RESUMO

The first description of ligand-independent activating mutations in the KIT gene, which encodes the tyrosine-kinase KIT, greatly improved our understanding of gastrointestinal stromal tumour (GIST) biology. The therapeutic success in GIST has made tyrosine kinase inhibitors a "paradigm of targeted therapy". Deciphering resistance mechanisms in GIST has had implications for many other kinase-driven cancers. To exchange current knowledge within the field of GIST, the German GIST Meeting has taken place for now 10 years, traditionally in Göttingen. Subjects discussed include clinical diagnostics, pathology, surgery, and medical therapy. The following presentation gives an overview of the last meeting held in December 2013, including distinctive features in GIST and current data on the different topics.

RESUMO

The aim of this article is to demonstrate a right lobe hemihepatectomy to obtain a partial liver transplant for an adult organ recipient. Partial liver resection is carried out in the form of an ultrasound dissection of the parenchyma via a conventional (open) approach with a completely retained perfusion during the resection phase. The gall ducts are separated in the parenchymal phase and the peripheral segment of the middle hepatic vein is resected with the donor liver lobe. The surgical technique is demonstrated in detail with the help of a video of the operation that is available online. Furthermore, the advantages and disadvantages of the technique are discussed.

We report diagnosis and treatment problems encountered in 4 cases of adenocarcinoma in situ (AIS) of the uterine cervix. The patients' ages ranged from 24 to 45 years. In 2 cases, Pap smears revealed strips of cells showing crowded nuclei and pseudostratification of AIS. In 3 cases, the diagnosis of AIS was confirmed from a colposcopically directed biopsy and, in the last case, from a surgical cone biopsy. In 3 cases, AIS was found to be associated with cervical squamous intraepithelial neoplasia. In all cases, it was difficult to rule out an early invasive adenocarcinoma. Three patients underwent total abdominal hysterectomy. The topographic data of lesions, notably the presence of endocervical AIS in hysterectomy specimen realised 4 years after conization, and a review of literature lead us to propose hysterectomy as a basic treatment. If conization is chosen, it should be cylindrical with adequate diameter and height.

RESUMO

Small cell carcinoma of the ovary is a rare histological form which is highly malignant. We have decided to consider this as an epithelial tumour after studying it with the optical and the electron microscope. It is however difficult to put it in to a definite type because it is highly undifferentiated. On the clinical level small cell carcinoma is different from other ovarian cancers in several respects. Whereas ovarian tumours usually tend to occur in menopausal women in the fifth or sixth decade of life, small cell carcinomas of the ovary occur mainly in younger women of ages between 10 and 38 years. Furthermore, women who are attacked by this kind of a carcinoma tend to die very soon after the diagnosis has been made without receiving any benefit from the many therapies that have been tried to improve the prognosis. We have studied the condition after seeing two cases of small cell carcinoma in the University Gynaecological Department of Tours, and we have made a study of the literature. We have studied the therapeutic angle for these tumours and we have tried to find out what the best technique will be to cope with this awful prognosis. We discuss whether it might be worthwhile in future to intensify therapy in combination with bone marrow autotransplantation.

BACKGROUND: Image and video-based results and process control are essential tools of a new teaching concept for conveying surgical skills. The new teaching concept integrates approved teaching principles and new media. METHOD: Every performance of exercises is videotaped and the result photographically recorded. The quality of the process and result becomes accessible for an analysis by the teacher and the student/learner. The learner is instructed to perform a criteria-based self-analysis of the video and image material by themselves. RESULTS: The new learning concept has so far been successfully applied in seven rounds within the newly designed modular class "Intensivkurs Chirurgische Techniken" (Intensive training of surgical techniques). Result documentation and analysis via digital picture was completed by almost every student. The quality of the results was high. Interestingly the result quality did not correlate with the time needed for the exercise. The training success had a lasting effect. CONCLUSION: The new and elaborate concept improves the quality of teaching. In the long run resources for patient care should be saved when training students according to this concept prior to performing tasks in the operating theater. These resources should be allocated for further refining innovative teaching concepts.

BACKGROUND: Small bowel adenocarcinoma is a rare disease. The diagnosis is often disguised by nonspecific and varied presenting symptoms. Adenocarcinoma of the small bowel is typically detected at a late stage and with a poor prognosis. PATIENTS AND METHODS: The records of 42 patients with small bowel adenocarcinomas diagnosed in 2 surgical clinics between 1995 and 2009 were reviewed for patient and tumor characteristics, treatment effects and survival. RESULTS: The tumor locations were the duodenum (50%), jejunum (38%) and ileum (12%). In comparison to patients seen before 2004 the observed frequency of emergency operations or palliative procedures as well as stage distribution did not change. The median overall survival was 19 months with a 5-year overall survival of 20%. For patients with resections, reduced performance status, higher stage and residual disease after resection predicted decreased overall survival in univariate analysis. Residual disease and pT category were predictive of survival in multivariate analysis. CONCLUSION: Complete resection provides the only means of a cure. In cases where curative resection cannot be performed the prognosis remains poor. Further study on the methods for early detection and effective adjuvant chemotherapy should be investigated, however, the available data are limited.
