AIMS: To compare the efficacy of pelvic floor muscle exercises (PFME) with and without electromyographic biofeedback (BF) in increasing muscle strength, improving myoelectric activity, and improving pre-contraction and quality of life in postmenopausal women with stress urinary incontinence. METHODS: Randomized controlled trial of 49 postmenopausal women with stress urinary incontinence. Participants were allocated across three groups: control, PFME alone, and PFME + BF. Forty-five women completed the study (14 control, 15 PFME, 16 PFME + BF; mean age 58.26 years). Outcome assessment was carried out using digital palpation (modified Oxford grading scale), electromyography, and the International Consultation Incontinence Questionnaire-Short Form (ICIQ-SF) quality of life instrument. The treatment protocol consisted of eight twice-weekly, 20-min one-on-one sessions. Controls were assessed only at baseline and after 1 month. RESULTS: The PFME and PFME + BF groups exhibited significant increases in muscle strength (Oxford scale) (P < 0.0001), precontraction while coughing (P < 0.0001), maximum voluntary contraction, duration of endurance contraction, and ICIQ-SF scores (P < 0.0001). PFME + BF was associated with significantly superior improvement of muscle strength, precontraction while coughing, maximum voluntary contraction, and duration of endurance contraction as compared to PFME alone (P < 0.05). CONCLUSIONS: This preliminary study suggests that pelvic floor muscle training, with and without biofeedback, is associated with increased muscle strength, myoelectric activity, precontraction of pelvic floor muscles, and improved quality of life in postmenopausal women with stress urinary incontinence.

The aim of this narrative review was to determine effects of whole body vibration exercises (WBVE) on the pelvic floor muscle (PFM) of healthy and unhealthy individuals. Searches were performed in the databases PubMed, Scopus, Science Direct and PEDRo. The level of evidence and methodological quality of the selected papers were determined. It was included six studies with a total of 189 participants (95.76% women) with ages ranging from 18 to 68 years. It was reported that WBVE: (I) improves the PFMs strength and quality of life (QOL) in individuals with urinary incontinence; (II) does not cause (PFM) fatigue in nulliparous continent women; (III) leads to higher (PFM) activation in subjects with weakened (PFM) and achieves higher pelvic floor (PF) activation than maximum voluntary contraction alone; (IV) in an individual with postprostatectomy stress urinary incontinence (SUI), over a period of 6 weeks after starting treatment, the patient regained continence (usage of 1 safety pad) and (V) has a significant effect on the electromyographic response and additionality and the rating of perceived exertion (RPE) significantly increased with increased frequency of the mechanical vibration. Relevant findings are presented and demonstrated that the WBVE might be highly relevant to the management of clinical disorders of the (PFM). Nevertheless, this intervention must be more understood and known to be used in the management of individuals with impairment of the (PFM) and there is the necessity of more research in this area.

Abstract Introduction: The climacteric is one of the transition periods in the life cycle of women between the reproductive and non-reproductive phase, and it is characterized by various metabolic, psychological and social changes, either by psychic disorders or sexual dysfunctions promoting changes in quality of life. Objective: To analyze the repercussions of electrical stimulation on quality of life, lubrication and myoelectric activity of pelvic floor muscles in postmenopausal women. Methods: This was a pilot study in which the sample consisted of ten menopausal volunteers aged 48-60 years. We used the Female Sexual Function Index questionnaire to determine the improvement in lubrication and the WHOQOL-BREF questionnaire to assess quality of life. The pelvic floor muscles were evaluated by electromyography with an intracavitary electrode. The protocol used was: phasic contraction evaluated by three contractions of two seconds and six of rest; three tonic contractions for ten seconds with ten-second rest; and endurance for thirty seconds. The intervention was performed by another researcher, through circulatory stimulation with the Dualpex 961 Quark®. Results: Electromyography after circulatory stimulation showed statistically significant results with an increase in Fast Fourier Transform in tonic and phasic contractions, obtaining a positive effect on the perception and awareness of contractions due to increased blood flow. Also, there was improvement in lubrication and quality of life in all areas. Conclusion: Circulatory stimulation had repercussions on pelvic floor muscles in menopausal women in both muscle physiology and lubrication, with an influence on the quality of life of volunteers.


Resumo Introdução: O climatério constitui um dos períodos de transição no ciclo vital da mulher entre a fase reprodutiva e a não reprodutiva, caracterizado por várias alterações metabólicas e psicológicas, seja por distúrbios psíquicos ou disfunções sexuais, promovendo alterações na qualidade de vida. Objetivo: Analisar as repercussões da eletroestimulação na qualidade de vida, lubrificação e atividade mioelétrica dos músculos do assoalho pélvico em mulheres menopausadas. Métodos: Trata-se de um estudo piloto no qual a amostra foi composta por dez voluntárias em menopausa com idade entre 48 e 60 anos. Para avaliar a melhora da lubrificação foi utilizado o questionário Female Sexual Function Index. Quanto à qualidade de vida, utilizou-se o questionário WHOQOL-bref. Já para a avaliação dos músculos do assoalho pélvico, utilizou-se a eletromiografia com eletrodo intracavitário. O protocolo utilizado foi: contração fásica avaliada por três contrações de dois segundos e seis de repouso; três contrações tônicas sustentadas por dez segundos com repouso de dez segundos; endurance durante trinta segundos. A intervenção foi realizada por outra pesquisadora, através de estimulação circulatória com o aparelho Dualpex 961 Quark®. Resultados: A eletromiografia pós-estimulação circulatória obteve resultados significativos, com aumento na Fast Fourier Transform nas contrações tônicas e fásicas, obtendo efeito positivo na percepção e conscientização das contrações devido ao aumento do fluxo sanguíneo. Houve, também, melhora na lubrificação e na qualidade de vida em todos os domínios. Conclusão: A estimulação circulatória apresentou repercussões nos músculos do assoalho pélvico em mulheres menopausadas tanto na fisiologia muscular quanto na lubrificação, influenciando a qualidade de vida das voluntárias.

Introduction: Anal incontinence is defined as the loss of voluntary control of fecal matter or gases with a recurrence period longer than 3 months in individuals aged ≥ 4 years; it has a female predominance. Among the treatment modalities is pelvic physiotherapy, the second line of treatment, which promotes the reeducation, coordination, and strengthening of the muscles of the pelvic floor to enable patients to return to their regular activities of daily living. Objective: To perform a systematic review on the physiotherapeutic treatments used in women between the ages of 18 and 65 years with a diagnosis of anal incontinence. Material and methods Clinical studies written in Portuguese, Spanish and English were searched on the the following databases: Science Direct, Medical Literature Analysis and Retrieval System Online (Medline) via PubMed, Physiotherapy Evidence Database (PEDro), Scientific Electronic Library Online (SciELO), and Scopus. Results: Of the 998 articles found, only 4 studies met the inclusion criteria of the present systematic review. The physiotherapeutic approaches to treat women with anal incontinence are biofeedback, Kegel exercises, electrostimulation, and training of the pelvic floor muscles. The average score on the PEDro scale was of 6.25, which indicates that the methodological quality was good. Conclusion: Although pelvic physiotherapy is effective to treat anal incontinence, it must be promoted through the performance of evidence-based scientific research. (AU)
