RESUMO

Radionuclide transport with groundwater flow and subsequent doses to people are an aspect to be studied when assessing the long-term safety of geological nuclear waste repositories. A scenario where the radionuclide release migrates through a three-layer sediment structure of a lake in a farming environment is presented in this paper. The sediment column consists of deep (till), intermediate (glacio-aquatic sediment) and top layers (clay). The radionuclide release is assumed to enter the deep sediment layer from a bedrock fracture system at a rate of 1 Bq yr-1. The main objectives of the paper are to investigate the most contributing parameters, especially linked to the sediment layers, to the overall dose estimates for humans. The sensitivity analysis was conducted in two phases where the Morris method was used for screening and the Extended Fourier Amplitude Sensitivity Testing and Sobol's methods were used for estimating total-order indices. The studied radionuclides, 36Cl, 135Cs, 129I, 94Nb, 237Np, 90Sr, 99Tc and 238U, exhibit differences in how the sediment layers affect the concentration in the lake water used for drinking, irrigation and watering cattle and subsequently the dose conversion factors for humans through ingestion, inhalation and external radiation.

OBJECTIVES: Parental support has been shown to be important for children's self-esteem, which in turn is related to later important life outcomes. Today, an increasing number of children in the Western world spend time in both the parents' respective households after a separation. Children who live with both parents report more parental support than children who live only with one parent after a divorce. We took the opportunity of the commonness of children sharing their time between their parents' homes in Sweden to investigate children's self-esteem in relation to family type. STUDY DESIGN AND METHODS: With nationally representative survey data (ULF) collected from both parents and children, we analyze differences in children's self-esteem among 4823 10-18 year olds in nuclear families, joint physical custody and those living mostly or only with one parent after a separation using ordinary least squares regression, adjusting for demographic and socioeconomic characteristics. RESULTS: We found no significant difference in self-esteem between children who lived equally much with both parents, mostly with one parent and those in nuclear families, whereas children in single care showed lower self-esteem compared with children in the other living arrangements. The difference was not explained by socioeconomic factors. CONCLUSION: The self-esteem of children who share their time between their parent's respective homes after a separation does not deviate from that in their peers in nuclear families. Instead, those in single care reported lower self-esteem than those in the other living arrangements. These differences were not explained by socioeconomic factors. Longitudinal studies are needed to establish pre- and post-separation family characteristics that influence self-esteem and well-being in young people.

We demonstrate guided-mode resonance filters featuring an amorphous TiO(2) layer fabricated by atomic layer deposition on a polymeric substrate. The thermal properties of such filters are studied in detail by taking into account both thermal expansion of the structure and thermo-optic coefficients of the materials. We show both theoretically and experimentally that these two effects partially compensate for each other, leading to nearly athermal devices. The wavelength shift of the resonance reflectance peak (< 1 nm) is a small fraction of the peak width (~11 nm) up to temperatures exceeding the room temperature by tens of degrees centigrade.

Acute organ transplant rejection is characterized by a heavy lymphocyte infiltration. We have previously shown that alterations in the graft endothelium lead to increased lymphocyte traffic into the graft. Here, we demonstrate that lymphocytes adhere to the endothelium of rejecting cardiac transplants, but not to the endothelium of syngeneic grafts or normal hearts analyzed with the in vitro Stamper-Woodruff binding assay. Concomitant with the enhanced lymphocyte adhesion, the cardiac endothelium begins to de novo express sialyl Lewis(a) and sialyl Lewis(x) (sLea and sLex) epitopes, which have been shown to be sequences of L-selectin counterreceptors. The endothelium of allografts, but not that of syngeneic grafts or normal controls, also reacted with the L-selectin-immunoglobulin G fusion protein, giving further proof of inducible L-selectin counterreceptors. The lymphocyte adhesion to endothelium could be significantly decreased either by treating the lymphocytes with anti-L-selectin antibody HRL-1, or by treating the tissue sections with sialidase or anti-sLea or anti-sLex monoclonal antibodies. Finally, we synthetized enzymatically several members of the sLex family oligosaccharides and analyzed their ability to block lymphocyte adhesion to cardiac endothelium. The monovalent sLex (a tetramer), divalent sLex (a decamer), and tetravalent sLex (a 22-mer) could all significantly reduce lymphocyte binding, but the inhibition by the tetravalent sLex-construct was clearly superior to other members of the sLex family. The crucial control oligosaccharides, sialyl lactosamines lacking fucose but being otherwise similar to the members of sLex family, had no effect on lymphocyte binding.

We derive explicit analytical relations to describe paraxial light beams that represent a particular case of the hypergeometric (HyG) laser beams [J. Opt. Soc. Am. A25, 262-270 (2008)JOAOD60740-323210.1364/JOSAA.25.000262]. Among these are modified quadratic Bessel-Gaussian beams, hollow Gaussian optical vortices, modified elegant Laguerre-Gaussian beams, and gamma-HyG beams. Using e-beam microlithography, a binary diffractive optical element capable of producing near-HyG beams is synthesized. Theory and experiment are in sufficient agreement. We experimentally demonstrate the ability to rotate dielectric microparticles using the bright diffraction ring of a HyG beam.

The decay of (133m)Xe has been re-measured using an electron-transporter spectrometer and a planar HPGe detector. The sample of (133m)Xe was produced by means of proton-induced fission using an ion-guide based on-line mass separator. The deduced K and L+M+... shell conversion coefficients, alpha(Kappa)=6.5(9) and alpha(L+M+...)=2.9(4), agree within the uncertainty limits with the theoretical values and remove the inconsistencies between the previous experimental studies of (133m)Xe.

An electron spectrometry set-up was built at IRMM consisting of a vacuum chamber with a moveable source holder and windowless Peltier-cooled silicon drift detector (SDD). The SDD is well suited for measuring low-energy x rays and electrons emitted from thin radioactive sources with low self-absorption. The attainable energy resolution is better than 0.5keV for electrons of 30keV. It has been used to measure the conversion electron spectra of three plutonium isotopes, i.e. (238)Pu, (239)Pu, (240)Pu, as well as (241)Am (being a decay product of (241)Pu). The obtained mixed x-ray and electron spectra are compared with spectra obtained with a close-geometry set-up using another SDD in STUK and spectra measured with a Si(Li) detector at IRMM. The potential of conversion electron spectrometry for isotopic analysis of mixed plutonium samples is investigated. With respect to the (240)Pu/(239)Pu isotopic ratio, the conversion electron peaks of both isotopes are more clearly separated than their largely overlapping peaks in alpha spectra.

PURPOSE: As reported earlier, a chemotherapy regimen that consisted of dacarbazine, vincristine, lomustine, and bleomycin (DOBC) combined with natural leukocyte interferon (IFN) has been administered with favorable results to patients with metastatic melanoma. In this study, lymphocyte subsets (CD4+ and CD8+) were analyzed before and during treatment to elucidate if alterations in the CD4+/CD8+ ratio had any prognostic value. MATERIALS AND METHODS: Blood samples were systematically obtained from 54 patients with metastatic melanoma who received this chemoimmunotherapy. The frequencies of peripheral-blood lymphocyte subsets were monitored by flow cytometry using the monoclonal antibodies OKT4 (CD4+, T-helper cells) and OKT8 (CD8+, T-suppressor cells). RESULTS: Twenty-seven patients had a constantly increasing ratio, while the remaining 27 patients had a fluctuating or constantly decreasing ratio. The former group had a median survival time of 11.8 months, as compared with 6.5 months for the latter (P = .008, log-rank test). This difference was generated among patients who had an objective response. Responding patients with a constantly increasing ratio had a median survival time of 21.7 months, as compared with 10.2 months for patients with no constant increase in the ratio (P = .038, log-rank test). In nonresponders, no difference in survival was observed between the two groups. CONCLUSION: The monitoring of early changes in the CD4+/CD8+ ratio can provide valuable information that predicts the prognosis of metastatic melanoma patients receiving chemoimmunotherapy.

The UV-B-absorbing compounds of herbarium specimens of 10 subarctic bryophyte species collected during the years 1926-1996 and available at the Botanical Museum, University of Oulu, were studied. We studied whether herbarium specimens reflect changes in the past radiation climate through their methanol-extractable compounds. The order of gametophytes based on the average amount of total compounds (sum of A280-320 nm) per mass from the lowest to the highest was Polytrichum commune, Pleurozium schreberi, Hylocomium splendens, Sphagnum angustifolium, Dicranum scoparium, Funaria hygrometrica, Sphagnum fuscum, Sphagnum warnstorfii, Sphagnum capillifolium and Polytrichastrum alpinum, and the amount of UV-B-absorbing compounds per specific surface area correlated with the summertime daily global radiation and latitude. P. alpinum, F. hygrometrica and three Sphagnum species seem to be good indicators for further studies. The amount of UV-B-absorbing compounds revealed no significant trends from the 1920s till the 1990s, with the exception of S. capillifolium, which showed a significant decreasing trend.

OBJECTIVE: The objective of this study was to investigate the effect of selegiline first as monotherapy and then in combination with levodopa in the early phase of PD. METHODS: A total of 157 de novo PD patients were randomized to receive either selegiline or placebo in a double-blind study until levodopa therapy became necessary. Thereafter, the drugs were withdrawn for an 8-week washout period to evaluate the possible symptomatic effect of selegiline. RESULTS: Analysis of Kaplan-Meier survival curves for each group showed that selegiline delayed significantly the need for levodopa therapy (p = 0.028). The semiannual rate of disability progression was slowed down significantly in the selegiline group analyzed with the Unified Parkinson's Disease Rating Scale (total and motor scores; p < 0.001). Selegiline had a "wash-in" effect (i.e., an initial symptomatic amelioration of PD at 6 weeks and 3 months). However, after the 8-week washout period, no significant differences in the deterioration of disability between the groups was revealed in any of the scales, suggesting that besides having a slight symptomatic effect, selegiline may also have neuroprotective effects. Similarly, the progression of symptoms from baseline to the end of the washout period was significantly slower (p = 0.033) in the selegiline group when the progression was adjusted by the time to reach the end point. Selegiline was well tolerated. CONCLUSIONS: Selegiline delayed significantly the need to start levodopa in early PD. After a 2-month washout period (before the start of levodopa therapy) no significant symptomatic effect of selegiline was seen in comparison with the placebo group, supporting the concept of neuroprotective properties of the drug.

RESUMO

Spontaneous regression of advanced breast cancer is a rare phenomenon. Efforts have been made in order to explain it by means of immunological mechanisms. Corticosteroids have demonstrated important efficacy in the treatment of breast cancer. We present a patient with stage IV breast cancer in whom large tumour masses dramatically regressed during treatment with dexamethasone alone. In this patient, histological and hormonal findings, with results of analyses on surface and intracellular blood cells markers demonstrated significant redistribution of lymphocytes and accumulation of natural killer cells in tumour masses. It seems that dexamethasone has acted through the hypophyse against cancer.

RESUMO

In this paper we demonstrate that the vascular endothelium of a normal kidney and a syngeneic kidney graft binds only poorly syngeneic or allogeneic lymphocytes in an in vitro binding assay. On the contrary, the peritubular capillary endothelium of an allograft binds almost 7 times more lymphocytes than the peritubular capillary of a normal kidney. These findings suggest that the site of influx of inflammatory capillary white cells into the graft during rejection is the peritubular capillary endothelium.

Acute cardiac allograft rejection is characterized by infiltration of leukocytes into tissue parenchyma, but the site of entry and endothelial adhesion molecules involved are not yet defined. Lymphocyte binding to frozen sections prepared from day-3 rejecting cardiac allografts was significantly increased compared with sections made from normal hearts (number of bound lymphocytes, 983 +/- 216 per mm2 vs. 309 +/- 121, respectively, P < 0.001) or syngeneic grafts. The bound lymphocytes were located exclusively only on the top of the capillary structures and not on any other sites on the heart vasculature. We further wanted to analyze which of the cloned endothelial adhesion molecules and their counterreceptors would be involved in the increased lymphocyte binding. Lymphocyte pretreatment with mAb anti-CD11a or anti-CD49d inhibited this binding more than 50%. This inhibition on lymphocyte binding could not be increased by combining these two antibodies. Lymphocyte binding to endothelium has been shown to be at least partly organ specific; therefore, we asked whether increased lymphocytes adhere to cardiac allografts could be organ specific. Lymphocyte binding to lymph node high endothelial venules (HEV) has been shown to be inhibited by mannose-6-phosphate (M6P) and to kidney peritubular capillaries by mannose-1-phosphate (M1P). In the present study neither of these carbohydrates had any effect on lymphocyte binding to cardiac allograft endothelium. Monosaccharide inhibition studies demonstrate that the mechanism of lymphocyte adhesion to cardiac capillary endothelium differs from adhesion to kidney allografts or peripheral lymph node high endothelium.

The concept of scalar fields with uniformly rotating intensity distributions and propagation-invariant radial scales is extended to the case of electromagnetic fields with rotating but otherwise propagation-invariant states of polarization. It is shown that the conditions for field rotation are different for scalar and electromagnetic fields and that the electromagentic analysis brings in new aspects such as the possibility that different components of a rotating electromagnetic field can rotate in opposite directions.

The electromagnetic theory of self-imaging fields is considered. Several features are presented, which have no counterparts within the scalar theory of self-imaging. For example, the electromagnetic field self-images at one half of the classical self-imaging distance for scalar fields, the electric and magnetic energy densities can self-image while the scalar field components do not, and the self-imaging distances of the electric and magnetic energy densities can be different. In addition, general expressions for TE and TM polarized fields are presented by using the concept of the angular spectrum of the field.

When endothelial cells (EC) were stimulated with IL-1 and/or lymphocytes with rIL-2 or PHA, the binding of lymphocytes to EC was increased. PMA treatment of lymphocytes alone did not increase their binding to EC, but when EC were additionally induced with IL-1 the binding was increased. The expression of LFA-1 was constant, whilst the expression of CD49d was increased after rIL-2 and PHA stimulation. The PMA- and rIL-2-induced lymphocyte binding to IL-1-induced EC was inhibited by anti-CD11a, CD18 and CD49d mAbs; on the other hand, the enhanced binding of PHA-stimulated lymphocytes to EC could not be blocked by these mAbs. These results show that activation of lymphocytes by various stimuli leads to different usage of adhesion pathways in their binding to inflammatory EC.

Biopsy specimens from 12 patients with metastatic melanoma were longitudinally analysed to evaluate changes in proliferation activity and CD4+/CD8+ ratios during the course of the disease. The primary tumours of the patients who subsequently had metastatic disease were also each matched with tumours from two controls whose disease remained localized, and were compared with regard to tumour proliferation. Immunohistochemistry was performed using the avidin-biotin complex (ABC) immunoperoxidase technique, using bcl-2, p53, mdm-2 and Ki-67 as the primary monoclonal antibodies, and the percentage of positively stained melanocytic cells was calculated. Frozen sections were also available from metastatic lesions excised from eight of our patients before treatment initiation and at the time of disease progression. These specimens were prepared for microscopy, and quantitative characterization of CD4+ (OKT 4a) and CD8+ (OKT 8) cells was performed. Compared with the localized melanomas bcl-2 expression was higher in those primary melanomas that later metastasized (P = 0.068, Wilcoxon; P = 0.038, median test). Mdm-2 and Ki-67 expression did not differ in the primary tumours of patients and controls, but a statistically significant trend was observed towards increasing expression with the progression of the disease (two-sided exact P-values: 0.04 and 0.05, respectively). Patients with a low Ki-67 index in their first metastasis had a better prognosis when compared with patients with high indexes (P = 0.008, log-rank). Furthermore, most patients with decreasing CD4+/CD8+ ratios had increasing p53 immunoreactivity. Our findings suggest that Ki-67 and bcl-2 may be useful for predicting the prognosis of melanoma patients. Mdm-2 is a new but promising marker in melanoma and deserves further evaluation.

RESUMO

PURPOSE: To assess the Doppler indices best suited for detecting a stenosis of the internal carotid artery. MATERIAL AND METHOD: 358 patients (234 men, 124 women, mean age 59 years, range 25-83 years) were examined via duplex carotid sonography (US) and arteriography. Flow indices and B-mode real-time results of stenoses were compared with arteriographic findings. RESULTS: The accuracy of US in differentiating a 50% or more severe ICA stenosis was 93% and the correlation coefficient between angiographic and US stenosis was 0.94. Peak systolic velocity of the internal carotid artery (vpICA) and its ratio to the systolic (vpICA/vpCCA) and diastolic velocity of the common carotid artery were most accurate at 70% stenosis. The vpICA/vpCCA ratio was even slightly more accurate in cases of less severe stenosis. B-mode real-time measurement of diameter stenosis was most accurate at the < 30% level. There was large variation in the flow values of the ICA, mostly due to the variability of flow in the common carotid artery. The vpCCA was low in wide and higher in medium-sized or narrow common carotid arteries. Contralateral ICA stenosis also affected the flow and thus on the vpICA/vpCCA ratio and the differentiation between significant and non-significant stenosis. CONCLUSIONS: At duplex US, B-mode real-time measurement is suitable for screening small carotid plaques and flow indices, especially vpICA and vpICA/vpCCA in severe stenoses.

We reviewed the results of 187 consecutive ultrasound-guided fine-needle biopsies of the pancreas in 171 patients to assess the diagnostic accuracy of the method. The final diagnosis obtained at operation, autopsy or follow-up were: adenocarcinoma (n = 83), metastatic cancer (n = 11), cystadenocarcinoma (n = 2), lymphoma (n = 2), malignant gastrinoma (n = 1), pseudocyst (n = 25), cyst (n = 13), chronic pancreatitis (n = 9), normal pancreas (n = 10), abscess (n = 7), benign islet-cell tumour (n = 5), cystadenoma (n = 3). Sufficient cytologic material was obtained in 95.3% of biopsies and the overall accuracy in distinguishing benign from malignant disease was 85.4%. False negative results were obtained in 12 patients (13.1%). Inconclusive results (CIII) were found in aspirates from one cyst and two islet cell tumours. There were no false-positive results. The only complication was a post-biopsy haematoma around the head of pancreas, which resolved spontaneously. Ultrasound-guided pancreatic fine-needle biopsy is a safe method and allows of a high degree of diagnostic accuracy. It has a high specificity. Its sensitivity in the detection of malignancy improves if biopsies are repeated in doubtful cases. It further permits tumours to be graded and allows complications of pancreatitis to be diagnosed.

RESUMO

It appears that allograft rejections can be considered as a series of cellular and molecular cascades of inflammation triggered by the immune response. In acute rejection, inflammatory changes with a prominent blast cell component dominate. The main result is toxic damage of several parenchymal components, particularly the microvascular endothelium, resulting in necrosis of the parenchyma and loss of the allograft within days after transplantation. In the chronic type of rejection, inflammation also exists, but the major manifestations are different. Adventitial inflammation and perivascular cuffing of leukocytes result in concentric intimal proliferation and atherosclerosis. Very little experimental data exist, at present, on the mechanisms of transplant atherosclerosis and chronic rejection.
