and disease prevention, aiming to minimize the effects of stress in this population.

OBJETIVOS: Investigar se o sexo feminino é preditor independente de risco para o estresse e comparar os níveis de estresse entre mulheres e homens com IAM. MÉTODOS: Estudo transversal de uma série de casos, realizado entre janeiro de 2017 e junho de 2018. Pacientes com idade entre 18 e 65 anos, atendidos na instituição por IAM nesse período. A existência de estresse foi avaliada por meio do Inventário de Sintomas de Stress para Adultos de LIPP (ISSL), que o categoriza em quatro fases: alerta, resistência, quase exaustão e exaustão, com base em uma lista de sintomas físicos e psicológicos. Os dados foram analisados pelo programa estatístico Statistical Package for Social Sciences (SPSS) versão 24.0. O nível de significância adotado foi um valor de p < 0,05. RESULTADOS: Dos 330 entrevistados, 89% das mulheres e 70% dos homens apresentaram estresse; o sexo feminino quase triplicou as chances de sofrê-lo (EXP (B) 2,79; p = 0,02). Quanto às quatro fases, as mulheres mostraram-se mais em quase exaustão e exaustão, e os homens, mais em resistência. CONCLUSÕES: Este estudo evidenciou que as mulheres se encontram na terceira e quarta fases do estresse, ou seja, em situações de estresse psicossocial duradouras. Tais resultados podem auxiliar no desenvolvimento de estratégias específicas para prevenção e promoção da saúde conforme os sexos, visando minimizar os efeitos do estresse nesses pacientes.

In 2 studies, the authors explored preadolescent and adolescent use of personal homepages in relation to mastery and identity formation. In Study 1, the authors attempted to determine the prevalence of personal homepage and online journal (blog) construction among a random sample (N=500) of preadolescents and adolescents. Adolescents were more likely to create personal homepages or blogs than preadolescents. Creation was related to feelings of mastery, expressions of identity, and a means to socialize. In Study 2, the authors explored the relationship of homepages to mastery and identity formation by content analysis of a random sample of homepages. Results suggest children use personal homepages to express and explore their forming identities.

Recruiting children and families for research studies can be challenging, and re-recruiting former participants for longitudinal research can be even more difficult, especially when a study was not prospectively designed to encompass continuous data collection. In this article, we explain how researchers can set up initial studies to potentially facilitate later waves of data collection; locate former study participants using newer, often digital, tools; schedule families using recruitment phone/email/mail scripts that highlight the many benefits to continued study participation; and confirm appointments with other digital tools. We draw from prior methodological and longitudinal pieces to provide suggestions to others wishing to re-recruit families for longitudinal studies. In addition, we draw upon our own experience conducting a non-prospective longitudinal study 6 years after an educational intervention, in which we successfully re-located 122 (90%) and interviewed 101 of 136 (83% of the located sample and 74% of the full original sample) parents and their early adolescent children. Although the majority of participants were recruited via original contact information (especially phone numbers), using a range of strategies to recruit (e.g., search engines focused on contact information, social media) and motivate participation (e.g., multifaceted phone/email/mail scheduling scripts, flexibility in location and means of participation) yielded a more desirable sample size at relatively low costs.

BACKGROUND: Bosentan, a dual endothelin-receptor antagonist, is registered for the treatment of pulmonary arterial hypertension. Little is known about the effects of bosentan in children. This study was conducted to investigate the pharmacokinetics, safety, and efficacy of bosentan in pediatric patients with pulmonary arterial hypertension. METHODS: In this 2-center, open-label study, 19 pediatric patients with pulmonary arterial hypertension were enrolled and stratified for body weight and epoprostenol use. Patients weighing between 10 and 20 kg, between 20 and 40 kg, or greater than 40 kg received a single dose of 31.25, 62.5, or 125 mg, respectively, on day 1, followed by 4 weeks of treatment with the initial dose. The dose was then up-titrated to the target dose (31.25, 62.5, or 125 mg twice daily). Pharmacokinetic and hemodynamic parameters were obtained at baseline and after 12 weeks of treatment. Six-minute walk distance and cardiopulmonary exercise testing results were measured at baseline and at week 12 in children aged 8 years or older. RESULTS: The variability in exposure among the 3 groups was less than 2-fold after single- and multiple-dose administration. The exposure to bosentan decreased over time in all groups. The covariates body weight, gender, age, and the use of epoprostenol had no significant effect on the pharmacokinetics of bosentan. Bosentan produced hemodynamic improvement and was well tolerated. The mean change from baseline in mean pulmonary artery pressure was -8.0 mm Hg (95% confidence interval, -12.2 to -3.7 mm Hg), and that in pulmonary vascular resistance index was -300 dyne x s x m(2)/cm(5) (95% confidence interval, -576 to -24 dyne x s x m(2)/cm(5)). CONCLUSIONS: The pharmacokinetics of bosentan in pediatric patients with pulmonary arterial hypertension and healthy adults are similar, and treatment with bosentan resulted in hemodynamic improvement. These results suggest that the applied dosing regimens may be appropriate to treat pediatric patients.

BACKGROUND: The course of 12-year-old, homozygotic twins with primary pulmonary hypertension (PPH) treated with different vasoactive agents, beraprost vs epoprostenol, is described. METHODS: Clinical, exercise, and hemodynamic assessments were made at baseline, and at 9 months and 24 months of treatment. FINDINGS: Twin A had a rapid improvement with epoprostenol. In contrast, twin B, initially treated with beraprost, had progressive worsening with subsequent improvement on epoprostenol. INTERPRETATION: Epoprostenol was efficacious for identical twins with PPH. A 9-month delay in initiating epoprostenol for twin B did not appear to have irreversible short-term detrimental effects.
