RESUMO

PURPOSE: Continuous positive airway pressure (CPAP) is the gold standard treatment for obstructive sleep apnea. However, the physiologic impact of CPAP on cerebral blood flow (CBF) is not well established. Ultrasound can be used to estimate CBF, but there is no widespread accepted protocol. We studied the physiologic influence of CPAP on CBF using a method integrating arterial diameter and flow velocity (FV) measurements obtained for each vessel supplying blood to the brain. METHODS: FV and lumen diameter of the left and right internal carotid, vertebral, and middle cerebral arteries were measured using duplex Doppler ultrasound with and without CPAP at 15 cm H(2)O, applied in a random order. Transcutaneous carbon dioxide (PtcCO(2)), heart rate (HR), blood pressure (BP), and oxygen saturation were monitored. Results were compared with a theoretical prediction of CBF change based on the effect of partial pressure of carbon dioxide on CBF. RESULTS: Data were obtained from 23 healthy volunteers (mean ± SD; 12 male, age 25.1 ± 2.6 years, body mass index 21.8 ± 2.0 kg/m(2)). The mean experimental and theoretical CBF decrease under CPAP was 12.5 % (p < 0.001) and 11.9 % (p < 0.001), respectively. The difference between experimental and theoretical CBF reduction was not statistically significant (3.84 ± 79 ml/min, p = 0.40). There was a significant reduction in PtcCO(2) with CPAP (p = <0.001) and a significant increase in mean BP (p = 0.0017). No significant change was observed in SaO(2) (p = 0.21) and HR (p = 0.62). CONCLUSION: Duplex Doppler ultrasound measurements of arterial diameter and FV allow for a noninvasive bedside estimation of CBF. CPAP at 15 cm H(2)O significantly decreased CBF in healthy awake volunteers. This effect appeared to be mediated predominately through the hypocapnic vasoconstriction coinciding with PCO(2) level reduction. The results suggest that CPAP should be used cautiously in patients with unstable cerebral hemodynamics.

OBJECTIVES: Liquid smoke rice husk has been researched and proved to consist of antibacterial, analgesic, anti-inflammatory, and growth factor stimulatory properties. By these complex properties, the liquid smoke rice hull is able to purpose as a novel coronavirus disease 2019 (COVID-19) inhibitor. The research was conducted to analyze the role of the dominant compound in rice husk liquid smoke against one of the main proteases in complex with inhibitor N3 of COVID-19 and 6LU7 protein data bank (PDB) ID. MATERIAL: AND METHODS: The Molecular Operating Environment (MOE) 2020.0901 (Chemical computing group) was used to analyzed the interaction. The molecular structure test, including phenol, mequinol, 2-methoxy-phenol, 6-octadecenoic acid, oleic acid, 9-cctadecenoic acid, was chosen. The lopinavir as positive control and 6LU7 as COVID-19 protein were chosen. All the protein analyses were conducted using docking molecular. RESULT: The phenol, 2-methoxy-phenol, mequinol and 9-octadecenoic acid have higher binding free energy that causes difficult to bind to the active site of protein 6LU7 (-3.4758, -3.5509, -3.6845, and -5.0173 kcal/mol, respectively). The minor component of liquid smoke, such as 6-octadecenoic acid and oleic acid, has the binding free energy (-5,5327 and -5,4208 kcal/mol) and more factor of binding presumably as active COVID-19 inhibitor. CONCLUSION: The liquid smoke rice husk has active component like 6-octadecenoic acid and oleic acid are presumably as active COVID-19 inhibitor.

OBJECTIVE: Rice husk liquid smoke nanoparticles have the potential to be developed as a drug because they have anti-inflammatory effects that can modulate the process of osteoblast stimulation through osteoblast stimulation by thorough small size and enter cells easily. The osteoblast is the key of alveolar regeneration in periodontitis treatment. This present study analyzed the differences of liquid smoke rice husk and nanoparticles of liquid smoke rice husk on osteoblast viability as periodontitis treatment MATERIALS AND METHODS: The liquid smoke rice husk was obtained from the pyrolysis process. The nanoparticles were made with chitosan, maltodextrin, and difference of concentration of liquid smoke rice husk (such as 1, 2.5, 5, 7.5, 10, 12.5, 15, and 17.5%). The viability of osteoblast was analyzed by 2,5-diphenyl-2H-tetrazolium bromide (MTT) assay. STATISTICAL ANALYSIS: The data were analyzed using independent t-test to analyze the differences between liquid smoke rice husk and nanoparticles of liquid smoke rice husk, the significant was set a p<0.05. RESULT: The nanoparticles of liquid smoke rice husk showed higher osteoblast viability compared liquid smoke rice husk. The nanoparticles' concentration of 5, 7.5, and 10% showed higher osteoblast viability compared liquid smoke rice husk (p = 0.002, 0.000, and 0.001, respectively). CONCLUSION: The nanoparticles of liquid smoke rice husk showed higher viability of osteoblast. This confirmed that the nanoparticles were able to reduce the toxicity in the higher concentration of liquid smoke of rice husk.

BACKGROUND: Currently, there is a high interest in 177Lu targeted radionuclide therapies, which could be attributed to favorable results obtained from 177Lu compounds targeting neuro-endocrine and prostate tumors. SPECT based dosimetry could be used for deriving dose values for individual voxels, as is the standard in external-beam radiation-therapy (EBRT). For this a time-activity-curve (TAC) at voxel resolution and also a voxel-wise modeling of radiation energy deposition are necessary. But a voxel-wise determination of TACs is problematic, since several confounding factors exist, such as e.g. poor count-statistics or registration inaccuracies, which add noise to the observed activity states. A particle filter (PF) is a class of methods which applies regularization based on a model of the temporal evolution of activity states. The aim of this study is to introduce the application of PFs for de-noising of per-voxel time-activity curves. METHODS: We applied a PF for de-noising the TACs of 26 patients, who underwent 177Lu-DOTATOC or -PSMA therapy. The TACs were obtained from fully-quantitative, serial SPECT(/CT) data, acquired at 4h, 24h, 48h, 72h p.i. The model used in the PF was a mono-exponential decay and its free parameters were determined based on objective criteria. The time-integrated activities (TIA) resulting from the PF (PFF) were compared to the results of a mono-exponential fit (SF) of individual voxels in several volumes of interest (kidneys, spleen, tumors). Additionally, an organ-averaged TIA was derived from whole-organ VOIs and subsequent curve-fitting. This whole-organ TIA was also compared to the whole-organ TIAs obtained from summation of the voxel-wise TIAs from PFF and SF. RESULTS: The number of particles was set to 1000. Optimal values for noise of observations and noise of the model were 0.25 and 0.5, respectively. The deviation of whole-organ TIAs from conventional organ-based dosimetry and the summation of the voxel-wise TIAs was substantial for SF (kidneys -22.3%, spleen -49.6%, tumor -60.0%), as well as for PFF (kidneys -37.1%, spleen -57.9%, tumor -70.9%). The distribution of voxel-wise half-lives resulting from the PFF method was considerably closer to the organ-averaged value, and the number of implausibly long half-lives (>physical HL) was reduced. CONCLUSION: The PFF leads to voxel-wise half-lives, which are more plausible than those resulting from SF. However, one has to admit that voxel-wise fitting generally leads to considerable deviations from the organ-averaged TIA as obtained by conventional whole-organ evaluation. Unfortunately, we did not have ground-truth TIA of our patient data and proper ground-truth could even be impossible to obtain. Nevertheless, there are strong indicators that particle filtering can be used for reducing voxel-wise TAC noise.

The STRiDE project sets out to support the development of effective dementia policy in middle-income countries (Brazil, India, Indonesia, Jamaica, Kenya, Mexico, and South Africa). As part of this it will generate new data about the prevalence of dementia for a subset of these countries. This study aims to identify the current estimates of dementia prevalence in these countries and where the gaps lie in the current literature. A systematic review was completed on 30th April 2019 across electronic databases, identifying dementia prevalence literature originating from any of the seven countries. Four hundred and twenty-nine records were identified following de-duplication; 28 studies met the inclusion criteria and were included in the systematic review. Pooled estimates of dementia prevalence ranged from 2% to 9% based on DSM-IV criteria; these figures were generally higher in studies using other diagnostic criteria (e.g. the 10/66 algorithm). Available prevalence data varied between countries. Only Brazil, Mexico and India had data derived from studies judged as having a low risk of bias. Irrespective of country, studies often were not explicit in detailing the representativeness of their sample, or whether there was non-response bias. Further transparent and externally valid dementia prevalence research is needed across the STRiDE countries.

BACKGROUND: Hepatitis B virus (HBV) genotype appears to show varying geographic distribution. Molecular epidemiological study of HBV in particular areas in Indonesia is still limited. This study was aimed to identify the prevalence of HBV genotype/subgenotype and mutations in basal core promoter (BCP) region in voluntary blood donors in Makassar, one of the biggest cities in east part of Indonesia. A total of 214 hepatitis B surface antigen (HBsAg)-positive samples were enrolled in this study. HBV genotype/subgenotype was identified by genotype-specific PCR method or direct sequencing of pre-S region. Mutations in BCP were identified by direct sequencing of the corresponding region. RESULTS: HBV/B and HBV/C were detected in 61.21% and 25.23% of the samples, while mix of HBV/B and HBV/C was found in 12.62% of the samples. Based on pre-S region, among HBV/B and HBV/C, HBV/B3 (95.00%) and HBV/C1 (58.82%) were predominant. Interestingly, HBV/D was identified in two samples (22.165.07 and 22.252.07). Complete genome sequences of two HBV/D strains (22.165.07 and 22.252.07) demonstrated that both strains belong to HBV/D6, and the divergence between the two strains were 1.45%, while divergences of both 22.165.07 and 22.252.07 strains with reference strain (AM422939/France) were 2.67%. A1762T/G1764A mutation was observed in 1.96% and 5.36%, whereas T1753V mutation was found in 2.94% and 1.79% of HBV/B and HBV/C, respectively. CONCLUSION: HBV/B and HBV/C are dominant in Makassar, similar to most areas in Indonesia. Mutations in BCP which might be associated with severity of liver disease are less common.

For the first time, inter-operator dependence of MRI based computational fluid dynamics (CFD) modeling of cerebrospinal fluid (CSF) in the cervical spinal subarachnoid space (SSS) is evaluated. In vivo MRI flow measurements and anatomy MRI images were obtained at the cervico-medullary junction of a healthy subject and a Chiari I malformation patient. 3D anatomies of the SSS were reconstructed by manual segmentation by four independent operators for both cases. CFD results were compared at nine axial locations along the SSS in terms of hydrodynamic and geometric parameters. Intraclass correlation (ICC) assessed the inter-operator agreement for each parameter over the axial locations and coefficient of variance (CV) compared the percentage of variance for each parameter between the operators. Greater operator dependence was found for the patient (0.19 < ICC < 0.99) near the craniovertebral junction compared to the healthy subject (ICC > 0.78). For the healthy subject, hydraulic diameter and Womersley number had the least variance (CV = ~2%). For the patient, peak diastolic velocity and Reynolds number had the smallest variance (CV = ~3%). These results show a high degree of inter-operator reliability for MRI-based CFD simulations of CSF flow in the cervical spine for healthy subjects and a lower degree of reliability for patients with Type I Chiari malformation.

Cerebrospinal fluid (CSF) dynamics in the cervical spinal subarachnoid space (SSS) have been thought to be important to help diagnose and assess craniospinal disorders such as Chiari I malformation (CM). In this study we obtained time-resolved three directional velocity encoded phase-contrast MRI (4D PC MRI) in three healthy volunteers and four CM patients and compared the 4D PC MRI measurements to subject-specific 3D computational fluid dynamics (CFD) simulations. The CFD simulations considered the geometry to be rigid-walled and did not include small anatomical structures such as nerve roots, denticulate ligaments and arachnoid trabeculae. Results were compared at nine axial planes along the cervical SSS in terms of peak CSF velocities in both the cranial and caudal direction and visual interpretation of thru-plane velocity profiles. 4D PC MRI peak CSF velocities were consistently greater than the CFD peak velocities and these differences were more pronounced in CM patients than in healthy subjects. In the upper cervical SSS of CM patients the 4D PC MRI quantified stronger fluid jets than the CFD. Visual interpretation of the 4D PC MRI thru-plane velocity profiles showed greater pulsatile movement of CSF in the anterior SSS in comparison to the posterior and reduction in local CSF velocities near nerve roots. CFD velocity profiles were relatively uniform around the spinal cord for all subjects. This study represents the first comparison of 4D PC MRI measurements to CFD of CSF flow in the cervical SSS. The results highlight the utility of 4D PC MRI for evaluation of complex CSF dynamics and the need for improvement of CFD methodology. Future studies are needed to investigate whether integration of fine anatomical structures and gross motion of the brain and/or spinal cord into the computational model will lead to a better agreement between the two techniques.
