OBJECTIVES: Sexual networks are known to structure sexually transmitted infection (STI) transmission among men who have sex with men (MSM). We sought to estimate the risks of STI diagnosis for various partnership types within these networks. METHODS: Our cross-sectional survey analysed data from 1376 MSM screened for a partner management intervention in Lima, Peru. Participants were tested for HIV, syphilis, gonorrhoea (NG) and chlamydia (CT) and completed surveys on their demographics, sexual identity/role, HIV status, partnership types and sexual network from the prior 90 days. χ2 and Wilcoxon rank-sum tests compared participants without an STI to those diagnosed with (1) syphilis, (2) NG and/or CT (NG/CT) and (3) syphilis and NG/CT coinfection (coinfection). RESULTS: 40.8% (n=561/1376) of participants were diagnosed with an STI (syphilis: 14.9%, NG/CT: 16.4%, coinfection: 9.5%). 47.9% of all participants were living with HIV and 8.9% were newly diagnosed. A greater proportion of participants with syphilis and coinfection were living with HIV (73.5%, p<0.001; 71.0%, p<0.001) compared with those with NG/CT (47.8%) or no STI (37.8%). Participants with syphilis more often reported sex-on-premises venues (SOPVs) as the location of their last sexual encounter (51.7%, p=0.038) while those with NG/CT tended to meet their last sexual partner online (72.8%, p=0.031). Respondents with coinfection were the only STI group more likely to report transactional sex than participants without an STI (31.3%, p=0.039). CONCLUSIONS: Sexual networks and partnership types of Peruvian MSM are associated with differential risks for STIs. Participants diagnosed with syphilis tended to meet single-encounter casual partners at SOPV, while MSM with NG/CT were younger and often contacted casual partners online. Coinfection had higher frequency of transactional sex. These findings suggest the potential importance of public health interventions through combined syphilis/HIV screening at SOPV, syphilis screening at routine clinic appointments for MSM living with HIV and directed advertisements and/or access to NG/CT testing through online platforms.

BACKGROUND: Syphilis co-infection among pregnant people living with HIV (PLH) may worsen pregnancy outcomes. We evaluated the impact of syphilis co-infection on pregnancies in south Brazil. METHODS: Data was extracted from hospital records between 1/1/2008 -12/31/2018. Preterm birth (PTB), low birth weight (LBW < 2500 g), and a composite adverse infant outcome [AIO: HIV vertical transmission, loss to follow-up before HIV diagnosis (LTFU), stillbirth, congenital syphilis] were evaluated among pregnancies without HIV and syphilis (PWOH+S), PLH mono-infection, syphilis mono-infection (PLS), and PLH with syphilis (PLH + S). RESULTS: Among 48,685 deliveries where patients were tested for HIV and syphilis, 1,353 (2.8%) occurred in PLH; of these, 181 (13.4%) were HIV/syphilis co-infected (PLH + S). Among PLH, 2.4% of infants acquired HIV and 13.1% were LTFU. Among all PLS, 70.5% of infants acquired congenital syphilis. Across the cohort, 1.2% stillbirths/neonatal deaths occurred. 37.0% of PLH + S did not initiate ART versus 15.4% of PLH mono-infection (p < 0.001). 37.6% of PLH + S had VDRL titers > 1:16 compared to 21.7% of PLS only (p < 0.001). Among PLH, syphilis co-infection and unknown/high VDRL titers ( > 1:16) increased AIO risk more (aRR:3.96, 95%CI:3.33-4.70) compared to low VDRL titers ( < 1:8) (aRR:3.51, 95%CI:2.90-4.25). Unsuppressed viremia ( > 50 copies/mL) was associated with risk of PTB (aRR:1.43, 95%CI:1.07-1.92) and AIO (aRR:1.38, 95%CI:1.11-1.70) but not LBW. Lack of prenatal care was significant in predicting PTB and LBW in all PLH and PLS mono-infection. CONCLUSION: Syphilis co-infection worsens adverse infant outcomes in all women and compounds negative effects of HIV infection during pregnancy. Effective syphilis treatment and HIV VL suppression are paramount for optimal obstetric care.

Background: Despite high rates of HIV transmission among men who have sex with men (MSM) in Lima, Peru, limited data exist on the sexual network characteristics or risk factors for secondary HIV transmission among MSM with uncontrolled HIV infection. We report the frequency of serodiscordant, condomless anal intercourse (CAI) and associated sexual network characteristics among MSM in Lima with detectable HIV viremia and compare to those with undetectable viremia. Methods: This cross-sectional analysis includes MSM who tested positive for HIV-1 during screening for a trial of partner management and STI control (June 2022-January 2023). Participants were tested for HIV, gonorrhoea, chlamydia, and syphilis, and completed questionnaires on their demographic characteristics, sexual identity and behaviour, sexual network structures and engagement in HIV care. Findings: Of 665 MSM, 153 (23%) had detectable (>200 copies/mL) viremia. 75% (499/662) of men living with HIV were previously diagnosed, with 94% (n = 469/499) reporting that they were on ART, and 93% (n = 436/469) virally suppressed. 96% (n = 147/153) of men with detectable viremia reported serodiscordant CAI with at least one of their last three sexual partners, and 74% (n = 106/144) reported the same with all three of their recent partners. In contrast, 62% (n = 302/489) of men with undetectable viral load reported serodiscordant CAI with all of their last three partners (p < 0.01). Interpretation: 23% of men living with HIV in Peru had detectable viremia, of whom almost all (96%) reported recent serodiscordant CAI. The primary gap in the HIV care cascade lies in awareness of HIV serostatus, suggesting that improved access to HIV testing could be a key prevention strategy in Peru. RESUMEN Introducción: El diagnóstico de várices esofágicas en pacientes cirróticos se realiza mediante la endoscopía digestiva alta. Se han estudiado predictores no invasivos para el diagnóstico de estas. Objetivo: El objetivo de este estudio es evaluar el desempeño del índice FIB-4 en el despistaje de várices esofágicas en pacientes con diagnóstico de cirrosis hepática. Materiales y métodos: Se realizó un estudio transversal analítico en cuatro hospitales nacionales utilizando historias clínicas de pacientes cirróticos. Se realizó el análisis univariado y bivariado, cálculo de sensibilidad, especificidad, valor predictivo positivo y negativo, razón de verosimilitud positiva y negativa del índice FIB-4 para el despistaje de várices esofágicas, tamaño de estas y profilaxis. Se construyeron curvas ROC para cada grupo de análisis. Resultados: Se incluyó 289 pacientes con diagnóstico de cirrosis hepática, la mayor parte fueron de sexo masculino (54,33%). 77,85% presentaron várices esofágicas. La distribución del tamaño de várices fue de 19,03%, 35,99% y 22,84% para várices grandes, medianas y pequeñas, respectivamente. En el análisis del índice FIB-4 con la presencia de várices se encontró una sensibilidad de 81,3% y una especificidad de 37,5% (AUC: 0,57). Para el tamaño de várices se encontró una sensibilidad 81,8% y una especificidad de 23,9% (AUC: 0,50). En el análisis de FIB-4 para grupos de profilaxis se encontró una sensibilidad de 81,8% y una especificidad de 28,5% (AUC: 0,54). Conclusiones: El índice FIB-4 no tiene un buen desempeño en el despistaje de la presencia várices esofágicas y su tamaño en pacientes con diagnóstico de cirrosis hepática.


ABSTRACT Introduction: The diagnosis of esophageal varices in cirrhotic patients is made by the upper gastrointestinal endoscopy. Multiple non-invasive predictors have been studied for the diag-nosis of esophageal varices. The objective of this study is to testthe FIB4 index as screening of esophageal varices in patients with liver cirrhosis. Materials and methods: A cross-sectional analytic study was developed in four national hospital using hepatic cirrhosis patient's medi-cal files. We assessed the information using univariate and bivariate analysis, sensitivity, speci-ficity, predictive positive and negative value, the positive and negative likelihood ratio calcu-lation of the esophageal varices screening and its size. We built ROC curve for every analysis group. Results: The study included 289 liver cirrhosis patients. Most of the patients were male (54.33%). 77.85% patients had esophageal varices. The distribution of varices was 19.03%, 35.99% and 22.84% for large, medium and small varices, respectively. In the FIB-4 index analysis for the presence of varices, it was found a sensitivity of 81.3%, specificity of 37.5% (AUC: 0.57). The calculation for variceal size showed a sensitivity of 81.8%, specificity of 23.9% (AUC: 0.50). In the analysis of FIB-4 index for prophylaxis groups was found a sensitivity of 81.8% and a specificity of 28.5% (AUC: 0.54). Conclusions: The FIB-4 index has no good performance in the screening for the presence of esophageal varices and its size in liver cirrhosis patients.

ANTECEDENTES Y PROPÓSITO DEL ESTUDIO: la hemorragia digestiva alta es una causa importante de ingreso hospitalario y constituye la principal emergencia gastroenterológica, con una tasa de mortalidad de hasta el 14%. En el Perú no existen estudios sobre el uso de la escala de Glasgow-Blatchford para predecir mortalidad por hemorragia digestiva alta. El objetivo de este estudio es realizar la validación externa de la escala de Glasgow-Blatchford y establecer su mejor punto de corte para predecir mortalidad por hemorragia digestiva alta en un hospital de Lima, Perú. Métodos: estudio de validación diagnóstica, analítico, longitudinal, de tipo retrospectivo, con datos de pacientes con diagnóstico clínico y endoscópico de hemorragia digestiva alta atendidos en la Unidad de Hemorragia Digestiva del Hospital Nacional Edgardo Rebagliati Martins, entre junio de 2012 y diciembre de 2013. Calculamos el área bajo la curva ROC (receiver operating characteristic) de la escala de Glasgow-Blatchford para predecir mortalidad, con un intervalo de confianza al 95%. Resultados: un total de 339 registros fueron analizados. El 57,5% fueron varones y la edad media (desviación estándar) fue de 67,0 (15,7) años. La mediana de la escala de Glasgow-Blatchford obtenida en la población fue de 12. El análisis ROC para mortalidad dio un área bajo la curva de 0,59 (IC95% 0,5-0,7). Se estratificó por tipo de hemorragia digestiva alta, obteniendo un área bajo la curva de 0,66 (IC95% 0,53-0,78) para el tipo no variceal. Conclusiones: en la población estudiada, la escala de Glasgow-Blatchford no posee una validez diagnóstica adecuada para predecir mortalidad


BACKGROUND AND AIM: Upper gastrointestinal bleeding is a major cause of hospitalization and the most prevalent emergency worldwide, with a mortality rate of up to 14%. In Peru, there have not been any studies on the use of the Glasgow-Blatchford Scoring System to predict mortality in upper gastrointestinal bleeding. The aim of this study is to perform an external validation of the Glasgow-Blatchford Scoring System and to establish the best cutoff for predicting mortality in upper gastrointestinal bleeding in a hospital of Lima, Peru. METHODS: This was a longitudinal, retrospective, analytical validation study, with data from patients with a clinical and endoscopic diagnosis of upper gastrointestinal bleeding treated at the Gastrointestinal Hemorrhage Unit of the Hospital Nacional Edgardo Rebagliati Martins between June 2012 and December 2013. We calculated the area under the curve for the receiver operating characteristic of the Glasgow-Blatchford Scoring System to predict mortality with a 95% confidence interval. RESULTS: A total of 339 records were analyzed. 57.5% were male and the mean age (standard deviation) was 67.0 (15.7) years. The median of the Glasgow-Blatchford Scoring System obtained in the population was 12. The ROC analysis for death gave an area under the curve of 0.59 (95% CI 0.5-0.7). Stratifying by type of upper gastrointestinal bleeding resulted in an area under the curve of 0.66 (95% CI 0.53-0.78) for non-variceal type. CONCLUSIONS: In this population, the Glasgow-Blatchford Scoring System has no diagnostic validity for predicting mortality

Introducción: durante los primeros días de vida la madre es la principal fuente de alimento para el recién nacido. Sin embargo, es común que la madre presente trastornos de la lactancia y se genere una pérdida de peso neonatal superior a la fisiológica. Objetivo: estimar la magnitud y asociación entre los trastornos de la lactancia y la pérdida de peso neonatal superior a la fisiológica en neonatos en el área de alojamiento conjunto de un servicio de neonatología en un hospital de la Seguridad Social en Lima, Perú. Métodos: estudio de tipo transversal analítico. Registramos el peso neonatal en una evaluación de rutina (entre las 24 y 72 horas de vida) y lo comparamos con el peso al nacer. La pérdida de peso excesiva fue definida como una diferencia igual o mayor al 7%. Mediante una encuesta y una verificación visual investigamos los trastornos de la lactancia materna (retraso en el inicio, posición de la boca, duración de la lactancia, frecuencia de la lactancia, sobreabrigo, dolor en el pezón y forma de la C). La asociación entre la pérdida de peso excesiva y los trastornos, ajustada por otros factores, fue cuantificada mediante un modelo lineal generalizado múltiple. Resultados: en 18,8% (74/393) de los neonatos, la pérdida de peso excesiva fue igual o superior al 7% del peso al nacer. La posición inadecuada de la boca en el pezón estuvo presente en el 53,7% (211/393) de los neonatos, mientras que el dolor en el pezón fue reportado en el 44,0% (173/393) de las madres. En el análisis ajustado, el dolor en el pezón [RP = 1,50 (IC95%:1,02-2,22)] y la posición inadecuada de la boca [RP = 1,67 (IC95%:1,09- 2,57)] estuvieron asociados a una mayor pérdida de peso excesiva. Conclusiones: los trastornos de la lactancia son comunes. Estos factores están directa y positivamente asociados a una mayor pérdida de peso excesiva. La introducción de mejoras en las prácticas de lactancia, por ejemplo mediante programas educativos, podría disminuir la pérdida de peso neonatal excesiva (AU)


Introduction: during the first days of life the mother is the main source of nutrients for the newborn. However, breastfeeding difficulties are common and may generate excessive neonatal weight loss. Objective: estimate the magnitude and association between breastfeeding problems and excessive neonatal weight loss beyond the physiological norm in the neonatology ward in a social security hospital in Lima, Peru. Methods: we conducted a cross sectional analysis of the recorded neonatal weight in a routine evaluation (between 24 and 72 hours of life) and compared it against birth weight. Excessive weight loss was defined as a difference greater than or equal to 7 %. Breastfeeding problems (defined as problems with breastfeeding initiation, mouth position, breastfeeding duration, breastfeeding frequency, too much clothing, nipple pain and C form) were investigated using a survey and visual verification. The association between excessive weight loss and breastfeeding problems, adjusted by other factors, was quantified using a multiple generalized linear model. Results: excessive weight loss was present in 18.8% (74/393) of the newborns. Improper positioning of the mouth on the nipple was present in 53.7% (211/393) of neonates while nipple pain was reported by 44.0% (173/393) of mothers. In the adjusted analysis, nipple pain [PR = 1.50 (95% CI: 1.02 to 2.22)] and improper positioning of the mouth [PR = 1.67 (95% CI: 1.09- 2.57)] were associated with an increased occurrence of excessive neonatal weight loss. Conclusions: breastfeeding problems are common. These difficulties are significantly associated with an increased occurrence of excessive neonatal weight loss. Improvements in breastfeeding practices, for example through educational programs, may decrease the occurrence of excessive neonatal weight loss (AU)

Determinar la concordancia entre las escalas PROCAM (Prospective Cardiovascular Münster) y Framingham en pacientes receptores de tratamiento antirretroviral. Materiales y métodos. Estudio transversal analítico en población masculina VIH positiva que recibe tratamiento antirretroviral en un hospital de referencia nacional en Lima, Perú. Para evaluar la concordancia entre ambas escalas se empleó el método gráfico de Bland y Altman, correlación con el coeficiente de Pearson y acuerdo entre pruebas mediante el coeficiente de Kappa. Resultados. Enrolamos 111 personas, con edad promedio de 47,0 años. La distribución de pacientes en riesgo cardiovascular bajo, moderado y alto fue de 81,2%, 13,6% y 5,4%, según PROCAM y de 71,2%, 25,2% y 3,6% según Framingham, respectivamente. Según el método de Bland y Altman, la concordancia fue adecuada en valores bajos y se fue perdiendo en riesgos altos. El coeficiente de Pearson mostró fuerte correlación (r=0,87 y p<0,05) y el coeficiente de Kappa fue de 0,56 (p<0,001). Conclusiones. La concordancia existente a riesgos bajos se va perdiendo a medida que el riesgo se va incrementando. Existe fuerte correlación entre ambas escalas. Se recomienda realizar más estudios que permitan conocer qué escala es la más óptima para ser utilizada en población adulta VIH positiva que recibe tratamiento antirretroviral...


Objectives. The aim of the study is to determine the concordance between the PROCAM (Prospective Cardiovascular Münster) and Framingham scales in patients receiving highly active antiretroviral therapy (HAART). Materials and methods. A cross sectional study was conducted in HIV-positive male population who use HAART in a national reference hospital located in Lima, Peru. To evaluate the concordance between the two scales the graphic method of Bland and Altman was used, for the evaluation of the correlation we used the Pearson coefficient and to measure the agreement we use the kappa coefficient. Results. 111 patients were enrolled, with an average age of 47.0 years. The distribution of patients according to the risk was low, moderate and high, 81.2%, 13.6% and 5.4% respectively for PROCAM and 71.2%, 25.2% and 3.6% for Framingham. According to the graphic method of Bland and Altman, the concordance was adequate in low values and was lost as the risk score increased. PearsonÆs test found a strong correlation (r=0.87 and p<0.05) and the kappa coefficient was 0.56 (p<0,001). Conclusions. The agreement we found at low risk decreases as the risk increases. Strong correlation was found between the two scales. We recommend further studies in order to know which scale of cardiovascular risk is the most optimal scale for clinical practice among HIV population who receive HAART...

Objetivos. Analizar la variación nutricional de los beneficiarios actuales y pasados del Programa de Alimentación y Nutrición al Paciente Ambulatorio con Tuberculosis y Familia (PANTBC). Materiales y métodos. Se realizó un análi-sis secundario a partir de la valoración antropométrica por medio del índice de masa corporal (IMC) en beneficiarios actuales y pasados del programa PANTBC. Las mediciones se realizaron al inicio, segundo y quinto o sexto mes y se comparó la distribución del estado nutricional al inicio respecto al final (prueba de McNemar-Bowker). Resultados. Se incluyó información de 409 beneficiarios actuales y 110 beneficiarios pasados. Tanto en beneficiarios actuales como pasados se observó disminución progresiva de la condición de bajo peso (IMC<18,5) así como el aumento de la condición de sobrepeso. Los resultados de la tercera medición mostraron que el 57,7% de los beneficiarios actuales que iniciaron con bajo peso terminaron con peso normal, mientras el 20,7% de los que iniciaron con peso normal terminaron con sobrepeso. La comparación del estado nutricional al inicio respecto al final mostró cambios en la distribución (p<0,05). Conclusiones. A lo largo del programa PANTBC existe variación del estado nutricional de los beneficiarios, con la tendencia de pasar de un nivel inferior a otro superior de IMC.


Objectives. Analyze the nutritional changes among current and old beneficiaries of the Food and Nutrition Program for Outpatients with Tuberculosis and their Family (PANTBC, Spanish acronym). Materials and methods. A secondary analysis was conducted based on the anthropometric measurements with the body mass index (BMI) in current and old beneficiaries of the PANTBC program. The measurements were taken at the baseline, second and fifth or sixth month, and the distribution of the nutritional status was analyzed at baseline as compared to the endpoint (McNemar-Bowker test). Results. Information about 409 current beneficiaries and 110 old beneficiaries were included. A progressive decline in low weight as well as an increase in overweight were observed in both current and old beneficiaries (IMC<18.5). The results of the third measurement showed that 57.7% of current beneficiaries who started with low weight ended up with normal weight, while 20.7% of those who started with normal weight ended up with overweight. The analysis of the nutritional status at baseline compared to the endpoint showed changes in distribution (p<0.05). Conclusions. Across the PANTBC program, there is a variation of beneficiaries’ nutritional status, showing a tendency to go from a lower level to a higher level of BMI.

Objetivos. Determinar las características clínicas y demográficas de la neumonía por el virus de influenza AH1N1/2009 pandémico en un hospital de referencia de Perú. Materiales y métodos. Se realizó un estudio serie de casos en niños hospitalizados por neumonía por influenza AH1N1/2009 pandémico en un hospital de referencia. Revisamos las historias clínicas entre los meses de junio a septiembre 2009. Todos los casos tuvieron confirmación virológica. Resultados. Se encontró 74 casos de neumonía por el virus de Influenza AH1N1/2009 pandémico (NVIp), de los cuales 50 tuvieron el diagnóstico de neumonía adquirida en la comunidad viral (NACv) y 24 con neumonía nosocomial viral (NNv) de los cuales 16 requirieron ventilación mecánica. Fallecieron 12, todos ellos con antecedentes de comorbilidad. Los casos NNv presentaron asociación estadística con mortalidad. En los casos NACv, los menores de 6 años representaron 72 por ciento (36/50). La mediana de tiempo de enfermedad fue de 5 días. Los síntomas más frecuentes fueron fiebre, tos, rinorrea. Recibieron oseltamivir el 82 por ciento. En la radiografía de tórax el 48 por ciento de los casos presentó infiltrado en parches y el 44 por ciento infiltrado intersticial en la radiografía de tórax. La proteína C reactiva (PCR) mayor a 10mg/L tuvo una asociación significativa con insuficiencia respiratoria (p <0,05). Conclusiones. Encontramos casos NNv quienes tuvieron mayor mortalidad, también los que presentaron el PCR elevado y los que presentaron condición preexistente.


ObjectiveTo determine the clinical and demographic characteristics of pneumonia with influenza virus AH1N1/2009 pandemic at the National Institute of Child. Methods. Retrospective case series in children hospitalized for influenza pneumonia pandemic AH1N1/2009 in a pediatric hospital. Reviewed the medical records between the months of June to September 2009. All cases had virological confirmation, we describe the clinical characteristics and conditions of severity. Results. A total of 74 children of pneumonia with influenza virus AH1N1/2009 pandemic (NVIp), of those 50 were community acquire pneumonia viral (NACv) and 24 pneumonia nosocomial viral (NNv), 16 required mechanical ventilation. 12 died, all had preexisting factors. NN cases showed statistical association with mortality. The most frequent factors were malnutrition, respiratory infections, congenital heart disease and neurological deficits In NACv cases the children under 6 years accounted for 72 percent (36/50). The median disease duration was 5 days. The most frequent symptoms were fever, cough, runny nose. Received oseltamivir 82 percent. The chest radiograph 48 percent of cases showed patchy infiltrates and 44 percent interstitial infiltrate on chest radiograph. Protein c reactive (CRP) more than 10mg / L was significantly associated with respiratory failure (p <0.05). Conclusions. Cases of NN found who had more mortality, even those who had the highest PCR and those with preexisting condition.

Existe poca literatura nacional sobre las percepciones que tienen las personas viviendo con VIH/SIDA (PVVS) acerca de los servicios de salud vigentes que reciben. Objetivos: Recoger las percepciones de las PVVS sobre los servicios de salud especializados y el tratamiento antirretroviral de gran actividad (TARGA). Materiales y métodos: Estudio transversal con aplicación de una encuesta ad hoc, previamente validada, en los usuarios PVVS de los principales hospitales que proveen TARGA en cinco ciudades del Perú. Se planificó un muestreo no probabilístico de 488 PVVS y los participantes fueron enrolados por conveniencia en forma sucesiva hasta alcanzar el tamaño muestral. Resultados: Desde enero a marzo de 2007 se encuestó 578 PVVS en siete hospitales de las cinco ciudades. El 27,5 y 7,5 por ciento de los encuestados pertenecieron al género femenino y transgénero respectivamente. El 27,2 por ciento refirió que recibió el diagnóstico durante el 2006. El 28,2 por ciento refiere que antes había sido atendido en otro establecimiento y los principales motivos para el cambio fueron por indicación médica (51,5 por ciento) y maltrato del personal de salud (8 por ciento). La mayoría (85,3 por ciento) recibe actualmente TARGA, de los que no reciben, aproximadamente un tercio, ya tiene indicaciones para el inicio del tratamiento. De los que no han recibido TARGA, 10 por ciento refiere que es por desabastecimiento de medicinas Conclusiones: La implementación del Programa TARGA en el Perú evoca nuevas necesidades de investigación sobre la calidad de atención, acceso a atención integral y la adherencia.


National literature on people living with HIV/AIDS (PLWHA) perceptions on existing health services they receive is scarce. Objectives: To gather the perceptions of PLWHA on specialized health care services and highly active antiretroviral therapy (HAART) Materials and methods: A cross-sectional study with application of an ad-hoc survey, previously validated, in PLWHA users of the main hospitals that provide HAART in five cities of Peru. A non-probabilistic sample of 488 PLWHA was planned and participants were enrolled for convenience in succession to reach the sample size. Results: From January to March 2007, 578 PLWHA from 7 hospitals in 5 cities were enrolled. 27,5 per cent and 7,5 per cent of the respondents were female and transgender respectively. 27.2 per cent commented that received the diagnosis in 2006. 28,2 per cent referred previous management in another health facility and the main reasons for the change were for medical referral (51,5 per cent) and abuse of health personnel (8 per cent). 85,3 per cent are currently receiving HAART and of those who did not receive about one-third had already received indication for initiation of therapy, 10 per cent referred having not received HAART by shortages of medicines. Conclusions: The implementation of the HAART program in Peru evokes new research needs on the quality of care, access to comprehensive care and adherence.
